Subconjunctival Granuloma after Manual Small Incision Cataract Surgery

Presented herein, is an unusual case of a subconjunctival granuloma formation after an uneventful small incision cataract surgery. Subconjunctival nodules present unique challenges to the ophthalmologists in terms of establishing the correct diagnosis. Excision biopsy of the nodule resulted in complete resolution of symptoms. Histopathology of the mass revealed granulomatous inflammation, consistent with the diagnosis of a foreign body granuloma.

A comparative study of effect of SICS and phacoemulsification on corneal endothelium

Introduction: Senile cataract has been documented to be the most significant cause of bilateral blindness in India. The aim of cataract surgery is no longer restricted to just visual restoration, but is now considered to be a refractive surgery i.e. to achieve a state of emmetropia. Preservation of corneal endothelial function is a major goal in cataract surgery. Objectives: Present study aims to highlight the importance of measurement of central corneal thickness and compares the effect of SICS vs Phacoemulsification on corneal endothelium. Material and Methods: This was a prospective study consisting of 101 patients who presented to the department of Ophthalmology, who fulfill inclusion criteria and are willing to enroll in the study. Standard uneventful small incision cataract surgery was done on 51 patients and standard uneventful clear corneal phacoemulsification was done on 50 patients. Change in central corneal thickness was observed post-surgery on day 7th and day 30th. This study was conducted over a period of two years. Results: Both groups showed increase in CCT values on post-operative day 7 indicating some endothelial cell disturbances, but the increase in CCT was comparable between the two groups. At day 30 there was a decrease in CCT value as compared to day 7, the decrease was more in SICS as compared to PHACO, but the difference between the two groups was statistically insignificant. Prospective study of incidence of cystoid macular edema in uneventful cataract surgery: a study of 100 cases

Background: Cystoid macular edema (CME) is the important postoperative complication of cataract surgery, which can compromise the result of a cataract surgery. It can occur even after uneventful cataract surgery, but the incidence rapidly increase after complicated cataract surgery. Fundus fluorescein angiography (FFA) has been used for long time for diagnosis of cystoid macular edema. Angiographic CME, which is the presence of fluorescein leakage within the macular area without visual impairment, must be differentiated from clinical CME. The incidence of the two forms is different, with the angiographic form being much more common. Angiographic CME was reported to occur in 3–70% of the patients 4–6 weeks after cataract surgery. Objective: To study the incidence of cystoid macular edema in uneventful cases of small incision cataract surgery (SICS). Materials and Methods: This prospective study comprised 100 eyes of 100 patients who have undergone SICS with IOL implantation. Incidence of CME was found with the help of FFA. Results: In the study, 40 male patients and 60 female patient who underwent uneventful SICS of whom 20% male were CME positive and 20% female were CME positive. Angiographic CME (20%) incidence was more than clinical CME (2%). The incidence of CME was more on 30th post-operative day as compared to 7th post-operative study. intracameral adrenaline increases the incidence of CME. Conclusion: Incidence of clinical cystoid macular edema is 2% in uncomplicated cataract surgery, incidence of angiographic cystoid macular edema is 20%.

Central retinal thickness measurement and observance of macular reaction using intraoperative OCT in cataract surgery before and after phacoemulsification

Purpose: To demonstrate feasibility of macular cube optical coherence tomography (OCT) scanning and intraoperative central retinal thickness (CRT) measurement with a novel combination of a Carl Zeiss MeditecTM Cirrus HD-OCTTM system adapted to the optical pathway of a Zeiss OPMI VISU 200TM surgical microscope (Intra-OP-OCT) during uneventful small-incision cataract surgery.

ABSTACT: AIM: To test the efficacy of topical betaxolol to recover unexplained visual loss after uneventful cataract surgery in hypertensive patients. Study design: Randomized clinical trial. MATERIALS AND METHODS: One hundred patients of decreased visual acuity (20\40 to 20/50 after best correction after 6 weeks of uncomplicated small incision surgery in hypertensive patients were enrolled. Fifty eyes received topical betaxolol twice daily and fifty eyes received placebo eye drops as a randomized comparison group and followed up for more than six months. Change in best-corrected visual acuity was considered as change of five letters (0.1 log MAR) or more. Statistical analysis: Significance of difference of results between study and control groups is determined by MannWhitney U test and Fisher Exact test. RESULTS: 14 (28 %) patients demonstrated three lines and 31 (62 %) patients showed two line improvement in best-corrected visual acuity under treatment group. Whereas one (0.3 to 0.0) and 11 (0.4 to 0.2) eyes of the placebo group demonstrated same amount visual improvement. CONCLUSION: Topical betaxolol significantly improves visual acuity in unexplained visual loss after uneventful small incision cataract surgery in hypertensive patients.

A Comparative Study of the Anti-inflammatory effect of Topical 1% Prednisolone and Topical 0.1% Dexamethasone Eye Drops after Cataract Surgery in Western Uttar Pradesh

Purpose: The aim of this study is to compare the anti-inflammatory efficacy of topical 1% prednisolone acetate and topical 0.1% dexamethasone sodium eye drops after uneventful small incision cataract surgery with posterior chamber intraocular lens implantation in patients of Western Uttar Pradesh. Method: This study was done at Saraswathi Institute of Medical Sciences (SIMS) Hapur. Data was collected from Ophthalmic OPD from October 2012 to March2013.72 patients who underwent uneventful small incision cataract surgery with posterior chamber intraocular lens implantation were taken for both study groups. The baseline parameters were same for both groups of patients. All the patients were followed up post operatively on day 1, day 3, 1 week, 2 week, 4 week and 6 weeks and post inflammatory response was graded according to severity. The anti-inflammatory efficacy of two study groups was compared by examination of operated eyes for anti-inflammatory response and visual acuity. Results: All results were correlated with final visual outcome, which showed that topical prednisolone acetate is clinically and statistically more effective than dexamethasone sodium in early post operative period to control the inflammation in uneventful small incision cataract surgery with posterior chamber intraocular lens implantation. Conclusion: This study conclude that topical 1% Prednisolone acetate is more effective than topical 0.1% Dexamethasone sodium eye drops in controlling postoperative inflammation and early visual rehabilitation after uneventful small incision cataract surgery with posterior chamber intraocular lens implantation.

Comparative evaluation of the antiinflammatory effect of topical 0.1% dexamethasone sodium and topical 1% prednisolone acetate eye drops after small incision cataract surgery in Indian eyes

Purpose: To compare the anti-inflammatory efficacy of topical 0.1% dexamethasone sodium and 1% prednisolone acetate after uneventful small incision cataract surgery (SICS)with in the bag posterior chamber intraocular lens( PCIOL) implantation in Indian eyes. Methods: 60 consecutive patients who underwent uneventful SICS with in the bag PCIOL implantation were randomly assigned to either of two study groups. Patients in both groups were similar in base line parameters. The anti-inflammatory efficacy of the two study drugs were compared by closely examining the operated eyes for inflammatory response and visual acuity. All the patients were followed up post operatively on day 1, day 2, day 7 and day 42nd and the severity of post inflammatory response was graded. Results: All results were correlated with final visual outcome, which showed that topical 1% prednisolone acetate is clinically and statistically more effective in early postoperative period than dexamethasone sodium to control the inflammation in uneventful SICS. Conclusion: The study showed that topical 1% prednisolone acetate is more effective than topical 0.1% dexamethasone sodium in controlling postoperative inflammation and in early visual rehabilitation in uneventful cataract surgeries in Indian eyes. However, the same cannot be said to be true in cases of cataract surgery with per operative complications.

Negative dysphotopsia associated with implantation of the Z9000 intraocular lens

Uneventful small-incision cataract surgery was performed in a 70-year-old man and a 62-year-old woman with implantation of a Tecnis Z9000 intraocular lens (IOL) (Pharmacia). No ocular pathology was present in the patients preoperatively. Unsolicited complaints of negative dysphotopsia were made postoperatively, and the symptoms have persisted for more than 1 year in both patients. Symptomatic negative dysphotopsia may be seen in association with the Z9000 IOL.

Effect of small incision cataract surgery on ocular blood flow in cataract patients

Purpose To evaluate the effect of small incision cataract surgery using peribulbar anesthesia on ocular blood flow in patients with senile cataract. Setting Department of Ophthalmology, University of Vienna, Vienna, Austria. Methods In 33 eyes of 33 consecutive patients scheduled for cataract surgery, ocular hemodynamics were measured preoperatively and 1 day, 1 week, and 1 month postoperatively. Measurements included fundus pulsation amplitude (FPA) with a laser interferometric method that assessed the pulsatile choroidal blood flow, mean blood flow velocity (MFV), and resistive index (RI) in the ophthalmic artery (OA) and the central retinal artery (CRA) with Doppler sonography. Systemic hemodynamics and intraocular pressure (IOP) also were measured. Results The FPA, MFV, and RI in the OA and the CRA did not change postoperatively from baseline values. Systemic hemodynamics, IOP, and as a consequence ocular perfusion pressure, also remained unchanged. Conclusion Uneventful small incision cataract surgery using peribulbar anesthesia did not affect ocular blood flow in patients with senile cataract between 1 day and 1 month postoperatively.

Cataract in Seckel Syndrome

Seckel syndrome is an extremely rare inherited disorder characterised by severe growth retardation in utero, which continues later in life, resulting in short stature. Seckel syndrome presents as microcephaly, mental retardation, and a beak-like nose. This report describes a patient with Seckel syndrome who had bilateral cataract and underwent uneventful small incision cataract surgery in both eyes. The association of cataract with Seckel syndrome has not been described in the literature to the best of the authors’ knowledge.
