Men's perceptions and perceived acceptability of their female partner's use of self-administered intravaginal therapies for treatment of cervical precancer in Kenya.

Cervical cancer continues to be a major health issue in low- and middle-income countries (LMICs). Despite increasing access to screening, access to precancer treatment remains a significant challenge in LMICs, highlighting a need for innovative, accessible and resource-appropriate treatment approaches, including self-administered therapies. A cross-sectional mixed-methods study was conducted among men aged 25-65 with a current female partner in Kisumu County, Kenya. Participants were sequentially recruited and surveyed to evaluate their understanding of human papillomavirus and cervical cancer, their views on screening and treatment and their attitudes toward self-administered therapies. Focus group discussions (FGDs) with a subset of the survey participants further explored their treatment preferences and perceptions. Two hundred fourteen men participated in the survey, and 39 men participated in FGDs. The median age was 39 years, and 51% had a primary school education or less. Most (96%) were in a committed relationship, and 74% earned $10 or less daily. There was strong support for self-administered topical therapies, with 98% willing to support their partners using such treatments if available. Additionally, most participants were open to supporting necessary abstinence or condom use, though 76% believed their partners might hesitate to request condom use. When given an option, most preferred their partner to self-administer such therapies at home compared to provider administration at a health facility, citing convenience, cost-effectiveness and privacy. Preferences varied between two potential therapies, 5-Fluorouracil and Artesunate, based on their administration frequency, duration and abstinence requirements. Qualitative findings largely supported the quantitative analysis. The study demonstrates strong support for self-administered topical therapies for cervical precancer among Kenyan men. Additional research on acceptability, feasibility and efficacy in different LMICs could pave the way for these therapies to help bridge current cervical precancer treatment gaps in these settings.

A qualitative study of ethnic Korean women and men's experiences of HPV and HPV vaccination in the United States

ABSTRACT Introduction Approximately 42.5% of adults aged 18–59 in the United States is estimated to be affected by human papillomavirus (HPV) infection. However, Asian Americans have the lowest HPV vaccination initiation rate compared to other racial groups. This study aims to explore the experiences of HPV and the HPV vaccination among ethnic Korean women and men in the United States. Methods A total of 33 ethnic Korean and Korean Americans aged 27–45 years living in the U.S. were recruited via word-of-mouth and social media using a purposive sampling strategy. They participated in an online survey. Of the 33 participants, 29 (14 females and 15 males) participated in in-depth interviews via password-protected Zoom. A content analysis approach was used to analyze the interviews. Results Only 32% of participants had received the HPV vaccine at least once (female: 35.3%, male: 12.5%). Six major themes emerged from data analysis: (1) awareness of HPV, HPV vaccine, and HPV-associated cancers; (2) attitudes toward the HPV vaccine; (3) barriers to HPV vaccination; (4) women's experiences and preferences for pap smear testing; (5) experiences with HPV diagnosis; and (6) HPV and HPV vaccination education preferences. Conclusion The findings highlight cultural factors that may impede the discussion about and uptake of HPV vaccination and HPV-associated cancer screening, which emphasize the need for culturally appropriate interventions to overcome stigma around HPV and enhance vaccination rates. Healthcare providers should consider ethnic and cross-cultural differences perceptions to effectively HPV-related health information. This study provides insight into the experiences and understanding of HPV and vaccination among ethnic Korean men and women, laying the groundwork for developing culturally-tailored programs that sim to increase HPV vaccination rates and mitigate the stigma and impact of HPV-related disease in this community.

Interactions among human papillomavirus proteins and host DNA repair factors differ during the viral life cycle and virus-induced tumorigenesis.

This review focuses on the impact of human papillomavirus (HPV) oncogenes on DNA repair pathways with a particular focus on how these relationships change as productive HPV infections transition to malignant lesions. We made specific efforts to incorporate advances in the understanding of HPV and DNA damage repair over the last 4 years. We apologize for any articles that we missed in compiling this report.

Patients’ knowledge and attitudes regarding human papillomavirus disease and the effectiveness of education in patients referred to the Dermatology Clinic of Baqiyatallah Hospital in 2020

"Introduction: Human papillomavirus infection considers one of the leading causes of infection-related cancer worldwide. The general public's range of knowledge and understanding of human papillomavirus is low; the present study was done intending to evaluate the knowledge and attitude of patients with human papillomavirus and the effectiveness of educational intervention in those patients. Methods: This was a quasi-experimental study. The study population included patients with genital human papillomavirus referred to Dermatology Clinic of Baqiyatallah Hospital in 2020. The patients filled out a pre-test questionnaire. In the next step, they were trained by the treating physician about the disease, transmission ways, complications, treatment methods, and laboratory tests, and available vaccines. After that, an educational pamphlet was given to the patient to complete and review the information. Moreover, a post-test was performed in the patient's next visit. Data were analyzed using SPSS software version 22. Results: 92 patients were included in the study. The levels of knowledge and attitude of patients were between 50 to 65% before training and training had a significant impact on improving the level of knowledge and attitude of patients (P 0.001) so that the level of knowledge and attitude of patients reached above 95%. Demographic factors such as age, gender, marital status, and level of education were not related to the initial knowledge and attitude of individuals based on the statistical analysis (P 0.05). However, the knowledge of married people about the complications of the disease was more than single ones (P = 0.032). Therapeutic strategies for systemic therapies of human papillomavirus-related oropharyngeal cancer.

Our understanding of human papillomavirus (HPV)-related oropharyngeal cancer (OPC) and its molecular basis continues to evolve and produce important insights into customized therapeutic strategies. Novel therapeutics exploiting HPV-related targets are being evaluated in the incurable setting, while the favorable prognosis of locoregionally advanced disease has stimulated investigation into de-escalation strategies. There is much opportunity for better personalization of standard therapy according to HPV status. This review discusses both current and investigational therapeutic strategies for HPV-related OPC.

HRRD: a manually-curated database about the regulatory relationship between HPV and host RNA

HPV (Human papilloma virus) is a kind of small double-stranded DNA viruses which is extremely associated with different cancers. The roles HPV plays in the host were gradually identified through the interaction between it (including its early genes) and host RNA. In recent years, increasing numbers of studies in HPV-related cancers have been published showing the relationship between HPV and host RNA. Here, we present a database named HRRD, which contains the regulatory relationship between HPV and RNA (mRNA, miRNA and lncRNA). The information was extracted from 10,761 papers in PubMed (up to December 1st, 2019). In addition, the sequence map of HPV (198 genotypes) is also contained. HRRD was designed as a user-friendly web-based interface for data retrieval. It integrated the information of interaction between HPV and RNA, which reflects the relationship between HPV and host. We hope HRRD will further provide a comprehensive understanding of HPV in carcinogenesis and prognosis. HRRD is freely accessible at www.hmuhrrd.com/HRRD.

Implementation of a targeted HPV educational program in a population with HIV.

Patients living with human immunodeficiency virus (PLWH) are at higher risk of developing human papillomavirus (HPV)-associated malignancies. This prospective, longitudinal study evaluated the baseline knowledge of PLWH regarding HPV infection and its association with head neck cancer, and it aimed to determine whether a focused educational session could promote both short- and long-term knowledge acquisition in this population. Twenty-seven subjects participated in an interactive educational session and completed pre-test and immediate and delayed (4-month) post-test questionnaires. When compared to their pre-test answers, subjects demonstrated significant improvements in all 28 questions immediately following education. Knowledge preservation was demonstrated 4 months after initial evaluation, with subjects performing significantly better than their pre-test scores in 24 of the original 28 questions. These results suggest that short, focused, educational programs for PLWH may promote a better understanding of HPV's association with human immunodeficiency virus (HIV) and HPV risk factors, methods of transmission, and prevention.

Awareness and knowledge of anal cancer in a community-recruited sample of HIV-negative and HIV-positive gay and bisexual men.

Following consultation with affected communities and relevant healthcare professionals, a questionnaire was developed that assessed knowledge, understanding and experience of anal HPV, HPV vaccination, screening and perceived personal risk of anal cancer. Participants were recruited through HIV community and GBM organisations and anonymously completed the questionnaire online. Of 1660 questionnaires returned, 1574 were analysed from men, of whom 1535 (97.5%) identified as GBM and 15.7% reported being HIV-positive. Most (51.8%) of the HIV-positive men and 68.1% of HIV-negative or unknown men thought their risk of anal cancer was the same, or lower, than that of the general population. Only a small minority (12.5%) reported ever having talked to their doctor about anal HPV and/or anal cancer and 11.6% reported ever having had an anal cancer examination. Less than one-third (31.5%) had heard of HPV vaccination and only 2.9% of men recollected receiving HPV vaccination. Knowledge and awareness of anal cancer was generally poor in a sample of HIV-positive and HIV-negative GBM. Specific information targeted at these people could potentially raise awareness, leading to earlier diagnosis, reduced burden of disease among GBM and less demands on the healthcare system. Young GBM might benefit from education regarding the importance of HPV vaccination.

Information needs of young women vaccinated against HPV attending colposcopy: a qualitative study

BackgroundPrevious studies have shown that woman attending their first cervical screening or colposcopy appointment experience negative emotions, primarily anxiety and fear. With the introduction of the Human Papillomavirus (HPV) vaccine, it is unknown whether these emotions will have altered or whether the information needs of vaccinated women will have changed. The objective of this study is to determine the knowledge, understanding and concerns that young women have about HPV when attending colposcopy and whether their information needs are met.MethodsThis is a qualitative study using semi-structured interviews which were audiotaped and transcribed. Data was analysed thematically, with recruitment until data saturation was reached. Women born after 01/09/1990 and attending colposcopy as a result of abnormal cytology were eligible to join the study. Recruitment took place in an out-patient regional colposcopy clinic, Aberdeen, Scotland.ResultsFifteen women were interviewed. The majority of participants had some knowledge and understanding of HPV, cervical screening and colposcopy. Knowledge about the HPV vaccine was more limited; a third of participants misunderstood the effectiveness of the vaccine believing that is provided complete protection, and were left feeling that it had failed them. Some also felt that they were “test cases” for the vaccine.ConclusionWith the introduction of the HPV vaccine, the information and support needs of young women attending colposcopy are not fully met, leaving women with unanswered questions. With increasing numbers of vaccinated women entering the screening programme, it is timely to review the information available to these women.

12 - Awareness and knowledge of anal cancer in a community-recruited sample targeting people with HIV and gay/bisexual men

Background Anal cancer disproportionately affects gay and bisexual men (GBM), especially those with HIV infection. We explored the awareness and understanding of human papillomavirus (HPV) and anal cancer in a community-based cohort, to inform future evidence-based public health interventions. Methods Participants recruited through advertisements at gay and HIV community organisations completed an anonymous online questionnaire that assessed knowledge, understanding and experience of anal HPV, HPV vaccination, anal cancer screening and perceived personal risk of anal cancer. Results Of 1660 questionnaires returned, 1535 (92.5%) were from GBM, of whom the majority thought their risk of anal cancer was the same, or lower, than that of the general population. 196 (13.5%) participants reported ever having talked to their doctor about anal HPV and/or anal cancer. The discussion was initiated by the patient in more than half (58.7%) of cases, but was more likely to be doctor-initiated by participants who were HIV positive than negative/unknown (51.6% vs 35.7%, p=0.037). Only a small minority (12.7%) had talked to their doctor about anal HPV and/or anal cancer and (11.8%) had an anal cancer examination. Less than one third (32.3%) had heard of HPV vaccination and only 3.0% of men aged ≤ 26 years had received HPV vaccination. Conclusions Knowledge and awareness of anal cancer was generally very poor in GBM, who are at elevated risk of anal cancer. Specific information targeted at this group could potentially raise awareness, leading to earlier diagnosis and improved outcomes. Young GBM need education around the importance of HPV vaccination.

Cervical Cancer Screening.

Cervical cancer screening has significantly reduced the incidence of cervical cancer in the United States. National recommendations have evolved to keep pace with advancement in our understanding of human papilloma virus (HPV) biology as well as improvements in HPV testing. This chapter summarizes present national screening guidelines, reviews information regarding recent development in HPV biology and testing, discusses some of the controversies related to screening and provides clinicians with talking points for their patients.

Human Papillomavirus Vaccine Awareness Among HIV-Positive Gay and Bisexual Men: A Qualitative Study.

This study explores HIV-positive gay and bisexual men's (GBM) understanding of human papillomavirus (HPV) and the HPV vaccine. Researchers conducted 15 in-depth interviews with HIV-positive GBM between the ages of 18-30 years old. Two participants had received a single dose of the HPV vaccine. Otherwise, the majority of participants had either never heard of the HPV vaccine or they perceived it as a resource for women only. Other commonly cited barriers to getting the vaccine included lack of provider recommendation to complete the vaccine series and vaccine costs. Future provider-driven interventions should focus on increasing HPV vaccine among age-eligible HIV-positive GBM.

Development of an in vivo infection model to study Mouse papillomavirus-1 (MmuPV1)

Preclinical model systems to study multiple features of the papillomavirus life cycle are extremely valuable tools to aid our understanding of Human Papillomavirus (HPV) biology, disease progression and treatments. Mouse papillomavirus (MmuPV1) is the first ever rodent papillomavirus that can infect the laboratory strain of mice and was discovered recently in 2011. This model is an attractive model to study papillomavirus pathogenesis due to the ubiquitous availability of lab mice and the fact that this mouse species is easily genetically modifiable. Several other groups, including ours, have reported that MmuPV1-induced papillomas are restricted to T-cell deficient immunosuppressed mice. In our lab we showed for the first time that MmuPV1 causes skin cancers in UVB-irradiated immunocompetent animals. In this report we describe in detail the MmuPV1-UV infection model that can be adapted to study MmuPV1 biology in immunocompetent animals.

Subcellular Trafficking of the Papillomavirus Genome during Initial Infection: The Remarkable Abilities of Minor Capsid Protein L2

Since 2012, our understanding of human papillomavirus (HPV) subcellular trafficking has undergone a drastic paradigm shift. Work from multiple laboratories has revealed that HPV has evolved a unique means to deliver its viral genome (vDNA) to the cell nucleus, relying on myriad host cell proteins and processes. The major breakthrough finding from these recent endeavors has been the realization of L2-dependent utilization of cellular sorting factors for the retrograde transport of vDNA away from degradative endo/lysosomal compartments to the Golgi, prior to mitosis-dependent nuclear accumulation of L2/vDNA. An overview of current models of HPV entry, subcellular trafficking, and the role of L2 during initial infection is provided below, highlighting unresolved questions and gaps in knowledge.

Rodent Papillomaviruses.

Preclinical infection model systems are extremely valuable tools to aid in our understanding of Human Papillomavirus (HPV) biology, disease progression, prevention, and treatments. In this context, rodent papillomaviruses and their respective infection models are useful tools but remain underutilized resources in the field of papillomavirus biology. Two rodent papillomaviruses, MnPV1, which infects the Mastomys species of multimammate rats, and MmuPV1, which infects laboratory mice, are currently the most studied rodent PVs. Both of these viruses cause malignancy in the skin and can provide attractive infection models to study the lesser understood cutaneous papillomaviruses that have been frequently associated with HPV-related skin cancers. Of these, MmuPV1 is the first reported rodent papillomavirus that can naturally infect the laboratory strain of mice. MmuPV1 is an attractive model virus to study papillomavirus pathogenesis because of the ubiquitous availability of lab mice and the fact that this mouse species is genetically modifiable. In this review, we have summarized the knowledge we have gained about PV biology from the study of rodent papillomaviruses and point out the remaining gaps that can provide new research opportunities.

‘Is it like one of those infectious kind of things?’ The importance of educating young people about HPV and HPV vaccination at school

The National Human Papillomavirus (HPV) Vaccination Program in Australia commenced in 2007 for girls and in 2013 for boys, using the quadrivalent HPV [4vHPV] vaccine. In Australia, students are primarily vaccinated en masse, on school grounds, after parental/guardian consent is obtained. Students most often receive little, or no, education at school about HPV or HPV vaccination prior to immunisation. There is also some uncertainty about where young people can and should obtain reliable information about the vaccine, outside of school. We conducted a cluster randomised controlled trial of a complex intervention in schools. This study aimed to improve: (1) student knowledge about HPV vaccination; (2) psycho-social outcomes and (3) vaccination uptake. In this paper, we briefly outline our educational intervention and discuss its implementation by educators including facilitators and barriers. We also discuss the study findings pertaining to student knowledge about HPV and HPV vaccination and their attitudes to vaccination across control and intervention schools. Study results showed students in intervention schools demonstrate greater knowledge and understanding of HPV and HPV vaccination. Greater knowledge and understanding of HPV and HPV vaccination appeared to promote positive attitudes towards vaccination and supported confidence with vaccination.

Prevalence of human papillomavirus, Chlamydia trachomatis, and Trichomonas vaginalis infections in Amazonian women with normal and abnormal cytology.

Sexually transmitted infections are an important cause of morbidity among sexually active women worldwide, and have been implicated as cofactors in the pathogenesis of cervical cancer. We investigated the prevalence of human papillomavirus (HPV), Chlamydia trachomatis (CT), and Trichomonas vaginalis (TV), and accessed the diversity of HPV in women with normal and abnormal cytology in Manaus, Brazil. We used polymerase chain reaction and HPV genotyping by direct sequencing. The chi-square test was used to calculate the absolute and relative frequencies of the categorical variables, and Fisher's test was used when P < 0.05. The level of significance was set at 5%. All statistical analyses were performed using R 2.9.0. There were statistically significant differences in age (P = 0.0395), education level (P = 0.0131), sexual partners (P = 0.0211), condom use (P = 0.0039), marital status (P < 0.0001), and pregnancy (P = 0.0003) between the normal and abnormal groups. HPV DNA was found in 36.56 and 93.88% of subjects in the normal and abnormal groups, respectively. A total of 19 genotypes were detected; HPV16 was the most common, followed by HPV58. The percentages of TV and CT DNA were 18.04 and 9.02% in the normal group, respectively. The percentages of HPV/TV and HPV/CT coinfection were 12.5% each in women with normal cytology. These findings improve our understanding of HPV, CT, and TV, and the distribution of HPV types, which may be relevant to vaccination strategies for protecting women from the north of Brazil from cervical cancers and precancerous lesions.

The 2014 Bethesda System and Pap-Smear

Dear Editor, Bethesda style reporting system, such as the system used in the cervix, is employed to report thyroid cytopathology and pancreaticobiliary tract cytology and urine cytology. Therefore, it is beneficial for gynecologists, endocrinologists, thyroid surgeons, gastroenterologists, general surgeons, and urologists (1). The Bethesda system (TBS) was modified in 2014, which altered compared to the 2001 version. According to the new evidence in 2014, the TBS is enhanced. The evidence includes an additional understanding of human papilloma virus (HPV) biology, HPV vaccination in at least hundred countries, and the high utilization of liquid base cytology in decades. It is also noteworthy that additional screening options such as Pap test with high-risk HPV (i e, cotesting) and primary high-risk HPV test are also considered (2). The following explanations can reveal the reason to modify TBS. 1. If the number of squamous cells is less than 5000 in the liquid base cytology, it is inadequate and HPV DNA test cannot be precisely analyzed (3). 2. The general categorization section declares that the reporting system of the endometrial cell for females is altered through 40 45 years old. Since the normal endometrial cell can exfoliate, detected in Pap smear, the reporting age altered. It leads to increase of the predictive value. It is noteworthy that the working group suggested that endometrial evaluation should be done only in postmenopausal situations and stated in the educational note. 3. The interpretation (result) section, non-neoplastic finding subsection and the following items are added which are optional to report. a. Keratotic changes: Keratin formation is not a normal process in a squamous cell of cervix and vagina. HPV, trauma and pessary are the main reasons of keratotic changes. It should be considered that cervical cancer are ruled out when the keratotic changes reported (4). b. Tubal metaplasia: While metaplasia is a benign process, an excess estrogen happens and turns it as a pathologic process. c. Pregnancy-associated changes: True stromal decidual nature of the cells in pregnancy, such as smudgy chromatin with nucleolus, can be mistaken with a high-grade squamous intraepithelial lesion. Therefore, changes associated with pregnancy should be reported as a normal process (5). 4. In the inflammation section, the lymphocytic (follicular) cervicitis was added. Due to the high-prevalence of sexually transmitted diseases, cellular changes consistent with herpes simplex and cytomegalovirus were added in the organism section. 5. In squamous epithelial abnormality section, the two-tiered classification system for low-grade squamous intraepithelial lesion (LSIL) and high-grade squamous intraepithelial lesion (HSIL) were approved again. On the other hand, squamous abnormality as LSIL and a few cells suggestive of concurrent HSIL were not added to the report. In glandular cell abnormality section, there is difference with the last Bethesda system. 6. The following notes were added after the other malignant neoplasm section: a. In adjunctive testing, provide a brief description of the test method(s); b. And report the result to be easily understood by the clinician.

Human papillomavirus infection mechanism and vaccine of vulva carcinoma

AbstractVulvar carcinoma is a rare tumor occurring in female patients. Though more than 40% of vulva cancers are due to the infection of human papillomavirus (HPV), understanding of HPV and vulvar carcinoma is insufficient. HPV expression is regulated by cellular and viral transcription factors that bind to specific elements within the ligase chain reaction. These proteins bind with different affinity to host cell proteins and disrupt normal epithelial differentiation and apoptosis. Immunotherapy does not target tumors, but instead targets the host immune system. Active immunotherapy is tumor-targeting or immune-targeting monoclonal antibodies and vaccines. Nonspecific active immunotherapy is mainly cytokine therapy. In the treatment and prevention of HPV, the most popular research projects were regarding peptide, recombinant protein and DNA-based vaccines, recombinant virus and other targets in HPV infection. Since the cervix and vulva are both susceptible areas, these studies may be able to help reduce prevalence of vulvar precancerous lesions and prevent all cancers caused by HPV.
