Peripheral vascular injuries in children

We report 94 arterial injuries in 91 children treated at the Department of Thoracic and Cardiovascular Surgery, Medical School of Dicle University, between 1978 and 1988. The average age was 10.3 (range, 3 to 14) years. Stab wounds were seen in 37 patients (40%). Gunshot wounds were seen in 21 patients (23%), and two cases were caused by iatrogenesis. Vein damage was present in 39 patients (44.4%), and nerve loss was observed in 20 patients (21.2%). Twenty-one cases (23.3%) were associated with fractures. Diagnosis of arterial injury was made by clinical findings. Arteriography was used in 15 patients (16.5%) as the diagnostic method. The surgical interventions performed for arterial repair in our series were end-to-end anastomosis in 61 (64.5%), interposition of autogenous reversed saphenous vein grafts in 22 (23.4%), lateral suture in 8 (8.3%), and ligation in 3 (3.9%) patients. Four patients (5.3%) underwent limb amputation.

Relationship of severity of lower limb peripheral vascular disease to mortality and morbidity: A six-year follow-up study

Among the considerations affecting the therapeutic approach to patients with atherosclerosis of the lower extremities is their associated risk of death, myocardial infarction, stroke, and limb loss. To investigate the relationship of these events to the severity of peripheral vascular disease we undertook a 6-year review of 247 consecutive patients undergoing lower extremity noninvasive vascular assessment. There were 130 men and 117 women with a mean age of 65 +/- 15 years. Patients were categorized into four groups according to their ankle-brachial pressure indexes at their first visit. Ninety-seven patients had normal indexes (greater than or equal to 0.92), 86 had indexes of 0.50 to 0.91, 39 had indexes of 0.31 to 0.49, and 25 had indexes within the ischemic range, less than or equal to 0.30. At 6 years 64% of the patients with ischemic indexes were dead. This incidence was significantly higher than that of any other patient category (p less than 0.01). Diabetes also had a significantly adverse effect on survival. The incidence of stroke and myocardial infarction was similar for all disease groups. Thirteen percent and 32% of patients with indexes of 0.31 to 0.49 and less than or equal to 0.30, respectively, underwent limb amputation. We conclude that patients with evidence of mild to moderate peripheral vascular disease have a survival rate and risk of vascular-related disorders similar to those of patients of similar age with little evidence of disease, whereas an ankle-brachial pressure index less than or equal to 0.30 is associated with a malignant prognosis.

Prostacyclin synthetase activity in human diabetic and nondiabetic vascular tissue

A decrease in the formation of prostacyclin (PGI2), a potent vasodilating and platelet antiaggregatory substance, has been implicated in the pathogenesis of diabetic vasculopathy. This defect, as well as others, may contribute to imbalances in the thromboregulatory system resulting in enhanced platelet aggregability, accelerated atherosclerosis, and subsequent vessel injury. Until recently the major thrust of relevant literature has been directed toward abnormalities in PGI2 quantity or function in vascular tissue from experimentally induced diabetic animal models. For the past 2 years our laboratory has studied prostaglandin metabolism in human diabetic and nondiabetic blood vessels. We determined prostacyclin synthetase (PGI2ase) activity in saphenous veins of diabetic and nondiabetic patients (HSV-D and HSV-ND) undergoing coronary artery bypass grafts and in tibial arteries and tibial veins of diabetic patients (HTA-D and HTV-D) and nondiabetic patients (HTA-ND and HTV-ND) undergoing limb amputation for arterial disease of the lower extremity. Carbon 14—labeled prostaglandin endoperoxide (PGH2) was incubated for 2 minutes with vascular microsomal protein. The products were separated via thin-layer chromatography and quantified by radiochromatographic scan. PGI2ase activity was determined by the formation of 6-keto-PGF1α, the stable breakdown product of PGI2. Results of this study indicate that (1) the microsomal fractions of all vascular tissues studied contain an active PGI2ase capable of forming PGI2; (2) formation is enzymatic, as the amount of product increased with increasing microsomal protein concentration; (3) there is no significant difference in PGI2ase activity between HSV-D and HSV-ND; (4) PGI2ase activity in HTA-D and HTV-D is less than in HSV-D and HSV-ND; and (5) PGI2ase activity appears to be lowest in HTA-ND and HTV-ND. These observations suggest one or more of the following conclusions. (1) A gradient in the pathophysiologic progression of diabetic vasculopathy may exist. (2) Differences in metabolic control of diabetes mellitus may influence PGI2ase activity and subsequent vasculopathy. (3) Differences in PGI2ase activity may be inherent in the specific type of vessel studied. (4) It cannot be distinguished from these data whether reductions in PGI2ase activity are a cause or effect of atheromatous disease.

Immediate and long-term phantom limb pain in amputees: Incidence, clinical characteristics and relationship to pre-amputation limb pain

In a prospective study 58 patients undergoing limb amputation were interviewed the day before operation about their pre-amputation limb pain and 8 days, 6 months and 2 years after limb loss about their stump and phantom limb pain. All but one patient had experienced pain in the limb prior to amputation. Pre-amputation limb pain lasted less than 1 month in 25% of patients and more than 1 month in the remaining 75% of patients. At the first examination the day before amputation 29% had no limb pain. The incidence of phantom pain 8 days, 6 months and 2 years after amputation was 72, 65 and 59%, respectively. Within the first half year after limb loss phantom pain was significantly more frequent in patients with long-lasting pre-amputation limb pain and in patients with pain in the limb immediately prior to amputation. Phantom pain and pre-amputation pain were similar in both localization and character in 36% of patients immediately after amputation but in only 10% of patients later in the course. Both the localization and character of phantom pain changed within the first half year; no further change occurred later in the course. The incidence of stump pain 8 days, 6 months and 2 years after limb loss was 57, 22 and 21%, respectively. It is suggested that preoperative limb pain plays a role in phantom pain immediately after amputation, but probably not in late persistent phantom pain.
