Assessment of ultrasound-guided tru-cut needle pleural biopsy in the diagnosis of unexplained exudative pleural effusion compared to medical thoracoscopy

Background Ultrasound-guided needle biopsy (USGNB) allows for precise and immediate viewing of pleural lesions, effusions, and needle placement without ionizing radiation. This study aimed to assess the efficacy of US-guided tru-cut needle pleural biopsy in diagnosing unexplained exudative pleural effusion compared to medical thoracoscopy (MT), focusing on safety, accuracy, time, and cost of the treatment. Patients and methods This observational cross-sectional research was conducted on 40 patients over 18 years old, of both sexes, with unexplained exudative pleural effusion. The patients were randomly separated into two equal groups. The girst group is USGNB and the second group is MT. Results Group II exhibited substantially greater costs, sedation levels, procedure duration, biopsy site discomfort, surgical emphysema, lung expansion failure, and psychological distress compared to group I (P<0.05). There was no notable disparity between the two groups regarding radiological detection of pleural effusion, pleural fluid properties, procedural observations, histological results, and diagnostic effectiveness. The duration of hospitalization was considerably shorter in group I compared to group II, assessed from the day of the operation to the day of release (P=0.001). Conclusion USGNB is an important tool in the diagnosis of pleural effusion after failure of pleural fluid analysis, which is performed by a chest physician is an accurate, safer, lower in cost, and shorter in duration with fewer complications compared with MT.

Comparison of Patients with Ultrasound Guided Tru-Cut Biopsy / Acid Cytology with Final Pathology Results from Patients Operated with Suspicious for Ovarian Cancer

Objective In women who are believed to have ovarian cancer but have poor performance status or have advanced disease believed to be beyond the scope of primary cytoreductive surgery, NACT can be given to patients with acid cytology and/or tru-cut biopsy referral. Our aim is to determine the accuracy, adequacy, safety and reliability of these minimally invasive interventional procedures. Material and Methods This is a retrospective analysis of 63 patients with a suspicioused of ovarian cancer in our hospital between 2014 and 2021, who underwent ultrasound-guided acid cytology and tru-cut biopsy, and also had postoperative final pathology results. Results Comparing acid cytology, tru-cut biopsy, acid cytology and tru-cut biopsy at the same time with the postoperative final pathology results, it was seen that the PPV was 100% in all groups. It was revealed that the sensitivity of acid cytology was 64%, the specificity was 100%, the NPV was 12%, and the accuracy of the test was 65%. The sensitivity of the tru-cut biopsy was 91%, the specificity was 100%, the NPV was 42%, and the accuracy of the test was 92%. In the case of both procedures, the sensitivity was calculated as 93% and the accuracy of the test was calculated as 93%. There were no false positive cytology and biopsy results that could lead to unnecessary NACT therapy in the study. Conclusion Minimally invasive procedures can be safely applied to patients with low adverse event and high accuracy rates, since they provide NACT in patients who are thought to be candidates for interval surgery.

Evaluation of transrectal ultrasound-guided tru-cut biopsy as a complementary method for predicting pathological complete response in rectal cancer after neoadjuvant treatment: a phase II prospective and diagnostic trial.

Patients with pathological complete response (pCR) of rectal cancer following neoadjuvant treatment had better oncological outcomes. However, reliable methods for accurately predicting pCR remain limited. To evaluate whether transrectal ultrasound-guided tru-cut biopsy (TRUS-TCB) adds diagnostic value to conventional modalities for predicting pathological complete response in patients with rectal cancer after neoadjuvant treatment. This study evaluated data of patients with rectal cancer who were treated with neoadjuvant treatment and reassessed using TRUS-TCB and conventional modalities before surgery. This study is registered with ClinicalTrials.gov. The primary outcome was accuracy, along with secondary outcomes including sensitivity, specificity, negative predictive value, and positive predictive value in predicting tumour residues. Final surgical pathology was used as reference standard. Between June 2021 and June 2022, a total of 74 patients were enroled, with 63 patients ultimately evaluated. Among them, 17 patients (28%) exhibited a complete pathological response. TRUS-TCB demonstrated an accuracy of 0.71 (95% CI, 0.58-0.82) in predicting tumour residues. The combined use of TRUS-TCB and conventional modalities significantly improved diagnostic accuracy compared to conventional modalities alone (0.75 vs. 0.59, P =0.02). Furthermore, TRUS-TCB correctly reclassified 52% of patients erroneously classified as having a complete clinical response by conventional methods. The occurrence of only one mild adverse event was observed. TRUS-TCB proves to be a safe and accessible tool for reevaluation with minimal complications. The incorporation of TRUS-TCB alongside conventional methods leads to enhanced diagnostic performance.

Effect of Metadoxine on Chronic Primary Hepatopathies in Canines: Number of Cases

Objective. To evaluate the effect of Metadoxine on paraclinical, ultrasound, and histopathological findings in canines with primary hepatopathies through a number of cases. Materials and Methods. Seven dogs aged 2 to 14 years with mild to moderate clinical signs of primary hepatopathy, abnormal bile acids, increased ALT or ALP values three times above the upper range for 4 weeks, underwent ultrasound evaluation and ultrasound-guided Tru-Cut liver biopsy to confirm hepatopathy according to the histological classification of the WSAVA. Prior to the biopsy, they were required to have fibrinogen >100 mg/dL, platelets >100 cells x 103/µL, and prothrombin time <11.5 seconds. Once the histological diagnosis was confirmed, they received metadoxine at 17±11.35 mg/kg every 12 hours for 4 weeks. Clinical, paraclinical, ultrasound, and histopathological follow-up was conducted at the end of this period. Results. A decrease in the occurrence of vomiting and anorexia was observed (p=0.03), a non-significant increase in ALT and ALP (p=0.84; p=0.43), a reduction in bile acids (p=0.4), and no change in ultrasound score (p=0.42) or histopathological diagnosis (p>0.05). Conclusions. Despite significant limitations, this number of cases provides preliminary observation suggesting that metadoxine may improve the clinical condition of dogs with liver disease without altering paraclinical, ultrasound, or histological variables; additional research with a Randomized Clinical Trial design is needed to propose metadoxine indication protocols for canines with hepatopathies in clinical practice.

Ultrasound-guided Tru-cut biopsy of pelvic tumors: First reported case series in the Philippines

This paper documents the first reported cases of patients in the country who underwent transvaginal ultrasound-guided Tru-cut biopsy of pelvic tumors in a tertiary level health institution in 2019. Different indications for Tru-cut biopsy were demonstrated highlighting its clinical utility in the diagnosis and management of malignant pelvic tumors. All patients warranted histologic diagnosis for further planning of the most appropriate management. Adequate tissue samples were obtained from all three patients with no procedure-related complications.

Primary hepatic angiosarcoma (PHA) is a sporadic and aggressive tumour of the liver that originates from mesenchymal cells and represents less than 2% of all primary liver tumours. It is known to be associated with several environmental and industrial carcinogens; however, in 75% of cases, aetiology remains unclear. Patients generally present with nonspecific symptoms and laboratory findings. Imaging has a limited role in the diagnosis. We herein present a case of a 52-year-oldman with a history of hepatitis B-related cirrhosis who was referred to our hospital for liver transplantation assessment. Magnetic resonance imaging (MRI) revealed two small nodular lesions of 5 and 6 mm in segment IV of the liver, categorised as Liver Imaging Reporting and Data System (LI-RADS) category 3. The patient was discussed at a multidisciplinary tumour meeting, and an MRI follow-up in three months was planned. Three months later, MRI depicted a substantial increase in the lesion size measured 8.5 cm. An ultrasound-guided tru-cut biopsy was performed, and the diagnosis of PHA was confirmed by pathology. In this report, we aim to highlight PHA's MRI features and underline this rare entity's rapid and fatal progression.

Outcome of Ultrasound Guided Trucut Biopsy of Adnexal Masses with Suspected Malignancy

Background: Ultrasound guided sampling techniques are frequently used in the tissue diagnosis of various tumours. Female patients commonly present with adnexal masses and require tissue diagnosis by trucut biopsy for initiation of the treatment. This study was done to determine the outcome of ultrasound guided trucut biopsy in patients presenting with suspected adnexal malignancy.
 Methodology: This cross sectional study was done at the Department of Radiology, MCH Centre, PIMS hospital, Islamabad from October 2018 to September 2020. In this study, all female patients aged 18 years and above with suspected adnexal malignancy were included. Trucut biopsy was performed under ultrasound guidance, sample sent for histopathology and various histopathological outcomes were assessed. Patients were kept under observation for 2 hours following biopsy. The data was entered and assessed by using SPSS version 24.0. Frequencies and percentages were calculated for nominal data and mean and standard deviation for numerical data.
 Results: Mean age of the subjects was 50.24±10.52 years and mean duration of symptoms was 2.97±1.23 months. Definitive diagnosis made in 59 (95.1%) out of 62 cases and only 3 (0.04%) cases were inconclusive; among which two showed inadequate sample and one showed normal tubo-ovarian tissue. 90% histopathological findings were conclusive and no complication was reported.
 Conclusion: Trucut biopsy has high diagnostic yield with no complications and most common malignancy detected was serous cystadenocarcinoma followed by mucinous carcinoma.

An uncommon case of primary synovial sarcoma of the lung with a rare intraoperative finding

We report a case of a 32-year-old male who was evaluated for right chest pain and cough with streaky hemoptysis. Chest X-ray and contrast-enhanced computerized tomography scan of the chest showed a large (20x16×16 cm), heterogeneously enhancing mass arising from the right lower lobe. Ultrasound-guided Tru-cut biopsy revealed spindle cell sarcoma. In immunohistochemistry, tumor cells expressed epithelial membrane antigen, CD99, and bcl-2. The final diagnosis of primary pulmonary synovial sarcoma was confirmed after positron emission tomography-computed tomography revealed that the lesion was confined to the right lung. The patient was managed by multimodal treatment. The patient underwent right anterolateral thoracotomy and lower lobectomy with staged systematic mediastinal lymphadenectomy. Intra-operatively, he was given 2 units of packed red blood cells (RBCs). The patient developed dark-colored urine near the end of the transfusion of the second unit of packed RBCs, but without any hemodynamic instability. All the tests for possible blood transfusion reactions were negative; thus, we concluded that this event was probably secondary to tumor lysis due to the handling of the large primary lesions during surgery.

Ultrasound-Guided Tru-Cut Biopsy in Gynecological and Non-Gynecological Pelvic Masses: A Single-Center Experience.

Aim: The aim of this study was to evaluate the feasibility of adequacy, accuracy, and safety of ultrasound-guided tru-cut biopsy in managing malignant and benign abdominopelvic masses in a selected population and critically discuss some issues in different situations, which deserve some reflections on those practices. Materials and Methods: This is a retrospective study involving 42 patients who underwent transvaginal or transabdominal tru-cut biopsy between August 2017 and November 2021. The inclusion criteria were poor health status or primary inoperable advanced tumor, suspicion of recurrence or metastasis to the ovaries or peritoneum in gynecological and non-gynecological pelvic malignancies. Tissue samples were considered adequate if it was possible to determine the origin of the tumor, and immunohistochemistry could be performed. Diagnostic accuracy was assessed considering the agreement between tru-cut biopsy histology and final postoperative histology. Results: It total, 44 biopsies were obtained from 42 patients (2 patients had repeat biopsies). The pathologist considered all pathological samples adequate (adequacy 100%). The final histology was consistent with tru-cut biopsy diagnosis in all but 2 cases (diagnostic accuracy 88.2%). If we consider only the cases that have carried out at least two diagnostic samples, accuracy rose to 94.1%. Pathological examinations from tru-cut samples showed 2 benign lesions (4.8%) and 40 malignant tumors (95.2%), divided into 19 advanced primary inoperable ovarian cancers, 7 primary advanced cervical cancers, 4 recurrent endometrial cancers, 3 recurrent cervical cancers, 3 recurrent ovarian cancers, 1 case of primitive peritoneal malignancy (leiomyosarcoma), and 3 non-gynecological cancers with a strong suspicion of metastases at ultrasound (2 cases of ovarian, colorectal cancer metastasis, and 1 case of pelvic site type B lymphoma metastasis). However, one case of minor complication related to the procedure was reported but not significant. Conclusions: The diagnostic adequacy, accuracy of the tru-cut biopsy, and safety were high. Pathological samples are representative of the disease and suitable for histological and immunohistochemical analysis.

Diagnostic yield of one-pore versus two-pore ultrasound-guided tru-cut needle pleural biopsy

Background Ultrasound (US)-guided tru-cut pleural biopsy is a rapid technique that is cheap, has low complication, provides real-time visualization of the biopsy needle, and is without radiation. Aim To increase diagnostic yield of US-guided tru-cut pleural biopsy by taking biopsy from two different sites and to study factors increasing this yield for an effective, safe, and noninvasive substitute of thoracoscopy. Patients and methods Randomly chosen 80 patients with undiagnosed pleural effusion were scanned by US and classified into two groups: one-pore group where tru-cut US-guided pleural biopsy was done using one opening in the most safe, accessible, thick nodular pleura and the other group is the two-pore group, taking the biopsy from two different site, making two pores instead of one. Results The two groups were comparable to each other in diagnostic yield, with no significant difference, and the factors affecting this yield were the more thickness of pleura, presence of nodularity or masses, and the caliber of the cutting needle. Conclusion There is no need for more than one pore for taking US-guided tru-cut pleural biopsy, and diagnostic yield depends on more needle caliber and the thickness of the pleura.

Diagnostic Value of Ultrasound-Guided Pleural Biopsy in Patients with Recurrent Pleural Effusion

Background: Establishing a definite diagnosis in cases of pleural effusion is sometimes challenging. Adding ultrasound-guided tru-cut biopsy at the time of obtaining pleural fluid for analysis may increase the diagnostic yield in such cases. Objective: The aim of the current work was to assess the diagnostic yield and the safety of percutaneous ultrasound guided tru-cut needle biopsy in patients with recurrent undiagnosed pleural effusion. Patients and Methods: A retrospective observational study including 45 patients with recurrent undiagnosed pleural effusion who underwent ultrasound-guided tru-cut needle biopsy in the period between January 2019 and October 2020. Results: By using ultrasound-guided needle biopsy, we were able to reach a final histopathological diagnosis in 40 patients with a diagnostic yield of 88.8%. Only 5 cases remained undiagnosed (11.1%). Mesothelioma was found in 13 cases (28.9%) and bronchogenic carcinoma in 7 cases (15.5%). Post-procedural complications included 1 case of pneumothorax (2.2%) and 1 case of hemothorax (2.2%) all resolving with the small caliber chest drain that was inserted at the conclusion of the procedure. Conclusion: It could be concluded that ultrasound-guided needle biopsy is a safe procedure that can be useful in establishing a diagnosis in patients with recurrent pleural effusion. Its high diagnostic yield, while being simpler and cheaper than CT guided biopsy or thoracoscopy, makes it a suitable option in our clinical settings. More studies are needed to validate our findings.

Role of Ultrasound-Guided Tru-cut Biopsy in the Diagnosis of Peripheral Lung and Pleural Lesions

Abstract Background Transthoracic needle biopsy is a well-established technique for diagnosing pulmonary lesions. Computed tomography (CT) is usually used as guidance. Ultrasound (US)-guided biopsy is a relatively affordable modality for diagnosis of peripheral lung lesions (PLLs; also known as peripheral pulmonary lesions [PPLs]) and peripheral pleural lesions. Objectives The purpose of this study was to study the diagnostic yield of US guidance sampling a consecutive series of peripheral lung and pleural lesions and potential factors influencing the diagnostic yield with recording the occurrence of any complications. Patients and Methods This was a prospective study that was conducted at Ain Shams University Hospitals upon a population of 60 patients, during the period from September 2018 to August 2019. A special puncture transducer is used to perform US-guided biopsy with visualization of the biopsy needle and the lesion; facilitating the sampling procedure. Results The use of US-guided transthoracic needle biopsies across 60 patients was shown to have a yield of 75% which found the occurrence of 45 conclusive and 15 non-conclusive results From this study population, 70% (n = 42/45) were found to have malignant manifestations, of which 26 were undifferentiated high grade adenocarcinoma, and 9 were moderately differentiated adenocarcinoma. As for complications arising from the biopsy procedure, twenty percent 20% (n = 12) of patients suffered from complications in the form of hemoptysis in 8 which was controlled by hemostatic measures and 4 patients acquired pneumothorax, three (¾) of them received high flow oxygen and conservative treatment and only one (1/4) patient had intercostal tube placement. Diagnostic yield was significantly increased with the presence of Wedge shaped hypoechoic lesions(p < 0.001), hard Mass consistency was significantly highly associated with conclusive results (p < 0.001), as well as a significant link between the longitudinal diameter of masses that had a mean length of 45.05mm ±12.93mm (#x0003D; 0.029).It also showed that more biopsies taken were highly significant correlation with conclusive outcomes (p < 0.001). Conclusion US-guided biopsy is a robust and accurate procedure to effectively diagnose peripheral lung lesions, with a low incidence of complications and gradually improving results with the mastery of the procedure.

Safety and efficiency of performing transvaginal ultrasound-guided tru-cut biopsy for pelvic masses

ObjectiveTo assess the safety, adequacy and accuracy of transvaginal ultrasound-guided tru-cut biopsy of pelvic masses. MethodsWe performed a retrospective analysis of consecutive women who underwent transvaginal ultrasound-guided tru-cut biopsies between June 2014 and October 2018 at the Department of Obstetrics and Gynecology of the University Hospitals Leuven. Main indications for tru-cut biopsy were tissue collection for diagnosis of pelvic tumors in cases of suspected disseminated disease or recurrence, or tissue banking for research purposes. Data about adverse events occurring within 2 weeks of the procedure (including bleeding, blood transfusion, hospital admission, urgent surgery, pelvic infection or death) were extracted from electronic medical records. Tissue samples were recorded as adequate if tumor identification and immunohistochemistry were possible. Accuracy was defined in patients who underwent surgery as the agreement between histology after tru-cut biopsy and final histology. Results176 tru-cut biopsies were performed in 155 patients. Procedure related events were limited to moderate blood loss (<50 ml) without the need for treatment in 4.5%. There were no major complications. Biopsies were deemed adequate for histological evaluation in 84.3% of biopsies performed for diagnostic purposes and in 71.4% of research cases in whom a single tissue cylinder was available for diagnosis. When at least two cylinders were available, diagnostic adequacy increased to >95%. Comparing final histology, the diagnostic accuracy of the tru-cut biopsies was 97.2%. ConclusionTransvaginal tru-cut biopsy for diagnosis of pelvic masses is a safe procedure. To allow an adequate and accurate diagnosis, we advise taking at least 2 core biopsies.

Ectopic cervical thymoma: A diagnostic dilemma

Thymic tumors are rare neoplasms that arise in the anterior mediastinum. The Incidence of ectopic cervical thymoma is very less. A 36-year-old female presented with a swelling in the front of the neck for the duration of around 1 year with a cytology report suggestive of lymphoma. She underwent positron emission tomography/computed tomography scan which showed only low-grade fluorodeoxyglucose uptake in the left neck node, no evidence of disease elsewhere. She underwent ultrasound-guided Tru-cut biopsy which was suggestive of round cell tumor favoring lymphoproliferative disorder which needed immunohistochemistry (IHC) for further confirmation. IHC was confirmative of thymoma, for which patient underwent surgery.. Final histology report was of Thymoma AB type, MASOKA Stage IIa. This case briefs about the diagnostic challenge in a case of neck swelling, for which none of the differential diagnosis was correlated. Surgery remains the mainstay of treatment for thymoma once the tissue diagnosis is confirmed.

Lesions in 224 spleens of splenectomized dogs and evalution of alternative techniques for previous microscopic diagnosis

ABSTRACT: When detecting a proliferative splenic lesion, veterinarians usually choose splenectomy before a conclusive diagnosis, which can provide a deleterious effect to the dog. The most appropriate would be to perform splenectomy as a therapeutic procedure only in cases with real surgical indication, which can be established after defining microscopic diagnosis and prognosis. The objectives of this study were: to determine the frequency of different types of lesions in spleens of splenectomized dogs in the period of 12 years (2006-2017); determine the representativity of neoplastic lesions (benign and malignant) and non-neoplastic lesions; to evaluate and compare the safety and efficiency of fine needle aspiration biopsy (FNA) and ultrasound-guided Tru-cut needle biopsy for cytological and histopathological diagnosis, respectively, of splenic nodular lesions. In the studied period 224 cases of lesions were found in splenectomized spleens. The frequency of non-neoplastic lesions (50,45%, 113/224) and neoplastic lesions (49,55%, 111/224) was very similar. Among the neoplastic lesions, the malignant ones were more frequent (79,27%, 88/111), and the hemangiosarcoma was the most common (52,25%, 58/111). The possibility of malignant neoplasm was about 74% greater than a benign one. In summary, it was verified that 60.71% (136/224) of the cases corresponded to benign lesions without indication to splenectomy. FNA techniques and Tru-cut biopsy showed a low risk of complications. Regarding the diagnostic efficacy, the FNA obtained 71.43% (15/21) of conclusive diagnoses, 60% (9/15) of which were compatible with the final result of the histopathological evaluation, after splenectomy (gold standard). The Tru-cut biopsy obtained 71.43% (5/7) of conclusive diagnoses and 28.57% (2/7) of inconclusive diagnoses. Among the conclusive one, in 100% of the cases the diagnosis was compatible with the gold standard. Thus, since the possibility of benign splenic lesions in dogs is 1.52 times greater than malignancies, splenectomy should be recommended as a therapeutic procedure only in cases with proven surgical indication, which can be established after definition of microscopic diagnosis and prognosis. The use of FNA and Tru-cut biopsy should be recommended, especially for small and focal splenic lesions, since such techniques are good alternatives for establishing diagnosis previously to splenectomy, which may reduce the number of unnecessary splenectomies. The importance of recommending such techniques is emphasized, especially for dogs with focal splenic lesions smaller than three centimeters.

IntroductionWhile computed tomography (CT) guided lung biopsy has been standard in histological diagnosis of pulmonary lesions, its use is limited to the interventional radiologists only. Ultrasound (US) guided biopsy of pulmonary lesions, which can be performed in-clinic by the pulmonologists only, is becoming a more popular technique. It also has the edge of real-time techniques, multi-planar imaging, and no radiation exposure to the patients.MethodsThis is a retrospective review of all the patients presenting with pleural-based lung lesions who underwent US-guided biopsy for diagnosis in the Department of Pulmonology, Liaquat University of Medical and Health Sciences Hospital, Hyderabad, Pakistan from 1st January 2013 till 31st December 2017. The diagnostic yield, sensitivity, specificity, and accuracy of US-guided biopsies were evaluated for diagnoses of peripheral lung malignancies.ResultsUltrasound-guided biopsies for lung lesions has a diagnostic yield of 88.3%, sensitivity of 95.80%, and specificity of 90% with an accuracy of 95.35%. Pneumothorax as an immediate complication was seen only in 1.5% cases.ConclusionUS-guided biopsies are a much safer diagnostic alternative to CT-guided biopsy for lung lesions and have high diagnostic yield. It doesn’t require special radiological interventionists, can be performed at patients' bedsides, and the equipment is not as expensive.

Differentiation of secondary involvement of the breast by lymphoreticular malignancy from fibroadenoma using ultrasound elastography: A report of two cases.

Extramedullary lymphoma infiltration of the breast by lymphoblastic lymphoma is very rare and most cases are of B-cell lineage; T-cell neoplasms represent less than 10% of all breast lymphomas. Here, we report one patient with lymphoblastic lymphoma and one patient with leukemia, who have similar lesions in breasts with different ultrasound elastography findings. Ultrasound-guided tru-cut biopsies were performed and the first lesion was confirmed as lymphoma infiltration and the second as fibroadenoma. In cases of breast mass presence in patients with a history of hematologic malignancies such as lymphoma or leukemia, breast infiltration should be kept in mind. Elastography findings can assist in the differentiation of these lesions and further investigations or biopsies can be avoided.
