Distance Learning Course for Healthcare Professionals: Continuing Education in Tuberculosis.

Continuing education of healthcare workers (HCWs) is an essential strategy for the control of tuberculosis (TB) transmission, enabling HCWs in early detection and appropriate treatment of TB cases. We developed a distance learning (DL) course on TB for nurses. We conducted a quasi-experimental before and after study to evaluate the DL community at the participant's learning level. In addition, to evaluate the DL community at the level of participant satisfaction, a cross-sectional study was carried out after the course. Nurses involved in active inpatient or outpatient care of patients were recruited to participate in the study. Sixty-six participants started and completed the course and they were included in the analysis. The overall mean pretest and post-test scores were 10.3 ± 2.2 and 11.4 ± 2.7, respectively. Participants increased their knowledge to a statistically significant degree (p < 0.0001). At baseline, the frequency of correct answers was very low in some questions: number of people infected by Mycobacterium tuberculosis in the world (10.6%); number of TB cases in Brazil (36.4%); contagiousness of latent TB infection (LTBI) (28.8%); and definition of active case finding (45.5%). Course feedback was mostly positive, with majority of users saying they were satisfied or totally satisfied. A brief DL course on TB was associated with some improvement in knowledge among nurses. The baseline knowledge was low regarding TB epidemiologic data, concepts on LTBI, and active case finding. This finding emphasizes the need to further improve the competencies and knowledge of nurses.

Age is associated with latent tuberculosis in nurses

ObjectiveTo evaluate risk factors for developing latent tuberculosis (LTB) in Thai nurses. MethodsA comparison study was conducted at Srinagarind Hospital, Khon Kaen, Thailand. Clinical factors were compared between persons with tuberculin conversion and those without tuberculin conversion identified by tuberculin skin test. ResultsThere were 173 eligible persons with the LTB (34.7%). There were five workplaces where participants worked regularly including the general ward, surgical ward, pediatric ward, medical ward and critical care ward. In a multivariate model, two factors were significantly associated with LTB including age and history of tuberculosis in colleagues. The adjusted odds ratio (95% confidence interval) of both variables were 1.056 (1.004–1.104) and 0.202 (0.044–0.941). ConclusionsOlder age is associated with latent tuberculosis in nurses. LTB should be screened routinely and treated if diagnosed for nurses.

Incidence of tuberculosis among nurses and healthcare assistants in France.

Health care workers (HCWs) are considered to be at higher risk of tuberculosis (TB) than the general population. However, a decreasing incidence in the general population as well as improvement in preventive measures in hospitals has reduced the risk for HCWs. To quantify the actual incidence of TB in nurses and health care assistants in a low-incidence country. We performed a retrospective study of 80 hospitals throughout France, employing 233389 health care staff (physicians excluded). We calculated the number of pulmonary TB cases over 3 years (700166 person-years) and the total number of staff members in each job category (nurses, health care assistants, administrative staff) in each hospital, to calculate the incidence. Overall, the incidence rate varied between 1.27 and 6/100000 for administrative staff and nurses, respectively (non-significant difference). The incidence varied according to the geographical area. However, the incidence in nurses and health care assistants was not different from the general population (7.5/100000). In a low-incidence country, such as France, the implementation of measures to prevent occupational TB among HCWs has been effective. These preventive measures should be maintained but medical follow-up could be revised.

Delays in diagnosis and treatment of smear-positive tuberculosis and the incidence of tuberculosis in hospital nurses in Blantyre, Malawi

There is little information about nosocomial transmission of tuberculosis (TB) in sub-Saharan Africa. A study was carried out to examine the process of diagnosis and treatment of smear-positive pulmonary TB patients in Queen Elizabeth Central Hospital, Blantyre, Malawi, and the incidence of TB in nurses working in specific departments of the hospital. Case notes of 1365 patients with smear-positive pulmonary TB, diagnosed and treated at the hospital in 1993 and 1994, were analysed. The number of qualified nurses who worked in specific departments of the hospital between 1993 and 1994 and the number who were diagnosed and treated for TB during this period were obtained from nursing records. 787 patients (58%) were diagnosed as out-patients and 578 (42%) were diagnosed in hospital wards, 544 from medical wards. In medical wards, there were long delays from the time of admission to diagnosis and start of anti-TB treatment in new and previously treated TB patients. Of 310 qualified nurses, 12 (4%) were treated for TB in 1993–1994; 4 (14%) of 29 nurses working in the medical wards developed TB. The results indicate the importance of finding simple measures in resource-poor countries to improve the diagnosis and treatment of TB in hospital patients in order to decrease the risk of nosocomial TB transmission.

Tuberculosis in nurses in a New Zealand hospital

Five hundred and three student nurses, entering Dunedin Hospital between 1942 and 1947, were followed up to June 1948. Among those Mantoux-negative on entry, annual rates of conversion to positivity were comparatively low. One-third may be expected to complete their four-year course without acquiring infection. Conversion carried an immediate morbidity rate of nearly 1 in 4. Of nurses leaving the staff, none subsequently developed tuberculosis attributable to their hospital service. Tuberculin sensitivity tended to fluctuate, some nurses Mantoux-positive on entry tending to become negative during training. Of 10 cases of Mantoux-reversion, 4 again became converted to positivity, with lesions typical of primary tuberculosis in 2 cases. Such tuberculosis as became evident in the nurses as a whole has had no mortality. In all cases but 1, restoration to working capacity has been achieved. Tuberculosis must none the less be regarded special nursing risk. Annual loss of service was 2·9 per cent in Mantoux-negative entrants and 0·7 per cent in Mantoux-positive entrants. Though this figure may not be high in relation to the total loss from all illness, there are good grounds for measures to reduce it still further.
