National Trends in Healthcare Expenditures for the Management of Skin Cancer in the United States.

Malignant melanoma and keratinocyte carcinomas account for a substantial proportion of healthcare expenditures in the United States. To estimate trends in the economic burden of skin cancer in the United States between 1996 and 2016. The Disease Expenditure Project and Global Burden of Disease databases were used to estimate annual total costs and population-standardized rates of change for skin cancer-related healthcare spending. Skin cancer expenditures totaled $23.4 billion in 2016, of which $1.4 billion (95% CI: $1.3-$1.6 billion) was melanoma-related and $22 billion (95% CI: $18-$28 billion) was keratinocyte carcinoma-related. Most spending on skin cancer management occurred in ambulatory care settings [60.7% (95% CI: 57.7%-64.3%) for melanoma and 87.8% (95% CI: 87.2%-88.2%) for keratinocyte carcinoma]. Pharmaceutical costs for melanoma have increased since 2010 to $365 million (95% CI: $327-$416 million), which represents 26.1% (95% CI: 22.6%-29.3%) of total melanoma expenditure. Skin cancer management in the United States is costly. Expenditures have increased substantially since 1996 without signs of plateauing in recent years.

Trends in health care expenditures and incremental health care cost in adults with atrial fibrillation in the United States

BackgroundAtrial fibrillation (AF) is associated with increased healthcare costs; however, comprehensive data on the financial burden of AF remain limited. ObjectivesTo delineate healthcare expenditures among patients with AF. MethodsWe used the longitudinal panels from the Medical Expenditure Panel Survey (MEPS) covering 2016-2019 to estimate healthcare expenditures associated with AF. We identified individuals aged ≥18 with AF in the first year of each panel (2016-2018) using the ICD-10 code. Covariates included sociodemographic characteristics and comorbidities. Healthcare expenditures were derived from the second year of each panel (2017-2019) to reflect the cost of having the condition for an entire year. Adjusted mean annual costs were calculated, including total healthcare expenditure, hospital inpatient, emergency room visits, office based, outpatient visits, home health visits, prescribed medicines, and other expenses. Adjusted models were used to estimate the mean annual incremental total healthcare cost associated with AF. ResultsThe weighted study population included 3,080,055 adults with AF (382 respondents, mean age 71.5, 89.9% White). The adjusted annual total healthcare cost among adults with AF totaled $14,083 (95% CI, $10,887 – 17,279) compared to $8,771 (95% CI, $ 8,106 – 9,436) for those without AF. Primary drivers of cost over time were hospital inpatient care, office based visits, and prescribed medications. The annual incremental total healthcare cost associated with AF was $5,312 per adult (in 2019 US dollars). ConclusionAdults with AF in the US face a higher financial burden across various healthcare services compared to those without the condition, with consistently increasing expenses in inpatient care and prescribed medicines. Further research is needed to identify the independent contribution of AF to these costs.

Chronic Myeloid Leukemia in Bulgaria in the New Millennium: Identification of Directions for Improvement in Management and Outcomes Reporting

Background: In the last two decades, tyrosine kinase inhibitors (TKIs) and advances in molecular diagnostics have revolutionized management and long-term clinical outcomes in chronic myeloid leukemia (CML). Real-world data from different countries allow for the identification of country-specific issues in the clinical management and development of specific plans for improvement. Here, we aimed to analyze the trend in overall survival in Bulgarian CML patients since 2000. Methods: We retrieved publicly available Bulgarian CML data from several sources such as the Bulgarian National Cancer Registry, Bulgarian National Statistical Institute, and National Health Insurance Fund since 2000. We used the retrieved data of a total of 1513 Bulgarian CML patients to describe the trends in overall survival (OS), conditional overall survival, life expectancy, and life years lost over five time periods. We also described the trends in healthcare expenditures for TKIs and CML patients’ coverage with TKIs since 2014. Results: In both uni- and multivariate models, we found a constant increase in OS over the three 5-year periods until 2014. The period 2015–2019 was not associated with an additional increase in OS. Identical dynamics in the improvement in life expectancy (LE) and in life years lost (LYLs) was observed. Additionally, conditional 5-year survival did not improve during 2015–2019 in comparison to 2010–2014. Population-level data did not show consistent changes in the documented number of deaths due to CML since 2013. The period after 2013 is marked by a constant increase in the annual expenditures for TKIs, reaching to about 2.0 EUR/capita. The number of patients who received at least one TKI also increased during that period. Conclusions: After the initial significant improvement in the clinical outcomes for Bulgarian CML patients until 2014, subsequent periods did not bring further benefit in spite of the improved coverage with second- and third-line TKIs. Multiple factors may contribute to these suboptimal outcomes. Therefore, one can propose several additional measures at the country level, which could lead to additional improvement in the OS of Bulgarian CML patients.

Healthcare expenditure trends among adult stroke patients in the United States, 2011-2020

BackgroundIn the US, between 2018 and 2019, approximately $57 billion were expended on stroke and related conditions. The aim of this study was to understand trends in direct healthcare expenditures among stroke patients using novel cost estimation methods and a nationally representative database. MethodsThis study was a retrospective analysis of 193,003 adults, ≥18 years of age, using the Medical Expenditure Panel Survey during 2009-2016. Manning and Mullahy's two-part model were used to calculate adjusted mean and incremental medical expenditures after adjusting for covariates. ResultsThe mean (Standard Deviation) direct annual healthcare expenditure among stroke patients was $16,979.0 ($16,222.0- $17,736.0) and was nearly 3 times greater than non-stroke participants which were $5,039.7 ($4,951.0-$5,128.5) and were mainly spent on inpatient services, prescription medications, and office-based visits. Stroke patients had an additional healthcare expenditure of $4096.0 (3543.9, 4648.1) per person per year, compared to participants without stroke after adjusting for covariates (P<0.001). The total mean annual direct healthcare expenditure for stroke survivors increased from $16,142.0 (15,017.0-17,267.0) in 2007-2008 to $16,979.0 (16,222.0-17,736.0) in 2015-2016. ConclusionOur study showed that stroke survivors had significantly greater healthcare expenses, compared to non-stroke individuals, mainly due to higher expenditures on inpatient services, prescription drugs, and office visits. These findings are concerning because the prevalence of stroke is projected to increase due to aging population and increased survival rates.

The Economic Burden of Disease in France From the National Health Insurance Perspective: The Healthcare Expenditures and Conditions Mapping Used to Prepare the French Social Security Funding Act and the Public Health Act.

Background:Identifying the most frequently treated and the costliest health conditions is essential for prioritizing actions to improve the resilience of health systems.Objectives:Healthcare Expenditures and Conditions Mapping describes the annual economic burden of 58 health conditions to prepare the French Social Security Funding Act and the Public Health Act.Design:Annual cross-sectional study (2015–2019) based on the French national health database.Subjects:National health insurance beneficiaries (97% of the French residents).Measures:All individual health care expenditures reimbursed by the national health insurance were attributed to 58 health conditions (treated diseases, chronic treatments, and episodes of care) identified by using algorithms based on available medical information (diagnosis coded during hospital stays, long-term diseases, and specific drugs).Results:In 2019, €167.0 billion were reimbursed to 66.3 million people (52% women, median age: 42 y). The most prevalent treated diseases were diabetes (6.0%), chronic respiratory diseases (5.5%), and coronary diseases (3.2%). Coronary diseases accounted for 4.6% of expenditures, neurotic and mood disorders 3.7%, psychotic disorders 2.8%, and breast cancer 2.1%. Between 2015 and 2019, the expenditures increased primarily for diabetes (+€906 million) and neurotic and mood disorders (+€861 million) due to the growing number of patients. “Active lung cancer” (+€797 million) represented the highest relative increase (+54%) due to expenditures for the expensive drugs and medical devices delivered at hospital.Conclusions:These results have provided policy-makers, evaluators, and public health specialists with key insights into identifying health priorities and a better understanding of trends in health care expenditures in France.

Trends in Health Care Expenditures and Patient Out-of-Pocket Expenses in Primary Anterior Cruciate Ligament Reconstruction

Background: Anterior cruciate ligament reconstruction (ACLR) is one of the most commonly performed orthopaedic procedures in the United States, and the number of procedures is increasing annually, as is the cost. Patients are expected to shoulder a larger out-of-pocket expenditure. Purpose: To answer the following questions: (1) How is reimbursement changing for ACLR, and how is this affecting patients’ out-of-pocket expenditures? (2) How are reimbursements from payers and patients’ out-of-pocket expenses for ACLR distributed, and how is this changing? (3) Does performing ACLR in an ambulatory surgery center (ASC) result in lower costs for payers and patients? Study Design: Economic and decision analysis study; Level of evidence, 4. Methods: A total of 37,763 patients who underwent outpatient primary arthroscopic ACLR in the United States between 2013 and 2017 were identified using the IBM MarketScan Commercial Claims and Encounters Database. Patients with concomitant procedures and revision ACLR were excluded. Recorded outcomes were total patient payments and reimbursed claim totals in US dollars. Results: Day-of-surgery reimbursement decreased 4.3% from $11,536 in 2013 to $11,044 in 2017, while patient out-of-pocket expenses increased 36% from $1085 in 2013 to $1480 in 2017. Day-of-surgery charges were the highest expense for patients, followed by physical therapy and magnetic resonance imaging (MRI) costs. Total reimbursement for MRI decreased 22.5%, while patient out-of-pocket expenses for MRI increased 166%. ACLR performed in an outpatient hospital resulted in 61% greater day-of-surgery expenditure for payers compared with ACLR performed in an ASC; however, the median total patient out-of-pocket savings for ACLRs performed in an ASC versus outpatient hospital was only $11. Conclusion: Out-of-pocket expenses for patients are increasing as they are forced to cover a larger percentage of their health care costs despite overall payer reimbursement decreasing. High-deductible health plans reimbursed the least out of all insurance types while having the highest patient out-of-pocket expenditure.

Trends in Healthcare Expenditures among Adults in the United States by Cancer Diagnosis Status, 2008–2016: A Cross-Sectional Study

This study aims to assess trends in direct medical expenditures and indirect costs between adults with and without a prior cancer diagnosis from 2008 to 2016. Nine years of data (2008-2016) from the Medical Expenditure Panel Survey (weighted N = 236,811,875) were used. The outcomes included medical expenditures (total expenditure, inpatient, office-based, medications, outpatient, dental, emergency room visits, home health, other) and health-related missed workdays. The predictor was prior cancer diagnosis. Covariates included demographic characteristics, comorbidities, and calendar year at time of survey completion. Two-part statistical modeling with a combination of binomial and positive distributions was used to estimate medical expenditures and missed workdays. Data were clustered into five timepoints: 2008 to 2009, 2010 to 2011, 2012 to 2013, 2014 to 2015, and 2016. Eleven percent of the sample (n = 25,005,230) had a prior cancer diagnosis. Compared with those without a prior cancer diagnosis, those with a prior cancer diagnosis had higher mean incremental total expenditures across all years. Between 2008 and 2016, the adjusted annual incremental total expenditures were $3,522 [95% confidence interval (CI), $3,072-$3,972]; office-based visits ($1,085; 95% CI, $990-$1180); inpatient hospitalizations ($810; 95% CI, $627-$992); outpatient appointments ($517; 95% CI, $434-$600); and medications ($409; 95% CI, $295-$523); and health-related missed workdays (0.75; 95% CI, 0.45-1.04) compared with adults without a prior cancer diagnosis. Adults with a prior cancer diagnosis had significantly increased healthcare expenditures and health-related missed workdays compared with those with no cancer diagnosis. Our findings highlight the need for increasing strategies to remedy the impact of increasing direct and indirect costs associated with cancer survivorship as the population grows and ages.

New Spending Programs and Old Frustrations: Where Is the Vision?

The spring has ushered in an unexpected number of major health policy announcements compared with the last 10 years. They are led by the federal government's outlines of a national pharmacare program, an unexpected dental care program (Prime Minister of Canada Justin Trudeau 2022), plus "top-up" funding for clearing provincial surgical and imaging backlogs. These announcements are on top of the voices expressing concerns about COVID-19-related healthcare expenditure trends (Bailey 2022). Without a doubt, taxpayer money is flowing freely into healthcare (Labby 2020).

Trends in healthcare expenditures and resource utilization among a nationally representative population with opioids in the United States: a serial cross-sectional study, 2008 to 2017

BackgroundPrevious reports on healthcare costs and expenditures associated with populations prescribed an opioid primarily focused on populations who chronically use opioids or have opioid use disorder. However, studies that characterize the healthcare and expenditures costs among the wider number of people prescribed opioids in a nationally representative population are unavailable. We sought to characterize the healthcare costs and expenditures associated with a population prescribed an opioid in the U.S. from 2008 to 2017.MethodsA serial cross-sectional design was used to compare the economic burden of adult household respondents who were prescribed and not prescribed an opioid using pooled data from the Medical Expenditure Panel Survey (MEPS) between 2008 and 2017. Respondents with an opioid prescription were matched to respondents without an opioid prescription using propensity score match methods with survey weights. Two-part generalized linear models were used to estimate the survey-weighted annual healthcare expenditures and resource utilization adjusting for multiple covariates. Additionally, 10-year trend comparisons between the groups were performed. Costs were adjusted to 2019 US dollars.ResultsThere was a weighted total of 31,696,671 respondents with an opioid and 31,536,639 respondents without an opioid after propensity score matching. The sample had a mean (SD) age of 50.63 years (18.03), 58.9% females, and 81.6% Whites. Total annual economic burden among RPOs was $524 billion. Annual total expenditures per respondent with and without an opioid were $16,542 and $7067, respectively (P < 0.001). Similarly, adjusted prescription, outpatient, emergency department, and inpatient expenditures were significantly higher for respondents with an opioid compared to respondents without an opioid. Average annual increases in expenditures were significantly greater among respondents with an opioid compared to respondents without an opioid for total (+$185; 95% CI: $37–$334) and prescription (+$78; 95% CI: $28–$128) expenditures. There were no differences in the average annual trends for outpatient, emergency department, and inpatient expenditures between respondents with and without an opioid.ConclusionsRespondents with an opioid prescription had higher healthcare expenditures and resource utilization compared to respondents without an opioid prescription from 2008 to 2017. Specifically, significant annual increases were observed for total and prescription expenditures. Additionally, 10-year trends in total and prescription expenditures were higher among respondents with an opioid than respondents without an opioid.

Incremental health care utilization and expenditures for sleep disorders in the United States.

To determine the incremental increases in health care utilization and expenditures associated with sleep disorders. Adults with a diagnosis of a sleep disorder (International Classification of Diseases, 10th Revision, code G47.x) within the medical conditions file of the 2018 Medical Expenditure Panel Survey medical conditions file were identified. This dataset was then linked to the consolidated expenditures file and comparisons in health care utilization and expenditures were made between those with and without sleep disorders. Multivariate analyses, adjusted for demographics and comorbidities, were conducted for these comparisons. Overall, 5.6% ± 0.2% of the study population had been diagnosed with a sleep disorder, representing approximately 13.6 ± 0.6 million adults in the United States. Those with sleep disorders were more likely to be non-Hispanic, White, and female, with a higher proportion with public insurance and higher Charlson Comorbidity Scores. Adults with sleep disorders were found to have increased utilization of office visits (16.3 ± 0.8 vs 8.7 ± 0.3, P < .001), emergency room visits (0.52 ± 0.03 vs 0.37 ± 0.02, P < .001), and prescriptions (39.7 ± 1.2 vs 21.9 ± 0.4, P < .001) vs those without sleep disorders. The additional incremental health care expenses for those with sleep disorders were increased in all examined measures: total health care expense ($6,975 ± $800, P < .001), total office-based expenditures ($1,694 ± $277, P < .001), total prescription expenditures ($2,574 ± $364, P < .001), and total self-expenditures for prescriptions ($195 ± $32, P < .001). Sleep disorders are associated with significantly higher rates of health care utilization and expenditures. By using the conservative prevalence estimate found in this study, the overall incremental health care costs of sleep disorders in the United States represents approximately $94.9 billion. Recent Health Care Expenditure Trends Among Adult Cancer Survivors in United States, 2009-2016.

The objective of this study was to understand recent trends in direct health care expenditures among cancer survivors using novel cost-estimation methods and a nationally representative database. This study was a retrospective analysis of 193,003 adults, ≥18 years of age, using the Medical Expenditure Panel Survey during the years 2009-2016. Manning and Mullahy two-part model was used to calculate adjusted mean and incremental medical expenditures after adjusting for covariates. The mean direct annual health care expenditure among cancer survivors ($13,025.0 [$12,572.0 to $13,478.0]) was nearly 3 times greater than noncancer participants ($4689.3 [$4589.2 to $4789.3]) and were mainly spent on inpatient services, office-based visits, and prescription medications. Cancer survivors had an additional health care expenditure of $4407.6 ($3877.6, $4937.6) per person per year, compared with noncancer participants after adjusting for covariates (P<0.001). The total mean annual direct health care expenditure for cancer survivors increased from $12,960.0 (95% confidence interval: $12,291.0-$13,628.0) in 2009-2010 to $13,807.0 ($12,828.0 to $14,787.0) in 2015-2016. Given the higher health care expenditures among cancer survivors and the increasing prevalence of cancers, cost-saving measures should be planned through multidisciplinary initiatives, collaborative research, and importantly, health care planning and policy changes. Our findings could be helpful in streamlining health care resources and interventions, developing national health care coverage policies, and possibly considering radically new insurance strategies for cancer survivors.

PIT2 PATTERNS OF OPIOID USE FOLLOWING FRACTURE BASED ON TIME BETWEEN PRESCRIPTION ORDER AND PATIENT FILL

Opioids are commonly prescribed for acute pain, including treatment of fractures. However, there is risk for dependence and abuse, particularly among opioid-naïve populations. This study used linked claims and electronic medical records to examine the impact of timing of opioid use on outcomes within a fracture population. Individuals with a fracture (index date) between 2012-2017 and continuous eligibility for 6-months preceding and following index were identified in the IBM MarketScan®Explorys®Claims-EMR Dataset. Evidence of fracture or opioid use/dependence during baseline resulted in exclusion. Patients had to have a prescription order for a study opioid (oxycodone, hydrocodone, or tramadol) in the 45 days following index, and a fill for a study opioid anytime during follow-up. Patients were stratified by age (<18; 18-64; and 65+) and time from prescription to first opioid fill (0-7, 8-14, 15-30, and >30 days). Opioid utilization and healthcare costs were compared between sub-groups. A total of 16,181 patients qualified for the study. Mean time from fracture to prescription was 3.5±8.2 days. The majority of the sample (83.6%) filled an opioid within 0-7 days; 7.7% filled >30 days. Patients aged 65+ were more likely than patients <18 to delay opioid fills 30+ days (16.2% vs. 1.6%) and evidenced greater total days’ supply of opioids (36.5±42.1 vs. 6.5±3.0). Compared to patients filling an opioid >30 days from the order, patients filling an opioid within 7 days presented significantly reduced opioid usage (21.2±31.2 vs. 34.9±45.5 days’ supply) and lower fracture-related costs ($9,907±$18,963 vs. $36,321±$85,055; ps< 0.01). Trends in healthcare expenditure were similar across age groups stratified by timing of opioid fill. Although serious risks of opioid use remain, timely utilization of opioids by fracture patients may help manage pain and contain healthcare costs. Once opioids were prescribed, prompt utilization was associated with improved outcomes.

Policy and Ethical Considerations for Widespread Utilization of Generic Orthopedic Implants.

In 2015, the United States spent $3.2 trillion on healthcare, representing 18% of the nation’s gross domestic product [1]. As prices continue to climb, physicians, healthcare policymakers, and industry leaders alike have sought varied means to lower costs while maintaining the quality of care. Coverage expansion, shifts toward bundled care payments, and the implementation of Medicare Payment Reform (Medicare Access and Children’s Health Insurance Program Reauthorization Act of 2015 [MACRA]) are indicative of shifting dynamics in the American healthcare system prioritizing outcomes, value, and cost-containment over the fee-for-service model [2], [3]. Orthopedic implant expenditures are considerable in the overall cost of care for surgical patients. The cost of total joint arthroplasty implants between 1996 and 2006 increased an estimated 130%. Implant and medical device costs comprise up to 60% of hospital reimbursements for primary procedures [4]. Most implants are proprietary in nature and vary substantially in their price and features depending on the industry manufacturer [5], [6], [7]. Control of implant costs is thus a necessary focus for cost containment in orthopedic surgery. To that end, as patents on many brand name implants begin to expire, “generic” implants present a unique opportunity to improve the value of orthopedic care. Although generic drugs have been used for decades in the pharmaceutical industry, the use of generic implants has only recently been considered in the field of orthopedic surgery [8], [9], [10]. In the pharmaceutical sector, an estimated $33 billion has been saved by Medicare in the United States through utilization of generic medications [11]. There could be potential for an analogous substantial reduction in healthcare expenditures via the use of generic orthopedic implants. There is some literature describing the favorable economic possibilities for use of generic orthopedic implants [9], [10], [12]. However, the ethical issues, policy implications, and potential unanticipated consequences of such a paradigm shift have not been investigated. This article discusses the pros and cons of generic implant usage from economic, clinical, and ethical perspectives.

Healthcare expenditures for privately insured US patients with cystic fibrosis, 2010-2016.

Published cost estimates for cystic fibrosis (CF) are based on older data and do not reflect increased use of specialty drugs in recent years. We assessed recent trends in healthcare expenditures for CF patients in the United States (US) with employer-sponsored health insurance. The study is a retrospective analysis of claims data for privately insured individuals aged 0-64 years who were continuously enrolled in non-capitated plans for at least 1 calendar year during 2010-2016. Mean annual expenditures during a calendar year were calculated for individuals who met a claims-based CF case definition. Average annual growth rates were calculated through linear regression of the natural logarithm of annual expenditures. The annual CF prevalence was 1.1-1.4 per 10 000 adults and 2.9-3.0 per 10 000 children. Average spending adjusted for inflation nearly doubled from roughly $67 000 per patient in 2010 and 2011 to approximately $131 000 per patient in 2016. Inflation-adjusted spending on outpatient and inpatient care increased by 0.5% and 2.5% per year, respectively, whereas pharmaceutical spending increased by 20.2% per year. Virtually all of the growth in pharmaceutical spending was accounted for by spending on specialty drugs; inflation-adjusted spending on other medications increased by 1.3% per year. The annual growth rate in pharmaceutical spending rose by 33.1% during 2014-2016, the years during which lumacaftor/ivacaftor was introduced. Per-patient expenditures for privately-insured patients with CF almost doubled during 2010-2016; specialty drugs were largely responsible for this increase, with a major contribution from new, genotype-targeted CFTR modulator medications.

A Case Study of Health Expenditure in India

The article aims in analysing healthcare expenditure of India, as it struggles to meet the health care requirements of all the citizens. In order to acheieve this, it is important to understand the trends in healthcare expenditure of the country to create a sustainable health expenditue model. Post millenium, with various reforms and increased awareness, a certain redistribution can be observed among different arms that contribute to total healthcare expenditure.This study reviews and summarise the expenditure trends between the years 2004 and 2014. Numerical illustrtions are also provided to show the government and private sectors contributions on the health care products.

Health Care Cost Analyses for Exploring Cost Savings Opportunities in Older Patients: Longitudinal Retrospective Study.

BackgroundHalf of Medicare reimbursement goes toward caring for the top 5% of the most expensive patients. However, little is known about these patients prior to reaching the top or how their costs change annually. To address these gaps, we analyzed patient flow and associated health care cost trends over 5 years.ObjectiveTo evaluate the cost of health care utilization in older patients by analyzing changes in their long-term expenditures.MethodsThis was a retrospective, longitudinal, multicenter study to evaluate health care costs of 2643 older patients from 2011 to 2015. All patients had at least one episode of home health care during the study period and used a personal emergency response service (PERS) at home for any length of time during the observation period. We segmented all patients into top (5%), middle (6%-50%), and bottom (51%-100%) segments by their annual expenditures and built cost pyramids based thereon. The longitudinal health care expenditure trends of the complete study population and each segment were assessed by linear regression models. Patient flows throughout the segments of the cost acuity pyramids from year to year were modeled by Markov chains.ResultsTotal health care costs of the study population nearly doubled from US $17.7M in 2011 to US $33.0M in 2015 with an expected annual cost increase of US $3.6M (P=.003). This growth was primarily driven by a significantly higher cost increases in the middle segment (US $2.3M, P=.003). The expected annual cost increases in the top and bottom segments were US $1.2M (P=.008) and US $0.1M (P=.004), respectively. Patient and cost flow analyses showed that 18% of patients moved up the cost acuity pyramid yearly, and their costs increased by 672%. This was in contrast to 22% of patients that moved down with a cost decrease of 86%. The remaining 60% of patients stayed in the same segment from year to year, though their costs also increased by 18%.ConclusionsAlthough many health care organizations target intensive and costly interventions to their most expensive patients, this analysis unveiled potential cost savings opportunities by managing the patients in the lower cost segments that are at risk of moving up the cost acuity pyramid. To achieve this, data analytics integrating longitudinal data from electronic health records and home monitoring devices may help health care organizations optimize resources by enabling clinicians to proactively manage patients in their home or community environments beyond institutional settings and 30- and 60-day telehealth services.

Trends in Healthcare Expenditures Among US Adults With Hypertension: National Estimates, 2003-2014.

BackgroundOne in 3 US adults has high blood pressure, or hypertension. As prior projections suggest hypertension is the costliest of all cardiovascular diseases, it is important to define the current state of healthcare expenditures related to hypertension.Methods and ResultsWe used a nationally representative database, the Medical Expenditure Panel Survey, to calculate the estimated annual healthcare expenditure for patients with hypertension and to measure trends in expenditure longitudinally over a 12‐year period. A 2‐part model was used to estimate adjusted incremental expenditures for individuals with hypertension versus those without hypertension. Sex, race/ethnicity, education, insurance status, census region, income, marital status, Charlson Comorbidity Index, and year category were included as covariates. The 2003–2014 pooled data include a total sample of 224 920 adults, of whom 36.9% had hypertension. Unadjusted mean annual medical expenditure attributable to patients with hypertension was $9089. Relative to individuals without hypertension, individuals with hypertension had $1920 higher annual adjusted incremental expenditure, 2.5 times the inpatient cost, almost double the outpatient cost, and nearly triple the prescription medication expenditure. Based on the prevalence of hypertension in the United States, the estimated adjusted annual incremental cost is $131 billion per year higher for the hypertensive adult population compared with the nonhypertensive population.ConclusionsIndividuals with hypertension are estimated to face nearly $2000 higher annual healthcare expenditure compared with their nonhypertensive peers. This trend has been relatively stable over 12 years. Healthcare costs associated with hypertension account for about $131 billion. This warrants intense effort toward hypertension prevention and management.

Trends in Healthcare Expenditures among Individuals with Arthritis in the United States from 2008 to 2014.

With the expected rise in the arthritis population, information is required regarding trends of healthcare expenditures among individuals with arthritis in the United States. We examined temporal trends in direct and out-of-pocket (OOP) healthcare expenditures among individuals with arthritis using a nationally representative database, the Medical Expenditures Panel Survey. The study population was composed of cross-sectional cohorts of individuals aged ≥ 18 years from 2008 to 2014. Two-part models were used to estimate the incremental total and types of annual direct and OOP healthcare expenditures (adjusted to 2014 US dollars) for arthritis, after controlling for predisposing, enabling, need, personal health practice, and external environmental factors, as per the Anderson Healthcare Behavioral Model. An annual weighted arthritis population rose from 56.1 million in 2008 to 65.1 million in 2014. Among individuals with arthritis, the annual average direct and OOP expenditure was $10,424 [standard error (SE) = $345, aggregate = $584.8 billion] and $1493 (SE = $50, aggregate = $83.8 billion) in 2008, respectively, and $910 (SE = $279, total = $645.1 billion) and $1099 (SE = $36, aggregate = $71.5 billion) in 2014, respectively. In the fully adjusted model, individuals with arthritis had significantly greater total and OOP expenditures from 2008 to 2014; however, the magnitude of incremental OOP expenditure declined from 2008 to 2014. Although the annual direct healthcare expenditures per person remained stable over the years, the rise in proportion of the arthritis population led to a huge increase in aggregate economic burden to the US healthcare system.

Nationwide trends in medical expenditures among adults with epilepsy: 2003–2014

ObjectiveHealthcare expenditure among adults with epilepsy is high. There is a paucity of published data on trends in the nationwide economic impact of epilepsy. This study examines trends in healthcare expenditures and components in U.S. adults with epilepsy between 2003 and 2014. MethodsWe analyzed 12years of data representing a weighted sample of 1,942,413U.S. adults aged ≥18years with epilepsy using Medical Expenditure Panel Survey Household Component (MEPS-HC), 2003–2014 data. We used a novel two-part model (adjusting for demographic, comorbidity, and time) to estimate the incremental healthcare expenditures by epilepsy status. Pre and post Affordable Care Act era costs were compared. ResultsOverall unadjusted annual mean medical expenditures for patients with epilepsy was $15,324. Individuals with epilepsy had nearly three times higher overall unadjusted mean expenditure than those without epilepsy ($15,324, 95%CI: 2778–17,871 vs. $5824, 95%CI: 5722–5926). The unadjusted annual mean medical expenditure decreased over time from $17,994 (95% CI $10,754–$25,234) in 2003/2006 to $13,848 (95% CI: $11,371–$16,324) in 2011/2014; a trend driven primarily by a decrease in inpatient expenditures from $5613 to $4113. Having a diagnosis of epilepsy increased health expenditure by $8598 which was 2.5 to 6 times greater than the equivalent incremental health expenditures for other selected comorbidities. Healthcare expenditure among adults with epilepsy was $4083 lower in the post- Affordable Care Act. ConclusionOver the last decade, individuals with epilepsy incurred significantly higher medical expenditures than those without epilepsy, but overall healthcare expenditure decreased over time due to a decrease in inpatient expenditures.
