The efficacy of immunotherapy using a Streptococcal preparation, OK-432, was evaluated in each clinical stage of uterine cervical cancer. The 382 eligible patients were stratified by clinical stage and presence/absence of surgery. Within each stratum, patient's were randomly allocated to OK-432 treatment or to control treatment. OK-432 significantly inhibited recurrence in patients with stage II cervical cancer; the recurrence-free interval and survival time were remarkably prolonged in patients with stage II disease who underwent surgery. However, OK-432 did not significantly prolong these parameters in patients with stage III disease. Retrospective analyses revealed that in patients with or without lymph node metastases who underwent surgery, the recurrence-free interval and survival time were significantly prolonged by OK-432 treatment. These results indicate that OK-432 is an effective and useful postoperative immunotherapeutic agent for uterine cervical cancer.

Prognosis of uterine cervical cancer with extensive lymph node metastases: Special emphasis on the value of pelvic lymphadenectomy in the surgical treatment of uterine cervical cancer

Our previous studies and the present study confirm the unfavorable effect of pelvic lymph node metastases on the 5 year survival rate after radical operation for uterine cervical cancer. As expected, bilateral lymph node involvement or lymph node involvement above the bifurcation of the iliac artery gives a less favorable prognosis for the patients after operation than unilateral involvement or involvement below the bifurcation. On the other hand, our previous data and the present data indicate the definite value of pelvic lymphadenectomy in the surgical treatment of uterine cervical cancer, even in patients with extensive lymph node metastases. A 48.5 per cent 5 year survival rate in the patients with one to four positive nodes, a 19.04 per cent 5 year survival rate in the patients with 5 to 83 positive nodes, and some 5 year survivals even among the cases with a high ratio of the number of positive nodes to the total number of pelvic nodes dissected (25 to 50 per cent and 76 to 97.1 per cent) have encouraged us to continue to do a complete and thorough pelvic lymphadenectomy for uterine cervical cancer regardless of the extent of pelvic lymph node metastases. Postoperative chemotherapy and/or irradiation, particularly the former, seem to improve the prognosis in patients with extensive lymph node metastases after radical operation. Some method to accelerate the function of the reticuloendothelial system was suggested to enforce the adjuvant therapy after radical operation.
