Acupuncture for shoulder-hand syndrome after stroke: An overview of systematic reviews.

To provide evidence, this review evaluated the methodological quality, risk of bias, and reporting quality of SRs/MAs in the treatment of shoulder-hand syndrome after stroke with acupuncture. Systematic reviews and Meta analyses (SRs/MAs) of acupuncture for shoulder and hand syndromes after stroke were retrieved from 6 databases from inception to May 1, 2022. Two reviewers independently screened the literature and extracted the data, then used Assessment of Multiple Systematic Reviews-2 (AMSTAR-2), Bias Risk in Systematic Review (ROBIS), and Preferred Report Item for Systematic review and Meta-analysis (PRISMA), Grading of Recommendations, Assessment, Development and Evaluation (GRADE) to assess methodological quality, risk of bias, quality of reporting, and quality of evidence. We included 7 SRs/MAs, of which all SRs/MAs had very low AMSTAR-2 assessment quality and one study had a very low assessment bias risk. According to the PRISMA checklist, Protocol and registration, Synthesis of results, Summary of evidence, Conclusions and Funding were the main reporting limitations. GRADE evaluation showed a total of 37 results, but no high-quality evidence results, 6 results (16.22%) of the evidence quality were moderate, and supported acupuncture combined with exercise rehabilitation and drug therapy was better than exercise joint drug rehabilitation and rehabilitation, we also found that the result of limitations were the main factors that influence the evidence of low quality, followed by imprecision, inconsistency, and publication bias. Acupuncture is a relatively safe and effective adjuvant therapy for shoulder and hand syndromes after stroke. However, because of the low quality of SRs/MAs evidence supporting these findings, high-quality randomized controlled trials should be conducted, and the quality of relevant SRs should be improved to provide evidence for clinical application.

Clinical Study of Qingpeng Ointment Treating Shoulder-hand Syndrome After Cerebral Hemorrhage During the Rehabilitation Period.

Shoulder-hand syndrome (SHS) refers to a syndrome causing sudden edema, shoulder pain and limited hand function. Qingpeng ointment, a kind of Tibetan medicine, can reduce swelling, relieve pain, tonify stagnation and clear the meridians, which is consistent with the pathological mechanism of SHS after stroke. Therefore, if clinical trials can be used to explore the effectiveness of Qingpeng ointment for the treatment of poststroke SHS and promote its application in clinical medicine, it may prove the specific significance for the treatment of poststroke SHS poststroke SHS. The aim of the study was to investigate the clinical efficacy and safety of Qingpeng ointment in the treatment of poststroke SHS and to provide an objective basis for a better therapeutic treatment for poststroke SHS. A prospective, randomized, controlled study was conducted. This study recruited 120 patients with poststroke SHS who met the inclusion criteria. They were randomized into the treatment group and the control group, with 60 patients allocated to each group. The treatment group received routine medical treatment and rehabilitative care after using the Qingpeng ointment, while the patients in the control group received only routine treatment without the ointment. All patients received clinical assessment with the Visual Analogue Scale (VAS), measurement of the range of motion (ROM) of the upper-limb joints, the Fugl-Meyer Assessment of Upper Extremity (FMA-U) and the Modified Barthel Index Score (MBI) before and after the whole treatment. After 4 weeks of treatment, the VAS scores of both the groups decreased significantly (P ＜0.05), and the difference between the two groups was statistically significant (P < 0.05). No statistical significance was observed for the difference between the treatment group and control group in terms of the FMA-U and MBI scores and the forward bend, backward, outstretch, external rotation and pronation angles after treatment. The increases in the values of VAS, FMA-M and MBI in the treatment group were greater than those in the control group, and the difference was statistically significant (P < 0.05). The increases in the values of the forward bend, outreach and external rotation angles in the treatment group were greater than those in the control group, and the difference was statistically significant (P < 0.05). The treatment group showed better results than the control group in terms of the relief of pain symptoms, the improvement of motor function and the improvement of the activities of daily living for patients with shoulder-hand syndrome after cerebral hemorrhage. Qingpeng ointment was found to be effective and safe for treating poststroke SHS.

Effect of electric stimulation combined with Ai salt bag on shoulder-hand syndrome after cerebral infarction

Objective To study the effect of electric stimulation combined with Ai salt bag on shoulder-hand syndrome after cerebral infarction. Methods A total of 188 patients with shoulder-hand syndrome after cerebral infarction admitted to Taizhou Central Hospital from September 2015 to August 2018 were enrolled in the study, they were randomly divided into study group (n=94) and control group (n=94) according to the random number table method.The control group was given uplink electrical stimulation treatment based on the conventional treatment for cerebral infarction and rehabilitation training, the study group was given Ai salt bag treatment on the basis of the control group.The clinical treatment effect, pain and edema VAS score, Fugl-Meyer motor function score and ADL score were observed and compared between the two groups. Results After 28 days of treatment, the total effective rate was 88.30%(83/94) in the study group and 76.60%(72/94) in the control group, the difference between the two groups was statistically significant (χ2=4.447, P<0.05). The VAS score of edema degree and pain degree in the study group decreased from (4.13±1.40) points, (3.43±2.20)points to (1.70±1.36) points, (1.52±1.22)points, which in the control group decreased from (4.10±1.46) points, (3.40±2.17) points to (2.79±1.58) points, (2.52±1.18)points, there were statistically significant differences between the two groups after treatment(t1=5.712, t2=5.069, all P<0.05). The Fugl-Meyer motor function score and ADL score of the study group increased from (19.91±13.32) points, (42.34±15.57)points to (54.21±11.71) points, (86.02±16.62)points, which in the control group increased from (20.74±11.30) points, (45.84±14.44) points to (45.44±12.61) points, (78.84±17.79)points, there were statistically significant differences between the two groups after treatment (t1=4.941, t2=2.859, all P<0.05). Conclusion Electrical stimulation combined with Ai salt bag in the treatment of shoulder-hand syndrome after cerebral infarction has significant clinical effect, which can effectively alleviate the pain symptoms and swelling of the affected limb, promote the activities of daily living and improve the function of the affected limb, and is worthy of clinical application. Observation of the effect of pregabalin capsules in the treatment of shoulder hand syndrome after stroke

Objective To investigate the effect of pregabalin capsules in the treatment of shoulder hand syndrome after stroke. Methods 90 patients with shoulder hand syndrome after stroke were divided into observation group and control group according to random number table method, 45 cases in each group.Rehabilitation training and acupuncture treatment were used in the control group, and on the basis of the control group, the observation group used pregabalin capsules treatment, initial dose 75mg/times, oral, 2 times/d.After 2 weeks, the dose was changed to 150mg/times, oral, 2 times/d.Maintenance therapy for 8 weeks.The overall efficacy and pain degree before and after treatment, upper limb function of the two groups were compared. Results After treatment, the VAS scores of the two groups were significantly decreased, and the FMA scores were significantly increased, the differences were statistically significant compared with before treatment(t=17.353, 15.777, 12.001, 10.432, all P<0.05). And the improvement degrees of VAS score and FMA score of the observation group were significantly better than those of the control group[(18.3±12.9)points vs.(29.8±12.1)points, (53.1±9.2)points vs.(42.3±8.9)points], the differences were statistically significant(t=3.294, 4.672, all P<0.05). The total effective rate of the observation group was 93.3%, which was significantly higher than 77.8% of the control group, the difference was statistically significant(χ2=4.406, P<0.05). Conclusion In the treatment of shoulder hand syndrome after stroke, pregabalin capsules has good effect. Key words: Stroke; Reflex sympathetic dystrophy; Pregabalin

Timeliness of the analgesic effect of superficial needling on shoulder-hand syndrome after stroke

ObjectiveTo observe the correlation between analgesic effect and duration of analgesic effect of superficial needling for shoulder pain of shoulder-hand syndrome (SHS) after stroke, so as to screen the best time period of analgesia. MethodsA total of 120 patients with SHS after stroke (stage I) were recruited and superficial needling therapy was applied. Two obvious tenderness points on the affected shoulder of patients were found out. The site 80–100 mm down each tenderness point was selected for superficial needling. Bimanual needling technique was applied after inserting needles. The surrounding of tenderness points was pinched and grasped by left hand above the needling, and the technique of green dragon swaying tail was applied by right hand. The needles at each acupoint were manipulated for 3 min and retained for 30 min. The analgesic effect was evaluated dynamically by visual analogue scale (VAS) in 120 patients before treatment, immediately after treatment, 30 min after treatment, 1 h after treatment and 24 h after treatment. The analgesic effects at different time were statistically analyzed by generalized estimating equation. ResultsThe mean values of VAS were 7.483, 3.950, 4.767, 5.917 and 7.217, respectively, before treatment, immediately after treatment, 30 min after treatment, 1 h after treatment and 24 h after treatment. The difference of analgesic effect at different time was statistically significant (P<0.01); the difference of analgesic effect of superficial needling in treatment of SHS after stroke was significant between immediately after treatment and 30 min after treatment (both P<0.05), while there was no significant difference between 1 h after treatment and 24 h after treatment (both P>0.05). ConclusionAnalgesic effect of superficial needling for shoulder pain of SHS after stroke was different at different time points and decreased over time; analgesic effect was the most significant immediately after treatment and the optimal duration of analgesic effect was from immediately to 30 min after superficial needling therapy.

Effects of balance acupuncture combined with function training in treatment of shoulder-hand syndrome after brain stroke

Objective To explore the effects of balance acupuncture combined with function training in treatment of shoulder-hand syndrome after brain stroke.Methods 120 cases were randomly divided into three groups:40 cases in treatment group were treated with the rehabilitation function training combined with balance acupuncture; 40 cases in control group A were treated with conventional functional training 40 cases in control group B were treated with balance acupuncture.All patients were assessed before and after therapy in two weeks,three weeks and four weeks by present pain intensity (PPI),FuglMeyer motor function assessment (FMA) of the upper limb and modified barthel index (MBI).Results The scores of the three groups were improved after two weeks(P ＜ 0.05),but the score of the treatment group was significantly better than those of the control group A and the control group B (P＜ 0.05).There was no significant difference between the control group A and B (P＞ 0.05).After three weeks,the score there was no significant difference between the treatment group and the control group B(P＞ 0.05),but the control group A and B was significantly different (P ＜ 0.05).After four weeks,the score of each group showed no significant difference (P＞ 0.05).Conclusion Balance acupuncture combined with functional training can rapidly improve the upper-limb function and shoulder-hand syndrome after brain stroke. Hemiplegic shoulder pain: implications for occupational therapy treatment.

Hemiplegic shoulder pain is common after stroke causing hemiplegia. It adversely affects the recovery of arm function and independence in activities of daily living. Subluxation, abnormal tone and limited range of motion or capsular constrictions have been reported as potential causes. Other factors such as rotator cuff tears, brachial plexus injury, shoulder-hand syndrome and other pre-existing pathological conditions may also be associated with hemiplegic shoulder pain. The etiology remains unclear, but hemiplegic shoulder pain may result from a combination of the above factors. This literature review examines the possible causes of hemiplegic shoulder pain and discusses the implications for occupational therapy treatment. Occupational therapy interventions include proper positioning, facilitation of movement through purposeful therapeutic activities, increasing passive range of motion, implementation of external supports and treatment of shoulder-hand syndrome. Understanding the processes involved will assist with effective assessment, treatment and prevention of hemiplegic shoulder pain. This will facilitate clients' participation in rehabilitation programs and move them towards attainment of optimal function.

Treatment of shoulder-hand syndrome with griseofulvin.

When griseofulvin (Fulvicin) was made available for the treatment of fungus diseases, there was some question whether it was fungistatic or fungicidal. This became evident when fungus infections of the finger or toe nails were treated.1The growth was apparently inhibited by continuous treatment for a number of months, while an old nail was replaced by a new one. If treatment was discontinued before the new nail had completely appeared, the fungus would start growing again and the prior condition would return. Since inflammation accompanying some fungus diseases of the skin1subsides rather abruptly, it was thought possible by us that griseofulvin might be another anti-inflammatory agent. Proceeding on this assumption, we decided to administer the drug to some of our patients with rheumatoid arthritis. Among the group of patients were a few with cases of shoulder-hand syndrome.
