MTA-An Elixir for Root Perforations-A Short Review

Root perforations are artificial connections between the root canal system and the periodontium and/or the oral cavity. They are undesired complications of endodontic treatment. It is very important to seal the site of perforation once diagnosed to minimize injury and prevent contamination of the surrounding periodontal attachment apparatus. Mineral trioxide aggregate is an endodontic cement used in treatment of root perforations and it is extremely biocompatible, hydrophilic and capable of stimulating the healing processes. Root perforation is a sign of endodontic failure and is the most common complication of endodontic therapy. The aim of this review is to understand the effectiveness of MTA in management of root perforations and to determine the prognosis of the treatment. Knowing the effectiveness of MTA may fasten the treatment and be more effective compared to other methods.

Interactions between Oral Tissues and External Light and Matters

We are currently in an era where the international scientific publication is highly valued, acting as a strong parameter for evaluation mechanism in ranking universities in the competitive scientific world. In this context, writing the editorial of this special issue gives us a great responsibility. After a cautious revision of the several articles submitted for publication in this special issue by the Editorial Board of this journal, the accepted papers translate the commitment of the authors to the scientific research. This special issue is a sample of the current research efforts addressing issues related to the interaction between the light and the oral tissues, highlighting subjects such as the treatment of pathologies associated with high intensity laser for better healing and welding, the use of high intensity laser treatment for cosmetic dentistry, evaluation of adhesive systems and dentin hypersensitivity, the detection and treatment decision of caries lesions with laser fluorescence, the treatment of root perforation using high intensity laser, and also the low level laser therapy for improving wound healing. Properties and applications of calcium hydroxide in endodontics and dental traumatology

Calcium hydroxide has been included within several materials and antimicrobial formulations that are used in a number of treatment modalities in endodontics. These include, inter-appointment intracanal medicaments, pulp-capping agents and root canal sealers. Calcium hydroxide formulations are also used during treatment of root perforations, root fractures and root resorption and have a role in dental traumatology, for example, following tooth avulsion and luxation injuries. The purpose of this paper is to review the properties and clinical applications of calcium hydroxide in endodontics and dental traumatology including its antibacterial activity, antifungal activity, effect on bacterial biofilms, the synergism between calcium hydroxide and other agents, its effects on the properties of dentine, the diffusion of hydroxyl ions through dentine and its toxicity. Pure calcium hydroxide paste has a high pH (approximately 12.5-12.8) and is classified chemically as a strong base. Its main actions are achieved through the ionic dissociation of Ca(2+) and OH(-) ions and their effect on vital tissues, the induction of hard-tissue deposition and the antibacterial properties. The lethal effects of calcium hydroxide on bacterial cells are probably due to protein denaturation and damage to DNA and cytoplasmic membranes. It has a wide range of antimicrobial activity against common endodontic pathogens but is less effective against Enterococcus faecalis and Candida albicans. Calcium hydroxide is also an effective anti-endotoxin agent. However, its effect on microbial biofilms is controversial.

Treatment of Endodontic Perforations Using Guided Tissue Regeneration and Freeze-Dried Bone Allograft: Two Case Reports with 2-4 Year Post-Surgical Evaluations

Clinicians often have difficulty with the diagnosis and treatment of root perforation. This paper reports two patients with root perforation treated with periodontal surgery associated with guided tissue regeneration (GTR) and demineralized freeze-dried bone allograft (DFDBA). This combined treatment resulted in minimal probing depths, minimal attachment loss, and radiographic evidence of bone gain after follow-up evaluations that ranged from 2 to 4 years. These case reports show a correct diagnosis and removal of etiologic factors can restore both periodontal and endodontic health.

Iatrogenic root perforations sometimes occur during root canal treatment or when preparing the root canal for a post. The inflammation in the periradicular tissues caused by root perforation reduces the prognosis of the involved tooth. The situation may be further complicated if by coronal leakage, microorganisms are allowed to infect the canal and the periradicular region. This clinical report presents a nonsurgical technique, based on the internal matrix concept, using both calcium hydroxide and SuperEBA together to repair root perforations. The author has experienced good clinical results with this perforation repair technique.
