Treatment of Left Ventricular Thrombus With Direct Oral Anticoagulants: A Retrospective Observational Study

BackgroundThere is limited data on the efficacy of direct oral anticoagulants (DOACs) for the treatment of left ventricular thrombus. Currently, vitamin K antagonists (VKAs) remain the preferred oral anticoagulant for left ventricular thrombus. In this retrospective study, we assessed the safety and efficacy of DOACs in comparison to VKAs in patients with a new diagnosis of left ventricular thrombus. MethodsWe retrospectively identified all patients admitted to the 5 Catholic Health Initiative Omaha hospitals with a diagnosis of left ventricular thrombus between January 2012 and March 2019 and were discharged on oral anticoagulants. Patients were stratified into 2 groups: VKAs or DOACs and followed for up to 1 year. We compared the outcomes of ischemic stroke, bleeding, and echocardiographic resolution of left ventricular thrombus between the 2 groups. ResultsA total of 99 patients were included in this study (mean age: 61 years, 29% females). Of these, 80 (81%) were discharged on VKAs and 19 (19%) on DOACs. Stroke within 1 year of diagnosis occurred in 2 patients in the VKA group and none in the DOAC group (P = 0.49). Bleeding events were observed in 5 patients (4 in the VKA group and 1 in the DOAC group; P = 0.96). Ninety patients had follow-up echocardiogram; resolution of left ventricular thrombus was similar between the 2 groups (VKAs vs DOACs: 81% vs 80%; P = 0.9). ConclusionIn patients with left ventricular thrombus, DOACs and VKAs had similar rates of stroke and bleeding. These findings need confirmation in randomized clinical trials.

Direct Oral Anticoagulants for the Treatment of Left Ventricular Thrombus-A New Indication? A Meta-summary of Case Reports.

Left ventricular thrombus (LVT) can be a consequence of cardiac diseases such as heart failure with reduced ejection fraction and acute myocardial infarction. Currently, the guidelines recommend the use of warfarin for the treatment of this condition. However, there are increasing reports of patients with LVTs being treated with direct oral anticoagulants (DOACs), for several reasons. We set out to review the available literature to assess the safety and the efficacy of this approach. We analyzed 52 cases, extrapolated by 34 papers contained in literature, focusing on the characteristics of patients, treatment, outcome, and follow-up. Rivaroxaban was the most commonly used DOAC, followed by apixaban. The diagnosis of LVT and the follow-up were mainly performed by transthoracic echocardiography. The thrombus resolved in 45 patients (92%) of 49 (there are no data available regarding the outcome of 3 patients) and failed to resolve in 4 patients treated with DOACs. The resolution occurred in a median of 32 days. DOACs are shown to be a reasonable and valid option for the treatment of LVT. Our study provides a rationale for a prospective randomized controlled trial.

Comparative Effectiveness of Direct Oral Anticoagulants and Warfarin for the Treatment of Left Ventricular Thrombus

Introduction Left ventricular (LV) thrombus is a complication of acute endomyocardial injury and chronic ventricular wall hypokinesis (e.g. dilated cardiomyopathy), resulting in increased risk of thromboembolic complications. Historically, observational studies support the general safety and efficacy of warfarin for this indication. To date, limited data exists regarding the use of the direct oral anticoagulants (DOACs) for LV thrombus. Methods A retrospective cohort study utilizing electronic medical records was conducted from September 2012 - October 2018 at the University of Colorado Hospital System. Patients were included if they had any diagnosis of LV thrombus and received either a DOAC or warfarin within 90 days of diagnosis. The primary outcome was to determine the incidence of thromboembolic stroke in patients receiving a DOAC compared to warfarin. Secondary outcomes consisted of the composite incidence of thromboembolic events (stroke and systemic embolism), bleeding rates defined by the The Global Use of Strategies to Open Occluded Arteries (GUSTO) criteria, and blood product administration. Chi square test was used to analyze categorical variables and the student's t-test and Wilcoxon Rank Sum test for continuous data. All statistical analyses were conducted using SAS. Results A total of 949 patients met inclusion, of whom 180 (19%) received a DOAC and 769 (81%) warfarin. Of the DOAC cohort, 77 (41.6%) received rivaroxaban, 79 (42.7%) apixaban, and 29 (15.7%) dabigatran. For baseline demographic information, see table 1. For the primary endpoint of new onset thromboembolic stroke on or within 90 days of LV thrombus diagnosis, no difference existed between treatment groups (DOAC: 7.8% vs warfarin: 11.7%, p=0.524). When compared to warfarin, no difference existed in the composite of thromboembolic events (33.1% vs 30.6%, P=0.524, respectively) or in GUSTO bleeding (11.1% vs 7.8%, p=0.397, for DOAC vs. warfarin, respectively). However, more patients on warfarin received blood products compared to those taking a DOAC (25.8% vs 13.9%, p Conclusions Compared to warfarin, DOACs may be safe and efficacious for the treatment of LV thrombus, however further studies are warranted.

Efficacy of Direct Acting Oral Anticoagulants in Treatment of Left Ventricular Thrombus

Direct acting oral anticoagulants (DOACs) are increasingly used as off-label alternatives to vitamin K antagonists for the treatment of left ventricular (LV) thrombus. However, efficacy data is limited to small case series and one meta-analysis of case reports. We aimed to determine the efficacy and safety of DOACs in treatment of LV thrombus utilizing transthoracic echocardiography (TTE) and clinical outcomes. We identified 52 patients (mean age = 64 years, 71% men) treated with a DOAC for LV thrombus (n = 26 apixaban, n = 24 rivaroxaban, and n = 2 dabigatran). Thirty-five of the 52 patients had a follow-up TTE after DOAC initiation. The primary end point was defined as resolution of LV thrombus (in patients with a subsequent TTE), or death, major bleeding requiring transfusion, intracranial hemorrhage, ischemic stroke, or peripheral embolization. An experienced echocardiographer (M.L.M.) reviewed all TTEs for presence or absence of LV thrombus without knowledge of time point or clinical data. Twenty-nine of the 35 (83%) patients who underwent follow-up TTE had resolution of LV thrombus, with a mean duration of 264 days. Of the total study population, there was 1 cardioembolic event (transient ischemic attack) 52 days after initiating DOAC, 3 gastrointestinal bleeds requiring transfusion, and 1 patient with epistaxis requiring transfusion. All patients with a hemorrhagic complication were receiving concomitant antiplatelet therapy. DOAC therapy appears promising for the treatment of LV thrombus. A larger, prospective study is warranted to confirm these results.

Use of dabigatran for treatment of left ventricular thrombus: A tertiary care center experience.

Objectives:Direct oral anticoagulants (DOACs) are now replacing vitamin K oral anticoagulants (VKAs) owing to superior efficacy, rapid action, less bleeding, wider therapeutic range,and fewer food and drug interactions.Unfortunately, the available data on the use of DOACs, particularly dabigatran, for treatment of left ventricular thrombus (LVT) issparse. We have hereby presented the largest study on use of dabigatran in LVT.Methods:Retrospective data of patients having LVT as diagnosed by transthoracic echocardiography (TTE) was screened. Patients on dabigatran were included in the study and follow up data of 6 months was obtained through medical records.Results:Of the 15 patients included in the study, the most frequent etiology was ischemic heart disease (67%), including 7 patients of STEMI (47%), followed by non-ischemic cardiomyopathy in 5 patients (33%). Only one patient, with STEMI, developed mild gastrointestinal bleeding at 3 months. Complete clot resolution was seen in 2 patients (13%) at first week of follow up and total 5 patients (33%) at the end of second week. The rate of clot resolution at 1 month, 3 months, and 6 months were 80%, 93% and 100%, respectively. The median duration required for complete clot resolution was 30 days (IQR=14-30).Conclusion:Dabigatran appears to be safe, highly efficacious and results in rapid LV clot resolution. DOACs may be a suitable alternative to warfarin in treatment of LV thrombus. However, larger studies are required to validate this hypothesis.

Direct Oral Anticoagulants for Left Ventricular Thrombus

Background: Left ventricular thrombus (LVT) is serious complication of myocardial hypokinesis resulting from acute myocardial infarction, aneurysms, or cardiomyopathic processes. LVT portends an increased morbidity and mortality due to thromboembolic sequelae. Current guidelines recommend warfarin for treatment of LVT and prevention of embolic complications. Direct oral anticoagulants (DOACs) including rivaroxaban have exploded in popularity in the last decade and are now more frequently prescribed than warfarin. Methods: A best evidence topic was written addressing the question “is left ventricular thrombus amenable to management with direct acting oral anticoagulants in terms of efficacy and safety outcomes?” Altogether, 27 papers describing 52 cases were found searching Medline, Embase, and Pubmed databases from 2014 to February 2019, which represented the best evidence to answer the clinical question. Results: Of the 52 cases described, DOAC thearpy resulted in LVT resolution in 88%. Rivaroxaban was the most commonly used agent (40%). Duration of therapy ranged from 1–12 months most commonly three months. Low-dose regimens were more common than standard-dose regimens owing to the co-administration of dual antiplatelet therapy in 58%. The rate of serious bleeding complications was low, occurring in only two patients (3.8%). Conclusions: We conclude that while data surrounding use of DOACs in the treatment and prevention of LVT are limited to case series, they appear to be highly efficacious. Randomised data is needed to demonstrate clear noninferiority to warfarin as well as optimum dosing and during of DOAC therapy for treatment of LVT prior to changing guideline recommendations.

Oral factor Xa inhibitors for the treatment of left ventricular thrombus: a case series

Left ventricular thrombus (LVT) formation usually necessitates short term anticoagulation for thrombus resolution and to prevent embolic events. Historically, vitamin K antagonist therapy has been the treatment of choice. However, with the advent of direct acting anticoagulants, their role in the management of LVT is not clear. Patients were included if they had received rivaroxaban or apixaban for more than 1 day for LVT documented on imaging. The primary objective was resolution of LVT at 3 months based on assessment by an independent cardiologist review of initial and subsequent imaging results. The principle safety objective was to assess major or clinically relevant non-major bleeding using GUSTO, TIMI, and BARC bleeding criteria. During the 2-year study period seven patients were treated with rivaroxaban and three with apixaban. Two patients who had received apixaban and one on rivaroxaban were lost to follow up. In those with an initial and follow-up ECHO (n = 6) the median time to follow up imaging was 214 (IQR 33-414) days and complete LVT resolution was observed in 83% of patients. One patient on rivaroxaban had a bleeding events that was minimal (TIMI), Type 2 (BARC), or classified as mild (GUSTO) due to pulmonary hemorrhage. In those deemed not to be a candidate for vitamin K antagonist the use of rivaroxaban or apixaban may be a considered in the treatment of LVT. Further research in this area is needed to assess the efficacy and safety of using FXAI for treatment of LVT.

Treatment of left ventricular thrombi with a low molecular weight heparin

Background: Once a diagnosis of left ventricular thrombus has been established, the classical attitude consists in the administration of unfractionated heparin relayed by oral anticoagulation therapy. However, the use of unfractionated heparins in this indication was only assessed in an open, non-randomized study with no control group, including 23 patients. On the other hand, although low molecular weight heparins are routinely used in some departments, there are no studies available concerning these agents in this indication. The aim of this study was to evaluate the feasibility of low molecular weight heparin therapy in patients with left ventricular thrombi. Methods: The study was a prospective, non-randomized, open-label trial. All patients with a new left ventricular thrombus diagnosed between September 2000 and September 2003 received enoxaparine 100 IU/kg twice daily for a mean duration of 13 days. A relay treatment with fluindione was initiated on day 5. The left ventricular thrombus outcome was followed for 3 weeks by bi-weekly transthoracic echocardiography. Results: 26 left ventricular thrombi were diagnosed over the 3-year study period: 19 in post-infarct patients with a history of anterior myocardial infarction and 7 in patients with dilated cardiomyopathy. The mean thrombus area decreased from 2.30±0.32 to 0.36±0.11 cm 2 ( p<0.0001). Nineteen thrombi out of twenty-six (73%) disappeared during the treatment period. No thrombocytopenia or hemorrhagic events were observed. One transient ischemic attack was reported. Conclusion: This preliminary study suggests that low molecular weight heparins are well tolerated and efficient in terms of left ventricular thrombi disappearance or size reduction.

Efficacy of high dose intravenous heparin for treatment of left ventricular thrombi with high embolic risk

Objectives. This was to assess the efficacy of high dose intravenous heparin to treat mobile or protruding left ventricular thrombi as detected by serial echocardiography.Background. The presence of mobile and protruding left ventricular thrombi greatly increases the risk of arterial embolization, yet optimal therapy, be it thrombolysis, anticoagulation or surgical removal, has not been defined.Methods. Full dose heparin, 31,291 ± 7,980 (mean ± SD) IU/day, to prolong partial thromboplastin time to at least twice normal, was administered intravenously to 23 consecutive patients with 25 mobile and protruding thrombi. Patients were prospeetively evaluated for hemorrhagic complications and embolic events during therapy. The presence or absence of thrombi and their size and characteristics were assessed by serial echocardiography.Results. In all 23 patients left ventricular thrombi decreased in size, with disappearance of the high risk features. The duration of high dose heparin infusion was 7 to 22 days (mean 14 ± 4). Thrombus size was reduced from 3.9 ± 2.6 to 0.16 ± 0.38 cm2, and thrombus disappeared entirely in 19 (83%) of 23 patients. No embolic events were detected during treatment, and the only complication was an upper gastrointestinal hemorrhage that was successfully treated medically.Conclusion. High dose intravenous heparin is a highly effective and safe treatment for completely resolving left ventricular thrombi with high risk features for embolization. Most such thrombi disappear completely within 1 to 3 weeks of this treatment without embolic or hemorrhagic complications.
