В роботі подані результати використання тренажерних пристроїв Гросса і МОТОmed gracile12 в реабілітації дітей із різними формами дитячого церебрального паралічу (ДЦП), формуванні й розвитку основних рухових навичок, підвищенні фізичної підготовленості та витривалості. Відновлювальне лікування дитини з ДЦП проводилось з урахуванням індивідуальних можливостей і потреб. У реабілітаційній програмі використовували метод чергування занять на тренажерах. Терапія базувалася на ігрових формах. Реабілітація адаптувалась до індивідуального рівня розвитку дитини, робився акцент на її сильні сторони та потреби. Використовувалися поєднані режими пасивного, активного й комбінованого тренування. Завдяки впровадженню методики використання тренажерних пристроїв Гросса і МОТОmed gracile12 в комплексну реабілітацію дітей із ДЦП збільшився загальний рівень рухової активності та маніпулятивної діяльності, досягнуті результати зберігалися тривалий час.

Clinical behavior of children with infantile cerebral palsy after ozone therapy.

Objective.
 The aim of this study was to determine the usefulness of ozone therapy in the treatment of Infantile Cerebral Palsy (ICP). 
 
 Patients and methods.
 A non-controlled clinical assay was made in the Ozone Research Center (CIO), Havana, Cuba from January 2013 to January 2014. The sample was constituted by patients remitted to pediatrics consultation of CIO, to whom inclusion and exclusion criteria were applied. The study group involved 45 patients, from 1 month of birth to 8 years, with cerebral palsy of hypoxic-ischemic cause. The evaluation criteria were: evolution of the motor disorder according to the Gross Motor Function Classification System (GMFCS) scale, modification of muscle tone (Ashworth modified scale) and response to treatment (O’Brien modified scale). The way of administration was rectal insufflation; concentrations between 15, 20, 25 and 30 mg/L were used, volumes varied according to age, making calculation of the dose of ozone according to kilograms of weight. Cycles of 20 sessions, every 3 months were indicated, until completing 4 in 16 months. Patients were clinically evaluated, according to the scales used, before and after each cycle. 
 
 Results and Discussion.
 The best answer to treatment was obtained in the group aged ? 4 years. The variables analyzed showed a significant improvement when the ozone treatment concluded. With respect to the evolution of the motor disorder, in 65 % of cases it improved. In the group of children below 4 years, the response was better in relation to the muscle tone. Response to treatment, according to the relatives’ criteria, was of 70 % of the children with marked improvement in the tone and muscle function. 
 
 Conclusions. 
 The greatest percentage of patients improved in the evolution of the motor disorder; when the Manual Ability Classification System (MACS) scale was applied, more than half the patients showed an improvement. A high percentage of children get a satisfactory result regarding muscle tone and motor function. No side effects were present in any of the cases during the study.

Progress in the treatment of infantile cerebral palsy with traditional Chinese and western medicine

Cerebral palsy is due to the development of the fetus or infant brain damage caused by non progressive, and the performance of continuous movement and postural abnormalities, which is one of the most common causes of disability in children.So far there is no specific treatment for cerebral palsy, mainly rehabilitation, which divided into traditional Chinese medicine rehabilitation therapy and modern western medicine rehabilitation therapy.This article reviews the research progress of traditional Chinese medicine and western medicine in the treatment of cerebral palsy. Discussion on the clinical treatment of infantile cerebral palsy with the differentiated intervention of meridian points on the spine and back

In view of the literature study and clinical practice, the rules of the distribution, the general function and the indication of the meridians points on the spine and back were explored. The TCM mechanism on the rehabilitation of infantile cerebral palsy was studied. The rules of acupoint combination on the spine and back were summarized with meridians, main points and supplementary points involved in the TCM rehabilitation of infantile cerebral palsy. In view of the theoretic study, the advantages on the early infantile motor function, such as body turning, sitting, standing and walking, were analyzed with the long-term intervention of meridian points on the spine and back. It is showed in the rehabilitation of infantile cerebral palsy that the intervention of meridian points on the spine and back improves the motor function on the back and relieves the incoordination in the development between each system and enhances the rehabilitation mechanism of the integration of Chinese and western medicine for the functional development of core stability, which provides much more effective and more precise approach to the diagnosis and treatment of infantile cerebral palsy in clinical practice.

Rehabilitation of Handicapped Children Suffering from Infantile Cerebral Palsy with Combined Embedding Acupuncture (YNSA + Body Acupuncture) and Complex Physiotherapy

Objectives: The standard treatment of infantile cerebral palsy contain following elements: medication, developmental behavior therapy, surgery, assistive technologies and some form of CAM. Acupuncture treatment is difficult to be carried out in children due to less tolerance of inserted needles. Therefore we were looking for atraumatic, not frequently disturbing methods for these patients. The aim of this prospective, assesor-blinded randomized control trial study was to investigate whether a special form of acupuncture, namely the embedding acupuncture, especially Yamamoto’s New Scalp Acupuncture (YNSA), combined with some effective body acupoints is of value for children suffering from cerebral palsy (CP) in addition to the different forms of motor and mental rehabilitation. Design: A prospective, assessor-blinded randomized control trial was carried out in the past 15 years with 585 children in an inpatient rehabilitation unit with day hospital service. After inclusion, patients were randomly stratified into a control group and an acupuncture group. The continous follow up was performed pediatric neurologists and physiotherapy, occupational and speech therapy in addition to skilled medical and nursing care. Outcome measures: changes in optimizing mobility, learning potential, mental development, changes of primary conditions, Qualtiy of Life (QoL) and the independent neurological investigation were used to follow-up the efficacy of the treatment. Results: In the permanent acupuncture group (combined YNSA and body acupuncture) all the sensory, motor and functional scores improved significantly during the examination period until 5 years after birth. A significant development during the 2-year follow-up was found due to developmental behavior pediatrician activity and special daily performed moving exercices, but the embedding acupuncture with YNSA and some important body points (they are motoric trigger points of muscles) treatment facilitated the mental and functional development. Improved moving and mental function, more flexible joints and ligaments were observed in comparison to the small patients’ condition prior to treatment. The number of seizures decreased, the reduction of antiepileptic drugs was possible, which resulted an elevated mental vigility. Statistical analyses showed significant improvement in Barthel index. Conclusion: The data suggest, that the embedding forms of acupuncture of YNSA and body acupoints are useful additional accessor methods in the rehabilitation of some forms of infantile CP. It enhances their quality of life, helping their integration into healthy child population.

The role of microcurrent reflexotherapy in combination with neuroprotector in the rehabilitation of the patients with infantile cerebral palsy

The objective of the present study was to estimate the therapeutic efficacy of the combined treatment of infantile cerebral palsy by means of microcurrent reflexotherapy (MCRT) in combination with the neuroprotector cortexin. This treatment including 15 sessions with the use of a MAKS apparatus was given to 69 children at the age from 3 to 7 years. They were randomly allocated to two groups. The patients of group 1 underwent three courses of MCRT and two courses of cortexin therapy in conjunction with massage and remedial gymnastics, those in group 2 were treated with massage and remedial gymnastics alone. Positive dynamics in the patients' clinical conditions was documented in the end of the rehabilitative program. Specifically, 60.5% of the children in group 1 developed the ability to walk unassisted compared with 38.6% in group 2. Positive changes in the brain functional status were documented, based on the results of the electroencephalographic study, in 21 (71%) and 16 (53%) children of groups 1 and 2 respectively.

Clinical Observation on Treatment of 29 Cases of Infantile Cerebral Palsy with Acupuncture and Massage

Objective To observe clinical effect of treating infantile cerebral palsy with acupuncture and massage. Methods 29 cases with cerebral palsy were randomly recurited into a control group and a treatment group. The treatment group was treated with acupuncture and massage, and the control group was treated with western medicine. The clinical effect of both groups was observed after the treatment. Results The total effective rate of the treatment group and the control group was 85.7% and 66.7% respectively, showing a significant difference (P<0.05) . Conclusion The treatment of infantile cerebral palsy with acupuncture and massage was better than treated with western medicine. Key words: Cerebral palsy; Acupuncture; Massage

Therapeutic observation on treatment of infantile cerebral palsy with integrative Chinese-Western medicine: A clinical report of 104 cases

To study the therapeutic effects of combined treatment with Chinese and western medicine in children with cerebral palsy (CP). 104 CP children were treated mainly with physiotherapy and acupuncture, together with occupational therapy, sensory integration and electrical stimulation. Gross Motor Function Measure (GMFM) was used to assess the effects of treatment. After treatment for 3.29±0.80 months, the majority of CP children improved their IQ and limb symptoms, with decreased abnormal posture, increased muscle strength and improved motor developmental function. GMFM88 and GMFM66 scores had significantly increased after treatment. Combined treatment with Chinese and Western Medicine has confirmed effects on CP children.

Treatment of infantile cerebral palsy mainly by acupoint-pressing and Tuina

Purpose: To investigate therapeutic methods and results of comprehensive convalescent method of acupoint-pressing, Tuina, acupoint-injection and exercise in the treatment of childhood cerebral palsy. Methods: Six hundreds and forty children under 7 years old with cerebral palsy were divided randomly into treatment and control groups, and they were both given routine acupoint-injection and functional exercise, children in treatment groups were also given Tuina and acupoint-pressing therapies following acupoint-injection and functional exercise. Results: Therapeutic effect was significantly better in treatment group than that in control group (P<0.05). Conclusion: Comprehensive convalescent method, mainly by acupoint-press, Tuina, and acupoint-injection is one of premium therapy for treatment of childhood cerebral palsy, and this comprehensive convalescent therapy is really full of Chinese features and worthy of popularization.

Treatment of infantile cerebral palsy by scalp acupuncture combined with point injection in 48 cases

The author treated forty-eight cases of infantile cerebral palsy with scalp acupuncture and point injection. After two courses of treatment the infants made progress in different degrees. Eight cases were cured, sixteen cases were obviously improved, twenty-two cases were improved and two cases were unchanged. The total effective rate was 95.8%.

A pilot investigation into the effects of extracorporeal shock waves on muscular dysfunction in children with spastic movement disorders

Extracorporeal shock waves (ESW) are being used increasingly frequently for the treatment of narrowly localized pain in the musculoskeletal system. Pain is normally associated with muscular dysfunction. Beginning with the observation that not only is the pain soothed by treatment with ESW, but that the muscular dysfunction itself is relieved, an attempt has been made to treat the impaired function in cases of infantile cerebral palsy with ESW. In 35 children and young people with mainly spastic disturbances of movement, the bellies of those muscles producing the flexion contracture of the lower limb were each treated with 500 non-focused pulses of low-energy ESW. The focus (focal pressure 10 MPA=0.06 mJ/mm) of the shock waves itself was placed either within the coupling cushion of the therapy source (Minilith, Storz Medical, CH-Kreuzlingen) or obliquely outside the patient's body. In this way ESW could be dispersed throughout the muscles and the risk of undesirable effects was minimized. The energy used amounted to about 30% of the threshold value at which cellular damage from focused treatment with ESW had been observed in the endothelium of the human umbilical cord. The degree of movement at the hip joint increased after one treatment by an average of 17 degrees, at the knee joint by 12.3 degrees and at the ankle joint by 7.1 degrees, each increase being statistically significant. Muscular stiffness, spastic co-contractions, and myofascial viscoelasticity, as well as dyskinetic and ataxic symptoms, decreased. It appeared as if a systemic action had entered into the motor control. Together with an increase in general proprioception, there was an improvement in body image, in economy of movement and also in the limbs that had not been treated. A few patients showed euphoric reactions for about 2 days after treatment. No undesirable effects were observed during a follow-up of 12 months. The treatment was painless and is therefore suitable for children. The biomechanical improvement remained for several weeks and this time could be used for initiating an improved pattern of movement. ESW is suitable for use as an additional instrument for physiotherapeutic treatment of infantile cerebral palsy. The effective action of ESW on disturbed peripheral muscles has been discussed in terms of positive pressure and tensile wave forces and/or forces acting by cavitation. The results obtained in this pilot investigation showed particular a decrease in muscle stiffness. A controlled investigation with gait analysis and psychological control is to be undertaken.

Experience of Modified Yamamoto Scalp Acupuncture with Simultaneous Rehabilitation and Soft-Laser Therapy in Children with Cerebral Palsy

This is an account of methods developed in Czechoslovakia for the treatment of infantile cerebral palsy. Fifty eight children have been treated using a combination of scalp acupuncture, soft laser therapy and intensive exercise. The acupuncture acts as an analgesic and muscle relaxant thus allowing both passive and active exercises to be practised more effectively. Scalp acupuncture is more practical than body acupuncture as the needles are well out of the way and do not interfere with muscular movement. The neurological benefits are not confined to the development of more normal physical capacity, but extend also to an improvement in mental ability.
