Isolated Limb Perfusion in the Treatment of In-Transit Melanoma Metastases: Are There Predictive Factors for the Outcome?

Introduction: Isolated limb perfusion (ILP) with delivery of high dose melphalan proved to be efficient in the treatment of in-transit metastases (ITM). Preoperative factors may carry an impact on patient outcome, including in-field or local progression-free survival (IPFS), time to distant metastases (TDM) and overall survival (OS). Materials and Methods: A retrospective analysis of 83 patients who underwent an ILP at our institution before the era of efficient upfront systemic therapy in high-risk cases. Patients were classified according to a modified M.D. Anderson score, with relevance for the outcome: 34 stage III A (patients with satellites and/or ITM), 31 stage III AB (patients with synchronous regional lymph node metastases and satellites and/or ITM), 11 at a new stage labelled III A(B) which takes into account a previous history of therapeutic regional node dissection and actual recurrence in the limb only, and 7 stage IV (metastatic cases with actual major problem of recurrence in the limb). Results: Our median follow-up time was 90.1 months (IQR 72.8-151.6). Median IPFS was 16.3 months (95% CI 9.5-78.5), median TDM 28.8 months (95% CI 15.4-69.6) and median OS 34.6 months (95% CI 21.1-59.5). The strongest significant prognostic factor regarding IPFS was LND before ILP (p=0.02). However, sex (p=0.03/0.07), LND before ILP (p=0.004/0.11) and some primary tumor characteristics (Clark level (p=0.15/0.07) and ulceration (p=0.006/0.04)) were prognostic regarding TDM and / or OS. Conclusion: ILP with melphalan can provide long-term regional and systemic tumor control in a selected group of patients and should be kept in mind for patients recurring after local surgery or radiotherapy and resistant to or ineligible for the newer systemic therapies.

Response and Toxicity of Repeated Isolated Limb Perfusion (re-ILP) for Patients With In-Transit Metastases of Malignant Melanoma

BackgroundIsolated limb perfusion (ILP) is a safe and well-established treatment for in-transit metastases of melanoma. In case of relapse or disease progression, ILP can be repeated (re-ILP). This study aimed retrospectively to analyze a large consecutive series of re-ILP and compare clinical outcomes with first-time ILP.MethodBetween 2001 and 2015, 290 consecutive patients underwent 380 ILPs. Of these, 90 were re-ILPs including 68 second ILPs, 16 third ILPs, 4 fourth ILPs, and two fifth ILPs. The study evaluated response (using World Health Organization [WHO] criteria), local toxicity (using the Wieberdink scale), and complications (using Clavien–Dindo).ResultsThe results were compared between the first ILP, the second ILP, and the third to fifth ILP. The overall response rate was respectively 83%, 80% and 68%, with a complete response (CR) rate of 60%, 41%, and 59%. In the re-ILP group, the patients with a CR after the first ILP had a 65% CR rate after the second ILP compared with 8% for the patients without a CR (p = 0.001). The risk for local toxicity or complications was not increased after re-ILP. The median overall survival periods were respectively 34, 41, and 93 months (p = 0.02).ConclusionAs a therapeutic option, ILP can be repeated safely for in-transit metastases of melanoma, achieving similar high response rates without increasing complications or toxicity. Re-ILP is mainly indicated for patients who already had a CR after the first ILP, whereas other treatment options should be considered for primary non-responders.

Long-term follow-up of 163 consecutive patients treated with isolated limb perfusion for in-transit metastases of malignant melanoma

Purpose: The aim of the present study is to describe our experience with isolated limb perfusion (ILP) in the treatment of in-transit metastases of malignant melanoma and to determine prognostic factors for response, local progression, survival and toxicity. Materials and methods: A retrospective follow-up of all patients (n = 163) treated between January 1984 and December 2008 using data collected from individual patient records and the Swedish National Patient Register. Results: Clinical response was evaluable in 155 patients, 65% had a complete response (CR) and 20% had a partial response (PR). Local progression occurred in 63% of the patients after a median time of 16 months. Negative prognostic factors in univariate analyses were proximal location of the primary tumour, >10 in-transit metastases and if there was no CR after ILP. In multivariate analysis, proximal location of the primary tumour and no CR after ILP were significant prognostic factors. Median cancer-specific survival was 30 months, and negative prognostic factors in univariate analyses were male gender, positive lymph node status, systemic metastases, bulky tumour, >10 in-transit metastases and if there was no CR after ILP. In multivariate analysis, positive lymph node status, bulky tumour and no CR after ILP were significant prognostic factors. A majority (97%) of the patients had a Wieberdink grade II–III local toxicity. Four patients underwent limb amputation after a median of 19 months, none because of toxicity. Conclusion: We found that ILP is a safe method with a high response rate for the treatment of patients with in-transit metastases of malignant melanoma.

Isolation perfusion in extracorporeal circulation with interleukin-2 and lymphokine-activated killer cells in the treatment of in-transit metastases from limb cutaneous melanoma

Background: Therapies of advanced melanoma patients with interleukin-2 (IL-2) and cytotoxic lymphocytes have produced interesting results, but a larger diffusion of these treatments is limited by the severe side effects due to IL-2 systemic infusion. A strictly regional administration of IL-2 and cells by an isolation perfusion (IP) in extracorporeal circulation (ECC) for the treatment of regional melanoma metastases could improve tolerability and efficacy of this specific modality of immunotherapy.
