Effect of different cone beam computed tomography settings on artifact production in titanium and zirconia dental implants: An in vitro study.

The use of dental implants in the treatment of edentulous patients is increasing. Zirconia implants are an alternative to titanium implants, offering advantages in terms of aesthetics and biological compatibility. However, the number of artifacts observed on radiographic images with zirconia implants compared to titanium implants is yet to be determined. The purpose of this study was to evaluate the impact of different cone-beam computed tomography (CBCT) parameters on the production of artifacts in zirconia and titanium implants. A dry human mandible was coated with wax to simulate human soft tissues and examined. Subsequently, titanium and zirconia implants were placed at the same points in the posterior region of the mandible. The production of artifacts on CBCT scans was evaluated using 2 parameters. The first parameter, the standard deviation within the region of interest (SDROI), is based on a comparison of the gray levels at implant and control areas. The second parameter was the contrast-to-noise ratio (CNR), which was evaluated for different protocols created by various combinations of the field of view (FOV) area, milliampere [mA] intensity and metal artifact reduction (MAR) programs. The study found that zirconia implants produced more artifacts than titanium implants. However, the production of artifacts in zirconia implants could be significantly reduced by increasing the mA values, performing CBCT scans with smaller FOV areas, and enabling MAR programs. The production of artifacts is a disadvantage of zirconia implants, but this drawback can be mitigated by selecting appropriate protocols for the CBCT device.

Exploring dimensions and shapes of edentulous patients consulting the prosthodontics centre at Ibn Sina University Hospital in Rabat, Morocco: a cross-sectional study

This cross-sectional study conducted at the Prosthodontics Centre, Ibn Sina University Hospital in Rabat, Morocco, investigates the dimensions and shapes characterizing maxillary and mandibular edentulous arches in a patient population seeking consultation. The objective is to provide insights crucial for optimizing outcomes in prosthetic therapy, informing decisions on impression trays, occlusion planes, prosthetic teeth, and implant attributes. Sixty-seven edentulous Moroccan patients (40 men, 27 women), aged 31 to 90, were recruited at the Prosthodontics Centre. Maxillary and mandibular casts were meticulously crafted, and precise measurements of anterior and posterior width and length were acquired for both arches. Arch shapes were categorized into O-shape, V-shape, and U-shape. Data were analyzed using Microsoft® Excel 2013 and SPSS software (IBM SPSS Statistics 23). Analysis revealed maxillary arch lengths ranging from 40 to 58 mm, maxillary arch widths from 38 to 58 mm, mandibular arch lengths from 33 to 56 mm, and mandibular arch widths from 30 to 60 mm. The predominant arch shape was the ovoid, constituting 34% (n=23) of maxillary cases and 54% (n=36) of mandibular cases. This study provides novel insights into the dimensions and shapes of maxillary and mandibular edentulous arches in patients seeking consultation at Ibn Sina University Hospital, Rabat, Morocco. The findings offer valuable guidance for prosthetic therapy decisions, emphasizing the importance of integrating established references and arch classifications for optimal outcomes in the treatment of edentulous patients in Morocco.

Evaluation and Comparison of Removable Dentures Constructed with The SR-IVOCAP System and The Traditional System.

The treatment of edentulous patients is still a challenging problem. Acrylic resin is the most used material forconstructing removable denture bases. The main disadvantage of this material is resin shrinkage, which occurs during and afterpolymerization. Our study aimed to evaluate the denture parameters regarding patient satisfaction regarding the removabledentures constructed with two different processing techniques: compression molding and injection. 80 patients, who came in theUniversity Dental Clinic, Tirana were examined and evaluated. Those who fulfilled the criteria were treated with removabledentures. The patients were divided into two groups: in the first group, 40 patients were treated with removable denturesconstructed with the compression molding technique, and in the second group, 40 patients were treated with removable denturesconstructed with the injection molding technique. All the clinical and laboratory procedures until the processing technique wereidentical. After denture delivery, all the patients were re-scheduled to fulfill a questionnaire regarding four parameters of theremovable dentures: denture stability, mastication efficiency, denture comfort, and quality of dentures in general. Results. Thedescriptive statistics of the data were done. Sample t-test showed that the SR IVOCAP dentures had higher mean values for allthe parameters than the traditional dentures. Based on our study, the satisfaction of the patients treated with removable dentureswith the SR-IVOCAP system for all the parameters evaluated was higher compared to the patient satisfaction treated withtraditional removable dentures.

The relationship between labial soft tissue changes and jumping spaces after immediate implant placement and restoration in the anterior maxilla: A prospective study.

Oral implants have been increasingly used in the treatment of edentulous patients or those with dentition defects due to reliable treatment procedure and favorable long-term prognosis. We investigated the changes of labial soft tissue contours with different jumping spaces after immediate implant placement and restoration (IIPR) in the maxillary esthetic area and also provided a long-term stability measurement for the changing trend of soft tissue contour. All patients had been separated into three groups based on the jumping space: group A (horizontal defect dimension [HDD] 2 mm), group B (2 mm < HDD 3 mm), and group C (HDD > 3 mm) and the digital impressions were obtained in the first, third, and sixth month after the operation. The changes of gingival mucosa levels, the average thickness of soft tissue contour volume, and the linear change of submarginal level decreased gradually across the three groups, with the largest change of submarginal level being at 5mm. The size of the jumping space was moderately negatively correlated with the level and average thickness of gingival mucosa and the linear changes of 3 mm and 5 mm under gingival margin, while there was no significant correlation with pink esthetic score (PES) and the linear change of the 1 mm under the gingival margin. Generally, IIPR of upper anterior teeth can achieve esthetic satisfaction, and the level of soft tissue around the implant can be well preserved.

Clinical performance of implant supported mandibular overdentures with cantilever bar and stud attachments: A retrospective study.

Treatment of edentulous patients with implant-supported over-dentures improves denture's retention and stability. Published data concerning implant-supported overdenture with cantilever bars that claimed that can affect the survival and bone loss of implants are scarce. The purpose of this study was to evaluate 5-year clinical performance of mandibular implant-supported over-dentures with different attachment systems. In this retrospective study, 103 patients who had received mandibular over-dentures supported by two implants were evaluated in a 5-year follow up. Studied groups were patients with Spherblock ball attachment (58 patients), Dolder bar with cantilever (36 patients), and Locator attachment (9 patients). Marginal bone-loss around implants, prosthetic complications, soft tissue status of the implants (gingival index, plaque index, pocket depth, and bleeding on probing) were used to compare studied groups. Visual Analogue Scale (VAS) criteria was used to assay patient's satisfaction. One-way ANOVA, Scheffe, Kruskal-Wallis, Mann-Whitney, and Fisher's exact tests, were used for the data analysis (α=0.05). One hundred and three patients (46 male, 57 female, mean age 64.7 ± 8.6) with 206 implants (Strauman) were studied. The implant survival rate was 100% with mean bone loss of 0.22 mm around implants in 5 years. Prosthetic complications including attachment wear and denture fracture occurred more often with ball attachments. The number of attachment replacement, and post insertion appointments were significantly less in patients with bar attachments (p < 0.05). Pocket depth and gingival index were less in the ball attachment (p < 0.05). Mandibular overdenture supported by two implants can be considered a successful treatment in edentulous patients. The frequency of prosthetic complication is higher in unsplinted than splinted superstructures.

RELATIONSHIP BETWEEN INTERPUPILARY DISTANCE AND VERTICAL DIMENSIONS OF OCCLUSION

Background: The determination of the vertical occlusion dimensions is one of the most important aspects in the treatment of edentulous patients. A widely discussed biometric indicator that is suggested for determination of the vertical occlusion dimensions is the interpupillary distance.The aim of this research was to study the relationship between the interpupillary distance and the vertical dimension of occlusion among individuals with natural dentition. Material and methods: Subjects of observation in the study were100 representatives of Bulgarian population, aged 20 to 25. Three reference points were marked: Pronasale, Subnasale and Gnathion. Every person was photographed enface with professional digital camera in strict standardized conditions. With help of the computer program were marked the pupil centres, connected the clinically marked points and then the distances between them were measured. Data were subjected to statistical analysis. Results:Stronger correlation was established between interpupillary distance and the distance Subnasale - Gnathion (for men: r = 0.6858 for women: r = 0.6584), compared to the correlation with distance Pronasale - Gnathion (for men: r = 0.5714 for women: r = 0.5805). Conclusions: Clearly expressed correlation was established between the interpupillary distance and the vertical dimensions of occlusion. The interpupillary distance could be applied as an additional landmark during determination of the vertical dimensions of occlusion. For achievement of optimal results, this method could be used along with other clinical methods.

Determination of intercanine distance from a proportion calculation: A pilot clinical study

Statement of problemThe optimal positioning of artificial teeth is essential for long-term success when providing removable complete dentures. However, information about the original tooth positions may be lacking, especially the canine teeth, which play a key role in the tooth arrangement. PurposeThe purpose of this pilot clinical study was to determine if the mandibular and maxillary canine position can be determined by proportioning to certain anatomic landmarks. Material and methodsFifty participants (32 women, 18 men) with a mean age of 19.18 years and Angle class 1 and 2 malocclusions were selected randomly from patients who had completed their orthodontic treatment. The distance between the sagittally bisecting lines of the maxillary tuberosity and the distance between the sagittal bisecting lines of the retromolar pads in the mandible were measured from casts and recorded in millimeters. The expected distances between the canine cusps in both jaws (e-DCCmand and e-DCCmax) were calculated, and the distance between the canine cusps in both jaws (DCCmand and DCCmax) was measured. The measurements and the calculated expectation values for maxillary and mandibular cast models were recorded and statistically compared. ResultsThe mean ±standard deviation value of the maxillary intercanine distance was 35.5 ±1.4 mm, and the calculated value was 35.52 ±1.43 mm. In the mandible, the mean ±standard deviation value of the mandibular intercanine distance was 26.73 ±1.25 mm, and the calculated value was 26.69 ±1.33 mm. The difference between the means of expected DDC for the maxilla and mandible was within the equivalence interval (P<.001). ConclusionsThe proportions evaluated were determined to provide accurate canine positions and should be suitable for use in the treatment of edentulous patients.

A multidisciplinary approach to the comprehensive treatment of edentulous patients with perioral wrinkles of the skin

Objectives: The purpose of this study was to comparative analysis of effectiveness of isolated use of intradermal injections of modified hyaluronic acid (Hyalorepair 04) and its combination with platelet-rich autologous plasma in edentulous patients with perioral wrinkles of the skin. Materials and Methods: A total of 56 patient’s presence of perioral wrinkles of the skin participate in the study after dental implant prosthodontics rehabilitations. They were randomly divided into 2 groups in accordance with the applied therapy method (29 isolated implementation of modified hyaluronic acid bio-repairing and 27 bio-repairing combined with autologic plasmotherapy). Treatment included implant-prosthetic rehabilitation followed by hyaluronic acid injections in order to correct cheek-zygomatic sulcus, nasolabial folds and marionette wrinkles. Preparations in an amount from 1 to 4 ml were injected into the face area, depending on the zones to be corrected at the request of the patients. Results: The complex treatment restored the aesthetic profile of the face and oral cavity and increased the effectiveness of the chewing function. On M03 and M05, 2/3 of patients had significant improvement as assessed by physician and patient according to GAIS. Most of the patients also showed significant improvement at visit M12. Conclusion: A multidisciplinary approach to the treatment of edentulous patients with perioral wrinkles increases the functional and aesthetic effect of the treatment. Implementation of combined of bioreparation and autologous plasmotherapy is significantly more effective comparatively to the isolated implementation of modified hyaluronic acid. Effect of Osteoporosis on Well-Integrated Bone Implants

The installation of dental implants has become a common treatment for edentulous patients. However, concern exists about the influence of osteoporosis on the final implant success. This study evaluated whether an ovariectomy (OVX)-induced osteoporotic condition, induced eight weeks postimplantation in a rat femoral condyle, influences the bone response to already-integrated implants. The implants were inserted in the femoral condyle of 16 female Wistar rats. Eight weeks postimplantation, rats were randomly ovariectomized (OVX) or sham-operated (SHAM). Fourteen weeks later, animals were sacrificed, and implants were used for histological and histomorphometric analyses. A significant reduction in the quantity and quality of trabecular bone around dental implants existed in OVX rats in comparison to the SHAM group. For histomorphometric analysis, the bone area (BA%) showed a significant difference between OVX (34.2 ± 4.3) and SHAM (52.6 ± 12.7) groups (p &lt; 0.05). Bone–implant contact (BIC%) revealed significantly lower values for all implants in OVX (42.5 ± 20.4) versus SHAM (59.0 ± 19.0) rats. Therefore, induction of an osteoporotic condition eight weeks postimplantation in a rat model negatively affects the amount of bone present in close vicinity to bone implants.

Prosthodontics is concerned with the impact of tooth or tissue damage and partial or complete loss of teethon oral function in its broadest sense. It deals with this largely through prosthetic replacement. The disciplineoccupies a major portion of a dental school curriculum, and dental practitioners usually devote much of theirpractice to prosthodontic services. It is continuously evolving consequent to the rapid advancements in dentalbiomaterials science, clinical and laboratory techniques and technologies, education, research, therapeutics,literature, and interdisciplinary developments. While the basic process of making dentures has changed littleover the past several decades, new materials and techniques can help laboratories and clinicians providefunctional, esthetic restorations that offer exceptional value to patients. Implant treatment is a tremendousadjunct to removable prosthodontics in the treatment of edentulous patients, but it is not within the financialreach of all dental patients.In future, prosthodontics has a very good scope and it is going to be beneficial forthe patients who are edentulous.Technology developments are very much needed in future for its very wellestablishments .This review is aimed to discover some fascinating facts about the trends that are prevailingin prosthodontics and its future scope.

Comparison of chewing activity in patients requiring complete denture with two different occlusions

Background and Objectives: The loss of natural teeth causes many problems; one of major problems that are associated with edentulousness is the hypotonicity of muscles of mastication and decreasing the ability of the patient to chew food properly. Treatment of edentulous patients with complete denture will increase the muscle tonicity and chewing activity. The purpose of this study is to clinically compare patient's chewing ability between lingualized occlusion complete denture and bilaterally balanced occlusion complete dentures. Materials and methods: Two sets of complete dentures with different occlusions were fabricated for ten patients. The patients received bilaterally balanced occlusion (BBO) complete denture and lingualized occlusion (LO) complete denture in random order. The patients wear each set of complete denture for one month. During this month, chewing activity tests were performed for the patients in four different time intervals. The results of these tests were analyzed using t-test and paired t-test to compare between the lingualized occlusion and bilaterally balanced occlusion complete denture and also to compare the results of each time interval with each other. Results: The mean amount of the walnuts that passed through the sieve in LO dentures were (1.78g), during the fourth visit while it was (1.18g) in BBO. There were statistically significant difference between LO and BBO schemes of complete denture when measuring the amount of the chewed walnuts that passed through the sieve in second, third and fourth visit (p=0.037, 0.001 and 0.000). Conclusion: Within the limitations of this study, it has beenconcluded that the chewing ability was higher in patients who were provided with complete dentures fabricated with the LO scheme as compared to those patients who received complete dentures made with BBO scheme. This was due to the fact that LO technique resulted in functionally better complete dentures as compared to the ones made by BBO technique

Unconventional Dentures: A Manifestation of New Vision in Prosthesis Construction

Removable prosthesis, that is, complete and/or partial dentures cannot function as a substitute for natural dentition, but they have been and will remain the staple treatment for edentulous patients. An edentulous patient is found to be satisfied if prosthodontic rehabilitation restores his/her function with superior esthetics. However, not every case of edentulism, either complete or partial, can be treated with conventional methods of denture fabrication. When a patient is clinically presented with complications such as severely resorbed ridge, microstomia, hyperplastic and/or flabby tissue, xerostomia, osteoma, bruxism, and labially inclined premaxilla, then it becomes difficult for the dentist to fulfill the patient’s desire for both esthetics and function. There is a need for slight modification in the regular clinical procedures by altering the impression technique or the design of the prosthesis so as to achieve best results in compromised cases, commonly known as unconventional approaches. This article intends to highlight the unconventional approaches for the fabrication of complete denture prosthesis that can be kept in mind when such cases are encountered in our daily practice.

RESULTS OF TREATMENT OF EDENTULOUS PATIENTS WITH DENTURES MADE OF «FTORAX»

Background. The problem of providing high-quality dental care to edentulous patients remains one of the most important and unresolved to this day in the clinic of prosthodontics. The causes of complete loss of teeth can be both periodontal tissue diseases and diseases of hard tooth tissues of carious and non-carious origin, trauma and the like. It becomes necessary to fabricate complete removable dentures to prevent the occurrence of pathologies in such situations. Complete secondary adentia leads to a large number of local and general complications. The chewing apparatus undergoes a number of functional and morphological changes. Therefore, the complete absence of teeth sets the dentist the task of restoring the functions of full chewing, the appearance of the patient, and speech. The complexity of high-quality prosthetics for edentulous patients is also because the clinical characteristics of supporting tissues are diverse, constantly changing, so a personal approach to creating stable denture is needed. It should be added that the fabrication of high-quality complete removable dentures largely depends on the properties of the base material. The main group of materials for the fabrication of such dentures is acrylic plastics.&#x0D; The aim. The work is focused on the study of the functional state of the masticatory muscles and the restoration of masticatory effectiveness in edentulous patients, for whom dentures were made in the prosthodontics clinic from the acrylic base material «Ftorax».&#x0D; Material and methods. Hot curing plastic based on fluorine-containing acrylic copolymers «Ftorax» was used in the work. Our evaluation of the quality of all constructions was carried out using the «BOFSAS» test, determining the biopotentials of the masticatory muscles using electromyography, and determining the masticatory effectiveness according to I.S. Rubinov.&#x0D; Results. Based on our work, it can be noted that the use of the basic acrylic material «Ftorax» allows us to achieve good fixation and stabilization of complete removable dentures, which is subjectively confirmed by the «BOFSAS» test, objectively – by the electromyographic studies and time indices during chewing test according to I.S. Rubinov.&#x0D; The amplitude during volitional compression is 556.90±8.72 μV, which approaches the norm (641.58±10.01 μV), and almost four times higher than before prosthetics (188.11±8,13 μV) after 1 year of constructions using. A similar pattern is observed with arbitrary chewing: the amplitude (547.32±8.43 μV) approaches normal (643.92±9.11 μV) compared with the results before prosthetics (201.40±9.39 μV). As for the coefficient «K», its value decreased almost twofold compared with the results before prosthetics (2.44±0.14) and equals to 1.25±0.03, which is significantly closer to normal (1,02±0.01).&#x0D; After the test by Rubinov the following results were obtained: the average time that patients with intact dentitions spent chewing on the stimulus was approximately 12.97±0.13 seconds and the average time for patients before prosthetics (with old dentures) was approximately 45.16±0,41 sec.&#x0D; The time that patients spent chewing a nut after 1 month was 28.97±0.42 seconds, after 6 months – 26.94±0.44 seconds, and after 1 year of using the entures – 25.48±0.45 sec.&#x0D; Based on this, it can be summarized that adaptation to such dentures takes place at the appropriate time with minor corrections.&#x0D; Conclusion. The results of our clinical study indicate the feasibility of using «Ftorax» in prosthodontics clinic for treatment of edentulous patients.

Oral rehabilitation with dental implants in patients with recessive dystrophic epidermolysis bullosa: A retrospective study with 2-15 years of follow-up

Background Epidermolysis bullosa (EB) comprises a group of hereditary disorders characterized by mechanical fragility of the skin and mucous membranes, with the development of blisters and vesicles in response to minimum tissue friction. Recessive dystrophic epidermolysis bullosa (RDEB) with generalized involvement is the most common subtype in the oral cavity. The present study was carried out to investigate dental implant survival, peri-implant tissue condition, patient satisfaction, and the impact of treatment upon the quality of life of patients with RDEB rehabilitated with implants and full-arch implant-supported prostheses. Material and Methods Thirteen patients with RDEB underwent dental implant treatment between September 2005 and December 2016. A retrospective study was made to analyze implant survival, peri-implant tissue health and patient satisfaction. Results A total of 80 implants were placed (42 in the maxilla and 38 in the mandible) in 13 patients between 20-52 years of age and diagnosed with RDEB. All the implants were rehabilitated on a deferred basis with 20 full-arch prostheses. Fifteen fixed prostheses and 5 implant-supported overdentures were placed. The implant survival rate was 97.5% after a mean follow-up of 7.5 years after prosthetic loading. Fifty percent of the implants showed mucositis at the time of evaluation. Probing depth was maintained at 1-3 mm in 96.2% of the implants, and bleeding upon probing was observed in 67.5% of the implants. There was a high prevalence of bacterial plaque (85%). Conclusions The treatment of edentulous patients with RDEB by means of implants and implant-supported prostheses is predictable as evidenced by the high success rate, and improves patient self-esteem and quality of life. Clinical Remount in Complete Denture

Proper occlusal relationships are a part of successful prosthetic treatment for edentulous patients. Fabrication of complete dentures involves clinical and laboratory procedures that should be performed precisely for achieving success with fabricated dentures. These occlusal errors can be corrected, (1) in a patient's mouth or (2) by recording new centric relation and remounting dentures on an articulator. The second method is more viable because the denture base moves over the oral mucosa which is unable to identify premature contacts in centric occlusion and tooth guided eccentric excursions. This article depicts the modest and effective clinical chairside remounting procedure.

Full-Arch Implant-Supported Fixed Dental Prosthesis Retained by a Combination of Galvano-Telescopic Copings and Screws: A Clinical Report.

Full-arch screw-retained implant-supported fixed dental prostheses have a high long-term success rate and are considered the gold standard by many clinicians. However, accurate fabrication of a passive fit long-span prosthesis can be challenging. A novel intraoral adhesion method using galvano-telescopic copings was proposed as a way of improving prosthetic fit for edentulous patients. This report describes the treatment of a 74-year-old female with a full-arch implant-supported dental prosthesis, supported by a combination of galvano-telescopic copings and screws to prevent retention loss. Four years have passed since this superstructure was placed, during this time she exhibited a good clinical course with no inflammation noted in surrounding tissues. Treatment with an implant-supported fixed dental prosthesis, retained by a combination of galvano-telescopic copings and screws, can be a useful alternative treatment for edentulous patients.

Surface roughness of biomaterials and process parameters of titanium dioxide gritblasting for productivity enhancement

Titanium is used in dental implants because of its properties as a biomaterial. Surface roughness is very important and fundamental for obtaining required results of biocompatibility. Dental implants with moderately rough surfaces are commonly used in the treatment of edentulous patients. Aiming for improvements in productivity, experiments were conducted in a dental implant industry to evaluate the influence of machining parameters on the surface of dental implants. Samples of different diameters were machined with specific parameters and their roughnesses were measured, before and after blasting, with and without asepsis chemical attack. The results indicate a random increase in roughness according to increase of feed parameter, evidencing that there is no relation between roughness and machining feed after gritblasting. Samples exposed to gritblasting that did not undergo chemical asepsis showed a decrease in roughness, compared to roughness of blasted samples and submitted to asepsis process. However, this trend remained constant leading to the conclusion that titanium-blasting process makes the surface roughness uniform, independently of first roughness variation. No relationship was observed between the variation of feed and the variation of roughness. Thus, in terms of productivity, cutting feed was increased with no harm to the biocompatibility. After more than twenty years of its first use in the industry, this study proves the possibility of changes in the process parameters of titanium dioxide gritblasting to increase productivity in the manufacture of dental implants.

Bone response around titanium alloy implants in osteoporosis

There has been a concerted effort among material scientists and clinicians worldwide to improve the performance of dental implants, with the aim of accelerating and maintaining their integration into hard and soft tissues and/or extend their range of application. The use of osseointegrated implants in the treatment of edentulous patients has become a common alternative to conventional prosthetic dentistry. The success of osseointegration depends in part on the state of the host bone and its healing capacity, and concerns have been raised about various conditions affecting its quality and quantity. Osteoporosis, which is a major health problem for the elderly population, is a reduction in the volume of bony tissue relative to the entire bone volume. The underlying clinical problem is a deficiency of bone mass. Because age and sex are reported to be important risk factors for osteoporosis, the rate of implant loss caused by the failure of osseointegration may also be expected to increase correspondingly. Thus, a large proportion of the target population for dental implants may be at high risk of implant failure. If so, the choice between conventional treatment and implant-supported prostheses in the prosthodontic treatment of older patients would need to be reassessed.
