The high prevalence of inflammatory processes in the periodontitis of children, the ineffective effectiveness of preventive and curative measures can contribute to the development of generalized periodontitis, therefore it remains urgent to develop pathogenetic effects in the treatment of chronic catarrhal gingivitis.The aim of the study – a comparative assessment of the traditional treatment of chronic catarrhal gingivitis in children aged 6–7 years with therapy using an anti-inflammatory gel with neovitin.Materials and Methods. 61 children aged 6–7 years, suffering from chronic catarrhal gingivitis, were taken under clinical supervision and divided into groups: the main (30 children) and the comparative (31 children). All children underwent conventional therapy in accordance with the protocols for the provision of medical care – basic therapy. In the main group, for a month, pathogenetic agents were used: an anti-inflammatory gel with neovitin, hygiene products were recommended. In the comparative group, 3 % hydrogen peroxide solution was used for local therapy, for rinsing of the mouth (during the first week of treatment) – Rotokan, a therapeutic and prophylactic toothpaste.Examination and control examinations were carried out with the hygienic state of the mouth determined by the hygienic index of Yu. A. Fedorov and V. V. Volodkina, prevalence and intensity of the inflammatory process in the gums with the help of the Schiller-Pisarev test and the PMA index, oral fluid index (lysozyme content, formation of oxydradicals, urease activity, degree of dysbiosis, pH and viscosity).Results and Discussion. After the treatment, elimination of gum inflammation in patients of the main group was accompanied by a decrease in the PMA index by 3.8 times; formation of oxyradicals decreased in children of the main group by 6.35 %; in the comparative group – by 6.15 %. Urease activity decreased in children of the main group by 16.37 % (p<0.05), the viscosity of the oral fluid of children decreased in 2.65 times in the main group and in 2.13 times in the comparative group (p<0.05). The lysozyme content increased in children of the main group by 15.61 % (p<0.05) in the comparison group – by 9.63% (p<0.05). The treatment after 0.5 years caused an increase in the lysozyme content in the oral fluid of the children of the main group to the level of the control group. The degree of dysbiosis decreased in the main group, in the comparison group tended to increase.Conclusions. The use of the proposed therapy with gel with neovitin contributed to the best results of treatment: reducing the degree of inflammation of the gums, the number of visits to the doctor, improving the indices of nonspecific protection in the oral fluid of 6-7 years-old children and, to a greater extent, six months after the treatment.

Prevention and treatment of chronic catarrhal gingivitis and gingival recession in patients with non-removable dental prosthesis designs

Schemes of prevention and treatment of chronic catarrhal gingivitis and gingival recession in patients with non-removable dental prosthesis designs. It is proved their clinical effectiveness and evaluation criteria of developed gum recession land. After application of preventive and therapeutic complexes, which observed in patients, improve the hygienic condition of the mouth, confirming the feasibility of their application.Methods. It is included 153 patients with various defects of individual teeth and dentition, including 85 women (55.6%) and 68 men (44.4%) aged from 25 to 55 years. The control group consisted of 35 healthy individuals of the same age without metallic inclusions in the mouth.Results. In analyzing the evaluation index hygienic condition of the oral cavity in patients 25-34 years of gingivitis against the backdrop of orthopedic treatment non-removable denture constructions the indices Green-Vermillion and Silness-Loe following data were obtained: patients of the main group before treatment, the rate Green Vermillion index was an average of 1,70 ± 0,07 points to 1,75 ± 0,08 points, indicating a satisfactory level of oral hygiene. After application of preventive and therapeutic complexes improve the hygienic condition of the oral cavity, which marked the studied index indicators hygiene.Analyzing the data should be noted that positive changes of hygienic condition of the oral cavity were observed throughout the study period in all age groups of patients in both subgroups of observation and comparison.Conclusions. We offer health care systems for the prevention and treatment of gum recession and chronic catarrhal gingivitis in patients after dental prosthetics of non-replaceable constructs allowed to suspend pathological processes in the gums and improve oral hygiene

Improving medical care in patients with chronic catarrhal gingivitis while using fixed orthodontic appliances

Aim. To study the immunostimulating effect of glucosaminyl muramyl dipeptide in the treatment of chronic catarrhal gingivitis in patients undergoing orthodontic treatment using fixed appliances. Methods. The data of clinical and laboratory examination of the oral health status in 54 patients aged 14-24 years with inflammatory periodontal diseases, undergoing orthodontic treatment using fixed appliances were analyzed. The first group included 26 patients in whom glucosaminyl muramyl dipeptide (1 mg sublingual tablets daily for 10 days) was additionally administered. The second group included 28 patients receiving standard treatment. The treatment efficacy estimation was based on subjective assessment, clinical, microbiological and immunological examinations. Results. After 6 weeks of treatment, there was a statistically significant reduction of simplified oral hygiene index (OHI-s) to 0.85±0.03 in patients of the first group, compared to 1.64±0,061 before treatment. Periodontal index (PI) and PMA index values were 0, reflecting good level of hygiene and no inflammation in periodontal tissues. In the second group, the OHI-s was 1.5±0.06, PI 0.71±0.04 and PMA 18±1.046%, reflecting mild gingivitis after the 6 weeks of treatment. Bacteriology showed that the number of colonies in the first group was reduced by 1-2 orders of magnitude on average after 6 weeks of treatment. Patients of the second group showed a significant (p 0.05) increase in the number of bacterial colonies potentially causing the periodontal inflammation. The level of secretory IgA after 6 weeks of combined treatment in the first patients group was 249.5±39.1 mg/ml, whereas in the second patients group it was 182.2±14.9 mg/ml, indicating the immunostimulating activity of the complex treatment administered in patients of the first group. Conclusion. This study shows the effectiveness of glucosaminyl muramyl dipeptide as immunostumulating drug in combined treatment of chronic catarrhal gingivitis in patients with dental braces.
