Implementing pharmacogenetic testing to optimize proton-pump inhibitors use among Indian population based on CPIC-CYP2C19-PPI dosing guidelines: The need of the hour.

Proton-pump inhibitors (PPIs) are widely prescribed to decrease stomach acid and treat various acid-related Gastrointestinal tract (GIT) diseases. However, genetic variations, particularly in the CYP2C19 gene, affect PPIs metabolism and efficacy. Variants in CYP2C19 can result in different rates of PPI metabolism, influencing their effectiveness. Personalized medicine strategies, such as genotyping for CYP2C19, have the potential to enhance the effectiveness of PPI therapy and patient safety. This review aims to describe the relevance of CYP2C19 genetic profiling in the indian population, including normal function (e.g. CYP2C19*1, *11, *13, *15, *18, *28, and 38), decreased function (e.g., CYP2C19*9, *10, *16, *19, *25, and 26), loss of function (e.g., CYP2C19*2, *3, *4, *5, *6, *7, *8, *22, *24, *35, *36, and *37), and increased function (e.g., CYP2C19*17) variants. This review also examines the clinical pharmacogenomics implementation consortium (CPIC)-CYP2C19-PPI guidelines to highlight the importance of pharmacogenomics (PGx)-informed personalized PPI therapy for gastroesophageal reflux disease and peptic ulcer disease treatment. On average, each person in India possesses eight pharmacogenetic (PGx) variants that can be clinically significant, underscoring the need for preemptive testing. Implementing CYP2C19 genetic testing in India requires expanding laboratory capacity, increasing accessibility in primary care, increasing public awareness, collaboration between pharmacovigilance and PGx programs, investing in advanced sequencing technologies, data management systems, and integration with electronic health records and clinical decision support systems. Addressing challenges such as genetic diversity, socioeconomic factors, health-care access issues, and shortage of trained professionals is essential for implementation. Due to the lack of definitive country-specific policies and PGx guidelines from Indian drug regulatory agencies, guidelines from international consortia such as the Clinical Pharmacogenetics Implementation Consortium and drug labeling offer crucial foundational evidence. This evidence can be used to enhance patient outcomes and ensure the safe and effective use of PPIs in India.

Surgical intervention remains vital, particularly in cases of perforated peptic ulcers, despite a shift towards conservative approaches. Understanding the impact of H. pylori eradication on surgical outcomes is crucial for optimizing PUD management. This systematic review was conducted to assess how H. pylori eradication treatment impacts surgical results in patients with PUD. The criteria for inclusion involved research studies on individuals aged 18 years and older with a diagnosis of PUD that necessitated surgical treatment. Important results comprised eradication rates, complications after surgery, recurrence rates, and overall outcomes for patients. Two researchers independently screened and extracted data from electronic databases using a thorough search strategy. The evaluation of quality employed standardized instruments for randomized controlled trials and cohort studies. Nine research projects met the requirements for inclusion, offering information on the effectiveness of H. pylori elimination treatment on surgical results. Different rates of eradication were noted, with a notable number of patients experiencing postoperative complications. Recurrence of ulcers was a concern, despite treatment, emphasizing the complexity of PUD management. Studies have shown that eradication therapy is effective in certain patient groups, like young men with perforated peptic ulcers. A Comprehensive Review on Peptic Ulcer

Abstract: Up to 10% people in the world are affected by a chronic condition known as peptic ulcer. Peptic ulcer development is influenced by the pH of gastric juice and a decline in mucosal defenses. Protonpump inhibitors, histamine (H2) receptor antagonists, prostaglandin analogues and sucralfate have all been used to treat peptic ulcer disease. There has been a continuing search for an appropriate, palliative, and curative agent for the treatment of peptic ulcer disease using natural materials of plant and animal origin due to the complexity, cost, and toxicity of these medications. The goal of this review was to examine medicinal plants, phytochemicals, pathways, and research models that have been applied to the treatment of PUD in order to assess the potential contribution of natural substances to the development of herbal treatments for PUD. A literature search was used to find information utilising electronic databases such as Web of Science, Google Scholar, PubMed, Sci Finder, Reaxys, and Cochrane.

The first successes in the treatment of peptic ulcer disease (PU) using antibiotics in combination with proton pump blockers contributed to the emergence of the hypothesis that H. pylori is the main cause of PU. Moreover, it was assumed that eradication of H. pylori could lead to elimination of ulcerative disease. However, 40 years after the discovery of H. pylori, it became clear that eradication therapy reduces the time to ulcer healing, improves the quality of life and prolongs remission, but does not eliminate PU. Nevertheless, the new Maastricht VI guidelines still proclaim H. pylori eradication as the main goal of therapy for H. pylori-associated diseases. The widespread use of antibiotics has saved millions of lives. At the same time, bacterial resistance to existing antibiotics due to the ongoing escalation of antibacterial therapy has been named by the World Health Organization as one of the main threats to humanity in this century. Therefore, the infectious concept of the pathogenesis of PU and the principles of its treatment require clarification.

Assessment of the pharmaceutical quality of various brands of omeprazole capsules

Introduction: Omeprazole is a first-line therapy drug for the treatment of peptic ulcer disease. The availability of numerous generic brands in the market raises doubts about their quality. Purpose: This study aimed to assess the pharmaceutical quality of various brands of omeprazole capsules marketed in three states of southern Nigeria. Methods: Organoleptic and physicochemical properties of ten (10) generic brands of omeprazole capsules were assessed according to official and unofficial standards. Quality assurance tests such as weight uniformity, disintegration time and drug content assay were carried out. The dissolution test was done in a double dissolution medium consisting of 0.1 N HCl for 1.0 h and phosphate buffer pH 6.8 for 2.0 h. Results: Capsules tested were suitably packaged with shells containing evenly coloured white pellets. All capsules were slightly soluble in water, sparingly soluble in ethanol and readily dissolved in alkaline medium. The 10 brands tested met the British Pharmacopeia (BP) specification for weight uniformity. All capsules had drug content within 90 - 110% and an average disintegration time of less than 15 min. All brands retained the drug in acid medium for 1 hour but released more than 75% within 40 min in phosphate buffer pH 6.8. Conclusion: Brands of omeprazole capsules passed Pharmacopeia tests, exhibited similar in vitro drug release profiles and may therefore be used interchangeably for the treatment of peptic ulcer. The study also showed that almost all brands of omeprazole capsules available in Nigeria are imported, hence the need arises for regular post-clinical analysis of imported drugs. The Interplay between Helicobacter pylori and Gut Microbiota in Non-Gastrointestinal Disorders: A Special Focus on Atherosclerosis.

The discovery of Helicobacter pylori (H. pylori) in the early 1980s by Nobel Prize winners in medicine Robin Warren and Barry Marshall led to a revolution in physiopathology and consequently in the treatment of peptic ulcer disease. Subsequently, H. pylori has also been linked to non-gastrointestinal diseases, such as autoimmune thrombocytopenia, acne rosacea, and Raynaud's syndrome. In addition, several studies have shown an association with cardiovascular disease and atherosclerosis. Our narrative review aims to investigate the connection between H. pylori infection, gut microbiota, and extra-gastric diseases, with a particular emphasis on atherosclerosis. We conducted an extensive search on PubMed, Google Scholar, and Scopus, using the keywords "H. pylori", "dysbiosis", "microbiota", "atherosclerosis", "cardiovascular disease" in the last ten years. Atherosclerosis is a complex condition in which the arteries thicken or harden due to plaque deposits in the inner lining of an artery and is associated with several cardiovascular diseases. Recent research has highlighted the role of the microbiota in the pathogenesis of this group of diseases. H. pylori is able to both directly influence the onset of atherosclerosis and negatively modulate the microbiota. H. pylori is an important factor in promoting atherosclerosis. Progress is being made in understanding the underlying mechanisms, which could open the way to interesting new therapeutic perspectives.

Systemic validation of Herbo- mineral formulation Udara Noi Nivarana Thiravagam by instrumental analysis

Siddha medicine is a complementary system of medicine, drawing global attention in recent times since it contains many formulations, able to treat and cure many diseases even degenerative disorders with no adverse or side effects. It comprises thirty-two types of internal formulations with distinct preparation techniques mentioned in various classical Siddha texts. These traditional medicines require newer standardization techniques to prove their safety and efficacy. Udara Noi Nivarana Thiravagam is a Siddha herbo-mineral formulation used for the treatment of peptic ulcer disease (PUD). This work was proposed to formulate and standardize Udara Noi Nivarana Thiravagam as per the guidelines set forth by PLIM. Udara Noi Nivarana Thiravagam was standardized by various parameters like FT-IR, ICP-OES and GC-MS. All parameters showed desirable results concerning the standard limits. The set parameters were sufficient to evaluate Udara Noi Nivarana Thiravagam and this could be used as reference standards for the quality control and quality assurance of the drug.

Pharmacological Investigation on Unraveling Mechanism of Action of Quisqualis indica Leaves for Predicted Treatment of Peptic Ulcer Disease

Background:: With the aid of various ulcer-induced models, the goal of this study was to assess the antiulcer ability of Qusqualis indica (Q. indica) leaf extracts in Wistar rats Method:: The induction of ulcers was done by different models like pylorus ligation method, ethanol-induced and stress-induced models. Group 1 (negative control), Group 2 (standard group) were treated with Sucralfate (8.6 mg/kg), Group 3 was treated with aqueous extract of Q. indica (AEQI,200 mg/kg), Group 4 was treated with aqueous extract of Q. indica (AEQI,400 mg/kg), Group 5 was treated with ethanolic extract of Q. indica (EEQI, 200 mg/kg) and Group 6 was treated with ethanol extract of Q. indica (EEQI, 400 mg/kg). All therapies were given orally twice every day. After the course of treatment was complete, blood and gastrointestinal contents were taken, and biochemical tests were run. The acetylcholine and histamine drug response curves were used to develop the mechanism of the extracts. Results:: The groups treated with extracts experienced a significant decrease in ulcer index. The antiulcer potential of the AEQI and EEQI is dose-dependent. Hematological, hepatic, and cardiac parameters were not significantly affected by the extracts, although high-density lipoprotein production was. Acetylcholine and histamine are blocked by AEQI and EEQI, according to the DRC analysis. The obtained scientific facts are amply supported by histopathological analysis Conclusion:: AEQI and EEQI have antiulcer potential in a dose-dependent manner, but further research must be needed.

Comparing the Safety and Efficacy of Proton Pump Inhibitors and Histamine-2 Receptor Antagonists in the Management of Patients With Peptic Ulcer Disease: A Systematic Review.

Peptic ulcer disease (PUD) refers to the occurrence of an open erosion in the inner lining of the stomach, duodenum, or sometimes lower esophagus. Treatments like proton pump inhibitors (PPIs) or histamine 2 receptor antagonists (H2RAs) are available on the market to efficiently treat the break in the mucosal lining. However, there is little evidence about the effects of the medication on the type and location of the ulcer and the epigastric pain caused by disintegration and increased acidity in the stomach. Given the above, we conducted a systematic review comparing the safety and efficacy of PPIs and H2RAs in various ulcer locations (gastric, duodenal, and pre-pyloric) and the effect of prolonging the treatment with the same medication or changing into a drug from another class in treatment-resistant ulcers. We employed major research literature databases and search engines such as PubMed, Medical Literature Analysis and Retrieval System Online (MEDLINE), Science Direct, and Google Scholar to find relevant articles. After a thorough screening, a quality check using various tools, and applying filters that suited our eligibility criteria, we identified eight articles, of which five were random clinical trials (RCTs), two review articles, and one meta-analysis. This study compares the different side effects of PPIs and H2RAs. Most studies concluded that omeprazole is superior in healing ulcers and bringing pain relief and that patients resistant to H2RAs can be treated better when switched to a PPI. This study also discusses the adverse effects of chronic use, such as diarrhea, constipation, headaches, and gastrointestinal infections. Patients on long-term PPI therapy are required to take calcium supplements to prevent the risk of fractures in older adults. Regarding long-term outcomes, PPIs remain the mainstay of treatment for peptic ulcer disease, based on the papers we reviewed.

Modern approaches to the treatment of peptic ulcer disease and prospects for the use of biological therapy

Digestive diseases are the sixth leading cause of death in the world and accounted for more than 2.5 million deaths in 2019. In Ukraine, mortality from diseases of the digestive system is recorded as one of the highest in the world, and the leading place in the general structure of diseases of the digestive system is occupied by peptic ulcer disease (РUD).
 The aim of the study. Summarize current information on the etiology and the pathogenesis of РUD and characterize modern approaches to the treatment of patients with РUD and the prospects of biological therapy.
 Materials and methods. Publications were selected based on the PubMed, Clinical Key Elsevier, Cochrane Library, eBook Business Collection, and Google Scholar databases, which covered information on the etiology, pathogenesis, and approaches to the treatment of РUD.
 Results. Standard first-line anti-Helicobacter therapy consists of a proton pump inhibitor and two antibiotics, such as clarithromycin and amoxicillin or metronidazole. A promising direction in the treatment of РUD is the use of biological therapy. According to literature sources, the prophylactic use of cryoextract of the placenta in indomethacin gastric lesions has an anti-ulcer effect at the level of 69.1 % and 92.1 % in diclofenac sodium gastric lesions. In addition, the specified cryoextract is capable of leveling the gastrotoxic effect of acetylsalicylic acid, ibuprofen, stress factor and chemical ulcerogens.
 Conclusions. Today, there is a great need for cell therapy that could be put into practice in clinically relevant volumes. The most promising directions of biological therapy in gastroenterology are considered to be the use of mesenchymal stem cells and agents obtained from the fetoplacental complex.

PDE5 inhibitors and gastric mucosa: implications for the management of peptic ulcer disease.

Peptic ulcer disease (PUD) continues to be a cause of significant morbidity and mortality worldwide. Almost two-thirds of PUD cases are asymptomatic. In symptomatic patients, epigastric pain is the most common presenting symptom of PUD, which is manifested by nausea, abdominal fullness, bloating, and dyspepsia. Most PUD cases are associated with the use of COX inhibitors or Helicobacter pylori infection, or both. The traditional management of PUD includes the use of proton pump inhibitors to reduce the gastric acid secretion and antibacterial drugs to combat H. pylori. Timely diagnosis and treatment of PUD are vital to reduce the risk of associated morbidity and mortality, as is prevention of PUD among patients at high risk, including COX inhibitors users and those infected with H. pylori. PDE5 inhibitors have been used for the management of erectile dysfunction and pulmonary hypertension for decades. In recent years, studies have mentioned tremendous pleiotropic effects of PDE5 inhibitors on gastrointestinal, urogenital, musculoskeletal, reproductive, cutaneous, and neurologic disorders. Recent data shows that PDE5 inhibition augments gastric mucosa protection, and here, we review the most recent findings regarding the use of PDE5 inhibitors for the prevention and management of PUD.

Anti-inflammatory effect of hyperbaric oxygen in the treatment of peptic ulcer disease: review

OBJECTIVE: Based on the analysis of literature data, highlight the mechanisms of anti-inflammatory action of hyperbaric oxygenation (HBO) in peptic ulcer of the stomach and duodenum (PUS and D).MATERIALS AND METHODS: The subject of study was scientific publications on the use of hyperbaric oxygenation (GBT) in treatment (PUS and D). The search was carried out on the international databases Elibrary, Pub Med, Scopus, Google Scholar from 1980–2022. A total of 38 literature sources were selected, of which at least 60% were published within the last five years. RESULTS: The effectiveness of the therapeutic effect of HBO (ulcer healing rate) in PUS and D depends on the location and size of the ulcer defect on the wall of the stomach or duodenum. At the moment, there is no conclusive data on the optimal number of HBO sessions required to achieve the maximum therapeutic effect in the treatment of PUS and D. DISCUSSION: The leading link in hyperoxic sanogenesis in this pathology is the anti-inflammatory effect of hyperbaric oxygen (HBO2), which is realized by: a) through stimulation of mucocyte formation; b) through regulation of HBO2 qualitative composition of mucocyte secretion; c) by stimulating the formation of bicarbonates by additional cells and increasing the sensitivity of these cells to their endogenous stimulant -solar acid (HCl); d) through the effect of HBO2 on gastrin formation (HCl stimulant), as well as on the diffusion of HCl from the gastric lumen into its capillary network. Together, this leads to a rapid cessation of pain syndrome, which creates conditions for the elimination of dyspeptic syndrome. This is realized through the restoration of the myoelectric activity of the stomach wall and duodenum, the regulation of the myogenic tone of their walls, as well as the elimination of the violation of gastric accommodation in the conditions of HBO. CONCLUSION: The anti-inflammatory effect of HBO2 in the treatment of PUS and D allows us to consider hyperbaric oxygenation as an integral part of the treatment of ulcerative lesion of the upper gastrointestinal tract.

The efficacy of triple and quadruple therapy for thetreatment of peptic ulcer disease

This study was designed to determine the efficacy of triple and quadruple therapy for the treatment of peptic ulcer disease. Seventy five patients underwent Helicobacter Pylori associated peptic ulcer ,as determined by endoscopy and rapid urease test of biopsy were paiiicipated in this study. They were divided to 4 groups. Three of them were treated with triple therapy. The triple therapy in the first group consisted of omprazole 20 mg BID + amoxicillin 500 mg QID + metronidazole 500 mg TID . For the second group , only omprazole was replaced by ranitidine 150 mg BID , while for the third group metronidazole was replaced by clarithromycin 500mg BID . All triple therapies were given for 2 weeks. However , for the fourth group , a quadruple therapy consisted of omprazole 20mg BID + amoxicillin 500mg QID + clarithromycin 500mg BID + metronidazole 500mg TID , was given for one week. The efficacy in the four groups, was 91.3, 88.89, 90, and 100% respectively. All treatmentregimens were well tolerated and no sever side effects necessitated cessation of the treatment.

Peptic Ulcer Disease and Its Implications

Peptic ulcer disease (PUD) is one of the commonest diseases seen throughout the world. Peptic ulcers are open sores that develop on the inside lining of your stomach and the upper portion of your small intestine. The most common symptom of a peptic ulcer is stomach pain. There are various risk factors for the development of peptic ulcer disease, but the most important ones are Helicobacter pylori infection and nonsteroidal anti-inflammatory drugs (NSAIDs). Patients generally present with dyspepsia or peptic ulcer bleeding. Acid suppressant therapy, H. pylori eradication, and avoidance of nonsteroidal anti-inflammatory drugs are the cornerstones of treatment of peptic ulcer disease. Peptic ulcer bleeding could be life-threatening. It is managed by appropriate supportive care, intravenous proton pump inhibitor therapy, and endoscopic homeostasis. Trans arterial embolization (TAE), Herbal treated and surgery are rarely required if endoscopic therapy fails.

Eradicate Helicobacter pylori first, or treat peptic ulcer disease?

Helicobacter pylori infection is very common worldwide, and about 10%-16% of these patients develop peptic ulcer disease. However, there is limited research on the impact of H. pylori eradication and peptic ulcer disease treatment sequencing. We conducted a retrospective study of adult patients diagnosed with H. pylori infection and peptic ulcer disease between October 2020 and April 2021 at our center. Data on primary treatment outcomes, including H. pylori eradication and peptic ulcer disease healing, were collected, and factors that may influence treatment outcomes were analyzed. A total of 306 patients were included in this study. The sequence of H. pylori eradication and peptic ulcer disease treatment did not significantly affect the outcomes of H. pylori eradication and peptic ulcer disease healing. In addition, patient age, peptic ulcer disease type, clinic type and treatment regimen (including choice of proton pump inhibitor) had no significant impact on H. pylori eradication. However, patient gender and the choice of antibiotic combination proved to be key factors, as eradication rates were lower in female patients compared to males, and the combination of levofloxacin and clarithromycin was the least effective in eradicating H. pylori. Regarding peptic ulcer disease healing, the peptic ulcer disease type was an important influencing factor, since gastric ulcers being more likely to get cured completely compared to duodenal ulcers. The sequence of H. pylori eradication and peptic ulcer disease treatment does not significantly affect the primary outcomes. Patient gender and the choice of antibiotic combination are important factors in H. pylori eradication, whereas peptic ulcer disease type plays a key role in ulcer healing.

PREVALENCE AND CAUSES OF RECURRENT PEPTIC ULCER

Background:Peptic ulcer disease (PUD) is a common disease of the gastrointestinal tract characterized by mucosal damage due to the secretion of pepsin and gastric acid. The current study aimed to determine the prevalence and causes of recurrent peptic ulcer. Methods:The current study adopted an exploratory study design in order to determine the prevalence and causes for recurrent peptic ulcer disease in individuals and predict an outcome. The participants for the current study were individuals belonging to the age group 18-75. This age group is chosen as the major influence of peptic ulcer disease is observed within this group.For the current study, questionnaire was adopted for data collection, which was also categorized as a study tool. Results:Study included 589 participants. It is noticed that most of participants (n= 530, 90.1%) strongly agree that they visit the doctor periodically to check on their health. Previous item has the highest rank followed by participants follow smoking lifestyle (n= 526, 89.4%). On the other hand, the least rank item on which participants strongly disagreed was participants suffered from soreness or bleeding from their stomach (n= 249, 42.4%). This gives a prevalence of 42.4% of peptic ulcer disease among study participants. Conclusion:Peptic ulcer illness burdens health care systems, which urge for adequate treatment to limit recurrence and effects. H. pylori, smoking, and aspirin usage are risk factors for 5-year peptic ulcer recurrence. Long-term PUD hazards include smoking and aspirin. Avoiding risk factors reduced the recurrence rate of H. pylori eradication, acid suppression medication, and surgery. This shows how eliminating risk factors may enhance long-term performance. Proper PUD treatment requires multicenter research to prevent recurrence and repercussions.

Phytochemical screening, acute toxicity and anti-ulcer activity of the stem bark of Anogeissus leiocarpus (DC.) Guill. & Perr. (Combretaceae)

Peptic ulcer disease constitutes a major health challenge resulting in morbidity and in some cases, mortality. This study evaluated the phytochemistry, acute toxicity, and anti-ulcer activity of the methanol extracts of the stem bark of Anogeissus leiocarpus (DC.) Guill. & Perr. Flavonoids, tannins and saponins are some phytochemicals found in the extracts. From the results, the LD50 of the aqueous and methanol extract was >5000 mg/kg and 3807 mg/kg. The mean ulcer indices for the aqueous extract was 0.38 ± 0.07, 0.38 ± 0.10 and 0.36 ± 0.07 respectively for the 100, 200 and 400 mg/kg), while those of the methanol extract were 0.32 ± 0.09, 0.10 ± 0.03 and 0.12 ± 0.10 respectively for 100, 200 and 400 mg/kg). These values were not significant at P>0.05 when compared with control. However, the percentage ulcer inhibition for methanol extract in group treated with 200 and 400 mg/kg, were both significantly (p<0.05) higher compared with 58.8% from omeprazole (20 mg/kg) treated group. These findings showed that the methanol extract at 200 mg/kg and 400 mg/kg. significantly (p<0.05) increased the anti-ulcer activity, suggesting that A. leiocarpus possesses anti-ulcer property which could be useful in the treatment of peptic ulcer disease.
