Using the follicular unit extraction technique in treatment of male androgenetic alopecia.

The incidence of androgenetic alopecia in males is on the rise annually, with hair transplantation using follicular unit extraction (FUE) gaining increasing acceptance as an appropriate treatment for these individuals. A retrospective study was undertaken, involving 158 male patients diagnosed with androgenetic alopecia, who underwent treatment between January 2016 and December 2020 at the Medical Cosmetology Department of the First Affiliated Hospital of Xi'an Medical University. Demographic data and treatment characteristics were documented and analyzed. Clinical efficacy, operation-related complications, patient satisfaction rates, and quality of life scores were assessed and analyzed. Following FUE hair transplantation, over 90% of the hair follicles survived in 158 patients, with more than 85% of patients achieving a hair follicle survival rate exceeding 95% at 12 months post-operation. Patient satisfaction rates exceeded 98%, while the complication rate was below 6%. Our findings demonstrate that FUE is a minimally invasive hair transplant technique associated with a high hair follicle survival rate and optimal hair density. This approach proves effective in treating male androgenetic alopecia and merits further clinical application.

A Comparative Study on the Application of Robotic Hair Restoration Technology Versus Traditional Follicular Unit Excision in Male Androgenetic Alopecia

ABSTRACTBackgroundThe robotic hair transplant technology, ARTAS, has a series of fine mechanical structure and an intelligent recognition system that allows it to independently select hair follicular units (FUs) and effectively harvest hair. After entering China in 2016, ARTAS has attracted the attention of hair transplant surgeons and hair loss patients given its advantages in a short learning curve and simple operation.ObjectiveTo compare the efficacy and safety between the ARTAS system and follicular unit excision (FUE) in the treatment of male androgenetic alopecia (AGA) and to further promote the improvement and upgrading of ARTAS technology concerning hair transplantation.MethodsThirteen Chinese male patients with Norwood–Hamilton II–IV AGA aged 25–35 years were enrolled in this study. The donor site of each patient was randomly divided into left and right regions, receiving ARTAS on one side and FUE on the other. Yield, transection, and discard rates of hair FUs from both sides were compared. Safety of the procedures in whole, as well as follow‐up results were investigated and evaluated.ResultsThe total yield rate on the ARTAS side was lower than on the FUE side (82.05% vs. 90.03%, p > 0.05); the total discard rate on the ARTAS side was higher than on the FUE side (10.71% vs. 5.46%, p < 0.05); the total transection rate on the ARTAS side was lower than on the FUE side (13.17% vs. 13.96%, p > 0.05). No significant difference was found in patient satisfaction (efficacy), and no side effects or complications were detected during or after all surgeries.ConclusionThe current iteration of Robotic hair transplant technology is effective and safe, and can be recommended for AGA hair transplantation surgery.

A Randomized Controlled, Single-Observer Blinded Study on the Safety and Efficacy of a Combination of 678nm Helmet-Type Low-level Laser Therapy and Topical Minoxidil 5% Versus Topical Minoxidil 5% Alone in the Treatment of Male Androgenetic Alopecia

Male pattern androgenetic alopecia is a chronic, progressive condition with significant social and psychological impact. Few preliminary researches have been done on the role of low-level laser therapy as a treatment modality for male pattern hair loss. This is a randomized, controlled, single-observer blinded clinical trial conducted from November 2019 to October 2021 at the East Avenue Medical Center Dermatology Out-Patient Department on the use of low-level laser therapy and topical minoxidil for the treatment of androgenetic alopecia. Twenty-four male adults with Norwood Hamilton Hair Loss Scale of IIa to V were randomly assigned into 2 groups: a control group (monotherapy with topical minoxidil 5%) and a treatment group (678 nm helmet-type low-level laser therapy and topical minoxidil 5%). A statistically significant higher increase in hair density was observed in the treatment group (14.58 and 15.08 for the right and left sides, respectively) compared to the control (5.83 on the right and 5.50 on the left, respectively, p=.000 <.05). In addition, the investigator’s global photographic assessment and patient satisfaction were also significantly higher. Both treatments were well tolerated. The results of this study show that 678 nm helmet-type low-level laser therapy is an effective adjunctive treatment modality to topical minoxidil 5% solution for the treatment of male androgenetic alopecia.

Oral Minoxidil vs Topical Minoxidil for Male Androgenetic Alopecia

There has been increased interest in low-dose oral minoxidil for androgenetic alopecia (AGA) treatment. However, the efficacy of oral minoxidil for male AGA is yet to be evaluated in comparative therapeutic trials. To compare the efficacy, safety, and tolerability of daily oral minoxidil, 5 mg, vs twice-daily topical minoxidil, 5%, for 24 weeks in the treatment of male AGA. This double-blind, placebo-controlled randomized clinical trial was conducted at a single specialized clinic in Brazil. Eligible men with AGA aged 18 to 55 years classified using the Norwood-Hamilton scale as 3V, 4V, or 5V were included and randomized. Data were collected from January to December 2021, and data were analyzed from September 2022 to February 2023. Participants were randomized 1:1 into 2 groups: oral minoxidil, 5 mg, daily and topical placebo solution; or 1 mL of topical minoxidil, 5%, twice daily and oral placebo for 24 weeks. The primary outcome was change in terminal hair density on the frontal and vertex regions of the scalp. The secondary outcomes were change in total hair density and photographic evaluation. Among 90 enrolled participants, 68 completed the study; of these, the mean (SD) age was 36.6 (7.8) years. A total of 33 participants were enrolled in the oral minoxidil group and 35 in the topical treatment group. Both groups were homogenous in terms of demographic data and AGA severity. For the frontal area, the mean change from baseline to week 24 between groups was 3.1 hairs per cm2 (95% CI, -18.2 to 21.5; P = .27) for terminal hair density and 2.6 hairs per cm2 (95% CI, -10.3 to 15.8; P = .32) for total hair density. For the vertex area, the mean change from baseline to week 24 was 23.4 hairs per cm2 (95% CI, -0.3 to 43.0; P = .09) for terminal density and 5.5 hairs per cm2 (95% CI, -12.5 to 23.5; P = .32) for total hair density. According to the photographic analysis, oral minoxidil was superior to topical minoxidil on the vertex (24%; 95% CI, 0 to 48; P = .04) but not on the frontal scalp (12%; 95% CI, -12 to 36; P = .24). The most common adverse effects in the oral minoxidil group were hypertrichosis (22 of 45 [49%]) and headache (6 of 45 [14%]). In this study, oral minoxidil, 5 mg, once per day for 24 weeks did not demonstrate superiority over topical minoxidil, 5%, twice per day in men with AGA. Role of oral minoxidil in the treatment of androgenetic alopecia

Patterned hair loss is the most common cause of alopecia, typically presenting with progressive thinning, miniaturization, and loss of hair at classical topography depending on the sex of the patient. In the 1970s, Minoxidil was used to treat severe refractory hypertension due to its vasodilator properties. It became popular after its coincidental finding on the promotion of hair growth and stimulation of new hair production. In 1988, the Food and Drug Administration approved topical minoxidil (TM) 2% for the treatment of male androgenetic alopecia and in 1992, for female pattern hair loss. It is also used as an off-label treatment for other hair loss conditions such as telogen effluvium and alopecia areata. However, there are frequent reports of TM-induced contact dermatitis and its reversible effect, which has reduced compliance in the patients and resulted in a poor outcome. Hence, they have studied using low-dose oral minoxidil (LDOM), which showed good efficacy and safety profile in the treatment of patterned hair loss along with avoidance of high-risk adverse cardiac effects that resulted due to doses used for hypertension. The added advantages of LDOM over topical formulation are good compliance, cost savings, effective in people with low hair follicle sulfotransferase activity, and the possibility of cotherapy with other topical or oral medications.

Finasteride and Dutasteride for the Treatment of Male Androgenetic Alopecia: A Review of Efficacy and Reproductive Adverse Effects

Finasteride and dutasteride are 5-α-reductase inhibitors (5-ARIs) used to treat androgenetic alopecia (AGA). This review evaluates the efficacy of 5-ARIs for treatment of men with AGA and the potential adverse effects on reproduction including sexual dysfunction, infertility, and teratogenicity. A broad literature review was conducted to search for publications on 5-ARI treatment in men with AGA. Hair counts, hair growth assessments, sexual adverse effects (erectile dysfunction, ejaculatory dysfunction, and decreased libido), change in sperm parameters (decreased sperm count, semen volume, sperm motility), and teratogenic drug concentration levels in semen were the measured outcomes of studies included in this literature review. Both finasteride and dutasteride are effective at treating hair loss in male AGA, with studies finding dutasteride was more efficacious than finasteride. Many studies reported sexual adverse effects of 5-ARIs that are uncommon and resolve spontaneously, although there remains no consensus with respect to the presence, severity, and duration of sexual adverse effects. 5-ARIs may have a negative impact on spermatogenesis although the clinical significance of this is unclear and discontinuation of these medications results in improved sperm parameters for most patients. Teratogenicity after paternal exposure is unlikely due to the low concentration of 5-ARIs absorbed in semen. Further research is needed to evaluate the effects of 5-ARI use on reproduction.

Efficacy of Ablative Fractional Carbon Dioxide Laser Alone and Combined with Platelet-Rich Plasma in the Treatment of Androgenetic Alopecia

Abstract Androgenetic alopecia (AGA) is a common form of hair loss in both men and women. In men, this condition is also known as male-pattern baldness. Fractional CO2 laser treatment has effectively induced hair regrowth through the wound-healing process. Platelet-rich plasma (PRP) ensures local delivery of growth factors, modification of the inflammatory response, and stimulation of cell proliferation and differentiation within the target tissue. PRP therefore showed a beneficial effect in treatment of AGA. Aim of the Study The aim of this study is to evaluate and compare the efficacy and safety of ablative carbon dioxide (CO2) fractional laser alone or combined with platelets rich plasma therapy in the treatment of Male Androgenetic Alopecia. Patients and Methods The study included 35 patients suffering from AGA. All patients will receive bilateral ablative fractional CO2 laser treatment on the alopecic scalp and the right or left side of the scalp will be randomly selected to be covered with platelet-rich plasma and the other half of the scalp will be covered with saline after the laser at the same session. Results Combined ablative fractional CO2 laser treatment and platelet-rich plasma showed statistically significant improvement compared to ablative fractional CO2 laser alone.

Injections of platelet-rich plasma prepared by automatic blood cell separator combined with topical 5% minoxidil in the treatment of male androgenetic alopecia.

Platelet-rich plasma (PRP) has been increasingly accepted as a potential therapy in the treatment of androgenetic alopecia (AGA), However, there remains a dearth of data on the effectiveness of PRP prepared by automatic blood cell separator with a combination of topical minoxidil for the treatment of AGA. To evaluate the efficacy and safety of PRP prepared by automatic blood cell separator combined with topical 5% minoxidil therapy in male AGA. Thirty male patients with mild/moderate AGA were enrolled in a randomized double-blind controlled study. Patients were randomly divided into two treatment arms: (group A) PRP prepared by automatic blood cell separator combined with topical 5% minoxidil group; (group B) PRP prepared by automatic blood cell separator combined with a topical placebo group. Trichoscopic assessments regarding hair density/quantity and mean hair diameter were performed at baseline and follow-up. Clinical efficacy of global photography and patient satisfaction were conducted to verify the therapeutic efficacy of the treatment, and the occurrence of adverse reactions was recorded. We detected a significant increase in all patients in hair density and quantity after PRP treatment (p < 0.05), and there was no significant difference in mean hair diameter. Although hair density/quantity was more pronounced in group A than in group B, the difference between groups was not statistically significant (p > 0.05). In terms of clinical efficacy and patient satisfaction, group A was superior to group B, and no serious adverse reactions occurred. We hereby conclude that the injections of PRP prepared by an automated method are effective and safe in the treatment of mild-to-moderate male AGA patients, and its combination with topical 5% minoxidil therapy was superior to PRP monotherapy with better clinical efficacy and higher patient satisfaction.

Androgenetic alopecia (AGA) has been one of the most common progressive hair loss in the world, which affects 80% of white males. To date, only minoxidil and finasteride have been approved by FDA for the treatment of AGA. However, limited therapeutic effect and the toxic adverse events of these drugs limit their applications. Therefore, it is still an urgent clinical problem to find effective therapeutic drugs and medication regimen. The goal was to explore the efficacy and side effects of basic fibroblast growth factor (bFGF) combined with minoxidil in the treatment of male patients with early stage of androgenetic alopecia (AGA). Using a randomized control method, 80male patients with androgenetic alopecia in Hamilton grade II-IV were randomly divided into two groups, with 40 patients in each group. The Group A: 1ml minoxidil for external use twice a day; Group B: 3500IU basic fibroblast growth factor (bFGF) and 1ml minoxidil for external use twice a day. The selected patients received global photograph evaluation before treatment, 3months after treatment, and 6months after treatment, and the curative effect was judged according to the changes in the area and degree of hair loss on the top of the head and anterior parietal area of the patients shown in the photographs before and after treatment. At the same time, each patient had a satisfaction questionnaire survey before treatment, 3months after treatment, and 6months after treatment. During the research period, the adverse reactions of the patients were recorded. After 3months and 6months of treatment, the effective rate of the two groups was statistically significant (p<0.05), and the patients' hair conditions in the Group B improved significantly compared with those in the Group A. After 6months of treatment, the difference in treatment satisfaction between the two groups was statistically significant (p<0.05). The patients in the Group B were more satisfied than those in Group A. During the patient's medication, no serious adverse reactions occurred in the two groups, and the incidence of adverse reactions between the two groups was not statistically significant (p>0.05). Compared with 5% minoxidil alone, the combination of basic fibroblast growth factor (bFGF) +5% minoxidil in the treatment of male patients with early stage of androgenetic alopecia improved treatment efficiency and patient satisfaction.

Epidemiology and Treatment of Male Androgenetic Alopecia

Hair loss affects self-image and is a significant contributor to anxiety and depression in some men, despite the fact that male androgenetic alopecia is frequently thought of as a relatively minor dermatological problem. Male androgenetic alopecia is also considered a risk factor for several other diseases including cardiovascular diseases. It affects 30-50% of men by the age of 50. Male androgenetic alopecia is a genetically transferred disease. Midfrontal scalp, vertex and temples are the commonly affected areas. Follicular shrinkage, inflammation, and altered hair development are the main pathophysiological characteristics the disease. The anagen phase gets shorter with each cycle whereas the telogen phase stays the same length or gets longer during the course of the disease. The purpose of this research is to review the available information about the epidemiology and treatment of male androgenetic alopecia. Male androgenetic alopecia shows variable prevalence among different ages and ethnicities. Caucasians have much high reported prevalence than Asians. The prevalence of male androgenetic increases with age. Minoxidil and finasteride are the drugs of choice and most commonly used in practice for the treatment. Even while the existing therapies for androgenetic alopecia are effective in slowing the disease's progression, they only allow for partial hair restoration. Early treatment leads to the most ideal outcome. Hair transplantation remains the option if medical treatment fails. Further research can aid in developing new treatment strategies.

Evaluation of the efficacy and safety of topical procyanidin b2 and placebo in the treatment of androgenetic alopecia in men; A randomized, double-blind, placebo-controlled study

Objective: Androgenetic alopecia (AGA) is a common type of alopecia characterized by the shortening of the anagen phase of hair growth and the miniaturization of hair follicles. TGF-β is a well-known hair cycle catagen phase inducer and is involved in the catagen phase in AGA. Inhibition of TGF-β is recognized as a therapeutic option in the treatment of AGA. Procyanidins are a type of polyphenol that has been shown to inhibit TGF-β activity in vivo, but there haven’t been many studies on their effectiveness. In this study, we aimed to evaluate the efficacy and safety of topical procyanidin B2 in the treatment of male AGA. Materials and Methods: Patients aged between 18 and 50 years who applied to our dermatology outpatient clinic with the complaint of AGA and had Hamilton-Norwood type II-V AGA were included in our study. Those who had received 5-reductase inhibitor or isotretinoin treatment in the previous year, those who had used any medicinal or herbal product that stimulated hair growth, particularly topical minoxidil, in the previous six months, those who had used systemic steroids for more than two weeks in the previous three months, those who had undergone a transplant or scalp reduction, and those who had received radiotherapy or chemotherapy at any point in their lives were excluded from the study. A total of 40 patients who met the current criteria were included in the study. Patients were randomized into two groups to receive 16 weeks of topical procyanidin B2 (n = 20) or placebo (n = 20) therapy. At the end of the treatment, the patients were called for control. The efficacy of topical procyanidin B2 after treatment was evaluated by trichoscan and global photographic evaluation. Results: A total of 40 male patients (mean: 33.32, range: 21–44) with AGA type II-IV were included in the study. There was no significant difference between the two groups in terms of age, duration of hair loss, and AGA type (P > 0.05). At the end of the study, there was a significant increase in total hair count in the topical procyanidin B2 group compared to the placebo group compared to baseline (P < 0.05). Anagen hair count was also significantly increased in the topical procyanidin B2 group (P < 0.05). Conclusion: In this placebo-controlled study, we think that topical procyanidin B2 is an effective and safe treatment option in the treatment of AGA patients.

Combination therapy with injectable platelet-rich fibrin and microneedling for male androgenetic alopecia: an evaluation of current practice

Introduction: Within the past 20 years, significant advances have taken place within the realm of aesthetic medicine to manage hair loss. Several human blood preparations have been found to contain regenerative properties suitable for hair rejuvenation and, subsequently, have emerged onto the market. An advanced and liquid version of platelet-rich fibrin, injectable platelet-rich fibrin (iPRF) is one such product. This article evaluates the combination of iPRF with microneedling (MN) for the treatment of male androgenetic alopecia (AGA) compared to other therapies. To knowledge, it is the first evaluation of its kind. Methods and materials: Secondary research was conducted via EMBASE, MEDLINE, PubMed and Cochrane Library databases to explore a preferred blood preparation and combination treatment using MN. Results and discussion: Secondary research shows that there is more in-depth evidence that suggests platelet-rich plasma (PRP) is the preferred blood preparation for treating male AGA at present. However, studies show that iPRF has greater regenerative potential. More research currently exists to suggest that MN in combination with minoxidil (MX) therapy is preferred over iPRF combined with MN alone. No validated patient-reported outcome measures (PROMs) currently exist for male AGA, although PROMs do exist for facial aesthetic treatments and alopecia areata. Conclusions: Studies show that iPRF combined with MN is a safe and effective option for the treatment of male AGA, but comparison studies against other combinations are lacking. Other monotherapy and combination methods are generally more evidenced. The secondary research also found that no specific PROM tool for measuring male AGA exists. Unless a specific PROM is developed and validated for male AGA, challenges around treatment comparison will continue to limit evidence.

Efficacy of Minoxidil Combined With Photobiomodulation for the Treatment of Male Androgenetic Alopecia. A Double-Blind Half-Head Controlled Trial.

Androgenetic alopecia (AGA) is a hair loss disorder that frequently affects the male population. Conventional treatment modalities are limited to minoxidil, 5α reductase inhibitors, and hair transplantation procedures. The efficacy of low-level laser therapy (LLLT), also known as photobiomodulation, in the treatment of AGA has been reported, yet little is known about the outcomes of combining photobiomodulation with other conventional therapies. To evaluate hair growth improvement in males with AGA, during the administration of minoxidil with and without photobiomodulation, using a half-head model. Twenty-one men with AGA agreed to undergo 12 minutes of low-level laser irradiation (using a modified Capellux®), followed by topical minoxidil application (1 mlof 5% solution), to the affected scalp two times per day for 6 months. The photobiomodulation devices were modified such that the left half emitted light, and the right half did not. Efficacy was assessed by blinded analyses of clinical photos and automated phototrichograms (Trichoscan®) taken before treatment and after 3 and 6 months of therapy. None of the study participants experienced any adverse events. All patients showed improvements in hair coverage on both sides of the scalp at 3 and 6 months. On the side with combined treatments, the number of total hairs was significantly increased after 3 (P < 0.001) and 6 months (P = 0.001). A similar increase was also observed on theminoxidil-only side, at both 3 (P < 0.001) and 6 months (P < 0.001). No statistically significant differences were detected between sides (P > 0.05). Additional improvement was not observed with the association of photobiomodulation to topical minoxidil in male AGA. Differences from previous studies that might have influenced our result includenon-collimated light source, higher dosimetry, and a cohort with darker skin phototype and more severe alopecia. Update Treatment of Male Androgenetic Alopecia

Background: Male androgenetic alopecia (MAGA), also known as androgenetic alopecia, is the most common hair loss in males who have a genetic predisposition. The pattern of baldness in MAGA starts from the frontal area in a triangular pattern, followed by progressive thinning of the vertex until baldness occurs. Generally, the diagnosis of MAGA is established by clinical examination. FDA has approved a combination of topical minoxidil and oral finasteride for MAGA treatment. Currently, there is another treatment option like dutasteride, a prostaglandin analog, ketoconazole, and co-adjuvant therapy like laser therapy, hair transplantation, and so on. Purpose: To provide an updated treatment for MAGA. Review: Etiopathogenesis of MAGA is influenced by genetic susceptibility and hormonal factors. The European Consensus Group set the evaluation diagnosis of MAGA to include a historyof hair fall, physical examination, hair examination, supporting examination, and clinical documentation. There are therapeutic options for MAGA, including antiandrogen therapies, androgen-independent therapies, and co-adjuvant therapies. The FDA has approved a combination of topical minoxidil and oral finasteride for MAGA treatment. MAGA may affect patients’ quality of life and self-esteem. In general, patients expect higher. Conclusion: MAGA is the most common progressive hair loss in males. The MAGA therapy is expected to achieve cosmetically significant regrowth and to slow additional hair loss.

Efficacy of Autologous Platelet Rich Plasma in the Treatment of Male Androgenetic Alopecia

Objective: To assess the outcome of autologous platelet rich plasma(PRP) for the treatment of androgenetic alopecia in male patients. Methods: A total of 61 male patients from Department of Dermatology, Lahore General Hospital Lahore with ages between 18-60 years were included in the study during a period of 6 months. PRP was prepared by double spin method and CaCl2 was used to activate platelets. PRP was injected, with a 30-G needle using Nappage technique. This descriptive case series study was carefully monitored. Protocol included three treatment sessions with an interval of four weeks. Number of hairs, and hair strength was assessed by dermoscopy and hair pull test. SPSS Version-21 was used for data entry and analysis. Results: The mean age of patients was 42.67±8.63 years. Mean number of hair was 21.50±4.57 per dermatoscopic field, Post treatment number of hair was 78.10±16.24. Mean duration of disease was 10.25±3.52 months. Majority of patients were in stage iv 19(31.1%) and in stage ii 16(26.2%). There was significant difference in number of hair before and after treatment.
