Management of Transjugular Intrahepatic Portosystemic Shunt (TIPS)-associated Refractory Hepatic Encephalopathy by Shunt Reduction Using the Parallel Technique: Outcomes of a Retrospective Case Series

To investigate the reproducibility and technical and clinical success of the parallel technique of transjugular intrahepatic portosystemic shunt (TIPS) reduction in the management of refractory hepatic encephalopathy (HE). A 10-mm-diameter self-expanding stent graft and a 5-6-mm-diameter balloon-expandable stent were placed in parallel inside the existing TIPS in 8 patients via a dual unilateral transjugular approach. Changes in portosystemic pressure gradient and HE grade were used as primary end points. TIPS reduction was technically successful in all patients. Mean±standard deviation portosystemic pressure gradient before and after shunt reduction was 4.9±3.6mmHg (range, 0-12 mmHg) and 10.5±3.9mmHg (range, 6-18 mmHg). Duration of follow-up was 137±117.8days (range, 18-326 days). Clinical improvement of HE occurred in 5 patients (62.5%) with resolution of HE in 4 patients (50%). Single episodes of recurrent gastrointestinal hemorrhage occurred in 3 patients (37.5%). These were self-limiting in 2 cases and successfully managed in 1 case by correction of coagulopathy and blood transfusion. Two of these patients (25%) died, one each of renal failure and hepatorenal failure. The parallel technique of TIPS reduction is reproducible and has a high technical success rate. A dual unilateral transjugular approach is advantageous when performing this procedure. The parallel technique allows repeat bidirectional TIPS adjustment and may be of significant clinical benefit in the management of refractory HE.

Stent-graft Narrowed with a Lasso Catheter: An Adjustable TIPS Reduction Technique

The authors describe an endovascular technique to reduce shunt diameter in the management of transjugular intrahepatic portosystemic shunt (TIPS)-induced refractory hepatic encephalopathy. Five patients were treated with a constrained stent-graft by using a commercially available balloon-expandable stent-graft narrowed by using a lasso catheter. This offers the possibility of an adjustable reduction of the shunt diameter. All procedures were technically successful, resulting in a significant increase in the portosystemic gradient and reopacification of the intrahepatic portal vein branches, findings that correlated with clinical improvement. This technique provides the ability to titrate the portosystemic gradient to the desired endpoint during shunt reduction.

Rescue of a Marginal Liver Graft by Sequential Treatment With Molecular Adsorbent Recirculating System and Transjugular Intrahepatic Portosystemic Shunt: A Case Report

A 53-year-old man with alcoholic liver cirrhosis underwent orthotopic liver transplantation (OLT) using a marginal graft. Persistent cholestasis post-OLT was successfully treated using a molecular adsorbent recirculating system (MARS). Afterwards, the patient developed refractory ascites, which was controlled by a transjugular intrahepatic portosystemic shunt (TIPS). TIPS reduction and eventually occlusion was necessary due to the development of encephalopathy. Despite TIPS occlusion, the ascites did not relapse probably because of the onset of other adaptive mechanisms. MARS and TIPS used sequentially were capable of rescuing a liver graft, thereby avoiding the morbidity and mortality associated with early retransplantation and sparing a liver graft from the donor pool.

Transjugular Intrahepatic Portosystemic Shunt Flow Reduction with Adjustable Polytetrafluoroethylene-covered Balloon-expandable Stents

Creation of a transjugular intrahepatic portosystemic shunt (TIPS) can effectively treat complications of portal hypertension, but excessive shunting can cause life-threatening hepatic encephalopathy and hepatic insufficiency. The present report describes a novel technique that allows for controlled and adjustable flow reduction through the TIPS via partial closure of the shunt with a balloon-mounted covered stent. The method results in clinical improvement of hepatic encephalopathy and hepatic insufficiency and immediate increase in the portosystemic pressure gradient. However, among the four patients described herein, survival beyond 1 year was seen in only one, who underwent liver transplantation after TIPS reduction.

Portosystemic pressure gradient during transjugular intrahepatic portosystemic shunt with Viatorr stent graft: What is the critical low threshold to avoid medically uncontrolled low pressure gradient related complications?

Inappropriately decreased portosystemic pressure gradient (PSG) during transjugular intrahepatic portosystemic shunt (TIPS) can cause fatal complications but the critical low threshold of PSG is still not clear. The aim of the present study was to evaluate the critical low threshold of PSG during TIPS. Sixty-six patients with cirrhosis who successfully underwent de novo TIPS with Viatorr stent grafts were studied. Medically uncontrolled low pressure gradient (LPR) complication was defined as when a patient died, or when acute transplantation or a TIPS reduction procedure was performed due to refractory encephalopathy or the deterioration of hepatic function within 3 months after the procedure. For the determination of the risk group for medically uncontrolled LPR complications, the Child-Pugh score and the model of end-stage liver disease (MELD) score showing a 100% negative predictive value was decided on as a threshold for each score. The risk group was defined when either of both scores was higher than its threshold. For the determination of a critical low post-TIPS PSG, a value of post-TIPS PSG showing the highest discrimination power on the receiver operating characteristic (ROC) curve in the risk group was decided on as a critical low threshold of PSG. The medically uncontrolled LPR complication rates of the patients with the determined threshold or lower were evaluated for the risk group. Medically uncontrolled LPR complications developed in nine patients (13.6%). Five patients died and four patients had TIPS reduction procedures. Patients with more than 10 on the Child-Pugh score or more than 14 on the MELD score were determined to be the risk group and 34 patients were included. The critical lower threshold of the post-TIPS PSG showing the highest discrimination power on the ROC curve was 5 mmHg (sensitivity 100%, specificity 72%), and the medically uncontrolled LPR complication rates of the patients with 5 mmHg or lower on the post-TIPS PSG were 56.3% (9/16) in the risk group. The critical threshold of the post-TIPS PSG to avoid the medically uncontrolled LPR complications of TIPS was >5 mmHg. The PSG should not be reduced below this level in the risk group.

Stent graft in TIPS: technical and procedural aspects

Transjugular intrahepatic portosystemic shunt (TIPS) is a nonoperative therapeutic option for the management of portal hypertension, variceal bleeding, recurrent ascites, Budd-Chiari syndrome. In view of the many issues surrounding the use of TIPS, in 1994 the US National Digestive Diseases Advisory Board convened a scientific conference to review the current data available and to establish the indications and controindications for this procedure. However there are still unsolved problems especially short primary patency of the shunt due to intimal hyperplasia, which causes a reduction of the shunt lumen thus favoring a return of the portal hypertension with recurrent variceal bleeding. Several study were performed in the last years to evaluate the efficacy of covered stent in order to reduce shunt disfunction secondary to intimal hyperplasia. PTFE seems to be more efficient in the prevention of restenoses. In our experience more then 100 patients were treated with the Viatorr stent-graft. After a follow-up ranging from 1 to 50 months we reported a 1- year primary patency rate of 83.8%. However the use of the stentgraft is correlate with a high rate of hepatic hencefalopathy (46.6 %). In case of hepatic hencefalopathy refractory to the conventional medical therapy, TIPS reduction should be performed.
