Introduction/Background: In this case report we describe the efficacy and outcome from rapid theta burst transcranial magnetic stimulation (TBS) to treat a hospitalized patient with schizophrenia post-suicide attempt. The patient was a 67-year old woman with a history of chronic schizophrenia. She was admitted to the inpatient unit after endorsing suicidal ideation, increasing depressive symptoms, auditory hallucinations and multiple recent suicide attempts. During the course of her 95-day hospitalization, she was noted to have depression with neurovegetative symptoms and periods of catatonia, severe psychosis with auditory hallucinations and continued suicidal ideation. Changes in medication were not effective. Electroconvulsive Therapy (ECT) was ruled out due to comorbidities of 5mm internal carotid aneurysm and anticoagulation post pulmonary embolism. 77 days into her hospital stay, the patient consented to a 5-day course of rapid TBS.
