Lingguizhugan Decoction in the Treatment of Non-Alcoholic Fatty Liver Disease: A Systematic Review and Meta-Analysis.

This study systematically evaluated the efficacy and safety of Ling Gui Zhu Gan Decoction for treating non-alcoholic fatty liver disease. Registered under CRD42024501460 on the PROSPERO platform, we searched eight major databases, including Web of Science, PubMed, Cochrane, Embase, China National Knowledge Infrastructure, Wanfang database, Chinese Scientific Journals Database, and Chinese Biomedicine Database, from inception to December 2023 for randomized controlled trials on Ling Gui Zhu Gan Decoction in non-alcoholic fatty liver disease treatment. We extracted data on total efficiency, TC, TG, ALT, AST, GGT, and HOMA-IR, analyzing results with RevMan 5.4 software. Twelve studies met the inclusion criteria, encompassing 970 cases. Ling Gui Zhu Gan Decoction, alone or combined with standard therapy, significantly improved non-alcoholic fatty liver disease outcomes, regardless of treatment duration. Only one study reported adverse events, including bloating, diarrhea, nausea, vomiting, and rash. Ling Gui Zhu Gan Decoction appears to be an effective and safe option for non-alcoholic fatty liver disease treatment. However, due to limited studies and methodological weakness, further rigorous randomized controlled trials are necessary for conclusive results.

MS O04 - OGC Outcomes following gastrectomy for cancer: A 15-Year Retrospective Study examining the impact of regional service centralisation, benchmarking against international standards

Abstract Background Gastrectomy remains the cornerstone of curative treatment for gastric cancer but is associated with significant risks of complications. This study aimed to explore the impact of service centralisation in Greater Manchester on outcomes following gastrectomy for cancer. Outcomes were compared against international benchmarks to evaluate impact of centralisation and identify areas for improvement. Method This was a retrospective study of all patients undergoing gastrectomy (total gastrectomy TG; partial gastrectomy PG) for cancer at a tertiary OG cancer centre between 2007-2022. Patients were allocated to three time periods (2007-2012; 2013-2018; 2018-2022). Regional centralisation took place in August 2018. Outcome selection was guided by a core outcome set for gastrectomy and international benchmarks including the GastroData consortium and the Dutch Upper Gastrointestinal Cancer Audit (DUCA). Results 379 patients undergoing gastrectomy (TG; 201, PG; 172) were included. Median length of stay shortened (p=0.023) and median lymph node yield improved (p<0.001) following centralisation. This was consistent with findings from the DUCA audit. There was no difference in R0 resection rates (p=0.668), surgical reinterventions (p=0.717) or readmissions within 90-days (p=0.428) after centralisation. Whilst post-operative mortality improved (30-day 4.5% versus 0%; 90-day 5.4% versus 1.2%), this was not statistically significant (p=0.09). Conclusion Centralisation of surgical OG cancer services in our region resulted in improved short-term outcomes for patients undergoing gastrectomy. This may reflect a greater emphasis on standardised peri-operative surgical and pathology protocols. These findings were partly consistent with international benchmarks. Further evaluation of the impact on longer-term outcomes is required.

Studies of Lipid Profiles Among Infants with Respiratory Distress Syndrome

Background: Respiratory distress syndrome (RDS) is known as hyaline membrane disease (HMD). It is the major cause of neonatal respiratory distress, especially in preterm infants Infants with RDS present with tachapnea, cyanosis, grunting, subcostal and intercostal retractions and nasal flaring. Oliguria with mild generalized edema may be present. Oxygen requirement may increase rapidly and is typically higher than that seen in infant with transient tachapnea of the newborn (TTN). Objective: to evaluate the relationship between the maternal and cord blood lipid profiles in preterm babies with respiratory distress syndrome. Methods: This is a case control study which was conducted on 50 preterm neonates admitted to the neonatal intensive care unit of Shebin El-Kom teaching hospital between January 2020 and October 2021. Results: the mean of total cholesterol was significantly higher in infants whose mothers had received antenatal steroid for lung maturity than those whose mothers had not (P<0.05). But there was no significant difference in the mean of levels of TG, HDL-cholesterol, LDL-cholesterol and VLDL cholesterol between both groups. Also, There is a positive correlation between weight gain during pregnancy and neonatal cord blood levels of (Total cholesterol, TG, HDL-C, LDL-C and VLDL-C), There is a positive correlation between maternal blood levels of (total cholesterol, TG, LDL-C and VLDL-C) and their neonatal cord blood levels of (total cholesterol, TG, HDL-C, LDL-C and VLDL-C), There is a positive correlation between the maternal blood level of HDL-C and their neonatal cord blood levels of (total cholesterol and HDL-C).also, the cutoff values of cord blood (total Cholesterol, HDL-C, and LDL-C) below which RDS can be predicted are 80, 45.5 and 23.2 mg/dl respectively with a sensitivity of 83.3%, 86.7% and86.7% and with specificity of 95%, 80% and 70% respectively. Conclusion: RDS is accompanied with lipid alteration in the infants and their mothers. The results of this study point to the importance of measuring maternal serum and cord blood lipid profile as a predictor for the occurrence of RDS.

Effect of electronic smoking (Vaping) on thyroid hormones level and lipid profile in men

In the last period, the market quickly became saturated with vaping devices available in many flavors and forms appealing to youth. Both traditional cigarette and e-cigarette smoking are known to potentially alter metabolic processes, including hormone production and to increase the risk of lung, heart and kidney diseases. The aim of the study was to estimate the level of thyroid hormones and lipids in the blood of young men who smoked traditional or e-cigarettes. A case-control study involved 200 men aged 24-25 years who smoked 5-7 h per day, divided into two groups (100 e-cigarette smokers and 100 cigarette smokers) and 50 healthy men who did not smoke. The levels of thyroid-stimulating hormone (TSH), free thyroxine (FT4), and free triiodothyronine (FT3) were measured using COBAS E411. The levels of total cholesterol, TG, LDL and HDL were estimated spectrophotometrically. No significant changes were found in thyroid hormone levels or lipid profiles, except for an increased TG content in the group of traditional cigarette smokers compared to the control group. Whereas in the blood of e-smokers, the increase in the level of FT3 and FT4 and a significant decrease in that of TSH, as well as the elevated content of total cholesterol, TG, and LDL, were detected compared to the control group. Correlations between dyslipidemia and retinal parameters measured with Angio-OCT in type II diabetics without diabetic retinopathy.

To analyze the relationship between lipoproteins such as total cholesterol, LDL-c, TG, and retinal parameters in patients with DM type II without signs of DR. A case-control study, consisting of 2 groups. A group of 64 patients with type II diabetes without signs of DR and a control group of 24 healthy subjects. Patients with DM type I, those who showed signs of DR, and those who had associated other eye diseases were excluded. The patients of the two studied groups had a similar average age: 65 years in the DM type II group and 64 years in the control group. In the group with DM, the average CRT was 241.31 µm, a significantly lower value compared to the control group, 252.51. The average value of DVFC was 19.19%, in patients with DM and 24.29% in the control group. An indirect correlation with moderate intensity was established between total cholesterol and CRT, (rs=-0.442, p≤0.001), thus it tended to decrease as total cholesterol increased. With increasing total cholesterol level, DVFC had a mild tendency to decrease (rs=-0.381, p≤0.001). An indirect correlation, but weak in intensity, existed between the LDL/HDL ratio and the DVFC S value (rs=-0.240, p=0.001). Central retinal thickness and central vascular density of the superficial capillary plexus were significantly lower in patients with type II diabetes, compared to control subjects. Total cholesterol had higher values in the DM group and an indirect correlation was established with CRT and DVFC, these having a moderate tendency to decrease as the total cholesterol values increased. An indirect and moderate relationship in intensity was also present between LDL and retinal parameters studied. These results were similar to those of other studies conducted, such as that of Chen et al. or Bernaous et al., who showed an association between various lipid classes and the frequency of DR. However, other studies, such as Ausdiab, found that this association did not hold. Type II diabetes patients tend to have elevated serum lipid levels compared to normal subjects, but the impact of dyslipidemia on the onset and progression of DR is incompletely elucidated.

Sex differences in circulating metabolites across glycemic status and risk of coronary heart disease.

Women with type 2 diabetes (T2D) have a 50% excess risk of coronary heart disease (CHD) than men with T2D. We compared circulating metabolites and their associations with CHD in men and women across glycemic status. We used metabolomic data (lipoproteins, fatty acids, amino acids, glycolysis, ketones, inflammation, and fluid balance) for 87,326 CHD-free UK Biobank participants. We used linear regressions to examine the association of sex and metabolites (log) in newly diagnosed T2D (diagnosis<2 yrs from baseline), prediabetes (A1c 5.7-6.5%), and euglycemia, accounting for age, race, Deprivation Index, income, smoking, alcohol drinking, obesity, physical activity, medications for hypertension, hyperlipidemia, and diabetes. We used Cox models to evaluate the association of metabolites and CHD risk by sex, adjusting the same covariates and menopausal status (women). All analyses were FDR-adjusted. We included 1250 individuals with new T2D, 12,706 with prediabetes, and 83,315 with euglycemia. In adjusted linear regressions, women showed a progressive increase in atherogenic lipid and lipoprotein markers and inflammatory marker, glycoprotein acetyls, compared to men as their glycemic status advanced. However, women had lower levels of albumin during this transition. Menopausal status did not alter these sex differences. In a 10-year follow-up, an SD higher total TG, TG in VLDL, LDL, and HDL, saturated fatty acids (SFA) were positively associated with a higher risk of CHD in women with T2D but not in men (p-interactions 0.03-0.15). With advancing glycemic status, women exhibited higher levels of atherogenic lipids and lipoproteins, as well as inflammatory markers, but lower circulating albumin. Women with T2D appear to be at a higher risk of CHD associated with TG, VLDL-TG, LDL-TG, and HDL-TG, and SFA than men with T2D.

Serum Total Bilirubin and Oxidative Stress Status in Diabetic Retinopathy – A Hospital-Based Observational Study

Diabetic retinopathy (DR) is one of the common microvascular complications of Type 2 Diabetes Mellitus (T2DM). As an antioxidant, the serum total bilirubin is associated with vaso-occlusive disorders. Oxidative stress parameters such as Erythrocyte Glutathione (GSH) as an antioxidant and Malondialdehyde (MDA) as an oxidant are critical in the pathogenesis of diabetic complications. This study aimed to explore the possibilities of the endogenous protective role of serum total bilirubin on the retinal vasculature in DR patients by estimating and correlating the levels of serum total bilirubin, GSH, and MDA in DR cases. In this hospital-based case-control study, 288 participants were selected from R.L. Jalappa Hospital and Research Centre, Kolar, divided into three groups with 96 subjects per group. Group I: Controls, Group II: T2DM, and Group III: DR subjects. The fasting blood sugar, glycated hemoglobin, liver function test, and lipid profiles were estimated by standard methods. Oxidative stress parameters viz, GSH and MDA were assayed by chromogen 5,5'- di thiobis 2-nitrobenzoic acid (DTNB) and thiobarbituric acid reactive substances (TBARS) methods, respectively. The prevalence of DR was significantly lower among subjects with the highest bilirubin quartile than those with the lowest. There was a significant mean difference with p&lt;0.001 between the groups for total bilirubin, FBS, HbA1c, GGT, TC, TG, LDL, GSH, and MDA. A Negative correlation of serum total bilirubin with FBS (r = - 0.375), HbA1c (r = -0.351), and MDA(r=-0.323), and a positive correlation with GSH (r = 0.335) was observed in DR group with a significant p-value. T2DM subjects with higher levels of bilirubin within biological reference intervals were less likely to develop retinopathy. The severity of DR was inversely proportional to the total bilirubin levels. Therefore, serum total bilirubin levels could be a biomarker to predict the risk of developing retinopathy in people with T2DM.

The Relationship of Gallestone Disease with Serum RBP4 Level, Vitamın D, Lipid Profile, Insulin Resistance and Uric Acid Levels

Abstract Aim The metabolic parameters associated with gallstones are the subject of numerous studies. RBP4, an adipokine, has been linked to various metabolic diseases; however, there is currently no study in the literature that clearly establishes its relationship with gallstone disease. Therefore, in our study, we aimed to evaluate the biochemical parameters associated with gallstone disease, primarily serum RBP4. Methods Between 2015 and 2016, abdominal ultrasound, serum biochemical tests, lipid profiles, uric acid, insulin, and fasting glucose values were available for 80 patients who presented to our hospital's gastroenterology clinic, were included in the study. RBP4 levels were analyzed in the serum samples obtained from the patients. Results Out of the 80 participants in the study, 42 had gallstones, while 38 did not. Among the biochemical parameters, no significant difference was found between the groups in terms of total cholesterol values (p= 0.483), LDL values (p=0.224), and TG values (p=0.764). A significant difference was observed between the two groups in terms of HDL values(p=0.017). No significant difference was found between the two groups in terms of serum uric acid (p=0.411), fasting glucose (p=0.214), fasting insulin, and HOMA-IR score (p=0.157), vitamin D levels (p=0.340). The mean±SD values of the studied serum RBP-4 levels in the participants were determined as 40.24±7.12 in the control group and 39.75±8.55 in the patient group. No statistically significant difference was found between the two groups (p=0.776). AN OBSERVATIONAL STUDY OF EFFECT ON LIPID PROFILE BY SHORT TERM USE OF LOW DOSE STEROID THERAPY IN COMMON ORTHOPAEDIC CONDITIONS

Introduction:Corticosteroids, potent anti-inflammatory drugs used for decades, provide symptomatic relief but carry risks like infection and chronic conditions. Despite complications, theyre prescribed for orthopedic issues. Glucocorticoids influence lipid levels, with conflicting effects. Steroids are termed a double-edged sword. This study aims to assess lipid profile effects of short-term, low-dose steroid therapy in Orthopedics and Rheumatology, evaluating glucocorticoid therapy impact and investigating other effects on musculoskeletal patients. Materials and Methods:This prospective, non-randomized observational study was conducted, focusing on patients with orthopedic and rheumatological issues. Over 150 patients were included, selected through convenient sampling. Inclusion criteria involved patients aged 18-65 without dyslipidemia or cardiovascular conditions, while exclusion criteria included those with specific medical conditions or high BMI. Patients provided written informed consent before undergoing blood tests for lipid profile and blood sugar levels, with vital signs measured. They received Medrol 4 mg thrice daily for ten days, after which outcomes including lipid levels, blood sugar, and blood pressure were assessed. Results:At baseline, 88.67% had desirable cholesterol levels and 11.33% had borderline levels. At follow-up, 20.67% had borderline levels and 79.33% had desirable levels, a statistically significant difference. Mean cholesterol levels decreased from 169.16 ± 19.70 mg/dl to 161.11 ± 27.60 mg/dl, also significant. For triglycerides, 92.67% had normal levels at baseline, decreasing to 65.33% at follow-up, with mean levels increasing from 131.33 ± 6.97 mg/dl to 139.12 ± 23.88 mg/dl. HDL levels showed improvement, with 84.67% having desirable levels at baseline compared to 43.33% at follow-up, with mean levels increasing from 41.09 ± 5.13 mg/dl to 50.71 ± 4.21 mg/dl. LDL levels decreased from 132.43 ± 16.86 mg/dl to 122.59 ± 16.93 mg/dl, with significant changes in distribution. Conclusions:Short-term steroid use for orthopedic conditions affects lipid profiles: Total cholesterol, LDL, and TG rise significantly, countered by increased HDL, offering protection. Long-term effects on lipid profiles and cardiovascular risk require further study. Risk of avascular necrosis due to subtle lipid profile changes also warrants investigation.

Association of Meibomian Gland Dysfunction with Serum Lipid Profile: A Hospital-based Cross-sectional study.

Meibomian gland dysfunction (MGD) frequently leads to ocular surface disease, yet its impact on patients' overall health is often overlooked. With increase in sedentary lifestyle, decreased physical activities and exposure to the digital world people have been facing increasing incidence of dyslipidemia. To investigate the association between serum lipid profile with severity of meibomian gland dysfunction. This was an observational, analytical, cross-sectional study carried out in the department of Ophthalmology of Shree Birendra hospital from January 2019 to January 2020. Oxford grading scale was used to grade the severity of MGD as none (grade 0), mild (grade I and II), moderate (grade III), and severe (grade IV and V). Serum lipid profile was obtained from all patients with MGD and analysis was carried out. Among 100 patients in this study majority of the participants were from the age group 61-70 (n, 40%) among whom 54 (54%) were females and 46 (46%) were males. Statistically significant association was noted between increase in total cholesterol, LDL, and TG levels with severity of MGD. However no significant association was found between HDL and the stage of MGD. This study demonstrated that patients with increasing severity of meibomian gland dysfunction had greater abnormalities in their serum lipid profiles with respect to total cholesterol, LDL and triglycerides.

A 4-week Randomized, Double-Blind, Placebo-Controlled Clinical Trial on the Use of Grape Seed Extract for Reducing Plasma Glucose, Lipid Profile, and Blood Pressure in Patients with Type 2 Diabetes Mellitus

: Diabetes mellitus (DM) is a common chronic disease that can significantly reduce life expectancy due to its complications. Grapes (Vitis vinifera L.) and their byproducts, including grape seeds, have been used worldwide for the treatment of various ailments. This clinical trial investigated the effect of grape seed extract (GSE) on fasting blood sugar (FBS), lipid profile (including total cholesterol, high-density lipoprotein [HDL], low-density lipoprotein [LDL], and triglycerides [TG]), and systolic blood pressure (SBP) and diastolic blood pressure (DBP) in patients with type 2 diabetes mellitus (T2DM). A total of 74 patients with T2DM were divided into two groups, including 38 patients in the treatment (GSE) group receiving 263.2 mg of GSE (standardized to 250 mg proanthocyanidin) twice daily and 36 patients in the placebo (control) group receiving 263.2 mg of the placebo twice daily for 30 days. At the end of the intervention, a statistically significant decrease was observed in SBP (from 125.83 ± 13.39 in the placebo group to 121.94 ± 7.49 in the treatment group, P = 0.002) and FBS (from 144.75 ± 30.82 in the placebo group to 129.87 ± 31.79 in the treatment group, P = 0.001). However, there were no significant changes in total cholesterol, HDL, LDL, TG, and DBP between the two groups. The use of Iranian GSE for a short period might lead to lower FBS and SBP. Therefore, GSE might play a significant role in improving blood pressure and fasting blood glucose levels in patients with T2DM.

Identification of predictive biomarkers for endometrial cancer diagnosis and treatment response monitoring using plasma metabolome profiling

BackgroundEndometrial cancer (EMC) is the most common female genital tract malignancy with an increasing prevalence in many countries including Japan, a fact that renders early detection and treatment necessary to protect health and fertility. Although early detection and treatment are necessary to further improve the prognosis of women with endometrial cancer, biomarkers that accurately reflect the pathophysiology of EMC patients are still unclear. Therefore, it is clinically critical to identify biomarkers to assess diagnosis and treatment efficacy to facilitate appropriate treatment and development of new therapies for EMC.MethodsIn this study, wide-targeted plasma metabolome analysis was performed to identify biomarkers for EMC diagnosis and the prediction of treatment responses. The absolute quantification of 628 metabolites in plasma samples from 142 patients with EMC was performed using ultra-high-performance liquid chromatography with tandem mass spectrometry.ResultsThe concentrations of 111 metabolites increased significantly, while the concentrations of 148 metabolites decreased significantly in patients with EMC compared to healthy controls. Specifically, LysoPC and TGs, including unsaturated fatty acids, were reduced in patients with stage IA EMC compared to healthy controls, indicating that these metabolic profiles could be used as early diagnostic markers of EMC. In contrast, blood levels of amino acids such as histidine and tryptophan decreased as the risk of recurrence increased and the stages of EMC advanced. Furthermore, a marked increase in total TG and a decrease in specific TGs and free fatty acids including polyunsaturated fatty acids levels were observed in patients with EMC. These results suggest that the polyunsaturated fatty acids in patients with EMC are crucial for disease progression.ConclusionsOur data identified specific metabolite profiles that reflect the pathogenesis of EMC and showed that these metabolites correlate with the risk of recurrence and disease stage. Analysis of changes in plasma metabolite profiles could be applied for the early diagnosis and monitoring of the course of treatment of EMC patients.

Increased Osteoclastogenesis in Absence of TG2 Is Reversed by Transglutaminase Inhibition-Evidence for the Role for TG1 in Osteoclast Formation.

Osteoclasts are multinucleated, bone-resorbing giant cells derived from monocyte-macrophage cell lines. Increased bone resorption results in loss of bone mass and osteoporosis. Osteoclast and bone marrow macrophages have been shown to express three TG enzymes (TG2, Factor XIII-A, and TG1) and TG activity to regulate osteoclast differentiation from bone marrow macrophages in vitro. In vivo and in vitro studies have demonstrated that the deletion of TG2 causes increased osteoclastogenesis and a significant loss of bone mass in mice (Tgm2-/- mice). Here, we confirm that TG2 deficiency results in increased osteoclastogenesis in vitro and show that this increase can be reversed by a TG inhibitor, NC9, suggesting that other TGs are responsible for driving osteoclastogenesis in the absence of TG2. An assessment of total TG activity with 5-(biotinamido)-pentylamine, as well as TG1 and FXIII-A activities using TG-specific Hitomi peptides (bK5 and bF11) in Tgm2-/- bone marrow flushes, bone marrow macrophages, and osteoclasts, showed a significant increase in total TG activity and TG1 activity. Factor XIII-A activity was unchanged. Aspartate proteases, such as cathepsins, are involved in the degradation of organic bone matrix and can be produced by osteoclasts. Moreover, Cathepsin D was shown in previous work to be increased in TG2-null cells and is known to activate TG1. We show that Pepstatin A, an aspartate protease inhibitor, blocks osteoclastogenesis in wild-type and Tgm2-/- cells and decreases TG1 activity in Tgm2-/- osteoclasts. Cathepsin D protein levels were unaltered in Tgm2-/-cells and its activity moderately but significantly increased. Tgm2-/- and Tgm2+/+ bone marrow macrophages and osteoclasts also expressed Cathepsin E, and Renin of the aspartate protease family, suggesting their potential involvement in this process. Our study brings further support to the observation that TGs are significant regulators of osteoclastogenesis and that the absence of TG2 can cause increased activity of other TGs, such as TG1.

Role of FGF21 in Gestational Diabetes Mellitus in a Sample of Egyptian Population

Abstract Background Gestational diabetes (GDM) is defined as carbohydrate intolerance that begins or is first recognized during pregnancy. Gestational diabetes mellitus (GDM) affects approximately 5 to 7% of pregnancies and is associated with increased risk for fetal overgrowth, cesarean delivery, birth trauma, and pre-eclampsia. Objective To investigate serum FGF21 concentrations in a sample of Egyptian patients with GDM as compared with healthy pregnant controls matched for gestational age And its correlation to clinical and biochemical changes (Glucose, insulin, lipids) and fetal wellbeing using obstetric ultrasound to assess fetal size and development of congenital malformation. Methods This cross-sectional study was conducted on 80 participants collected from Obstetrics and Gynecology clinic at Ain Shams University Hospital during the period from November 2019 to March 2021. Results Shows that there is statistically significant increase in FGF21 in female pregnants with GDM as compared to female pregnants without GDM. The high FGF21 level didn’t affect fetal wellbeing. And both groups had normal fetal development with absence of macrosomia and congenital malformation as shown on results of obstetric ultrasound. In addition, there is statistically significant positive correlation between fasting FGF21 level and each of total cholesterol, LDL, HDL and TG (R +ve, P.&amp;lt; 0.05). Conclusion This high level of FGF21 may be FGF21 resistance or compensatory response to the underlying metabolic stress. FGF21 may have reduced an even higher level of blood glucose than the measured one, blood glucose level could have been in the Range of 400-500 mg/dl instead the measured range 200-300 mg/dl. Obstetric ultrasound results revealed normal fetal wellbeing and no change in fetal size nor development of congenital malformation between both groups.

Establishment and validation of serum lipid-based nomogram for predicting the risk of prostate cancer

BackgroundThis study aimed to explore the value of combined serum lipids with clinical symptoms to diagnose prostate cancer (PCa), and to develop and validate a Nomogram and prediction model to better select patients at risk of PCa for prostate biopsy.MethodsRetrospective analysis of 548 patients who underwent prostate biopsies as a result of high serum prostate-specific antigen (PSA) levels or irregular digital rectal examinations (DRE) was conducted. The enrolled patients were randomly assigned to the training groups (n = 384, 70%) and validation groups (n = 164, 30%). To identify independent variables for PCa, serum lipids (TC, TG, HDL, LDL, apoA-1, and apoB) were taken into account in the multivariable logistic regression analyses of the training group, and established predictive models. After that, we evaluated prediction models with clinical markers using decision curves and the area under the curve (AUC). Based on training group data, a Nomogram was developed to predict PCa.Results210 (54.70%) of the patients in the training group were diagnosed with PCa. Multivariate regression analysis showed that total PSA, f/tPSA, PSA density (PSAD), TG, LDL, DRE, and TRUS were independent risk predictors of PCa. A prediction model utilizing a Nomogram was constructed with a cut-off value of 0.502. The training and validation groups achieved area under the curve (AUC) values of 0.846 and 0.814 respectively. According to the decision curve analysis (DCA), the prediction model yielded optimal overall net benefits in both the training and validation groups, which is better than the optimal net benefit of PSA alone. After comparing our developed prediction model with two domestic models and PCPT-RC, we found that our prediction model exhibited significantly superior predictive performance. Furthermore, in comparison with clinical indicators, our Nomogram’s ability to predict prostate cancer showed good estimation, suggesting its potential as a reliable tool for prognostication.ConclusionsThe prediction model and Nomogram, which utilize both blood lipid levels and clinical signs, demonstrated improved accuracy in predicting the risk of prostate cancer, and consequently can guide the selection of appropriate diagnostic strategies for each patient in a more personalized manner.

FEATURES OF THE COURSE OF ATRIAL FIBRILLATION IN PATIENTS WITH CORONAVIRUS DISEASE

Objective: The aim of our study was to study the course of the most common and potentially dangerous arrhythmia - atrial fibrillation (AF) in patients within 6 months after infection with COVID-19 (CI). Design and method: The study included 187 pts aged 62.5±0.9 years (47% men, 53% women). The first three groups formed 116 pts who had AF and underwent coronavirus infection (CI). The first group (G1) consisted of 36 patients who did not have AF before CI. The second group (G2) was represented by 25 patients in whom the transition of the paroxysmal form of AF into persistent, or persistent AF into its permanent form occurred. The third group (G3) consisted of 55 patients in whom the form of AF did not change. Results: Older age, female gender, hypertension, and coronary heart disease were clinical factors associated with the occurrence of AF or the worsening of the course of previously diagnosed AF in patients who underwent COVID-19. In addition, there were more pronounced signs of damage to the functional properties of the endothelium, the severity of systemic insufficient content in the blood of total cholesterol and TG, proatherogenic fractions of blood lipoproteins, which can be the cause of more clinically pronounced arrhythmias. Conclusions: A proportion of COVID-19 patients with newly diagnosed AF may have a pre-existing substrate for AF, and acute COVID-19 infection may be a trigger for the initiation of AF, consistent with a temporal relationship between new-onset AF and COVID-19 infection.

Influence of Georgian Saperavi Grapevine Shoots Extract on Lipid Profile in Diet-Induced Hypercholesterolemia in Rats

Hypercholesterolemia raises the risk of cardiovascular diseases (CVD), the leading cause of morbidity and mortality worldwide. Numerous clinical trials of lipid-lowering diets and medicines have been established for proper lipid profile regulation to reduce the significant level of pathological conditions of cardiovascular origin. Winemaking is a well-developed part of Georgian agriculture. Besides usual products such as grapes and wine, it is characterized by a significant amount of waste products like leaves and shoots, which can be used as a source of bio-active ingredients, namely polyphenols, which are wellknown to have hypocholesterolemia effects. Therefore, the decision was made to use polyphenols derived from wasted shoots and assess their potential protective and treating effect on lipid profile in the Vivo model of dietary-induced hyperlipidemia. During previous studies, 14 individual polyphenols were extracted and detected from Georgian grapevine shoots, and current research demonstrates the cholesterol-lowering effects of this polyphenol-rich extract in laboratory rats. Study results demonstrated the positive protective effect of Georgian Vitis Vinifera (Saperavi) polyphenols on the lipid profile of rats. Along with relatively low values of total cholesterol (5.47%), LDL (16%), and TG (9.9%), better results for HDL value were observed for the group receiving Saperavi shoots extract, than the group receiving only a hypercholesterolemia diet. Compared with a 10 mg/kg Atorvastatin daily dose, Saperavi shoots extract showed a smoother reduction of serum cholesterol; however, the normal range was reached. These findings support the potential of using currently wasted Georgian grapevine shoots as a source of biologically active ingredients. Based on the study results, Saperavi shoot extract can be considered a cost-effective candidate for phytomedicine supplementing and treating cardiovascular diseases associated with lipid profile disorders.

Effect of the Interaction between Seaweed Intake and LPL Polymorphisms on Metabolic Syndrome in Middle-Aged Korean Adults

This study aimed to examine the effect of the interaction between seaweed (laver, kelp, and sea mustard) intake and lipoprotein lipase gene (LPL) rs17482735 genotypes on the incidence of metabolic syndrome (MetS). The Korean Genome and Epidemiology Study (KoGES) data of Korean adults aged 40–69 years were used in this study. Information on seaweed intake was obtained from the food frequency questionnaire. To investigate the interaction between seaweed intake and LPL rs17482735 genotypes on the incidence of MetS, multivariable Cox proportional hazard models were used after adjusting for confounding variables. There was no significant association in women, but men with TG and TT genotypes of rs17482753 had lower incidence of MetS (HR 0.83, 95% CI 0.71–0.95, p-value = 0.01), low HDL-cholesterol levels (HR 0.81, 95% CI 0.69–0.95, p-value = 0.01), high triglyceride levels (HR 0.83, 95% CI 0.70–0.99, p-value = 0.0471), and high blood pressure (HR 0.79, 95% CI 0.67–0.93, p-value = 0.004). Furthermore, the incidence of MetS was lower in men with the highest laver and total seaweed intake and TG and TT genotypes of rs17482735 (HR 0.60, 95% CI 0.43–0.84; HR 0.57, 95% CI 0.41–0.79, respectively). High seaweed intake was negatively associated with MetS, suggesting that LPL genetic variations, particularly in men, may be helpful in preventing MetS. These results demonstrate that seaweed intake considering LPL genotypes may be beneficial for preventing and treating MetS.

EFFECT OF PASSIVE SMOKING ON LIPID LEVELS IN HEALTHY FEMALES - A RETROSPECTIVE ANALYSIS

Background &amp; Objectives: A retrospective study was performed to study the effects of passive smoking on the lipid levels of a healthy female. Materials and Methods: A retrospective case record based study was carried out amongst the healthy female attenders attending outpatient department for regular medical health check-up. Atotal of 200 women t the inclusion criteria. They were divided into two groups- A(case- at least one family member that smokes) and B (control non-smoking family members). The women in group were divided into three groups based on the number of cigarettes smoked by the family member. The lipid prole of the healthy women was then analysed. Results: signicant rise in the total cholesterol, LDL, TG and VLDL was noted. There was a statistically signicant reduction in HDL levels. This was also found to be directly proportional to the number of cigarettes smoked per day. Conclusion: Passive smoking is atherogenic in nature and can cause signicant derangement in the lipid levels in otherwise healthy women.

Serum bilirubin levels are negatively associated with atherogenic lipids in Saudi subjects with type 2 diabetes: A pilot study

&lt;b&gt;Background: &lt;/b&gt;Recent research has demonstrated the possible relevance of bilirubin in metabolic and cardiovascular disorders. Lipid abnormalities are a major problem that is related with an increased risk of cardiovascular disease in diabetics. This study examined the relationship between serum bilirubin and direct bilirubin concentrations and atherogenic lipids in patients with type 2 diabetes (T2DM).&lt;br /&gt; &lt;b&gt;Methods&lt;/b&gt;: This cross-sectional included 67 patients with type 2 diabetes and 39 matched healthy control. The lipid profile, including total cholesterol, HDL-C, and TG levels, fasting blood glucose, total bilirubin, direct bilirubin, ALT, AST, and ALP were measured using a dimension EXL clinical chemistry analyzer (Siemens Healthcare Diagnostics). Cholesterol in VLDL, LDL, and sdLDL were calculated from standard lipid assay results by the equations of Sampson et al.&lt;br /&gt; &lt;b&gt;Results&lt;/b&gt;: Serum bilirubin was lower in non T2DM subjects nearly significant (p=0.0.51) whereas direct bilirubin concentrations were lower in T2DM (p=0.008). ALT, AST, and ALP levels were higher in T2DM groups. The mean values of LDL-C, sdLDL-C, non HDL-C and VLDL-C were significantly increased in T2DM group and lower HDL-C. An inverse relationship could be observed with increase in serum total bilirubin and serum levels of LDL-C (r&lt;sup&gt;2&lt;/sup&gt;=0.139, p&amp;lt;0.005), sdLDL-C (r&lt;sup&gt;2&lt;/sup&gt;=0.137, p&amp;lt;0.005), VLDL-C (r&lt;sup&gt;2&lt;/sup&gt;=0.074, p&amp;lt;0.044), and non HDL-C (r&lt;sup&gt;2&lt;/sup&gt;=0.166, p&amp;lt;0.002) in T2DM group. The same inverse relationship was observed with serum direct bilirubin and serum levels of LDL-C (r&lt;sup&gt;2&lt;/sup&gt;=0.133, p&amp;lt;0.006), sdLDL-C (r&lt;sup&gt;2&lt;/sup&gt;=0.172, p&amp;lt;0.001), VLDL-C (r&lt;sup&gt;2&lt;/sup&gt;=0.118, p&amp;lt;0.01), and non HDL-C (r&lt;sup&gt;2&lt;/sup&gt;=0.182, p&amp;lt;0.001) in T2DM group.&lt;br /&gt; &lt;b&gt;Conclusions&lt;/b&gt;: A significant negative association was found between serum bilirubin levels and direct serum bilirubin with atherogenic lipids, suggesting that serum bilirubin may protect T2DM patients from development of cardiovascular disease. These findings indicate the need for additional research in a large cohort.
