The incidence of total hip arthroplasty after hip arthroscopy in osteoarthritic patients

ObjectiveTo assess the incidence of total hip arthroplasty (THA) in osteoarthritic patients who were treated by arthroscopic debridement and to evaluate factors that might influence the time interval from the first hip arthroscopy to THA.DesignRetrospective clinical seriesMethodsFollow-up data and surgical reports were retrieved from 564 records of osteoarthritic patients that have had hip arthroscopy between the years 2002 to 2009 with a mean follow-up time of 3.2 years (range, 1-6.4 years). The time interval between the first hip arthroscopy to THA was modelled as a function of patient age; level of cartilage damage; procedures performed and repeated arthroscopies with the use of multivariate regression analysis.ResultsNinety (16%) of all participants eventually required THA. The awaiting time from the first arthroscopy to a hip replacement was found to be longer in patients younger than 55 years and in a milder osteoarthritic stage. Patients that experienced repeated hip scopes had a longer time to THA than those with only a single procedure. Procedures performed concomitant with debridement and lavage did not affect the time interval to THA.ConclusionsIn our series of arthroscopic treatment of hip osteoarthritis, 16% required THA over a period of 7 years. Factors that influence the time to arthroplasty were age, degree of osteoarthritis and recurrent procedures.

Geographical variation in incidence of primary total hip arthroplasty: a population-based analysis of 34,642 replacements

Considerable variation in total hip arthroplasty (THA) incidence between regions has been described. The aim of this study was to evaluate geographical variation in the incidence of primary THA for OA in Finland and to analyze variables associated with this variation. Using Hospital Discharge Register, 34,642 THAs performed for primary OA over the 1998 and 2005 period were identified. Incidence indices for 21 hospital administrative regions were determined. Logistic regression analysis and generalized linear models were used for studying the association of potential explanatory factors for the variation in the incidence of THA. Incidence indices of THA varied from 1.9- to 3.0-fold during the study period. A high ratio of primary THA for primary OA to primary THA for any reason was associated with a high absolute incidence of primary THA (P < 0.001). Neither average incomes nor morbidity were associated with the incidence of THA. When hip surgery was performed on a larger scale, threshold for performing THAs due to primary OA was higher. Socio-economic status had no apparent effect on THA rate. Surgeon decision-making related factors influence THA rates when there are only a few surgeons responsible for performing THAs in a region.

Hospitalizations for Total Hip Arthroplasty after Renal Transplantation in the United States

The national incidence of and factors associated with total hip arthroplasty in renal transplant recipients has not been reported. We conducted an historical cohort study of 42096 renal transplant recipients in the United States between 1 July 1994 and 30 June 1998. Primary outcomes were associations with hospitalizations for a primary discharge code of total hip arthroplasty (ICD9 procedure code 81.51x) within 3 years after renal transplant using Cox regression. Renal transplant recipients had a cumulative incidence of total hip arthroplasty of 5.1 episodes/1000 person-years, which is 5-8 times higher than reported in the general population. Avascular necrosis of the hip was the most frequent primary diagnosis associated with total hip arthroplasty in this population (72% of cases). Repeat surgeries were performed in 27% of patients with avascular necrosis, vs. 15% with other diagnoses. Total hip arthroplasty was more frequent in transplant recipients who were older, African American, or who experienced allograft rejection. Mortality after total hip arthroplasty was 0.21% at 30 days and 15% at 3 years, similar to the mortality of all transplant recipients. The most common indication for total hip arthroplasty after renal transplant is avascular necrosis of the hip, in contrast to the general population. Although repeat surgeries are common, total hip arthroplasty is well tolerated and is not associated with increased mortality in this population.

Changing incidence of primary total hip arthroplasty and total knee arthroplasty for primary osteoarthritis

This study reports on the incidence of primary total hip arthroplasty (THA) and total knee arthroplasty (TKA) for primary osteoarthritis in Australia. Age-specific and gender-specific numbers for Australia, 1994 through 1998, and South Australia, 1988 through 1998, were obtained. Incidences were calculated per 100,000 population. In Australia, primary THA increased from 50.9/105 (1994) to 60.9/105 (1998). TKA increased from 56.4/105 to 76.8/105. Stratified by age and gender, changes in incidence for South Australia with respect to time were tested using regression analysis. South Australia showed a significant increase in the overall incidence of THA (P=.012) and TKA (P<.001), although this was not uniform across all age groups. No significant gender differences were found. The incidence of THA is increasing, and the incidence of TKA is increasing at a greater rate.

Incidence and Outcomes of Total Hip Arthroplasty among U.S. Army Aviators

Among Army aviators, the incidence of total hip arthroplasty (THA) is unknown. This study analyzes the incidence and aeromedical disposition of THA among Army aviators. The U.S. Army Epidemiology Data Register was queried for a 10-year period from calendar year 1987 to 1996. The aviators selected for this study cohort were all qualified and on active flight status before undergoing THA. Data collected included age, gender, diagnoses, and aeromedical dispositions. There were 214,003 aviator-years of observation. Eleven aviators underwent 14 THAs. The overall incidence of THA was 0.05 per 1,000 aviator-years of observation. Of the 11 aviators who underwent THA during the study period, 4 received aeromedical suspension from flying duties (36%). THA is a rare medical event among Army aircrew members. Most are able to return to full flying duties with a waiver. Aircrew members younger than 50 years with THA are more likely to be suspended from aviation duties.

TREATMENT OF OSTEO-ARTHRITIS OF THE HIP WITH INDOMETHACIN. A CONTROLLED CLINICAL TRIAL.

<h3>Objective</h3> The aim of this prospective cohort study was to determine whether dairy product consumption was associated with the incidence of total hip arthroplasty for osteoarthritis (OA). <h3>Methods</h3> There were 38,924 participants from the Melbourne Collaborative Cohort Study who had dairy product consumption recorded in 1990–1994. The incidence of total hip arthroplasty for OA during 2001–2013 was determined by linking cohort records to the Australian Orthopaedic Association National Joint Replacement Registry. <h3>Results</h3> Over an average of 11.8 years of followup, 1505 total hip arthroplasties for OA were identified (524 in men, 981 in women). In men, a 1 SD increase in dairy product consumption was associated with a 21% increased incidence of total hip arthroplasty for OA (HR 1.21, 95% CI 1.10–1.33), with a dose-response relationship observed for quartiles of dairy product consumption (p for trend = 0.001). These results were independent of age, body mass index, country of birth, education, smoking status, vigorous physical activity, calcium supplementation, energy consumption, circulating 25-hydroxy vitamin D, hypertension, and diabetes. No significant association was observed for women (HR 1.02, 95% CI 0.95–1.09). <h3>Conclusion</h3> Increasing dairy product consumption was associated with an increased risk of total hip arthroplasty for men with OA, with no significant association observed for women. Understanding the mechanisms may help identify strategies to prevent hip OA, particularly for men.
