Microbial Adhesion to Different Thermoplastic Denture Base Materials in Kennedy Class I Partially Edentulous Patients.

Introduction Since the polished and fitting surface of the denture base may promote the colonization of microorganisms, it is essential to know how the different types of denture bases prevent or encourage the adhesion of microorganisms. This study aimed to compare the microbial adhesion to the polished and fitting surfaces of thermoplastic nylon, thermoplastic acetal, and thermoplastic acrylic denture bases in Kennedy Class Ⅰ, partially edentulous patients. Materials and methods Thirteen patients were included in the study. The group consisted of eight males (61.54%) and five females (38.46%), with an age range of 41-50 years (mean age 46.1 years). Three types (groups) of removable partial dentures will be made for each patient using different thermoplastic denture base materials: thermoplastic nylon; thermoplastic acetal; and thermoplastic acrylic. The polished and fitting surfaces of the denture bases were swabbed after a one-month follow-up period. Microbial adhesion was evaluated by counting the microorganisms' colony-forming units (CFU) in the collected specimens. The data were collected and statistically analyzed. Results The study revealed no statistically significant difference in microbial adhesion to both polished and fitting surfaces between all types of studied thermoplastic denture base materials. However, the results showed that for the polished surface, the microbial adhesion median of thermoplastic acrylic denture base (40.5 CC x 102/ml) was higher than that of thermoplastic acetal (29.0 CC x 102/ml) and thermoplastic nylon (16.0 CC x 102/ml). Regarding the fitting surface, the microbial adhesion median of thermoplastic acrylic (51.0 CC x 102/ml) is higher than that of thermoplastic acetal (41.0 CC x 102/ml) and thermoplastic nylon (23.0 CC x 102/ml). Conclusion The thermoplastic nylon denture base materials showed less microbial adhesion among the studied thermoplastic materials, so it may be recommended to be used as a denture base material for individuals at high risk of denture stomatitis.

Evaluation Of Silano Adhesive Efficiency in Improvement of Shear Binding Strength of Synthetic Teeth to Thermo-Plastic Denture Material

Background: One of the most serious problems concerning denture performance is the possibility of prosthetic tooth displacement due to increased chewing efficiency. The nature of the mechanical bonding between thermoplastic denture base materials and acrylic teeth has aggravated the problem of tooth debonding. The study aiming to examine the strength of shear bond between prosthetic teeth and thermoplastic base material utilizing silano (silane coupling agent) and mechanical surface modifications (Alumin-ablasting, T. Shape hole) of tooth ridge lap pre and post (100 hrs.) of accelerated ageing conditions. Materials and method: The surface treatments of (120) maxillary central incisors were chosen and grouped into 6 groups of 20 samples each. Each group split into before and after (100hrs.) according to ageing (n=10). Measurement of shear bonding strength applied by a universal testing equipment with 0.5mm/min crosshead speed. Statistical analyses were done using one-way ANOVA test and Tukey’s (HSD) test. Results: Statistics showed high significant variation in average binding force after several surface manipulations in contrast to the control group at (P < 0.000). Regarding surface manipulations, dual treatment of “Alumina-blasting + silano “revealed significant elevation of binding strength in compare to “alumina-blasting” alone (P < 0.001). Similarly, with “T. hole + silano” yielded in greatly increased bonding strength compared to using “T. hole” alone (P < 0.001). a great reduction in bonding strength value was indicated along all treated groups after ageing process (P < 0.000) except the group treated with “T. Hole + silano” show non-significant variations at (p = 0.112). Conclusion: combination treatment (chemo-mechanically) with “T. Hole + silano” improve shear binding strength and not effected with ageing process.

Subject and Methods: Tenpatients having completely edentulous maxillary ridge and bilateral canine in the lowerarch were selected. Root canal treatment and preparation were performed to obtainnon coping abutment with amalgam plugs that covered with over denture. Maxillarycomplete dentures were constructed for all patients. Patients were divided into two equalgroups. Group I: received thermoplastic mandibular over denture. Group II: receivedconventional heat cure acrylic resin mandibular over denture. Clinical and radiographic evaluation were performed at denture insertion , after 6month and after 9monthfor both mandibular over denture. Results: Statistical analysis of bone height showedthat group I (thermoplastic denture) has less bone resorption than group II (acrylicdenture) which was statically significant. Statistical analysis of change in pocket depthshowed that group I (thermoplastic denture) has less change in the pocket depth thangroup II (acrylic denture) which was not statistically significant. Conclusion: Withinthe limitation of this study thermoplastic over denture preserved pocket depth and boneheight more than conventional heat cured acrylic resin.

Thermoplastic dentures are mainly used for the therapy of partial edentulism, but not only. They are made of flexible materials, that are more resistant to breaking, and thinner than classic resin prostheses or prostheses with metal con- struction. Due to their chemical composition and the man- ner of fabrication (by injection), they are suitable for cases that show allergies to the residual monomer of classic RPD or to allergens from the metals used for the fabrication of RMD with metal construction. They are more aesthetic because their clasps are made of the same material as the prosthesis, with the color of the tooth or gingiva. Not only RPD can be fabricated from thermoplastic materials, but also full dentures, clasps, fiber-reinforced fixed partial den- tures, temporary crowns and bridges, obturators, speech therapy devices, briquettes and orthodontic retainers, trays for taking a functional impression, occlusal splints, anti “sleep apnea” devices, gingival simulators and pros- thetic implant holders (abutments).

The Effect of Pineapple Peel Extract As a Denture Cleanser on Flexural Strength of Nylon Thermoplastic Denture Base Materials

Background: Nylon thermoplastic has been widely used as denture base material because of its esthetic property, high flexibility, and flexural strength. It is very important to maintain the removable dentures’ cleanliness, which is frequently done with the chemical technique by immersing the denture in a solution. The composition of malic acid and citric acid from pineapple peel extract has antibacterial and antifungal properties, so it can be used not only as a denture cleanser but also affect the flexural strength of denture base material. Objective: To determine the effect of pineapple peel extract as a denture cleanser on the flexural strength of nylon thermoplastic denture base material. Method: This is experimental laboratory research with a post-test-only group design using 36 samples, namely 18 Valplast samples and 18 Lucitone FRS measuring 64mm X 10mm X 2.5mm divided into 4 groups (Valplast control, Valplast experiment, Lucitone FRS control, and Lucitone FRS experiment). The samples were treated by immersing them in sterile distilled water as control, and in 3.5% pineapple peel extract for 15 minutes 3 times a day for 10 days as treatment. Flexural strength measurement using Universal Testing Machine. The data obtained were analyzed by the One Way ANOVA test. Result: There was a decrease in flexural strength in the Valplast and Lucitone FRS groups immersed in 3.5% pineapple peel extract. Lucitone FRS groups had higher flexural strength than Valplast’s. Conclusion: There is a decrease in flexural strength of nylon thermoplastic denture base after being immersed in 3.5% pineapple peel extract.

Relevance. Theadaptation to removable partial dentures is a multilevel process. At the beginning of usage, removable dentures irritate the tissues that interact directly with the prosthesis and are perceived as a foreign object by the patient. The irritating effect of prostheses is caused by their non-physiological pressure on the tissues that interact directly with the prosthesis, elimination of the bases plastic monomer, and change of the microbiological status of the oral cavity. The goal of the study. To determine the features of clinical and functional adaptation of patients in orthopedic rehabilitation with thermoplastic and acrylic prostheses. Materials and methods. For this clinical study, 60 patients with partial absence of teeth were examined. The patients underwent orthopedic rehabilitation with partial removable dentures (1st group – 30 patients used thermoplastic removable dentures; 2nd group – 30 patients used acrylic removable dentures).The number of patient visits for each of the studied groups for the correction in the adaptation period to fix partial removable dentures with bases of thermoplastic and acrylic materials treated with polishing pastes Blue Shine, ThermoGloss was analyzed. Results. Clinical studies have shown clear advantages of thermoplastic prostheses. The obtained data indicate a much smaller negative impact on the removable denture bases made of thermoplastic polymer compared to the acrylic polymer bases. Also, thermoplastic dentures show better results in patient adaptation. Patients with removable dentures made of thermoplastic and acrylic polymers showed better clinical results if the surface of the dentures was treated with ThermoGloss polishing paste. Conclusions. Using the ThermoGloss polishing paste for the final polishing of polymers has reduced the inflammatory response of the mucous membrane of the tissues that interact directly with the prosthesis and has increased its resistance to the negative effects of the partial removable dentures during adaptation. In turn, this helped reduce the number of visits to correct the prosthesis, as well as reduce the time of adaptation. The Relationship of Modulus of Elasticity and Micro-Hardness of Denture Bases to Pressure Transmission under the Thermoplastic Nylon Denture Base

Introduction: The purpose of this study was to evaluate the relationship of modulus of elasticity and micro-hardness of denture bases to pressure transmission under the thermoplastic nylon denture base.Material and Methods: Mandibular first molar acrylic denture teeth (Bioeco) embedded on 15x15x3 mm3 thermoplastic nylon (TCS and BioTone) and poly methyl methacrylate denture bases were evaluated (n=9). A 100N force applied on the specimen using universal testing machine and the pressure transmitted under the specimen observed using pressure transducer. The modulus of the elasticity (n=16) measured on 64x10x3,3 mm3 specimens and micro-hardness (n=16) measured on 10x10x3 mm3 specimens. Data were statistically analyzed using One-way Anova followed by Tukey HSD post hoc test. Pearson correlation test used to evaluate the relationship of modulus of elasticity and micro-hardness on denture bases to pressure transmission (α ≥ 0,05).Results: The results showed that poly methyl methacrylate has the highest pressure transmission compared to others. There was a significant relationship between modulus of elasticity and micro-hardness to pressure transmission under the denture base.Discussion: This study can help dentists to determine the design of dentures, especially in the selection of denture base materials.

Effect of Two Different Cleansing Agents on Color Stability of Thermoplastic Denture Base Material

Purpose: Researchers tried to improve the mechanical properties of poly methyl methacrylate(PMMA) denture bases. Development of alternative materials such as thermoplastic denture basematerials as polyamide had also been reported in the literature, however, it has not been well studiedregarding color stability. The aim of this in vitro study was to evaluate the effect of two differentcleansing agents on color stability of thermoplastic denture base material. Materials and methods:Thirty samples were fabricated from polyamide (Vertex Thermosens thermoplastic denture basematerial). Samples were divided into Three groups: 10 each. Group I (control group): soaked indistilled water, Group II: soaked in Corega cleansing agent and Group III: soaked in Efferdentcleansing agent. Results: Statistical non-significant results were found. A slight non-significanteffect on color stability was found more with Efferdent than Corega cleansing agent and water.Conclusion: Disinfecting polyamide denture base material (Vertex Thermosens) could be donesafely with Corega and Efferdent cleansing agents.

Effect of Occlusal Reactive and Thermoplastic Denture Bases on Supporting Structures in Lower Distal Extension Partial Dentures

Aim of study: was to evaluate and compare the effect of occlusal reactive versus thermoplastic partial denture bases on the supporting alveolar structure of mandibular distal extension partial dentures cases. Materials and methods: Ten partially edentulous patients were precisely selected to avoid factors that may adversely affect the results of this study. Removable partial dentures were constructed for all patients following the same procedures and the same design. According to denture base material the selected patients were randomly divided into two equal groups: Group I: five patients received cobalt-chromium partial denture with thermoplastic denture base. Group II: five patients received cobalt-chromium partial denture with occlusal reactive base. The patients were evaluated both clinically and radiographically, clinical assessment of the abutment was carried out through measuring its pocket depth, while radiographic assessment was done by recording bone height change distal to abutment teeth and bone high change of the residual ridge. Results: The results of this study showed that there were a significant differences between flexible and occlusal reactive bases on their effect on supporting structures. Conclusion: The occlusal reactive base exhibited better effect than flexible base regarding bone resorption in abutment as well as residual ridge

To Compare and Evaluate the Bond Strength of Acrylic Denture Teeth with Heat Cure Acrylic Resin and Thermoplastic Denture Base Material after Different Surface Treatments of Acrylic Teeth - An In Vitro Study

Background: Denture teeth made of acrylic resin are preferred because they chemically bond to denture base material but the problem of debonding is also common with the use of acrylic teeth. Many attempts have been done to improve bond strength of acrylic teeth which involves mechanical and chemical means. With the use of recently introduced thermoplastic denture base materials, the problem of tooth debonding has been increased due to the mechanical nature of the bond between these materials and the acrylic teeth. There is limited literature on bond strength of acrylic teeth and flexible denture materials and the methods to enhance it. The present study evaluates and compares the bond strength between acrylic teeth and heat cure resins and thermoplastic resins after different known denture tooth surface treatment methods, including use of methyl methacrylate; sandblasting and prefabricated vertical groove. Method: A total of 180 maxillary right central incisors were selected and divided into six groups of 30 samples each, according to the surface treatments and denture base material (heat cure or thermoplastic resins) used for processing. The bond strength of all specimens were tested in Universal Testing Machine. The obtained data were subjected to statistical analysis. Result: Results showed that there was statistical significant difference in mean bond strength after various surface treatments and processing of acrylic teeth with either heat cure or flexible denture material. Interpretation & Conclusion: The present findings suggest that vertical groove in the ridge lap area has highest bond strength amongst the groups processed with flexible denture material, and treatment of specimens with methyl methacrylate monomer for 5 sec has highest bond strength amongst all groups.

Effect of Two Cleansing Agents on Water Sorption and Solubility of Two Thermoplastic Denture Base Materials

Aim: Some patients experience adverse reactions to poly (methyl methacrylate)-based (PMMA) dentures. Polyamide (PA) as an alternative to PMMA has, however, not been well documented regarding water sorption and water solubility. The aim of this in vitro study was to evaluate the effect of two different cleansing agents on water sorption and solubility of two different thermoplastic denture base materials. Materials and methods: A total of 60 samples were fabricated from two different thermoplastic denture base materials (i.e. vertex thermosens and breflex). The samples were divided into 3 groups. Each group was including 20 samples, 10 samples from each denture base material. Samples of Group I were subjected to the distilled water as a control group. While samples of Group II were subjected to the Corega as a cleansing agent and samples of Group III were subjected to Fitty dent as the other cleansing agent. Effect of the two cleansing agents on the two-different thermoplastic denture base materials were evaluated and compared with regards water sorption and solubility. Results: Non-significant results were found regarding water sorption and solubility. The fittydent cleansing agent had a slight more non-significant effect on color stability of vertex thermosens and breflex denture base materials than corega cleansing agent and water. The corega and fittydent cleansing agents had a nonsignificant effect on water sorption and solubility of vertex thermosens and breflex denture base materials. Conclusion: Corega and fittydent cleansing agents could be used safely for disinfecting denture base materials (vertex thermosens and breflex) as they had non-significant effect on color stability, water sorbtion and solubility.

Bite Force Evaluation in Complete Denture Wearer with Different Denture Base Materials: A Randomized Controlled Clinical Trial.

Aim:The aim of this study is to evaluate the maximum bite force (MBF) with different denture base materials in completely edentulous patients.Materials and Methods:MBF was evaluated in randomly selected 100 completely edentulous patients. Patients were selected from the Outpatient Clinic, College of Dentistry, Aljouf University, Saudi Arabia. The patients were randomly divided into two equal groups: Group I (control group): Patient received a conventional heat-cured acrylic complete denture (Vertex™ Regular, Vertex-Dental B.V., The Netherlands) and Group II (experimental group): Patient received a thermoplastic complete denture (Vertex™ ThermoSens, Vertex-Dental B.V., The Netherlands). MBF measurements were taken at the time of new denture placement and after 6 months of denture use. Statistics were analyzed using independent t-test to compare the MBF measurements between both groups.Results:At the time of denture placement, there was no significant difference in bite force measurements between both groups. Bite force is increasing considerably after 6 months of denture use, and it was higher than MBF recorded at the time of new prosthesis placement in the same group. The MBF values were considerably higher in patients with a thermoplastic denture than patients with conventional acrylic denture with statistically significant difference after 6 months of denture use.Conclusion:After 6 months of denture use, patients with a thermoplastic denture have a higher biting force than patients with a conventional acrylic denture. Therefore, it could be considered a treatment plan option according to the ridge nature and quality to solve the problem of diminished bite force in old age patients.

MASTICATORY EFFICIENCY OF CANINE GUIDANCE OCCLUSION AND BILATERAL BALANCED OCCLUSION IN THERMOPLASTIC COMPLETE DENTURES

ABSTRACT The purpose of this study was to evaluate the masticatory efficiency of two occlusal schemes; canine guidance occlusion and bilateral balanced occlusion in thermoplastic complete dentures. Ten male completely edentulous patients whose ages ranged from 55 to 70 years were selected. The selected patients were free from any temporomandibular joint disease in order not to affect their masticatory efficiency. Half of the selected patients received a complete denture with canine guidance occlusal scheme and the other half of the patients received a complete denture with bilateral balanced occlusal scheme. One month was left as an adaptation period then the masticatory efficiency of the first occlusal scheme was tested, then the occlusal scheme was changed to the other one. After another one month, the masticatory efficiency of the second occlusal scheme was tested. The data were collected, tabulated and analyzed by using Paired-sample t-test. The results of this study showed that there was no significant difference between the canine guidance occlusal scheme and the bilateral balanced occlusal scheme in the number of strokes, the number of swallows and the time (in seconds) elapsed during chewing all test foods (banana, peanut and carrot). It was concluded that both bilateral balanced occlusion and canine guidance occlusion can be used successfully in thermoplastic complete dentures without affecting the masticatory efficiency. A Comparative Assessment of the Surface Roughness of Thermoplastic Denture Base Resins Following Adjustment and Re-Polishing

Purpose: This study assessed the roughness of two injection-molded, thermoplastic materials used for denture bases compared with a polyamide material and compression molded Polymethylmethacrylate (PMMA) after the adjustment and re-polishing with either a laboratory protocol or a chair side protocol. Methods: Forty specimens, each of PMMA, Valplast, DuraFlex, Dura Cetal were fabricated and finished according to individual manufactures’ instructions. These materials were adjusted with tungsten carbide (TC) burs to mimic gross adjustments, and then re-polished either on a lathe or bonded silicon carbide (B-SC). Following instrumentation, the specimens were assessed using contact profilometry and scanning electron microscopy. Two-factor ANOVA was used to determine significant differences in mean surface roughness (Ra and Rmax), with included factors being material type and re-polishing regimen. Results: Mean Ra values ranged from 0.26 (DuraFlex control) to 1.82 (Valplast adjusted with TC burs). Mean Rmax values ranged from 1.88 (Dura Flex control) to 13.76 (Valplast adjusted with TC burs). Two-factor ANOVA revealed that interaction of both factors was significant (p < 0.05) for Ra and Rmax. There was a statistically significant increase in both Ra (p < 0.05) and Rmax (p < 0.05) for all material types following the gross adjustment. With the exception of DuraFlex, re-polishing of samples that were previously adjusted with TC burs, on the dental lathe produced surfaces that were comparable to control samples. Conclusion: Adjustment of DuraFlex should be kept to a minimum since the adjustment produced the significant surface detriment that could not be corrected with either of the polishing regimens.

Impact of Thermoplastic Acrylic Denture Base Versus Conventional Base on the Patient Satisfaction in Implant Supported Mandibular Overdenture: A Systematic Review

Objective: The aim of this systematic review was to study the impact of thermoplastic acrylic denture base on the patient satisfaction in implant supported mandibular overdenture. Methods: Two electronic databases and hand searching were performed through May 2016 without limitations. The scientific terms: “Dental prosthesis”, “Implant supported”, “Denture bases”, “Overdenture”, “Thermoplastic acrylic resin”, “Acrylic bases’ and “Flexible acrylic resin” were used in this study. Initially, screening was performed for the retrieved articles through title and abstract after that, a full-text assessment was done to those articles that fulfilled the inclusion criteria. Findings: Sixty one potentially eligible studies were retrieved from the database search, after duplicate removal the studies became thirty-two. Titles and abstracts that fulfilled the inclusion criteria were selected for full-text assessment. Finally, one study met the inclusion criteria, and so its references were also screened. Applications/Improvements: This study would increase the application of the versacrylic resin to the denture base as a new material to utilize all its merits to improve the patient satisfaction.
