Territorial Post-Revascularization Remodeling of Chronic Coronary Syndromes: Cardiac Magnetic Resonance Feature Tracking Study.

MRI feature-tracking (MRI-FT) can accurately assess ventricular myocardial deformation and regional function and may be a better predictor of mortality than ejection fraction and infarct extension. However, role of MRI-FT in assessing coronary revascularization is unclear. To assess coronary revascularization effect on territorial left ventricle (LV) function of chronic coronary syndrome (CCS) patients by MRI-FT. Prospective. 50 CCS patients (age: 62.22 ± 8.70 years) scheduled for elective percutaneous coronary intervention (PCI), and 30 healthy controls (age: 35.33 ± 11.57 years). 1.5T with balanced steady-state free precession cine sequence. Global and segmental peak systolic longitudinal, circumferential, and radial myocardial strains were quantified in both patient and healthy control groups by an experienced operator using dedicated software. Patients were studied both pre-PCI and 6-month post-PCI and LV territorial myocardial strain values were calculated by averaging the segmental values of each revascularized territory. Student's t-test, paired t-test, Mann Whitney test, and Wilcoxon signed ranks test. Significance was judged at the 5% level. Territorial longitudinal strain showed significant 6-month post-PCI improvement in the left anterior descending (LAD) and right coronary artery (RCA) territories, but there was not in the left circumflex (LCX) territory (LAD: mean - 11.41% ± 3.45% pre, -13.01% ± 3.53% post; RCA: mean - 11.11% ± 2.65% pre, -13.25% ± 2.81% post; and LCX: mean - 15.43% ± 3.97% pre, -16.17% ± 4.38% post, P = 0.215). Territorial circumferential strain showed significant post-PCI improvement in all revascularized territories (LAD: mean - 13.73% ± 6.56% pre, -16.98% ± 6.01% post; LCX: mean - 13.23% ± 4.23% pre, -16.34% ± 3.45% post; and RCA: mean - 11.24% ± 3.36% pre, -13.80% ± 3.51% post). Territorial radial strain showed no significant post-PCI improvement (LAD: mean 22.73% ± 12.38% pre, 21.79% ± 11.55% post, P = 0.541; LCX: mean 27.73% ± 7.95% pre, 29.0% ± 7.25% post, P = 0.264; and RCA: mean 36.68% ± 11.10% pre, 31.75% ± 10.95% post, P = 0.208). Left ventricular longitudinal strain: a useful tool in the evaluation of patients with Takotsubo syndrome

Introduction In contrast with the usual type of Takotsubo syndrome (TS), where apical wall motion abnormality occurs, the less common patterns (mid-ventricular, basal, and focal) are often misdiagnosed. Purpose In this retrospective study we examined the role of global (GLS) and territorial longitudinal strain with speckle tracking in the evaluation of patients with TS. Methods Patients with TS were examined at the echocardiography laboratory of our general hospital, from August 2019 until August 2022. Left ventricular ejection fraction (LVEF) and GLS were calculated and territorial longitudinal strain was assessed. Results Out of 21 patients with TS (19 females), of median age 67.5 years (range 33.0 – 87.0 years), 11 (52.4%) had apical distribution, 6 (28.6%) mid-ventricular, 2 (9.5%) basal and 2 (9.5%) focal. Of those, 3 were excluded from the study due to insufficient echocardiographic image quality. Patients with apical pattern had median LVEF 37% (range 23–51%) and GLS −7.6% (−5.8 to −12.5%), patients with mid-ventricular pattern had median LVEF 40% (35–50%) and GLS −14.2% (−9.3 to −16.5%) and patients with basal pattern had median LVEF 52% (47–56%) and GLS −16.5% (−14.6 to −18.4%). In the first case of focal distribution, wall motion abnormality was found at the anterior septal wall, and LVEF was calculated 46% and GLS −12.8%. In the second case, abnormality was found at the inferoposterior wall and LVEF was calculated 56% and GLS −17.3%. Patients with apical TS had median strain of basal territory −11.7%, mid territory −3.7% and apical territory +3.6%. Patients with mid ventricular TS had median strain of basal territory −10.2%, mid territory −6.5% and apical territory −19.7%. Patients with basal TS had median strain of basal territory −4.3%, mid territory −16.4% and apical territory −29.1%. Conclusion GLS and territorial longitudinal strain reflect the reduction in left ventricular systolic function and the distribution of wall motion abnormalities in patients with TS, which further supports the initial diagnosis.

Accuracy of Global Longitudinal and Territorial Longitudinal Strain in Determining Myocardial Viability in Comparison to Single-Photon Emission Computed Tomography in Out of Window Period Anterior Wall Myocardial Infarction Patients.

Background: To determine the accuracy of global longitudinal strain and territorial longitudinal strain in determining myocardial viability in comparison to single-photon emission computed tomography in out of window period anterior wall myocardial infarction patients.Methods: This was a single-center, prospective study carried out in a tertiary care center in northern India. All patients presenting with anterior wall myocardial infarction–out of window period without ongoing chest pain and akinetic left-anterior descending territory on echocardiography were recruited. All patients underwent strain echocardiography and the determination of both global longitudinal strain and territorial longitudinal strain within 12-48 hours of anterior wall myocardial infarction. In addition, all underwent single-photon emission computed tomography to determine the viability status of the anterior myocardium.Results: Fifty-one patients of anterior wall myocardial infarction–out of window period were enrolled and underwent strain imaging with speckle tracking and single-photon emission computed tomography for viability determination. Gobal longitudinal strain and territorial longitudinal strain were significantly reduced in patients with nonviable myocardium (P < .001). On receiver-operating curves, a gobal longitudinal strain of <10.45% had a sensitivity of 77.8% and specificity of 93.9% (AUC = 0.889) in predicting nonviability on single-photon emission computed tomography. Similarly, a territorial longitudinal strain of <7.60% had a sensitivity of 77.8% and specificity of 84.8% (AUC = 0.825) in predicting nonviability.Conclusions: Treatment strategies in patients presenting with anterior wall myocardial infarction, outside the window period is largely guided by the hemodynamic status and influenced by the viability status of the myocardium. Strain echocardiography using speckle tracking provides gobal longitudinal strain and territorial longitudinal strain, both of which have good sensitivity and specificity in predicting viability and can be performed safely and quickly in high-risk group of patients.

Coronary-specific quantification of myocardial deformation by strain echocardiography may disclose the culprit vessel in patients with non-ST-segment elevation acute coronary syndrome.

AimsTo compare the diagnostic accuracy of speckle tracking echocardiography technique using territorial longitudinal strain (TLS) for the detection of culprit vessel vs. vessel-specific wall motion score index (WMSI) in non-ST-segment elevation–acute coronary syndrome (NSTE-ACS) patients scheduled for invasive coronary angiography (ICA).Methods and resultsOne hundred and eighty-three patients (mean age: 66 ± 12 years, male: 71%) diagnosed with NSTE-ACS underwent echocardiography evaluation at hospital admission and ICA within 24 h. Culprit vessels were left anterior descending (LAD), left circumflex (CX), and right coronary arteries (RCAs) in 38.5%, 39.6%, and 21.4%, respectively. An increase of affected vessels [1-, 2-, and 3-vessel coronary artery disease (CAD)] was associated with increased WMSI and TLS values. There was a statistically significant difference of both WMSI-LAD, WMSI-CX, WMSI-RCA and TLS-LAD, TLS-CX, TLS-RCA of myocardial segments with underlying severe CAD compared to no CAD (P = 0.001 and P < 0.001, respectively). Moreover, a significant difference of TLS-LAD, TLS-CX, TLS-RCA, and WMSI-CX of myocardial segments with an underlying culprit vessel compared to non-culprit vessels (P < 0.001, P < 0.001, P = 0.022, and P < 0.001, respectively) was identified. WMSI-LAD and WMSI-RCA did not show statistical significant differences. A regression model revealed that the combination of WMSI + TLS was more accurate compared to WMSI alone in detecting the culprit vessel (LAD, P = 0.001; CX, P < 0.001; and RCA, P = 0.019).ConclusionTerritorial longitudinal strain allows an accurate identification of the culprit vessel in NSTE-ACS patients. In addition to WMSI, TLS may be considered as part of routine echocardiography for better clinical assessment in this subset of patients.

Accuracy of global longitudinal and territorial longitudinal strain in determining myocardial viability in out of window period Anterior wall myocardial infarction patients

However, a large fraction of patients especially in the developing countries present outside the window period (OWP) with no chest pain and akinetic left anterior descending (LAD) territory on echocardiography. Revascularization in these patients is primarily guided by viability status. Purpose This study was conducted to determine the accuracy of strain echocardiographic parameters compared to single-photon emission computed tomography (SPECT) in predicting myocardial viability in patients of AWMI presenting OWP. Methods All patients presenting with AWMI-OWP without ongoing chest pain and akinetic LAD territory on echocardiography from December 2020 to June 2021 were recruited. All patients underwent determination of both Global longitudinal strain (GLS) and territorial longitudinal strain (TLS) within 24-72 hours on AWMI. In addition, all underwent SPECT to determine the percentage of non-viable myocardium. On SPECT, a defect &amp;gt;5% myocardial territory showing severe reperfusion defect was considered as a marker of non-viability. Results A total of 27 patients were recruited. The mean age of the population was 61.66 + 12.6 years. Acute left ventricular failure (LVF) and cardiogenic shock (CS) was present in 6 (22.2%) and 5 (18.5%) patients respectively. Out of 22 patients who underwent angiography, majority of the patients 19 (86%) had single vessel disease involving the LAD. The mean left ventricular ejection fraction (LVEF) of the study group was 29.6 + 6.5%. The mean GLS and LAD territory TLS of the study group was -11.13 + 3.14% and -7.2 + 2.75% respectively. On SPECT, 8 (29.6%) patients had non-viable underlying myocardium. Parameters that very significantly associated with non-viability on SPECT included past history of CAD (p = 0.004), smoking (p = 0.05) and presentation with LVF (p = 0.0008) and CS (p = 0.001). The mean LVEF was significantly lower in the group with non-viability on SPECT (32.3 + 5.1% vs 23.1 + 4.5%; p &amp;lt; 0.001). Both GLS (-12.8 + 1.4% vs -7.1+ 2.3%; p &amp;lt; 0.001) and TLS (-8.7 + 1.4% vs -3.8 + 1.8%; p &amp;lt; 0.001) were significantly lower in group showing non-viability on SPECT. On receiver operating curves, a GLS of &amp;gt; -10.45% and TLS of &amp;gt; -6.65%, both had a sensitivity of 87.5% and specificity of 89.5% in predicting non-viability on SPECT. Both showed good accuracy in predicting non-viability on overall quality model. (Figures 1 and 2) Conclusion Modalities like cardiac magnetic resonance, SPECT or positron emission tomography are resource dependent and take time to be performed. Hence, performing these investigations is challenging in unstable patients. Strain echocardiography provides GLS of the myocardium which has good sensitivity and specificity in predicting viability and can be performed safely and quickly in this high-risk group. Application of Layered Strain Technique in NSTE-ACS.

Background To explore the application value of layered strain technique in non-ST elevation acute coronary syndrome (NSTE-ACS). Methods 120 patients with suspected NSTE-ACS undergoing coronary angiography in our hospital from December 2018 to December 2019 were prospectively selected. According to the results of coronary angiography, the patients were divided into the significant CAD group and the nonsignificant CAD group. Echocardiography was performed 1-2 hours before invasive coronary angiography. The long axis and circumferential strain of the endocardium, myocardial layer, and epicardium were evaluated by the layered strain technique. The territorial longitudinal strain (TLS), the global longitudinal strain (GLS) of the three myocardial layers, and the global circumferential strain (GCS) were calculated based on the perfusion region of the three coronary arteries and the 17-segment model of the left ventricle. The primary endopoints were TLS and GCS of the three-layer myocardium. Results Compared with the nonsignificant CAD patients, the TLS and GCS of three-layer myocardium in significant CAD patients were decreased, especially in the endocardium. The absolute values of TLS and GCS of the endocardium and epicardium in significant CAD patients were lower than those in nonsignificant CAD patients. This indicates a significant decrease in endocardial function. Receiver operating characteristic (ROC) curve analysis showed that endocardial TLS was superior to LVEF, Troponin I (TnI), and other strain parameters in evaluating the extent of coronary lesions. Conclusions The layered strain technique of 2D-STE can evaluate the severity of coronary lesions in patients with NSTE-ACS, and for significant CAD patients, endocardial function is significantly more impaired than epicardial function.

6 The role of speckle tracking echocardiography in the identification of culprit lesion in patients with non-ST-segment elevation acute coronary syndrome

Abstract Aims An early evaluation of patients with non-ST elevation acute coronary syndrome patients (NSTE-ACS) is important to choose the appropriate treatment strategy. In this setting of patients, conventional echocardiographic assessment may reveal normal myocardial kinesis in 25–76% of cases. Global and territorial longitudinal strain (GLS and TLS, respectively) may be an early and accurate non-invasive tool for prediction of multivessel CAD in patients with NSTE-ACS. To evaluate the ability of TLS to predict culprit lesions in patients with NSTE-ACS. Methods and results We studied 183 patients diagnosed with NSTE-ACS, in our Institution over 2 years of time. Conventional echocardiography and 2 D speckle tracking echocardiography (STE) imaging were performed by two experienced echocardiographers, who were blinded to patient characteristics. The TLS was identified as the mean value of the segments’ strain as respect to each vessel territory. Coronary angiography was performed in all patients. Significant CAD (luminal stenosis more than 70% in a major epicardial coronary vessel) and culprit lesion were identified and threated by PTCA when appropriate. A significant difference between mono- and tri-vessel CAD in the variation of WMSI has been demonstrated. There was a statistically significant difference between both 3-vessels vs. 1-vessel disease and 2-vessels vs. 1-vessel disease in changing of TLS-LAD, TLS-RCA, and TLS-Cx values (P-value &amp;lt;0.001). There was a significant difference between 3-vessels vs. 2-vessels disease for TLS-RCA values. There was a statistically significant difference for WMSI-LAD, WMSI-CX, and WMSI-RCA values whether the respective artery was involved or not. Variations of TLS were statistically significant both when the territorial tributary artery was involved and also if the artery represented the culprit lesion (P-value TLS-LAD &amp;lt;0.001, TLS-LAD culprit &amp;lt;0.001, TLS-CX &amp;lt; 0.001, TLS-cx culprit &amp;lt;0.001, TLS-RCA &amp;lt;0.001, P-value TLS-RCA culprit 0.022). Role of territorial speckle tracking echocardiography in identifying the localization of significant coronary artery disease in patients with non-ST-segment elevation acute coronary syndromes

Abstract Background In non-ST-segment elevation acute coronary syndromes (NSTE-ACS) patients, several studies demonstrated that 2D speckle tracking echocardiography (STE) is able to predict the presence of coronary artery disease (CAD). Conversely, the role of STE for the localization of significant CAD is less well established. Purpose To investigate the role of territorial longitudinal (TLS) and circumferential strain (TCS) assessed with STE as a non-invasive predictor of localization of significant CAD in patients with NSTE-ACS. Methods We retrospectively enrolled NSTE-ACS patients with significant stenosis (≥70%) at least in one major epicardial coronary artery and without previous cardiovascular events over two years of time. Echocardiography was recorded before coronary angiography and myocardial strain was evaluated offline by an operator blinded to clinical data. Territorial strain was calculated grouping and averaging the strain values of the segments perfused by the 3 major coronary arteries. Results 150 patients were included (age 66.3±11.8 years, 71% male; 90.7% NSTEMI and 9.3% unstable angina). ROC curve analysis demonstrated the ability of TLS and TCS to identify the presence of coronary stenosis of LAD, LCX or RCA (AUC for TLS-LAD 0.74 [0.66–0.82] p=0.0001; LCX 0.73 [0.65–0.81] p=0.0001; RCA 0.69 [0.60–0.77] p=0.0001-AUC for TCS-LAD 0.80 [0.70–0.90] p=0.0001; LCX 0.76 [0.67–0.85] p=0.0001; RCA 0.65 [0.55–0.75] p=0.0001), superior to territorial wall motion score index (Figure 1). The diagnostic value was confirmed in the subgroup of patients without wall motion abnormalities for TLS and for TCS, except for RCA. Conclusion Territorial strain assessed with STE might be a non-invasive tool to localize coronary artery stenosis in patients with NSTE-ACS, even without wall motion abnormalities. Role of Left Ventricular Speckle Tracking Imaging in Detection of Coronary Artery Disease in Non-Diabetic Patients

This study aimed to determine the role of global longitudinalstrain (GLS) in early diagnosis and detection of severity of coronaryartery disease (CAD) in non-diabetic patients with preservedleft ventricular ejection fraction (EF) without regionalwall motion abnormalities presented with chronic coronary syndrome,and to identify the role of territorial longitudinal strain(TLS) in detection of the affected coronary artery. We enrolled60 non-diabetic patients with suspected CAD who underwentresting echocardiography and speckle tracking imaging andsubsequently coronary angiography then results were correlatedtogether. Patients with family history of CAD (68.3%), hypertension(68.3%), smokers (43.3%), and dyslipidemia (81.6%). Allparticipants had normal left ventricular systolic function withmean EF of 64.02±6.15% and no regional wall motion abnormalitiesat rest. Patients were classified angiographically accordingto the number of the diseased vessels into: 3 vessels disease(43.3%), 1-2 vessels disease (35%), and normal coronaryangiography (21.7%) with GLS mean values of -13.69±1.94%,-15.4±1.74%, and -18.80±2.14%, respectively. There was a negativesignificant correlation between GLS values and the numberof diseased vessels (P=0.001). The values of TLS were significantlylower in myocardial regions supplied by stenotic arteries thanthose supplied by non-stenotic arteries. Mean TLS values forstenotic left anterior descending, left circumflex, and right coronaryarteries were -15.51±3.19, -13.06±2.90, -13.27±2.60 withP=0.011, 0.001, and 0.001, respectively. Speckle tracking derivedGLS is an effective non-invasive method in predicting presenceand severity of CAD and in locating the affected vessels basedon the distribution of segments affected in TLS.

Myocardial strain analysis as a non-invasive screening test in the diagnosis of stable coronary artery disease

BackgroundCoronary artery disease (CAD) is one of the most prevalent diseases around the world; however, finding the best noninvasive, low-cost, and more easily accessible test for its screening has been a challenge for several years. Eighty-nine patients suspected of stable CAD underwent 2D-speckle-tracking echocardiography (2DSTE) at resting position and offline longitudinal myocardial strain analysis, followed by coronary angiography. The correlation of the global longitudinal strain (GLS) and territorial longitudinal strain (TLS) with significant CAD (70% and more stenosis in at least one coronary artery) was then evaluated.ResultsThe statistical analysis showed a significant correlation between low GLS and significant CAD (P=0.0001). The results also showed a significant correlation between low TLS and significant CAD in the left and right coronary artery territories. The optimal cut-off point of GLS for the detection of significant CAD was −19.25, with a sensitivity of 76.5% and specificity of 76.6%.ConclusionThis study confirmed the usefulness of 2DSTE myocardial strain analysis in diagnosis of CAD for detecting the affected coronary arteries using GLS and SLS.

Acute changes of left ventricular function during surgical revascularization by 3D speckle tracking.

Detecting early impact of coronary artery bypass grafting (CABG) on left ventricular (LV) function is important because such measures may contribute to meaningful improvement in clinical outcomes. We aimed to gain knowledge about acute changes of LV performance during surgical revascularization using three-dimensional speckle tracking echocardiography (3D STE). Thirty-five patients scheduled for CABG surgery who underwent intraoperative transesophageal echocardiography (TEE) were enrolled (mean age 68.9±7.3years). TEE was performed before and after surgery, as well as before and after grafting. 3D LV ejection fraction (LVEF), tissue motion annular displacement (TMAD) of the mitral valves, 3D global longitudinal strain (GLS), global circumferential strain (GCS), twist, and torsion were quantified. Regional longitudinal strain (LS) was calculated based on coronary perfusion territories in a 16-segment LV model. Despite the absence of change in TMAD and 3D LVEF, 3D GLS (-18.6±4.3% at baseline vs -16.0±4.0% after surgery, P=.01) was significantly decreased, followed with no significant effect on GCS, twist, and torsion during surgery. 3D GLS correlated significantly with 3D LVEF (r between -0.34 and -0.51, P<.05 for all) under the whole operation. Territorial LS did not increase immediately after surgery. 3D speckle tracking imaging allows for detailed and direct evaluation of myocardial deformation, though impaired LV longitudinal function is still apparent immediately after surgery. GLS is more sensitive to an acute reduction in LV function than conventional parameters, which can be potentially useful for serial monitoring of functional recovery.

Myocardial deformation after radiotherapy for breast cancer: A layer-specific and territorial longitudinal strain analysis (BACCARAT study)

Breast cancer (BC) radiotherapy (RT) and the resulting cardiac exposure is associated with early decrease of global longitudinal strain (GLS). Recent software allows multi-layer and segmental analysis of strain, which may be of interest to quantify and locate the impact of cardiac exposure on myocardial function and potentially increase the early detection of radiation-induced cardiotoxicity. To evaluate whether decrease in GLS after BC RT is layer-specific and if it varies according to the left ventricular regional level and the coronary arterial territories. LS was measured at baseline before radiotherapy and 6 months post-radiotherapy. The LS was obtained for each myocardial layer (endocardial, mid-myocardial, epicardial), left ventricular regional level (basal, mid, apical) and coronary artery territory (left anterior descending artery (LAD), circumflex artery, right coronary artery). The study included 64 left sided BC patients. Mean age was 58 ± 4 years, mean doses to the heart, the left ventricle and the LAD were respectively 3.0, 6.7 and 16.4 Gy. Despite unchanged LVEF between baseline and 6 months post-RT (61 ± 7% vs. 60 ± 9%, P = 0.07), decreased GLS was observed for the three layers (endocardial: −20.0 ± 3.2% to −18.8 ± 3.8%; mid-myocardial: −16.0 ± 2.7% to −15.0 ± 3.1%; epicardial: −12.3 ± 2.5% to −11.4 ± 2.8%, all P = 0.02), but only the endocardial layer GLS (GLS-endo) relative change was significant (−4.7%, P = 0.049). Indeed, the GLS-endo was significantly decreased for the most exposed parts of the left ventricle corresponding to the apical level (−26.3 ± 4.0% vs. −24.2 ± 7.1%, P = 0.03) and LAD territory (−22.8 ± 4.0% vs. −21.4 ± 4.8%, P = 0.03). Six months post-RT, GLS decreased predominantly in the endocardial layer of the most exposed part of the left ventricle. For precise evaluation of RT-induced cardiotoxicity and early left ventricular dysfunction, the endocardial layer-based GLS might be the most sensitive parameter.

Rapidly and Accurately Detecting Significant Coronary Artery Stenosis in Patients with Suspected Stable Coronary Artery Disease and Normal Segmental Wall Motion by Speckle Tracking Automated Functional Imaging

This study was performed to explore the value of a machine with built-in speckle tracking automated functional imaging (AFI) in predicting the severity of coronary artery lesions in patients with suspected stable coronary artery disease (SCAD) with normal wall motion. One hundred forty-three consecutive patients with clinically suspected SCAD with normal wall motion were included. Multiple parameters, including global longitudinal peak strain, post-systolic index (PSI) and peak strain dispersion, were automatically analyzed with AFI. Territorial longitudinal strain (TLS) and territorial PSI (TPSI) were also calculated. Receiver operating characteristic curve analyses revealed that global longitudinal peak strain and PSI were superior to other parameters in detecting left main or three-vessel SCAD. A combination of TLS and TPSI could improve diagnostic performance in identifying significant stenotic left anterior descending and right coronary arteries. In conclusion, 2-dimensional speckle tracking AFI can rapidly provide multiple parameters for detecting significant coronary artery stenosis with high accuracy in patients suspected of having SCAD with normal wall motion.

Value of territorial work efficiency estimation in non-ST-segment-elevation acute coronary syndrome: a study with non-invasive left ventricular pressure-strain loops.

Non-ST-segment-elevation acute coronary syndrome (NSTE-ACS) patients with normal left ventricular (LV) ejection fraction (LVEF) and wall motion require a non-invasive tool to detect LV risk areas. This study utilized non-invasive LV pressure-strain loops to evaluate territorial myocardial work efficiency (WE) for identifying obstructive coronary artery stenosis, in patients with non-obstructive or obstructive coronary artery stenosis NSTE-ACS, the latter with or without occlusion. Global and territorial longitudinal strain (LS) analyses were performed via speckle-tracking imaging before coronary angiography. LV pressure-strain loops estimated global and territorial myocardial work index (MWI), constructive work (CW), wasted work (WW), and WE. Receiver operating characteristic curve analysis was used to determine the optimal cutoff value of independent parameters to detect obstructive coronary artery stenosis. Compared with non-obstructive, obstructive coronary artery stenosis showed significantly lower global and territorial LS, MWI, CW, and WE, and higher WW. Territorial LS, MWI, CW, and WE were significantly worse in territories of coronary occlusion. Territorial WE was the best parameter for predicting obstructive coronary artery stenosis (AUC 0.80, cutoff < 96%, sensitivity 73%, specificity 70%, P < 0.001). In patients with NSTE-ACS with normal wall motion and LVEF, territorial WE is more accurate than territorial LS or MWI to identify LV risk areas.

P1319 Speckle tracking echocardiography in patients with acute coronary syndrome: the added value

Abstract Background Speckle tracking echocardiography with assessment of left ventricular global longitudinal strain (GLS) allows to identify early signs of cardiac dysfunction. The superiority of GLS in the prediction of all-cause mortality in the general population compared with LVEF is well known; in addiction GLS improves risk stratification and recognize early left ventricular dysfunction in several clinical setting. Purpose the aim of this study was to assess the diagnostic and prognostic role of global and segmental strain in the early identification of cardiac dysfunction in patients with myocardial infarction without persistent ST segment elevation (NSTEMI) and without obvious wall motion abnormalities and to assess the ability to predict the presence/extension of coronary artery disease in this population. Methods an observational study was carried out enrolling 56 patients (median age 66 ± 11,7 years) admitted for NSTEMI. Electrocardiogram, echocardiogram with global and segmental strain measurement (GE’s Automated Function Imaging) and coronary angiography were performed in all patients. In addition we measured territorial longitudinal strain (TLS) defined as the mean of peak systolic strain of the segments pertinent to the perfusion territories of each epicardial coronary arteries in a 17-segment LV model. Results mean LVEF was 53%. We found a significant relationship between GLS values and coronary artery disease extension: GLS -19,1 % ±5,7% in patients without significant coronary artery stenosis, -18,74% ±4,85% in patients with one coronary artery disease, -15,6% ±4,3% in patients with two coronary artery disease, -14,2% ±5,4% in patients with 3 coronary artery disease (p = 0,03). We also found a correlation between territorial strain value and corresponding coronary artery stenosis. TLS was significantly reduced in patients with significant coronary disease of the perfused territory for each artery (LAD TLS 15,1% ± 6,74 % vs -20,73± 4,47 p = 0,002; CX TLS -11,42 % ± 6,59 %,vs -17,06 ± 7,1, p = 0,0 05 , RCA -14,02 % ± 6,00 % vs-17,6 ± 5,21 p = 0,02). At ROC analysis we identified a TLS value of -16,9 % to identify patients with significant stenosis of LAD (sensibility 58,3% and specificity 89,4%, AUC = 0,74, P &amp;lt; 0,001), TLS -18,2% for circumflex artery (sensibility 86,2% and specificity 78,9%, AUC = 0,78, P &amp;lt; 0,001) TLS -18% for right coronary (sensibility 73,9% and specificity 56,2%, AUC = 0,67, P = 0,027) Conclusions global and segmental strain measurement in patients with NSTEMI help to identify the presence and extension of significant coronary artery disease.

P857 Diagnostic and prognostic value of 2D Strain in Non ST elevation myocardial infarction patients

Abstract Background Following an acute coronary syndrome, ischemic myocardial dysfunction has several degrees of severity and different outcomes from a total or partial recovery to an irreversible injury. In this study led in non-ST elevation myocardial infarction (NSTEMI) patients without otherwise previous non-ischemic cardiomyopathy (NICM), we investigated the correlation between 2D global longitudinal strain (GLS) and angiographic prognostic factors. The ability of territorial longitudinal strain (TLS), defined as the sum of segmental strain in a coronary territory,to identify culprit artery occlusion was also assessed. Methods 82 consecutive NSTEMI patients were prospectively screened for inclusion; 70 of them without NICM were enrolled. Severe coronary artery disease (CAD) was defined as three-vessel disease or a left main disease. Group 1 and 2 were defined by the presence or not of severe CAD. Statics ‘analyses was performed with IBM SPSS Statistics (version 22). Results mean age of patients was 60, 2 ±10 years. 37 patients had diabetes mellitus (53%), 31 had hypertension (44%), 21 had dyslipidemia (30%) and 5 had renal insufficiency (7%). Severe CAD was present in 24 patients (34%). The first ultrasound exam showed that mean EF was 49 ± 11, mean WMSI was 1.43 ± 0.4 and mean GLS was -14.9 ± 4. GLS was higher in group 1 (-12.82 ± 0.95 vs -16.04 ± 0.42; p &amp;lt; 0.001); LVEF and WMSI in group 1 and 2 were (43.3 ± 13.5% Vs 52.7 ± 7.9%; p &amp;lt; 0.001) and (1.64 ± 0.1 Vs 1.32 ± 0.04; p &amp;lt; 0.001) respectively. Correlations were found between LVEF and GLS (p = 0.004), and between WMSI and GLS (p = 0.002) . TLS was able to discriminate between coronary stenosis of LAD, LCX or RCA and to predict the occlusion of the culprit vessel: 7 patients had acute coronary occlusion (10%). TLS was -7.4 ± 5.1 in patients with coronary occlusion and -14.1 ± 6 in the absence of coronary occlusion (p &amp;lt; 0.001). A cut off of -9.5 was able to detect this occlusion with a specificity of 82% and a sensitivity of 85%. The second ultrasound exam, performed after a median of 10 ± 3.1 months, showed a statistically significant improvement of EF (53 ± 10, p =0.02), WMSI (1.35 ± 0.39, p= 0.01) as well as GLS (-17.1 ± 4.2, p =0.004). Patients who received only medical treatment (n = 11) had the lowest variation of EF (47% to 48 %; p = 0.7), WMSI (1.62 to 1.59; p = 0.69) and GLS (14.2 to 15.2; p = 0.2) with no statistical correlation between the two exams. While patients who had PCI or bypass revascularization, had the best outcome with improvement of EF (49% to 53%; p = 0.002), WMSI (1.4 to 1.32;p = 0.01) and GLS (15 to 17.4;p = 0.004). Conclusion GLS is a strong diagnostic and prognostic ultra sound parameter for NSTEMI patients correlated to CAD severity. Strain is a reliable parameter during follow up.TLS can be used to localize the culprit coronary artery and especially to predict its occlusion during the acute phase of NSTEMI which can lead to a different therapeutic strategy.

Predictive value of global and territorial longitudinal strain imaging in detecting significant coronary artery disease in patients with myocardial infarction without persistent ST-segment elevation.

Two-dimensional (2D) speckle-tracking echocardiographic (STE) imaging is frequently performed in the assessment of cardiovascular diseases. We aim to investigate the role of the global and territorial longitudinal strain (GLS and TLS) values assessed via 2D STE imaging to detect significant coronary artery disease (CAD) in non-ST-segment elevation myocardial infarction (NSTEMI) patients without wall-motion abnormalities. This study enrolled 150 patients with the diagnosis of NSTEMI. Patients who had typical chest pain with unstable angina characteristics within the last 24hours were 18-80years of age and had a typical rise and/or fall of cardiac biomarkers were included. Myocardial functions were assessed via myocardial deformation analyses of 2D STE images. The mean age of the CAD group was 52.91±9.11, vs 50.31±8.32 in the control group. In the CAD group, 56 patients were male (65%), whereas 21 were male (60%) in control group. GLS and TLS assessments demonstrated a statistically significant difference between CAD and control groups, with GLS values of -16.27±1.91 and -18.74±1.93 (P<0.001), TLS-LAD values of -15.67±1.83 and -18.54±1.97 (P<0.001), TLS-RCA values of -17.04±1.81 and -19.20±1.86 (P<0.001), and TLS-Cx values of -17.40±2.08 and -18.34±2.18 (P=0.028), respectively. Correlation analyses revealed that as high-sensitivity troponin (hsTnT) values increased, GLS decreased significantly, and further, an increase in severity of CAD resulted in decreased TLS-LAD, -CX and -RCA (TLS-LAD: P<0.001, r=-0.743; TLS-CX: P<0.001, r=-0.449; TLS-RCA: P<0.001, r=-0.737). Multivariate analyses indicated that GLS and GRACE ACS risk scores are independent predictors of CAD in patients with NSTEMI (GLS: OR=0.514, P<0.001; GRACE score: OR=0.938, P=0.007). Global longitudinal strain (GLS) assessed with 2D STE is a promising, easy to perform and quick imaging method to predict CAD in patients with NSTEMI.
