Teratomas in children and young adults.

A review of all teratomas seen at the University of Wisconsin Hospital between 1965 and 1977 revealed that sacrococcygeal and presacral teratomas were most common. In these cases survival was best in infants less than one year old, with the exception of two cases of malignant medulloepithelioma. Testicular teratomas were predominant in the young adult male, and survival was poor. Pathology and treatment of teratomas are discussed, with an accompanying discussion of congenital anomalies associated with teratomas.

Ovarian teratomas in children: A thirty-three year experience

During the thirty-three years from 1941 through 1973, forty-two children with ovarian teratomas were seen. The most common complaint was that of abdominal pain and the most common physical finding was palpable lower abdominal mass. Thirty-seven patients had ovarian teratomas with mature tissues only. Of these, two patients have been lost to follow-up and the remainder are alive and well. One patient had teratoma containing mature and immature tissues (embryonic); this patient has remained well since operation. Four patients had malignant teratoma. Of these, two patients are dead due to the tumor and the two are living and well for six and nine years, respectively.

Teratomas in childhood

Of 107 teratomas in children, 86 were benign and 21 malignant. Sacrococcygeal and pelvic teratomas predominated (51 cases) and these fell into 3 groups: post sacral, dumb-bell and presacral. The 34 purely posterior tumours were always congenital and benign, whilst the incidence of malignancy in dumb-bell and presacral teratomas increased as the tumour became more internal. Malignant teratomas were carcinomas, usually containing glandular, papillary and clear-cell areas, and metastases were similar. Immature tissues in benign teratomas were usually neural or connective tissue. They did not give rise to neuroblastomas or sarcomas, and did not indicate a worse prognosis. Only 2 originally benign teratomas later developed malignancy.

Of nine children with presacral teratomas four under the age of 6 mo, had benign tumors and are alive and well after surgery. Of five children older than 6 mo with malignant tumors, four were dead within 1 yr in spite of treatment. From these cases it is clear that survival of these infants is related to the age at which the diagnosis is discovered. Infants with urinary symptoms or constipation should have a digital rectal examination to exclude a pelvic mass. Early exploration is warranted to establish the diagnosis and once confirmed radical surgical excision of the tumor is mandatory.

Teratomas in children: Clinical and pathologic aspects

Teratomatous Tumors are interesting because of their obscure origin, bizarre microscopic appearance, and sometimes unpredictable behavior. Because they occur not infrequently in children, the clinician should be aware of their clinical features, natural history, pathology, and principles of treatment. In an attempt to document some of these features further, the clinical and pathologic aspects of 58 cases of teratomas in children seen at the Mayo Clinic during a 31-yr period were reviewed.

Acute thoracic distress in childhood due to spontaneous rupture of a large mediastinal teratoma

We have presented what we believe to be the first report of a mediastinal teratoma which ruptured into the pleural cavity of a child. The patient presented as an acute thoracic catastrophy with respiratory distress and severe chest pain. Despite extensive diagnostic procedures, the true nature of the lesion was not determined until a thoracotomy was performed. A review of the English literature pertinent to mediastinal tumors disclosed 120 teratomas in children, 23 per cent of which were malignant. Complications of mediastinal teratomas, including rupture into the pleural space, pericardium, tracheobronchial tree, great vessels, and esophagus, are discussed and diagnostic procedures are reviewed.
