Sir:FigureWe read the letter by Aveta et al. regarding the V-Loc (Covidien, Mansfield, Mass.) suture for breast surgery. We are pleased that the suture is being used more widely, as we have noticed the same in our country. This suture is very good for controlling closure tension, and we have used it for breast surgery and, in particular, for periareolar closure. We use the V-Loc suture for the periareolar closure described by Hammond et al.1 To control the shape and tension, we close two layers of the V-Loc and, if necessary, perform skin closure with a finer suture. We occasionally have spitting and extrusion of the suture, which we find occurs if we come to the end and go back on the suture. There is more likely to be some spitting at turning points, and we also place the suture in the deep dermis preferentially. Another simple option that we have now started to use is to reuse the remaining suture. The suture is often longer than we need, and we can reuse the suture by creating a “bobble” knot. The “bobble knot is a suture knot in the thread on one end of the V-Loc that is then used to stop the V-Loc from pulling through, and this can be placed either in the tissues or externally and later just cut flush. This allows us to reuse the V-Loc suture for smaller incisions. In general, the V-Loc suture will be a good adjunct for all body contouring procedures such as abdominoplasty, flankectomy, medial thighs lifts, arm reductions, and of course breast reduction and mastopexy. We are continuing to monitor our scars from this closure but need additional time to fully evaluate it objectively. We are pleased to see that Aveta et al. have noticed a good outcome with regard to the areola deformation and the resulting scars.
