Topical Antibiotic-Induced Otomycosis - a Systematic Review of Aetiology and Risk Factors.

Otomycosis is a chronic or subacute fungal infection of external ear accounting to 5 to 20% of external ear infection worldwide. Literature has suggested multiple local and environmental host factors associated with otomycosis, but their strength of association is not well established. We did this review to analyse otomycosis with respect to it's various common predisposing factors. Human studies reporting ototopical antibiotics with or without steroids as predisposing factors for otomycosis, or pooled data together with other predisposing factors were included. MEDLINE, EMBASE, CINAHL, the ISRCTN register, and Cochrane databases were searched for the data extraction. Critical appraisal was done using the standardized Joanna Briggs Institute's (JBI) Appraisal checklist. Random effects were used to calculate pooled estimate prevalence with 95% Confidence Intervals (CIs). Age varied in different studies with a mean age variation from 7 to 43.19. Most of the studies were conducted in tropical and sub-tropical countries. Aspergillus was the most common species isolated. The use of ototopical antibiotics with or without steroid drops [47% (95% CI, 0.38 - 0.56)] were found to be the commonest predisposing factor for otomycosis, followed by the use of a variety of oils and wax solvents [38% (95% CI,0.26 - 0.51)] and trauma to the External Auditory Canal (EAC) as a result of compulsive cleaning or instrumentation[37% (95% CI, 0.21 - 0.54)]. We suggest the need for caution and vigilance from clinicians treating patients with ototopical drops for ear infections and use of other alternative treatment like 2% acetic acid for mild cases whenever feasible.

Exploring the relationship between trunk flexibility and arterial stiffness measured by pulse wave velocity: A systematic review and meta-analysis.

As individuals age, the risk of cardiovascular disease (CVD) increases, largely due to progressive stiffening of the arteries. This relationship underscores the critical need to monitor arterial stiffness as a predictor of CVD outcomes. While aerobic exercise has demonstrated benefits for vascular health, the influence of flexibility, particularly trunk flexibility, on arterial stiffness remains underexplored. Thus, this study aimed to analyse the overall relationship between trunk flexibility and arterial stiffness across different age groups (young, middle-aged, and older adults) and according to sex. A systematic review and meta-analysis were conducted following the MOOSE and JBI Manual for Evidence Synthesis on systematic reviews of etiology and risk guidelines. Searches in PubMed, Scopus, and Web of Science identified studies investigating the association between trunk flexibility and arterial stiffness measured by pulse wave velocity. Data extraction, quality assessment, and statistical analyses were performed following predefined criteria. Five studies involving 2797 participants were included. Poor trunk flexibility was associated with increased arterial stiffness compared to high flexibility (pooled standardized mean difference = -0.27, 95% CI: -0.39, -0.14), with substantial heterogeneity observed. Subgroup analyses by sex and age revealed significant associations predominantly in men and older individuals. Sensitivity analyses confirmed the robustness of the findings, and meta-regression models showed no significant differences according to age, BMI, or blood pressure. No evidence of publication bias was found. Poor trunk flexibility is linked to elevated arterial stiffness across diverse demographic groups, highlighting its potential as a surrogate marker for cardiovascular health. Physiological mechanisms involving connective tissue integrity and neural regulation may underpin this relationship. Understanding the role of flexibility in arterial health could inform targeted interventions to mitigate age-related increases in arterial stiffness and reduce cardiovascular disease risk. However, further research is needed to validate these findings and explore potential sex-specific differences.

Correlação entre tempo no alvo e hemoglobina glicada de pessoas com diabetes mellitus: revisão sistemática

Objective: to analyze the correlation between time on target and glycated hemoglobin in people living with diabetes mellitus and carrying out continuous blood glucose monitoring or self-monitoring of capillary blood glucose. Method: systematic review of etiology and risk based on JBI guidelines and reported according to Preferred Reporting Items for Systematic Reviews and Meta- Analyses, covering six databases and grey literature. The sample included 16 studies and methodological quality was assessed using JBI tools. Protocol registered in the Open Science Framework, available at https://doi.org/10.17605/OSF.IO/NKMZB. Results: time on target (70-180 mg/dl) showed a negative correlation with glycated hemoglobin, while time above target (>180 mg/dl) showed a positive correlation. Correlation coefficients ranged between -0.310 and -0.869 for time on target, and between 0.66 and 0.934 for time above target. A study was carried out on a population that performed self-monitoring. Conclusion: there is a statistically significant correlation between time on target and time above target with glycated hemoglobin. The higher the proportion in the adequate glycemic range, the closer to or less than 7% the glycated hemoglobin will be. More studies are needed to evaluate this metric with data from self-monitoring of blood glucose.

Developmental learning disorders in children with prenatal/perinatal exposure to hypoxia: A systematic review protocol.

Developmental learning disorder (DLD) belongs to neurodevelopmental disorders because it results from the developmental neurodiversity of the brain. The main causes of DLD are genetics, but environmental factors, like inadequate supply of oxygen during pregnancy or labor, are considered. Our search strategy will consist of electronic databases (PubMed, PsycINFO, Web of Science, EMBASE, and Cochrane Library) and hand searching. The observational studies including cohort and case-control studies will be included. The primary outcome will be (DLD). Screening and eligibility will be done independently by two reviewers based on pre-specified eligibility criteria. Data extraction will be based on a pre-pilot data extraction form, and conducted by two authors independently. Study quality will be assessed by two authors independently. Any discrepancies identified at any stage of the review will be resolved by discussion or/and consultation with another reviewer. We plan a narrative and tabular summary of the findings. This systematic review of aetiology follows the traditional approach to evidence-based healthcare. This secondary research will assess the association between hypoxia and DLD by assessing the relationship of health-related event and outcome and examining the association between them. This review can provide information for healthcare professionals and policymakers indicating whether taking into account information about hypoxia should be permanently included in the diagnostic ontogenetic interview in the process of diagnosing neurodevelopmental disorders. Maternal and infant risk factors and risk indicators associated with early childhood caries in South Africa: a systematic review

ObjectivesTo evaluate the risk factors and risk indicators associated with early childhood caries in South Africa.DesignA systematic review of aetiology was performed. From 1366 papers found, 23 studies met the eligibility criteria and were included. All study designs were included. Healthy children under six who live in South Africa were eligible for the study. The study was registered with PROSPERO, registration number CRD42020216455.Data extractionEligible studies were selected, and data extracted independently by two reviewers. Published data on socio-economic status, dietary factors, oral hygiene knowledge and practices, breastfeeding and bottle-feeding practices, oral bacterial flora and other risk indicators were collected. Two authors appraised the studies independently using the Joanna Briggs Critical Appraisal tools.Data analysisHeterogeneity was assessed using the I2 statistics, and due to heterogeneity, extracted data were mostly presented narratively.ResultsMeta-analysis was performed using random-effects models and concluded that parents of children who had a tertiary education had a 1.77 [1.22–2.57] odds of experiencing dental caries compared to the children of parents with a secondary education. However, the unclear risk of bias of most included cross-sectional studies precluded definitive conclusions.ConclusionsMore high-quality cohort studies need to be performed to evaluate actual risk factors for ECC in a South African setting. Parental/caregiver oral educational knowledge needs promoting before the emergence of their children’s teeth. The social determinants of health need to be incorporated in future studies, and suitable targeted interventions need to be developed and implemented to mitigate early childhood caries in South Africa.

Sarcopenia and blood albumin: A systematic review with meta-analysis

ResumenLa sarcopenia se caracteriza por la pérdida de musculatura durante el envejecimiento, lo que puede traer consecuencias para la salud. Se detecta de diversas formas, una de ellas, el uso de biomarcadores sanguíneos como la albúmina, aunque todavía no se ha establecido dicha asociación de forma definitiva.Esta revisión sistemática y metaanálisis resume el conocimiento sobre la asociación entre sarcopenia y albúmina sérica en los adultos mayores, centrada en la etiología y los factores de riesgo. La revisión se hizo utilizando el programa del Joanna Briggs Institute y la búsqueda incluyó las bases de datos Medline, Embase, CINAHL, y LILACS; la búsqueda manual estuvo a cargo de dos revisores de forma independiente. Para el metaanálisis, se utilizó el programa EpiDat, versión 3.1; las diferencias de medias en los puntajes de albúmina desagregados por sarcopenia se analizaron mediante el modelo de efectos aleatorios. El grado de heterogeneidad se evaluó con la prueba Q de DerSimonian y Laird. Se analizaron 630 artículos, de los cuales 14 se incluyeron en la revisión. En el metaanálisis, se evidenciaron mayores cantidades de albúmina sanguínea en los adultos mayores que no presentaron sarcopenia frente a los que sí, una diferencia con significación estadística. Aunque hay estudios en los que se explora la asociación entre albúmina y sarcopenia, persiste la necesidad de evaluar la asociación entre los marcadores biológicos, comparándolos entre sí para determinar cuáles pueden utilizarse en la detección de sarcopenia en el adulto mayor.

Biliary Atresia: A Systematic Review of Etiology, Pathogenesis, Diagnosis and Treatment

Biliary Atresia (BA) is an uncommon, progressive and idiopathic fibro-obliterative cholangiopathy. The typical clinical features should be identified prompt in order to realize a Kasai Hepatoportoenterostomy (HPE) for restore the bile flow. However, despite HPE the BA remains the most common indication for liver transplantation in children. The lasts studies have shown the importance of virus, immunity and other environmental substances in pathogenesis of BA that is important in order to look for new therapeutic and preventive strategies. A systematic review of etiology, epidemiology, clinical manifestations, image findings, and medication of 2019 Corona Virus Disease-19 in Wuhan, China.

Background:Unknown origin pneumonia has been furiously spreading since the late of December 2019, subsequently spread to approximately all provinces and areas in China and many countries, which was announced as a Public Health Emergency of International Concern by World Health Organization (WHO). The studies on 2019 Corona Virus Disease-19 (COVID-19) conducted from various fields around the world. Herein, the objective of the present study is to summarize the etiology, epidemiology, clinical manifestations, image findings, traceability analysis, and drug development of COVID-19.Methods:The following electronic databases were searched: Cochrane Central Register of Controlled Trials (CENTRAL), PubMed, EMBASE, Cochrane Library, China National Knowledge Infrastructure (CNKI), Chinese Biomedical Literature Database, VIP Chinese Science and Technology Periodical Database, and Wanfang Data. Other relevant literature will be manually searched as a compliment. We have reviewed etiology, epidemiology, clinical manifestations, image findings, and medication from case reports and retrospective clinical studies relating to COVID-19 published since the outbreak.Results:The coronavirus is closely related to bat coronavirus and pangolin coronavirus. Besides, the infection pathway is confirmed to be the respiratory and digestive systems. The virus indicates person-to-person transmission and some patients present asymptomatic. The elderly have a higher mortality rate. Rapid and sensitive nucleic acid testing is usually used as a basis for diagnosis. Currently, there is no specific vaccine and antiviral drug. Intervention actions such as travel bans and quarantine adopted have effectively reduced the spread of the epidemic.Conclusion:This systemic review will provide high-quality evidence to summarize etiology, epidemiology, clinical manifestations, image findings, traceability analysis, drug development in patients with COVID-19.

Suprascapular Neuropathy in Overhead Athletes: A Systematic Review of Aetiology and Treatment Options

Suprascapular neuropathy is often overlooked as a cause for shoulder pain in overhead athletes and can be misdiagnosed for many months, even years. With recent advancements in the understanding of the condition as well as its treatment methods, suprascapular neuropathy is now being diagnosed more frequently. However with a multitude of treatment options, it can be difficult to choose the optimal management. In this systematic review, we have carried out a detailed literature search about suprascapular neuropathy looking into evidence-based diagnostic workup and treatment modalities available. Repetitive overhead activities, rotator cuff tear and direct compression of the nerve by space-occupying lesion are important etiologies for suprascapular neuropathy. Whilst MRI is widely used for the identification of space-occupying lesions and rotator cuff injury; Electromyography (EMG) and Nerve Conduction Studies (NCS) remain gold standards for confirming injury to the nerve. Conservative treatment modalities including physiotherapy and activity modification, nerve blocks, arthroscopic and open surgical interventions are the main treatment options for suprascapular neuropathy.

Frailty syndrome and cognitive impairment in older adults: systematic review of the literature.

Objective:to synthesize the knowledge about the association of frailty syndrome and cognitive impairment in older adults.Method:the Joanna Briggs Institute’s systematic review of etiology and risk factors was adopted. The search for the studies was conducted by two independent reviewers in the databases MEDLINE, Embase, CINAHL and LILACS and by manual search was performed by tow reviewers independently. The measures of association Odds Ratio and Relative Risk were used in the meta-analysis. The software R version 3.4.3 and the meta-analysis package Metafor 2.0 were used for figure analysis.Results:three studies identified the association of frailty syndrome and cognitive impairment through Odds Ratio values show that frail older adults are 1.4 times more likely to present cognitive impairment than non-frail older adults. Four studies analyzed the association through the measure of Relative Risk and found no statistical significance, and four studies used mean values.Conclusion:despite of the methodological differences of the studies and the lack of definition of an exact proportion in the cause and effect relationship, most studies indicate Frailty Syndrome as a trigger for Cognitive decline.

Acute colonic pseudo-obstruction: A systematic review of aetiology and mechanisms.

AIMTo critically review the literature addressing the definition, epidemiology, aetiology and pathophysiology of acute colonic pseudo-obstruction (ACPO).METHODSA systematic search was performed to identify articles investigating the aetiology and pathophysiology of ACPO. A narrative synthesis of the evidence was undertaken.RESULTSNo consistent approach to the definition or reporting of ACPO has been developed, which has led to overlapping investigation with other conditions. A vast array of risk factors has been identified, supporting a multifactorial aetiology. The pathophysiological mechanisms remain unclear, but are likely related to altered autonomic regulation of colonic motility, in the setting of other predisposing factors.CONCLUSIONFuture research should aim to establish a clear and consistent definition of ACPO, and elucidate the pathophysiological mechanisms leading to altered colonic function. An improved understanding of the aetiology of ACPO may facilitate the development of targeted strategies for its prevention and treatment.

Cervical and oral teratoma in the fetus: a systematic review of etiology, pathology, diagnosis, treatment and prognosis

The aim of the study was to produce a systematic review about etiology, pathology, diagnosis, prognosis and clinical management regarding oral and cervical teratomas. A systematic review of Pubmed/Medline using the following keywords was made: epignathus, cervical teratoma, fetus, oral teratoma, prenatal diagnosis, prognosis, treatment, ultrasound. The following clinical conclusions can be reached: (1) teratomas are rare, usually benign congenital tumors which recognized multifactorial etiology; (2) prenatal ultrasound diagnosis can be made early in pregnancy (15-16 weeks); (3) 3D ultrasound and MRI may enhance the accuracy of the antenatal diagnosis (location, extension and intracranial spread) and may aid in the selection of patients requiring treatment; (4) prenatal karyotype and search for associated abnormalities is mandatory in all teratomas; (5) delivery should involve elective Cesarean section with ex utero intrapartum treatment procedure or resection of the tumor mass, which may be performed on placental support operation on placental support procedure to increase the chances of postnatal survival.

Systematic review of aetiology and treatment of post-stroke hand oedema and shoulder-hand syndrome.

Studies on the aetiology and treatment of post-stroke hand oedema and shoulder-hand syndrome (SHS) published from January 1973 until August 1998 were identified. Eleven studies were included with at least some control for confounding. These were evaluated on 11 methodological criteria and by standardized effect sizes. There were five aetiological studies: four cohort studies and one study consisting of two case series using a within-subjects design. The matters investigated included lymph scintigraphy in hand oedema, bone scintigraphy, putative risk factors and the existence of autonomic dysregulation and peripheral nerve lesions in SHS. There were six therapeutic studies: one randomized controlled trial, one non-randomized controlled trial, one cohort study and three case series, of which two studies used a within-subjects design. These studies investigated continuous passive motion and neuromuscular stimulation in hand oedema as well as oral corticosteroids, intramuscular calcitonin and trauma prevention in SHS. A great diversity of pathophysiological and therapeutic insight was found. Based on systematic analysis of the literature, the following conclusions seem justified: (i) the shoulder is involved in only half of the cases with painful swelling of wrist and hand, suggesting a "wrist-hand syndrome" between simple hand oedema and SHS; (ii) hand oedema is not lymphoedema; (iii) SHS usually coincides with increased arterial blood flow; (iv) trauma causes aseptic joint inflammations in SHS; (v) no specific treatment has yet proven its advantage over other physical methods for reducing hand oedema; and (vi) oral corticosteroids are the most effective treatment for SHS.
