Exploring Health Care Professionals' Perspectives on the Use of a Medication and Care Support System and Recommendations for Designing a Similar Tool for Family Caregivers: Interview Study Among Health Care Professionals.

With the aging population on the rise, the demand for effective health care solutions to address adverse drug events is becoming increasingly urgent. Telemedicine has emerged as a promising solution for strengthening health care delivery in home care settings and mitigating drug errors. Due to the indispensable role of family caregivers in daily patient care, integrating digital health tools has the potential to streamline medication management processes and enhance the overall quality of patient care. This study aims to explore health care professionals' perspectives on the use of a medication and care support system (MCSS) and collect recommendations for designing a similar tool for family caregivers. Fifteen interviews with health care professionals in a home care center were conducted. Thematic analysis was used, and 5 key themes highlighting the importance of using the MCSS tool to improve medication management in home care were identified. All participants emphasized the necessity of direct communication between health care professionals and family caregivers and stated that family caregivers need comprehensive information about medication administration, patient conditions, and symptoms. Furthermore, the health care professionals recommended features and functions customized for family caregivers. This study underscored the importance of clear communication between health care professionals and family caregivers and the provision of comprehensive instructions to promote safe medication practices. By equipping family caregivers with essential information via a tool similar to the MCSS, a proactive approach to preventing errors and improving outcomes is advocated.

University student suicides in Türkiye: Insights from two decades of media reports

University student suicides have emerged as a significant societal concern, reflecting challenges inherent in the transition to adulthood and academic pressures. This study aims to analyze two decades of national media coverage on university student suicides in Türkiye, examining demographic trends, suicide methods, and social factors implicated in these tragic incidents. A comprehensive scan of Turkish national media archives from January 1, 2004, to December 31, 2023, identified 213 unique reports of university student suicides. Data extracted included age, sex, year of incident, prior suicide attempts, geographic distribution, housing status, academic major, psychiatric history, social background, suicide method, and presence of suicide notes or social media posts. Statistical analyses utilized Fisher's Exact Test and Pearson's Chi-square to examine categorical data. Male students accounted for 61.6% (n=125) of reported suicides, with a mean age of 22.2 years (SD=3.05). The most common suicide method was jumping from height, differing by sex, with hanging prevalent among males and jumping more frequent among females. Mobbing emerged as a predominant social stressor implicated in suicide incidents, surpassing other factors reported in the literature. The findings highlight an alarming increase in media-reported university student suicides, predominantly affecting males and characterized by distinct suicide methods. The pervasive impact of mobbing underscores the urgent need for targeted interventions addressing social stressors, alongside comprehensive medical, psychological, social, and economic support systems within academic settings. Efforts should prioritize proactive mental health initiatives, policy reforms, and community-based interventions to mitigate suicide risks among university students.

Summary. The purpose of the study is to analyze the problems, tasks and determine the main directions for further improvement of the training of medical specialists of the All-Russian Disaster Medicine Service (VSMK, Service) at the present stage. Materials and methods of the study. The materials of the study are regulatory legal acts, documentation and sources on the issues of training of VSMK personnel. Research methods are an analytical method and a generalization method. Research results and their analysis. The tasks and problems of training medical specialists of the All-Russian Disaster Medicine Service at the present stage are presented, the directions for its further improvement are determined taking into account new challenges and threats to the security of our country. It is noted that the modern conditions for training medical specialists of the Service are characterized by the level of their readiness for medical support of armed conflicts, which is due to the current stage of development of the system of medical support for the wounded, sick and victims. Various aspects of training medical specialists of the All-Russian Disaster Medicine Service are considered taking into account the experience of their participation in medical evacuation support of the said contingents in the context of the Special Military Operation (SMO), etc.

Redirection of low-acuity emergency department patients to nearby medical clinics using an electronic medical support system: effects on emergency department performance indicators

BackgroundOvercrowded emergency departments (EDs) are associated with higher morbidity and mortality and suboptimal quality-of-care. Most ED flow management strategies focus on early identification and redirection of low-acuity patients to primary care settings. To assess the impact of redirecting low-acuity ED patients to medical clinics using an electronic clinical decision support system on four ED performance indicators.MethodsWe performed a retrospective observational study in the ED of a Canadian tertiary trauma center where a redirection process for low-acuity patients was implemented. The process was based on a clinical decision support system relying on an algorithm based on chief complaint, performed by nurses at triage and not involving physician assessment. All patients visiting the ED from 2013 to 2017 were included. We compared ED performance indicators before and after implementation of the redirection process (June 2015): length-of-triage, time-to-initial-physician-assessment, length-of-stay and rate of patients leaving without being seen. We performed an interrupted time series analysis adjusted for age, gender, time of visit, triage category and overcrowding.ResultsOf 242,972 ED attendees over the study period, 9546 (8% of 121,116 post-intervention patients) were redirected to a nearby primary medical clinic. After the redirection process was implemented, length-of-triage increased by 1 min [1;2], time-to-initial assessment decreased by 13 min [-16;-11], length-of-stay for non-redirected patients increased by 29 min [13;44] (p < 0.001), minus 20 min [-42;1] (p = 0.066) for patients assigned to triage 5 category. The rate of patients leaving without being seen decreased by 2% [-3;-2] (p < 0.001).ConclusionImplementing a redirection process for low-acuity ED patients based on a clinical support system was associated with improvements in two of four ED performance indicators.

ISSUES OF PROVIDING GUARANTEES AND SOCIAL PROTECTION FOR EAEU LABOUR MIGRANTS

Background: Today, the Eurasian Economic Union (hereinafter the EAEU) has formed its own labour market; however, in the current international environment, issues related to ensuring the health protection of migrants and their access to employment persist. Despite ongoing efforts within the EAEU to develop and deepen Eurasian integration and improve the regulatory framework, there is a need in the age of digitalisation to develop better access for workers to social protection and medical support systems. The relevance of this scientific article lies in the presence of problematic issues related to the fact that many migrants are not registered and lack the documentation to protect them legally and financially; this situation contributes to the development of informal employment and low labour productivity. Although the EAEU countries have established working and socio- economic conditions, the issue of ensuring health protection, access to work, and social protection for labour migrants remains unsolved. Methods: This article employs various methods of scientific cognition, including historical and legal analysis, comparative legal study, induction and deduction, as well as analysis and synthesis. It also applies the acts and regulations of the ILO. What is the role of protecting the socio-economic and labour rights of migrant workers as the main task of receiving states? In law enforcement practice, to what extent do the provisions of international agreements on the organised hiring of workers between receiving countries and countries of origin of migrants ensure the adaptation of migrants to the legal regime of the host country, the participation of migrants in the functioning of civil society institutions, and the monitoring of migration processes? How does national regulation of labour migration in integration associations of states rationally organise their actions on the principles of national treatment for migrant workers? Labour migration strengthens integration unions, creating strong social ties and developing civil society institutions within the boundaries of integration associations. Since state sovereignty is inextricably linked to the protection of the interests of fellow citizens in regulating labour migration, all states participating in regional integration are called upon not only to protect the rights of migrant workers but also to provide all conditions for migrants to fulfil their obligations to civil society. The study examined the free movement and provision of labour and social guarantees for labour migrants in the EAEU and compared their situation in the EU. It also surveyed labour migrants in the EAEU by questioning workers in education, medical care, and various private sectors. This article is based on the results of this study. This article examined the challenges in providing guarantees and social protection for EAEU labour migrants. It proposed improvements to legal regulatory mechanisms and outlined recommendations for improving and expanding opportunities for free movement while better safeguarding the social and labour rights of migrants in the EAEU. The study’s primary objectives were as follows: - Examining the provisions of the ILO Convention on migrant workers. - Collecting and analysing data on labour movement across the EAEU - Analysing international EU treaties regarding the legal status of migrant workers - Interviewing migrant workers in the EAEU on issues related to their provision of rights and guarantees under the EAEU Treaty. Results and conclusions: This study highlights the need to organise safe labour migration, provide labour migrants with full access to digital services, improve the system of vocational and technical education in line with labour market demands, as well as develop and implement a unified system for ensuring the safety of workers and their families. Successful adaptation in the host country is identified as a critical factor for both labour migrants and their families. The authors recommend adding a dedicated chapter to the EAEU treaty that would comprehensively address labour and social rights, along with their guarantees.

Intelligent system based on multiple networks for accurate ovarian tumor semantic segmentation

Ovarian tumors, especially malignant ones, represent a global concern, with increased prevalence in recent years. More accurate medical support systems are urgently needed to support medical staff in obtaining an efficient ovarian tumors diagnosis since detection in early stages could lead to immediately applying appropriate treatment, and implicitly improving the survival rate. The current paper aims to demonstrate that more accurate systems could be designed by combining different convolutional neural networks using different custom combination approaches and by selecting the appropriate networks to be involved in the ensemble model to achieve the best performance metrics. It is essential to understand if combining all experimented networks or only the best-performing ones could always lead to the most effective results or not. The current paper is structured in three main phases. The first step is to propose the individual networks involved in the experiments. Five DeepLab-V3+ networks with different encoders (ResNet-18, ResNet-50, MobileNet-V2, InceptionResNet-V2, and Xception) were used. In the second step, the paper proposes a custom algorithm to combine multiple individual semantic segmentation networks, while the last step describes the iterative selection approach for selecting all individual networks to be combined so that the most accurate ensemble is obtained. The system performing semantic segmentation for different types of ovarian tumors, covering both benign and malignant ones, achieved 91.18 % Intersection over union (IoU), thus overperforming all individual networks. The proposed method could be extended so that more powerful deep learning models could be used.

Exploring Stress and Coping in Caregivers of Children with Pulmonary Vein Stenosis: A Mixed-Method Study.

(1) Background: This mixed-method study aims to identify and describe factors associated with acute and long-term parenting stressors and coping strategies in caregivers of children with intraluminal pulmonary vein stenosis (PVS). (2) Methods: Parents of children with severe PVS were recruited from a large quaternary hospital to complete a survey that included demographics, the Pediatric Inventory for Parents (PIP), and the Coping Health Inventory for Parents (CHIP). We determined the Social Vulnerability Index (SVI) based on self-reported home address. A subset of caregivers completed a 60 min semi-structured interview via Zoom exploring the impact their child's diagnosis had on their family; experience managing stress in the hospital and at home; current resources and processes for coping; and potential recommendations for hospitals to build resilience and coping. We used multivariable linear regression to examine the association between SVI and parental stress and coping while adjusting for possible confounders. Thematic analysis identified themes related to stress and coping. Finally, we assessed instances of convergence and difference between the qualitative and quantitative results. (3) Results: Participants included 32 caregivers who were 91% female with a mean age of 39 years. The children of participants were 66% female, with a mean age of five years. The parents reported a high amount of stress with an average PIP score of 120, nearly 46 points higher than similar studies in the congenital heart community. We observed no significant associations between SVI and either parental stress or coping in adjusted models. We identified 13 themes, including medical care, hospital, family, support systems, and home medical routine or support. (4) Conclusions: Our study found high levels of illness-related parental stress among caregivers of children with PVS. Stress evolved over time from what caregivers described as 'survival mode' to a future-oriented outlook. Currently, caregivers rely heavily on support networks that are not available to all caregivers or may experience strain over time. Caregivers indicated that communication and parental role functioning were coping strategies that could be better supported by providers and health systems.

The effectiveness of intraoperative ultrasound control in removing foreign bodies of soft tissues in military personnel with fragmentation wounds at the stage of specialized surgical care

BACKGROUND: The involvement of civilian medical institutions in medical care in military conflicts and counter-terrorism operations has become an important area of the medical support system. Modern methods of surgical navigation make possible minimal invasive, effective and safe removing foreign bodies from soft tissues. Clear understanding of injuries’ character allows better planning of procedures. AIM: Improving the results of treatment of military personnel with fragment wounds by taking into account the character of injury and the use of ultrasound control during removing soft tissue foreign bodies at the stage of specialized surgical care. MATERIAL AND METHODS: Data from 253 case histories who were treated at the State Budgetary Institution RO “Central City Hospital named Semashko”. Indications for the removal of fragments included the size of the fragment over 7 mm, localization near to large vessels and nerve trunks, and an acute purulent process. The patients were been randomized to two groups. Group I (control) included 118 patients in whom surgery was been performed in the traditional way. Group II (main) included 135 patients in whom procedures were performed with ultrasound navigation. The research methods are ultrasound, X-ray, clinical, and statistical. RESULTS: In 164 patients (64.8%), the wounds were of an isolated nature, where fragments were localized in various anatomical areas. 89 patients (35.2%) had combined shrapnel wounds. Of the 118 patients in group I, in whom surgery was performed in the traditional way, 65 patients (55.1%) underwent surgery under local anesthesia, 53 (44.9%) — under general anesthesia (anesthesia is always general). In 135 patients of group II, operations were performed by only one surgeon under local anesthesia. The duration of surgery, surgery invasion and wound infections rate were significantly lower in group II. CONCLUSION: The part of isolated fragment wounds is 64.8%, combined — 35.2%. The use of ultrasound navigation during surgery can significantly improve the effectiveness of surgical treatment of patients with fragmentation wounds: reduce the invasiveness and duration of surgery, blood loss, and the rate of wound infectious complications.

Expert gaze as a usability indicator of medical AI decision support systems: a preliminary study

Given the current state of medical artificial intelligence (AI) and perceptions towards it, collaborative systems are becoming the preferred choice for clinical workflows. This work aims to address expert interaction with medical AI support systems to gain insight towards how these systems can be better designed with the user in mind. As eye tracking metrics have been shown to be robust indicators of usability, we employ them for evaluating the usability and user interaction with medical AI support systems. We use expert gaze to assess experts’ interaction with an AI software for caries detection in bitewing x-ray images. We compared standard viewing of bitewing images without AI support versus viewing where AI support could be freely toggled on and off. We found that experts turned the AI on for roughly 25% of the total inspection task, and generally turned it on halfway through the course of the inspection. Gaze behavior showed that when supported by AI, more attention was dedicated to user interface elements related to the AI support, with more frequent transitions from the image itself to these elements. When considering that expert visual strategy is already optimized for fast and effective image inspection, such interruptions in attention can lead to increased time needed for the overall assessment. Gaze analysis provided valuable insights into an AI’s usability for medical image inspection. Further analyses of these tools and how to delineate metrical measures of usability should be developed.

Exploring medical students' experience of the learning environment: a mixed methods study in Saudi medical college

BackgroundIn medical education, the learning environment (LE) significantly impacts students' professionalism and academic performance. Positive LE perceptions are linked to better academic outcomes. Our study, which was conducted 15 years after curriculum reform at King Saud University's College of Medicine, aimed to explore students' perspectives on their LE and identify areas for improvement. By understanding their experiences, we strive to enhance LE and promote academic success.MethodsThis mixed-method study employed an explanatory sequential approach in which a cross-sectional analytical survey phase was collected first using the Johns Hopkins Learning Environment Scale (JHLES), followed by qualitative focus groups. Findings from quantitative and qualitative methods were integrated using joint display.ResultsA total of 653 medical students completed the JHLES. The total average score was 81 out of 140 (16.8), and the average subscale scores ranged from 2.27 (0.95) for inclusion and safety to 3.37 (0.91) for community of peers. The qualitative approach encompasses both inductive and deductive analyses, identifying overarching themes comprising proudness, high expectations and competition, and views about the curriculum. The integration of results emphasizes the need for continued efforts to create a supportive and inclusive LE that positively influences students' experiences and academic success.ConclusionThis research offers valuable insights for educational institutions seeking to enhance medical education quality and support systems. Recommendations include faculty development, the cultivation of supportive environments, curriculum revision, improved mentorship programs, and initiatives to promote inclusivity and gender equity. Future research should explore longitudinal and comparative studies, innovative mixed methods approaches, and interventions to further optimize medical education experiences. Overall, this study contributes to the ongoing dialog on medical education, offering a nuanced understanding of the complex factors influencing students' perceptions and suggesting actionable strategies for improvement.

Interaction of subjects of the educational process as a conditioned actualization of students’ personal abilities, taking into account the needs in education

The trend towards an increase in the number of students with disabilities necessitates the development of a system of psychological, medical and pedagogical support in educational institutions that takes into account the special educational needs of such children. The purpose of the article is to consider the model of functioning and interaction of subjects of the educational process within the framework of continuous psychological, medical and pedagogical support for students with special educational needs. Methodology. The methodological basis of the study was the theoretical and methodological aspects of the formation of a socially adaptive educational space (N. P. Abaskalova, E. M. Kazin, N. E. Kasatkina, O. G. Krasnoshlykova, I. A. Sviridova); the concept of socio-psychological and physiological adaptation of students (E. M. Kazin, N. E. Kasatkina, V. P. Kaznacheev, R. M. Baevsky). Research results: the article shows the specifics of psychological, medical and pedagogical support for students with disorders of the musculoskeletal system, discusses the tasks and ways of interaction between subjects of education in order to create an adaptive, developmental and health-saving environment in an educational institution. In conclusion, it is pointed out the need for constant interaction of participants in psychological, medical and social support of students in order to effectively actualize the personal potential of students with special educational needs.

Patient perspectives on liver transplant evaluation: A qualitative study

ObjectiveLiver transplant (LT) evaluation is a complex process for patients involving multi-step and parallel medical, surgical, and psychosocial assessments of a patient’s appropriateness for transplant. Patients may experience difficulties in navigating the evaluation process, potentially leading to disengagement and resulting in further health decline or death prior to completing evaluation. We aimed to identify and characterize patients’ perceptions of undergoing LT evaluation. MethodsWe performed fourteen 30–45 min, semi-structured interviews between 3/2021–5/2021 with patients at a large LT center. Using the constant comparison method, we individually noted themes within and across interviews and codes. ResultsOur analysis generated 5 thematic dimensions related to patient engagement (i.e., patient involvement/activation): (1) psychological impact of evaluation on patients’ lives; (2) information received during evaluation; (3) prior medical experience of the patient; 4) communication between patients and transplant providers; and (5) support system of the patients. Among these dimensions, we identified 8 themes. ConclusionLT patient engagement is a multi-dimensional component of LT evaluation that incorporates the psychological impact, information received, prior medical experience, communication, and support systems of patients. Practical implicationsThis work can inform targeted interventions for increasing patient engagement during the LT evaluation process.

Oleg Konstantinovich Bumay (in memory of a teacher, colleague, comrade)

January 1, 2024 marked the 70th anniversary of the birth of the former head of the organization and tactics of the medical service of the fleet (with a course of tactics and combat weapons of the fleet) department of the S.M. Kirov Military Medical Academy, effective director for modern topics of the Federal State Budgetary Institution Research Institute of Industrial and Marine Medicine FMBA of Russia, candidate of medical sciences, associate professor Oleg Konstantinovich Bumay (1954–2022). O.K. Bumay was born in the village of Bogudenka, Belarus. He was alumnus the faculty of training doctors for the Navy of the S.M. Kirov Military Medical Academy. After graduating from the academy in 1977, for a decade he provided and organized medical support for nuclear submarines as the head of the ship’s medical service, the cheef of medical a submarine forces in the Northern Fleet, and subsequently served as a senior medical officer in the Baltic Fleet. Over the course of two decades, O.K. Bumay was associated with the organization and tactics of the naval medical service (with a course of tactics and combat weapons of the fleet) department of S.M. Kirov Military Medical Academy. In 1991, he entered the adjunct program at the department; upon completion of his studies, he was appointed as a teacher, subsequently as a senior teacher, and since 2009 — deputy head of the department. He received the rank of Philosophy doctor of Medical Sciences (1994), associate professor (1998). After leaving the Armed Forces of the Russian Federation in 2007, he continued his career as deputy director for special topics at the Federal State Budgetary Institution Research Institute of Industrial and Marine Medicine of the Federal Medical and Biological Agency of Russia. O.K. Bumay developed origins medical support system for the domestic nuclear submarine fleet. Was appointed on the post the head of the ship’s medical service and after that officer of the command-and-control body of an operational-tactical formation. Throughout their career he left a multifaceted scientific and practical legacy, which is used by new generations of naval doctors.

Explainable AI in the military domain

Artificial intelligence (AI) has become nearly ubiquitous in modern society, from components of mobile applications to medical support systems, and everything in between. In societally impactful systems imbued with AI, there has been increasing concern related to opaque AI, that is, artificial intelligence where it is unclear how or why certain decisions are reached. This has led to a recent boom in research on “explainable AI” (XAI), or approaches to making AI more explainable and understandable to human users. In the military domain, numerous bodies have argued that autonomous and AI-enabled weapon systems ought not incorporate unexplainable AI, with the International Committee of the Red Cross and the United States Department of Defense both explicitly including explainability as a relevant factor in the development and use of such systems. In this article, I present a cautiously critical assessment of this view, arguing that explainability will be irrelevant for many current and near-future autonomous systems in the military (which do not incorporate any AI), that it will be trivially incorporated into most military systems which do possess AI (as these generally possess simpler AI systems), and that for those systems with genuinely opaque AI, explainability will prove to be of more limited value than one might imagine. In particular, I argue that explainability, while indeed a virtue in design, is a virtue aimed primarily at designers and troubleshooters of AI-enabled systems, but is far less relevant for users and handlers actually deploying these systems. I further argue that human–machine teaming is a far more important element of responsibly using AI for military purposes, adding that explainability may undermine efforts to improve human–machine teamings by creating a prima facie sense that the AI, due to its explainability, may be utilized with little (or less) potential for mistakes. I conclude by clarifying that the arguments are not against XAI in the military, but are instead intended as a caution against over-inflating the value of XAI in this domain, or ignoring the limitations and potential pitfalls of this approach.

"It made me feel like a shit parent": an intersectional analysis of pandemic mothering.

The COVID-19 pandemic brought to the fore the everyday and exceptional challenges for mothers. Rarely, however, did research or social commentary acknowledge the multiplicities of motherhood during this prolonged period of risk, disruption, and uncertainty. This paper draws upon interviews with 24 mothers living in Aotearoa New Zealand during the pandemic, including women who were pregnant and gave birth during lockdowns, teenage mothers, single and low-income mothers, and working mothers. The sample was intentionally diverse, including Māori, Pacific, Asian and migrant mothers. Engaging an intersectional lens on motherhood and women's health, this paper builds upon and extends feminist research on mothers' experiences during the pandemic, highlighting the many different challenges facing mothers of diverse social, cultural, and economic positionalities and during various stages of motherhood. Across the sample, we reveal the significant emotional toll on mothers, particularly with the absence of critical social, medical and health support systems during lockdown periods and sustained social restrictions. Many of the women described how the pandemic affected their feelings about motherhood, prompting new reflections on their relationships with the home, family, work, and broader society. Despite some similarities, the pandemic experiences of Māori, Pacific, migrant and single mothers were further intensified by various forms of isolation, judgement, and discrimination. In this way, the pandemic shed light on the gendering of everyday maternal life, but also the need for more intersectional culturally and gender-responsive policies that acknowledge the multi-layered complexities of mothers' lives.

СОВЕРШЕНСТВОВАНИЕ КЛИНИКО-ЛАБОРАТОРНОЙ ДИАГНОСТИЧЕСКОЙ СИСТЕМЫ В РАМКАХ МЕДИЦИНСКИХ РЕАБИЛИТАЦИОННЫХ МЕРОПРИЯТИЙ ДЛЯ ЛИЦ ОПАСНЫХ ПРОФЕССИЙ

Annotation. Risky professions are associated with working conditions in which production factors cause a threat to the life of an employee and a high risk of injury. Employees of law enforcement agencies (armed forces, law enforcement agencies, Ministry of Emergency Situations) are included in the list of dangerous professions. Their work is associated with the presence of threats to life and health, psychological and physical overstrain, overwork, suddenness of events with a shortage of time, information overload, etc. Their adaptive mechanisms are often deformed. Among them, one of the most vulnerable is the endocrine system. In order to fully implement all measures for the provision of medical care and rehabilitation of persons performing official tasks in conditions adversely affecting their health, special medical and psychological programs have been developed and are successfully functioning in law enforcement agencies. They include a variety of aspects, among which the leading ones are assessment, consideration of the psychophysiological condition, its pharmacological and non-drug correction by various methods. Modern realities dictate the need for continuous improvement of this system. The proposed model implies the improvement of the laboratory level of endocrinological care in law enforcement agencies within the framework of general medical preventive and rehabilitation measures. It includes several stages (preventive, curative and informational-analytical), which, in turn, are divided into blocks. Each of them serves as an integral part of medical support in the system of rehabilitation measures for persons of dangerous professions, forming a whole multi-level dynamic process. It is emphasized that the organization of comprehensive rehabilitation measures in relation to employees of law enforcement agencies exposed to various occupational risks, in addition to traditional medical and psychological support of activities, should include a mandatory laboratory endocrinological link as an integral part of the general system of departmental medical support.

Prospects for applying distributed database technology in medical property resources management within military health care

When considering the problems of digital transformation of various spheres of life support for the country and maintaining the defense capability of the state, it seems advisable to focus on the most effective digital technologies. Among them, in the context of an increasing risk of cyber threats, blockchain technology, which has a promisingly increasing potential for information security and demonstrates dynamic development in many areas of socio-economic life, primarily attracts at- tention. The article discusses the historical aspects of the development of blockchain tech- nology, presents the results of a study of its main characteristics, international and domestic experience of application, and also presents prospects for implementa- tion in the management of medical property resources. It has been determined that in the context of using distributed database technology in the field of military healthcare, the preferred solution is to use a closed type of blockchain, characterized by a high degree of digital security and scalability. Based on the results obtained during the study, the main advantages and limita- tions of implementing distributed database technology within military healthcare were highlighted. It has been determined that in order to increase the efficiency of the medical ser- vice, it is promising to build a single digital loop for managing medical property resources on the basis of blockchain technology in accordance with the territorial medical support system. It is concluded that digital transformation through the integration of blockchain technology into the practical activities of a medical service can be an effective tool for optimizing the processes of managing medical property resources. However, further comprehensive study of this issue is required, as well as justification and development of possible modifications of existing digital solutions, taking into ac- count the needs of military healthcare and the need to rationalize the provision of medical equipment to assigned contingents.

ИСПОЛЬЗОВАНИЕ МАТРИЧНОГО МЕТОДА ДЛЯ ОЦЕНКИ СИСТЕМЫ МЕДИЦИНСКОГО ОБЕСПЕЧЕНИЯ РАБОТНИКОВ ПРОМЫШЛЕННЫХ ПРЕДПРИЯТИЙ НЕФТЯНОЙ КОМПАНИИ

Staff health protection is an integral part of the corporate social policy of oil companies organized on a divisional basis. A medical support system based on outsourcing requires an effective system for monitoring its functioning. Since it includes dozens of objects, such control is possible only when using digital technologies. The matrix method of information representation was chosen for the implementation of the task. Two matrices were developed: the health risk management results Matrix and the health risk resource and process management Matrix. System was developed for quantitative risk assessment, key indicators, processes and outcomes of medical support personnel for each group of indicators selected a five-point scale by experts determined the weights for each factor. The developed methodology was applied to analyze the state of health and evaluate the medical support system at 49 enterprises. Special attention was paid to enterprises located in the "orange" zone of relative disadvantage (51-75 points). For three years of observations, 9-10 enterprises were registered in it, with 4 enterprises in both matrices. Based on the analysis of reports, a set of measures to correct the situation for specific enterprises was developed. Conclusion. Building health risk matrices using information technology allows you to monitor a significant number of enterprises, quickly obtain information, identify problem situations and make targeted management decisions.

Artificial Intelligence-Based Patient Monitoring System for Medical Support.

In this paper, we present the development of a monitoring system designed to aid in the management and prevention of conditions related to urination. The system features an artificial intelligence (AI)-based recognition technology that automatically records a user's urination activity. Additionally, we developed a technology that analyzes movements to prevent neurogenic bladder. Our approach included the creation of AI-based recognition technology that automatically logs users' urination activities, as well as the development of technology that analyzes movements to prevent neurogenic bladder. Initially, we employed a recurrent neural network model for the urination activity recognition technology. For predicting the risk of neurogenic bladder, we utilized convolutional neural network (CNN)-based AI technology. The performance of the proposed system was evaluated using a study population of 30 patients with urinary tract dysfunction, who collected data over a 60-day period. The results demonstrated an average accuracy of 94.2% in recognizing urinary tract activity, thereby confirming the effectiveness of the recognition technology. Furthermore, the motion analysis technology for preventing neurogenic bladder, which also employed CNN-based AI, showed promising results with an average accuracy of 83%. In this study, we developed a urination disease monitoring system aimed at predicting and managing risks for patients with urination issues. The system is designed to support the entire care cycle of a patient by leveraging AI technology that processes various image and signal data. We anticipate that this system will evolve into digital treatment products, ultimately providing therapeutic benefits to patients.

STATE POLICY OF SUPPORTING WAR VETERANS IN THE ACTIVE PHASE OF COMBATS IN UKRAINE

The article analyzes the state policy of supporting war veterans in Ukraine in the active phase of hostilities, prospects for the introduction of new mechanisms, problems of providing for veterans and ways to solve them. Among the main problems that currently exist in Ukraine: failure to identify the real needs of veterans and members of their families, as well as their dramatic changes since the full-scale invasion; ignorance by veterans and members of their families about opportunities from the state, budgetary institutions, and international organizations; support for veterans is organized as a system of payments and benefits, for which veterans must apply separately. The purpose of the article is to analyze the state policy of supporting war veterans in Ukraine in the active phase of hostilities, prospects for the introduction of new mechanisms, problems of providing for veterans and ways to solve them. Methods of analysis and synthesis were used (in the study of the system of medical and social support for combatants in Ukraine); theoretical methods - a systematic approach (analysis of state policy to solve problems in the field of medical, physical and psychological rehabilitation of combatants); empirical methods - comparison (experience in the field of managing medical and social support for combatants was analyzed). The key priorities of the Ministry of Veterans are summarized, including: rehabilitation and return of veterans to normal life; memory and respect; resocialization. The system of social protection, healthcare, psychological rehabilitation and socialization of war veterans, which need improvement, has been partially revealed.
