Numerous techniques have been developed over time to manage the unstable anterior cruciate-deficient knee, including extra-articular procedures, autograft reconstruction using fascia lata, hamstring tendons, and patellar bone-tendon-bone, and synthetic replacement or augmentation. In this article we describe a technique using an ultra-strong tendoachilles allograft, which has proven over the last 8 years to provide excellent functional stability, rapid rehabilitation, and return to activity with very low morbidity.
