Frequency of Mandibular Dysfunction Symptoms in Institutionalized Elderly People

Mandibular dysfunction symptoms were found in 74% of 375 elderly persons, aged 65 or over, 79% of the women and 59% of the men (p less than 0.001). 36% had one symptom, 26% two symptoms, 8.5% three symptoms, and 3% 4--5 symptoms. The most frequent were the temporomandibular joint (TMJ) sounds (48%). Deviation of mandible on opening and/or closing movement was found in 44%, in women more often than men (p less than 0.05). The frequency of limited opening was 14%, higher among women than men (p less than 0.05). Tenderness to palpation laterally over the TMJ was observed in 14%. 7% reported pain on opening movement, women more often than men (p less than 0.01).

Correlations of electromyographic silent period duration and the Helkimo dysfunction index

Fifty-eight patients seen for consultation concerning mandibular dysfunction symptoms were examined utilizing the Helkimo index of dysfunction and electromyographic silent period recordings from masticatory muscles. The data collected were analyzed for correlations between the two parameters. Several significant correlations were noted between certain variables of the dysfunction index and the duration of the silent period. The mean duration of the silent period increased with increasing dysfunction index value. However, the clinical significance of this correlation is not established as long as we do not know the validity of neither the dysfunction index nor the the EMG silent period duration.

Recording of the retruded position of the mandible in patients with mandibular dysfunction

The precision (reproducibility) of active and passive recordings of the retruded position of the mandible was studied by two examiners on 10 patients with mandibular dysfunction symptoms. The position of the mandible was recorded with an intra-oral graphic method, before and after treatment of the symptoms. The precision of the recording was highest when the retruded position was recorded by passive hinge movement and lowest when it was recorded by active hinge movement and when recording habitual closure. Both systematical and accidental errors tended to be somewhat larger among these patients than that previously found among individuals without signs or symptoms of mandibular dysfunction. The accidental errors in antero-posterior and medio-lateral directions were the same fo both examiners and of the same magnitude before and after treatment of the symptoms. Both examiners recorded the retruded position on the average 0.20 mm more posterior after treatment than before. The results showed that because of its good reproducibility the retruded position of the mandible can be recommended as a reference position in functional analysis of occlusion and for jaw recordings also in patients with TMJ muscle-pain dysfunction symptoms. During the recording the conventional technique with passive hinge movement and a posterior pressure should be used.

Subjective Symptoms in Temporomandibular Joint Osteoarthrosis

Questionnaires about subjective symptoms were sent by post to 120 patients referred to the clinic for mandibular dysfunction. A group of 20 patients with crepitation of the temporomandibular joint (TMJ) and a group of 19 patients with palpatory tenderness of the TMJ were compared with a reference group of 29 patients for any difference in local subjective symptoms, frequency of headache and symptoms in other joints. The patients with TMJ crepitation, which were considered to have TMJ osteoarthrosis, were older and reported a higher frequency of grating sound from the TMJ than the patients in the reference group. The patients with palpatory tenderness of the TMJ reported a higher frequency of toothgrinding, more severe symptoms of mandibular dysfunction and a higher frequency of symptoms in the hand joints. No correlation was found between subjective symptoms of mandibular dysfunction and those of other joints. The results of the study indicate that patients with TMJ osteoarthrosis do not differ from other patients with mandibular dysfunction with respect to subjective symptoms and that subjective symptoms of mandibular dysfunction in general develop independently of those in other joints.

Symptoms of mandibular dysfunction in complete denture wearers

Recent epidemiological studies have shown that symptoms of mandibular dysfunction are very common, but prosthodontists have thought that complete denture wearers are spared such complications. However, in recent reported series of patients who have sought advice for mandibular dysfunction complete denture wearers have been included. According to epidemiological data individuals with few remaining natural teeth had more symptoms of dysfunction than those with most of their natural teeth, and complete denture wearers had more symptoms of mandibular dysfunction than subjects with natural teeth. Mandibular and temporomandibular joint dysfunction symptoms have a heterogeneous background, but occlusal instability is an important aetiological factor. In complete denture wearers with mandibular dysfunction, symptoms often disappear after improvement of the occlusion.

Frequency of symptoms of mandibular dysfunction in young Swedish men

The occurrence of symptoms of dysfunction of the masticatory system was studied in 253 Swedish men from the region of Stockholm with an average age of 19 years. Besides inquiry by questionnaire regarding subjective symptoms a clinical functional examination of the masticatory apparatus was performed. Fourteen per cent were aware of clicking of the temporomandibular joint and 12% of other symptoms of dysfunction such as difficulties in opening the mouth wide, locking and pain on performance of various movements of the mandible. In the clinical investigation, symptoms of dysfunction were noted in 28% of those examined, mainly in the form of tenderness to palpation over the temporomandibular joints and of the masticatory musculature as well as difficult, painful and irregular movements of the mandible. Individuals with clinical symptoms of dysfunction of the masticatory system had a higher frequency of other joint and muscle symptoms, clicking of the temporomandibular joint and occlusal disturbances in the form of balancing side interferences, than those without clinical symptoms. Balancing sides interferences were the only occlusal disturbance that was significantly correlated with the symptoms of dysfunction (pain on movement and tenderness to palpation).

Symptoms of functional disturbances of the masticatory system. A comparison of frequencies in a population sample and in a group of patients.

With the aid of questionnaires symptoms of mandibular dysfunction and some general and oral conditions were studied in a group of previous patients and in a randomly selected population sample. From the two materials answers were obtained from 82 persons (81%) and 1.106 (91%) respectively. The results confirmed that women are heavily overrepresented in patient materials. This differs from the fairly equal sex distribution found in population studies of mandibular dysfunction. The most frequent symptoms of mandibular dysfunction were TMJ-sounds which appeared in 39 and 79% and pain on opening the mouth which appeared in 12 and 42% of the population and patient samples respectively. All symptoms of mandibular dysfunction, headache, clenching of the teeth and unilateral chewing appeared significantly more often in the previous patients, but the number of natural teeth did not differ in the two samples. The general state of health was poorer and general joint -- muscle symptoms were more common in the previous patients as well as in those of the population sample with symptoms of mandibular dysfunction. This finding suggests that functional disturbances of the masticatory system often may be related to impaired general health.
