Abstract TP104: Bridging the Education Gap: Strengthening Stroke Rehabilitation in the Midwest

Introduction: Post-acute care (PAC) facilities across Iowa, Montana, Nebraska, and North Dakota are engaged in the American Heart Association’s Mission: Lifeline Stroke PAC Initiative, a quality improvement initiative focused on enhancing stroke rehabilitation through evidence-based practice. Participants across these states collaborate to improve patient care, identify gaps in current practices, and enhance stroke specific education. Purpose: In a 2023 survey of healthcare professionals (HCPs) in Iowa, 48% (n=120) of respondents identified professional education as a missing resource that could help them provide rehabilitation care to stroke survivors. Through the initiative, education was developed to address this identified need. Methods: A variety of free educational opportunities were offered throughout the Mission: Lifeline Stroke Initiatives. These included in-person stroke conferences and workshops, virtual learning modules, online courses, and discipline specific rehabilitation certifications. 760 HCPs participated in 12 different educational offerings across Midwest states. Attendees were surveyed to assess the perceived benefits of the offerings, utility to their current role, and improvements in knowledge and confidence providing rehabilitation care to stroke patients. Results: Following the completion of the educational offerings, 93 attendees completed a post-survey. The large majority of respondents reported they were extremely or mostly confident in identifying signs of a stroke (95%), describing common deficits after a stroke (96%), and implementing evidenced-based practices to address these deficits (90%). Most respondents agreed the trainings improved their knowledge of the relationship between treatment and stroke rehab patient outcomes (91%) and their understanding about key issues related to working with stroke rehab patients (91%). Overall, 78% of respondents reported they always or often use the knowledge and skills learned from the trainings and 95% were satisfied with the content of the trainings. Respondents also identified additional educational needs for cognitive rehabilitation strategies, aphasia interventions, caregiver education, community integration, and general stroke rehabilitation. Conclusions: Mission: Lifeline Stroke educational offerings benefitted HCPs by improving their knowledge and confidence to provide care to stroke patients. Education on additional stroke specific topics were also identified.

Perceptions and experiences of healthcare professionals of implementing the Organ Donation (Deemed Consent) Act in England during the Covid-19 pandemic

ContextIn May 2020 during the COVID-19 pandemic, England implemented a ‘soft’ opt-out system of consent to deceased organ donation. As part of a wider evaluation, this analysis focused on the perceptions of health care professionals, specifically their experiences of implementation.MethodsMixed-methods study informed by Normalisation Process Theory, based on two national surveys of health care professionals and interviews, observations and document analysis, across two case study sites. Routine NHS Blood and Transplant’s audit data provided context.Findings70 interviews with 59 staff and 244 first and 738 second surveys. COVID-19 affected every aspect of implementation. Although supportive in principle, many staff were unconvinced that legislative changes alone would increase consent rates. Many staff were redeployed or left their jobs. As a result, staff were not able to work collectively as intended for implementation. Staff received routine donor audit data suggesting the law was yet to make a difference to consent rates, reducing their enthusiasm and commitment.ConclusionsImplementation could have been more impactful if delayed. The National Health Service needs to reprioritise organ donation and relaunch the implementation plan in the post-pandemic period, though it is unlikely the changes will bring about a significant increase in consent rates.

Assessing the role of education level on climate change belief, concern and action: a multinational survey of healthcare professionals in nephrology.

Climate change poses a significant risk to kidney health, and countries with lower national wealth are more vulnerable. Yet, citizens from lower-income countries demonstrate less concern for climate change than those from higher-income countries. Education is a key covariate. To examine its role in explaining this perception gap, we obtained the perspectives of a highly educated cohort of healthcare professionals. This was a cross-sectional survey of healthcare professionals involved in kidney care. Responses were compared by the income level of the participant's country (per World Bank). Of the 849 healthcare professionals from 107 countries (63.4% from lower and middle-income countries) that participated, most believed climate change was happening (97.9%), displayed a high level of concern (73.3%), and took personal action to combat climate change (62.0%). While the proportion who believed in climate change did not vary by income level (high:98.1%, upper-middle:97.2%, lower-middle:97.8%, low:100%, p = 0.73), the proportion with a higher level of concern (high:80.7%, upper-middle:74.9%, lower-middle:67.5%, low:53.8%, p < 0.001), and who took climate action (high:76.2%, upper-middle:63.1%, lower-middle:51.2%, low:30.8%, p < 0.001) decreased by national wealth. Barriers to involvement in sustainable kidney care were lack of time (54.4%), knowledge (39.7%), and peer support (30.3%). Only 34.0% were aware of national mitigation plans and barriers related to finances, technologies, tools, methods, research, and evidence were perceived as greater obstacles in lower-income countries. Our results highlight that predictors and correlates of climate change risk perception vary across countries. Education alone is unlikely to increase individual and group engagement in climate change. A better understanding of these factors can inform strategies towards climate action in different settings.

P-509. Impact of an Innovative Educational Initiative Using Community-Based Organization Partnerships to Reach Young Adults at Risk for HIV

Abstract Background Young adults (YA) are particularly vulnerable to acquiring HIV due to low self-perceived risk, low rates of HIV testing, and lack of knowledge/awareness about pre-exposure prophylaxis (PrEP). Methods Between 3/23 – 8/23, six education sessions for YAs were conducted in the Southeastern U.S. by local healthcare professionals (HCPs) along with community-based organizations (CBOs) in non-clinical settings (e.g. college campuses, breweries). Pre- and post-surveys were administered, with follow-up surveys 3 weeks later. A scale-up education toolkit was disseminated to HCPs nationwide. Results A total of 264 YAs participated in the education sessions (mean age: 24 years; 85% female; 54% Caucasian; 87% heterosexual) with 29% previously tested for HIV and 21% who previously discussed PrEP with a HCP. YA’s knowledge of different forms of PrEP improved significantly (38% pre- vs. 96% post-activity; p &amp;lt; 0.001) as did their confidence in protecting themselves from acquiring HIV (55% pre vs. 93% post; p &amp;lt; 0.001). In addition, awareness of local resources for HIV testing improved from 60% to 93% (p &amp;lt; 0.001) with a significant increase in YAs who stated they were likely to get tested for HIV (26% pre vs. 63% post, p &amp;lt; 0.001). Seventy-five HCPs and CBO leaders also participated. Interestingly, there was a disconnect between CBO/HCPs’ perceptions of barriers to HIV/STI testing compared to YAs’ reported challenges (Fig 1). All providers stated they were likely/very likely to organize another session. A follow-up survey of HCPs (N = 10) found they helped 58 YAs access HIV testing and 33 access PrEP since the education, with 70% of HCPs using the materials during 1:1 counseling with YAs, suggesting sustainability of the resources. HCPs (N = 867) who reviewed the toolkit had improvements in their confidence in differentiating between PrEP options and identifying who would benefit from each approach (p &amp;lt; 0.001) and counseling YAs about PrEP (p &amp;lt; 0.001). Conclusion Providing HIV prevention education to YAs in informal, community settings increases reach to those who may not have these discussions with their HCPs during regular visits. This type of educational initiative, which includes partnering with CBOs, is scalable and sustainable. Disclosures Meredith E. P-1547. Real-World Patient Insights Inform an Action-Oriented Healthcare Professional (HCP) Educational Initiative to Address Gaps in Uncomplicated Urinary Tract Infection (uUTI) Management

Abstract Background uUTIs are among the most common outpatient infections and reasons for antibiotic use. This project aimed to understand the real-world burden of UTIs on patients, uncover HCP challenges in uUTI management, and educate HCPs to improve knowledge of guidelines and align clinical practices with patient priorities.Table 1.Demographics of Women Who Experience UTIs Who Participated in the Patient Survey (N = 413). Methods Patient surveys were distributed nationwide to women who experience UTIs (Table 1). Baseline HCP surveys were administered in 6 clinics that treat UTIs in the southeast US (Table 2). Survey outcomes informed live education sessions with each clinic from 10/2023 – 2/2024, featuring pre-/post-session surveys and team-based action planning.Table 2.HCP Baseline Survey Participant Demographics (N = 94). Results Outcomes from the patient survey revealed the extensive burden of UTIs on quality of life (N = 413, Fig 1). Patients experienced 4 UTIs in the last year on average and reported the most difficult aspect of experiencing UTIs was fear of recurrence (49%), with 79% of patients seen in urology clinics and 66% of patients seen in infectious disease clinics agreeing they were frequently concerned about recurrence versus 58% of patients seen in other clinics (p &amp;lt; .001 and p = .217, respectively). HCPs (N = 94) also reported their top challenge in uUTI care was management of persistent or recurrent infections (54%) and felt they could most improve on antibiotic stewardship (47%) and aligning clinic protocols with the latest guidelines (42%). HCP education sessions led to improvements in knowledge and confidence in uUTI management. After sessions, more HCPs correctly selected 5 days of nitrofurantoin as a first-line uUTI treatment (85% vs 61%, p &amp;lt; .001) and reported high confidence in applying the latest guidelines (72% vs 40%, p &amp;lt; .001) than before. Patient survey data presented to HCPs during sessions showed patients thought their care teams could most improve on counseling about antibiotic side effects (50%) and education about risks of frequent antibiotic use (47%). In response, 93% of HCPs committed to counseling patients about antibiotic use and UTI prevention and clinic action plans included expansion of patient education efforts.Figure 1.Real-World Patient Insights on UTI Burden. Conclusion This initiative offers real-world insights on the burden of UTIs on patients and demonstrates the potential impact of patient-informed education on HCP knowledge and clinical practice in uUTI care. Advancing Healthcare Management and Patient Outcomes through Business Analytics: A Strategic Approach

Business analytics has started to find its way into the healthcare industry. It has proven to transform the sector by effectively using data in decision-making, enhancing patient experience, and increasing efficiency. This paper explores the use of analytics tools in clinical and administrative areas of focus, including hospital readmission rates, emergency department wait times, and cost savings. A qualitative survey of healthcare professionals complemented a quantitative approach to performance measurement to evaluate the impact of predictive, descriptive, and prescriptive analytics. The results show that readmission rates are down by 38.9%, and the average wait time in emergency departments is down by 25.6%. Logistics, workforce, and inventory showed a cost reduction, proving that adopting analytics was cost-effective. However, some concerns included workforce resistance, data integration issues, and data privacy and fairness issues. In order to overcome the challenges highlighted in the study, the following solutions are suggested: workforce training, improvement of IT facilities, and data governance policies. These measures will help to avoid complications in the adoption and achieve fair utilization of analytics tools. The paper establishes that analytics promises to revolutionize healthcare management, especially when deployed in convergence with other disruptive technologies such as Artificial Intelligence. Subsequent studies must examine the ramifications of analytics on healthcare systems in the international sphere and their role in meeting emerging concerns such as population aging and health emergencies. Consequently, this research highlights business analytics as the key tool that furthers patient care objectives and business sustainability in the hospital.

Cross-sectional survey of healthcare professionals' perceptions of bedrails as a measure of physical restraint in the hospital setting.

Although bedrails are considered a restraint measure under the legal regulations in Spain, no data on the related perceptions of healthcare professionals (HCPs) are available. Thus, this study aimed to calculate the proportion of HCPs in the hospital setting that consider bedrails to be a restraint measure. Between June and December 2022, a cross-sectional online survey was conducted among the HCPs at our institution. Basal information was collected in the first section, and seven questions related to bedrails were included in the second. The perception of bedrails as a restraint measure was evaluated with a multiple-choice question in which devices that can be considered restraint measures were listed. A total of 246 HCPs participated in the survey, 201 of whom passed quality control and were analysed. The mean age was 42.2years old, and 85% of the participants were women.The proportion of HCPs who considered bedrails as a restraint measure was 54.2% (95% confidence interval [CI]: 47.3-61.1) in the total group and 54.5% (95% CI: 44.1-65.0) in the intermediate care subgroup. These proportions were significantly lower for HCPs who worked night shifts and among those who believed that bedrails do not limit patient mobility. There is a considerable difference between what is established in legal regulations and the perceptions of HCPs in terms of bedrails being considered a restraint measure. Our results can help improve the content of training programs on the use of restraint measures by HCPs.

A technology-enabled collaborative learning model (Project ECHO) to upskill primary care providers in best practice pain care.

Background The South Australian (SA) Chronic Pain Extension for Community Healthcare Outcomes (ECHO) Network was established to upskill primary care providers in best practice pain care aligned to a patient-centred, biopsychosocial approach using didactic and case-based virtual mentoring sessions. The aims of this study were to assess: (a) participation, satisfaction (relevance, satisfaction with format and content, perceptions of the mentorship environment), learning (perceived knowledge gain, change in attitudes), competence (self-confidence) and performance (intention to change practice, perceived practice change) of the ECHO Network clinician participants; and (b) self-perceived barriers at the clinical, service and system level to applying the learnings. Methods A mixed methods, participatory evaluation approach was undertaken. Data sources included analysis of program records (learning needs assessment, enrolment survey data, program participation data and online surveys of healthcare professionals including a satisfaction survey after each ECHO session (n=106 across the ECHO series of 10 sessions; average response rate=46%), a case presenters survey (n=7, response rate=78%) and an outcome survey after all 10 ECHO sessions (n=11, response rate=25%). Results Forty-four healthcare professionals participated in the ECHO Network from a range of career stages and professional disciplines (half were general practitioners). One-third of participants practised in regional SA. Participants reported that the ECHO sessions met their learning needs (average=99% across the series), were relevant to practice (average=99% across the series), enabled them to learn about the multidisciplinary and biopsychosocial approach to pain care (a verage=97% across the series) and provided positive mentorship (average=96% across the series). Key learnings for participants were the importance of validating the patient experience and incorporating psychological and social approaches into pain care. More than one-third of participants (average=42% across the series) identified barriers to applying the learnings such as limited time during a consultation and difficulty in forming a multidisciplinary team. Conclusions The ECHO Network model was found to be an acceptable and effective interdisciplinary education model for upskilling primary care providers in best practice pain care aligned to a patient-centred, biopsychosocial approach to pain managment. However, participants perceived barriers to translating this knowledge into practice at the clinical, service and system levels.

Diagnosis and treatment of occupational burnout in the Swiss outpatient sector: A national survey of healthcare professionals' attributes and attitudes.

We aimed to describe the attributes and attitudes of Swiss health professionals who treat persons with occupational burnout (POB) in the outpatient sector and explore associated determinants. The study design was descriptive cross-sectional survey, distributed to the 16,883 general practitioners (GP), psychiatrist-psychotherapists (PP), occupational physicians (OP) and psychologists registered in the Swiss Medical Association, the Swiss Federation of Psychologists, and other specialized associations. Using an online questionnaire, we identified professionals who consult and treat POB, their attributes, volume of POB consultations, diagnostics and treatment modalities and outcomes (OB severity, average proportion of POB who returned to work and who relapsed). Multinomial regression analysis was conducted to identify attributes associated with these outcomes. Among 3216 respondents, 2951 reported to consult POB, and 1130 (713 physicians and 410 psychologists) to treat them. POB consultations constitute 5 to 25% of professionals' consultations, which varies across professionals' specialties and specializations and geographic regions. The profile of POB consulted also differs across professionals. Work psychologists reported more often consulting POB at early OB stage, GPs mostly reported having patients with moderate OB, while PPs reported having the largest proportion of patients with severe OB. The treatment practices depend on OB severity but neither latter nor former was associated with the proportion of relapsed POB or POB who return to work. Physicians with waiting time >3 months reported more often having a higher proportion of relapsed patients. Since the study had an exploratory nature using a cross-sectional survey design and aggregated outcomes, these findings should be considered as first descriptive data, motivating further research.

Family planning, reproductive health and pregnancy after bariatric surgery: a survey of bariatric healthcare professionals in Switzerland.

A wide range of reproductive health issues, including fertility, pregnancy outcomes and contraceptive practices can be affected by morbid obesity and weight loss subsequent to bariatric surgery. This study aimed to explore the attitudes and practices of bariatric healthcare professionals in Switzerland regarding reproductive health counselling in the context of bariatric surgery. We conducted a national, cross-sectional, 36-question online survey among bariatric professionals in Switzerland. Survey topics included demographic factors, baseline characteristics of bariatric patients, perioperative reproductive health practices, attitudes and knowledge about contraception and recommendations regarding pregnancy. The survey was open from 1 October 2022 to 30 April 2023. A total of 75 healthcare professionals participated in the survey.The majority of participants responded that female patients of reproductive age constitute more than half of the referred bariatric patients. Forty participants (57%) recommended contraception for a duration of 18-24 months following a bariatric operation. Only twenty-three respondents (31%) stated that they always refer their female bariatric patients of reproductive age to a gynaecologist prior to bariatric surgery. Fifty-six participants (75%) replied that they always discuss family planning prior to surgery. There was high variation regarding the recommended methods of contraception after surgery.Only thirty-nine participants (52%) reported that they always inform their patients about the occurrence of possible surgery-related complications during future pregnancies. More than half of the participants reported inadequate knowledge regarding absorption and safety of contraceptive pills after bariatric surgery. Although most professionals routinely follow up and provide counselling on maternal and foetal risks in pregnant women with previous bariatric surgery, forty-nine participants (65%) have no standardised protocol for such patients who present with acute abdominal pain in their practice. Despite acknowledging the importance of reproductive health counselling, bariatric professionals address perioperative and contraception issues inconsistently and mostly reported a lack of adequate knowledge in the latter. Therefore, a stronger collaboration between bariatric professionals and women's healthcare providers is needed to improve care of female bariatric patients of reproductive age.

Mapping challenges in the accessibility of treatment products for urea cycle disorders: A survey of European healthcare professionals.

Current management guidelines for urea cycle disorders (UCDs) offer clear strategies, incorporating both authorized and non-authorized medicinal products (including intravenous formulations and products regulated as food). These varying product categories are subject to specific accessibility challenges related to availability, reimbursement, and pricing. The aim of this study is to identify potential obstacles to optimal UCD treatment implementation in European clinical practice. A survey aimed at metabolic healthcare professionals (HCPs) managing patients with UCDs in Europe was disseminated through the European Reference Network for Hereditary Metabolic Disorders and the European registry and network for intoxication type metabolic diseases. Forty-eight survey responses were collected from 21 European countries. In 16 of these countries, at least one metabolic HCP reported challenges in accessing UCD products. Reimbursement issues were reported for most products (8/10), including both authorized and non-authorized products. Availability-related challenges were also reported for 8/10 products, although unavailability was limited to non-authorized products. Prices impacted accessibility for all authorized products (3/3) and one non-authorized IV product. The accessibility of UCD treatment products varied across Europe, although no clear regional variations could be discerned. Survey data revealed that metabolic HCPs experience challenges in accessing both authorized and non-authorized products for UCD management in the majority of European countries. This indicates that registering unauthorized products may not resolve all issues. Improved reimbursement policies and fair pricing models, as well as (adjusted) authorization procedures may help address these concerns, thereby optimizing treatment access for UCD patients.

Assessing Prenatal Alcohol Exposure History for Pediatric Patients: Practices Among U.S. Clinicians.

The American Academy of Pediatrics recommends clinicians who treat pediatric patients screen for prenatal alcohol exposure (PAE) to facilitate the identification of children with fetal alcohol spectrum disorders and promote timely access to behavioral and cognitive interventions. We evaluated how frequently clinicians inquire about PAE in their pediatric patient interactions and the methods used to ascertain this information. We analyzed data from the Fall 2020 DocStyles survey, a web-based survey of primary healthcare professionals (n = 1754). Distributions for frequency of assessing PAE history for five pediatric populations and the methods used were calculated by clinician specialty (family practitioners [FP], pediatricians, and nurse practitioners/physician assistants [NP/PAs]) and overall. Chi-square and Bonferroni post-hoc tests determined whether frequency of assessing PAE history varied by specialty. Among 779 clinicians serving pediatric patients, approximately 70.5%, 63.0%, and 60.7% reported often/always obtaining PAE history from parents of children with developmental/behavioral issues, adopted/foster children, and newborns, respectively. By contrast, less than half of respondents reported often/always collecting this information from parents of infants (47.6%) and new patients (38.2%). Most respondents reported collecting PAE history through interviews conducted by physicians or physician assistants (69.7%). Obtaining PAE history varied by specialty; pediatricians (71.5%) were more likely to collect PAE history for adopted/foster children when compared to FPs (57.7%, p = 0.003). PAE history is not routinely obtained for pediatric patients. These findings highlight the need for trainings and practice supports to aid clinicians in identifying and treating children at-risk of FASDs.

Parenteral prostacyclin utilization in patients with pulmonary arterial hypertension in the intermediate-risk strata: a retrospective chart review and cross-sectional survey

BackgroundCurrent clinical guidelines support use of parenteral prostacyclin therapy for patients with pulmonary arterial hypertension (PAH) at intermediate risk. The objective of this study was to assess parenteral prostacyclin therapy use among patients at intermediate risk according to the Comparative, Prospective Registry of Newly Initiated Therapies for Pulmonary Hypertension (COMPERA) 2.0 four-strata risk assessment model.MethodsThis was a retrospective chart review and cross-sectional online survey of healthcare professionals (HCPs). Included patients were classified as intermediate-low or intermediate-high risk per COMPERA 2.0 between 2016 and 2020 (index visit), initiated on a parenteral prostacyclin any time following intermediate risk assessment, and had World Health Organization (WHO) Functional Class (FC), 6-minute walk distance (6MWD), and B-type natriuretic peptide/N-terminal pro B-type natriuretic peptide (BNP/NT-proBNP) assessments at index and first comprehensive follow-up visits (follow-up).ResultsA total of 139 HCPs (53% community-based, 47% Pulmonary Hypertension Care Center-based) participated in the survey and provided 350 patient records; among these, mean age (SD) was 54.1 (15.3) years and 52% were female. Median (IQR) time from parenteral prostacyclin initiation to follow-up was 3.0 months (2.0, 7.0). At parenteral prostacyclin initiation for the 280 patient records with available COMPERA 2.0 assessments, 62% of patients were intermediate-high risk, 33% were intermediate-low risk and 3% were low risk, improving to 38%, 53%, and 8%, respectively, at follow-up.ConclusionsImprovements were seen for the individual COMPERA 2.0 risk calculator parameters and for several other clinical parameters. Findings from this study substantiate recent guidelines suggesting earlier use of this treatment in intermediate-risk patients with PAH.Clinical trial numberNot applicable.

Clinical team debriefing post-critical events: perceptions, benefits, and barriers among learners.

Clinical team debriefings (TD) following critical events are pivotal in promoting team learning and enhancing patient outcomes. Despite their importance, perceptions and practices surrounding these debriefings remain under-researched. The purpose of this study was to explore learners' perceptions and experiences regarding debriefing practices, investigate correlations or discrepancies within those perceptions and experiences, and identify recommendations and potential practice improvements for clinical educators. This was a cross-sectional anonymous survey of healthcare professionals, including medical students, medical residents, nursing students, and respiratory therapy students. The survey was sent to respiratory therapy programs, nursing programs, internal and emergency medicine and pediatric residency programs in southern California and Michigan. The variables surveyed included demographics, team debriefing experience, code experience, TD perceptions, emotional status, cognitive load, and the benefits and barriers of conducting post-code TD. Emotional status and cognitive load were assessed using validated surveys by Paas et al. and Barrett and Russell. Of the 184 participants, 56% (n = 104) were female. The mean cognitive load was 6.14 ± 1.6. A notable negative correlation was found between mental effort in recent real code experiences and emotional scales: "tense: calm" (r = -0.210; p = 0.018), "nervous: relaxed" (r = -0.234; p = 0.008), and "stressed: serene" (r = -0.258; p = 0.004). While 68.5% had attended a cardiopulmonary arrest event, only 34.9 had TD after their most recent code, and only 48.4% reported ever having a post-code TD. Notably, nurses (75.4%) and attending physicians (73.8%) predominated these debriefings. Debriefings averaged 9.30 min (SD = 7.30) with a median of 6 min. The most recognized benefits were identifying areas of systems/process improvement and promoting teamwork and solidarity within the code team participants. The most commonly recognized barriers were lack of time and wanting a more senior person to initiate TD. The results of this study show a relatively low TD occurrence despite the high value learners attribute to TD. Addressing this inconsistency requires structured approaches, dedicated time, and an understanding of barriers. Recognizing the significant cognitive and emotional loads on learners further accentuates the need for structured post-event debriefings. Addressing these challenges with multi-disciplinary participation can enhance debriefing outcomes.

Trends in the practice environment of Chinese healthcare professionals from 2008 to 2023: an age period cohort analysis

BackgroundHealthcare practice environment plays a vital role in evaluation and the development of health sector in China. However, there are few comprehensive reviews and studies focusing on its state and changing trends. This study aimed to examine the dynamic trends in Chinese healthcare professionals’ perceptions of their practice environment from 2008 to 2023 using age period cohort (APC) analysis.MethodsFour national cross-sectional surveys of healthcare professionals were conducted in 2008, 2013, 2018, and 2023. APC analysis was performed to distinguish effects of age, period and cohort. Covariates like gender, department, job satisfaction, and doctor–patient relationships were also analyzed.ResultsBetween 2008 and 2023, healthcare professionals' perceptions of their practice environment first declined and then improved. Those aged 28–38 during 2013–2018 and born between 1978 and 1988 had the most negative perceptions. After 2018, perceptions improved, peaking in 2023. Those under 23 and over 43 exhibited larger age effects. Birth cohorts after 1993 also had more positive effects. Controlling for covariates attenuated APC effects. Females, those in obstetrics and emergency medicine, nurses, technicians, and administrators perceived better environments. Higher job satisfaction and doctor–patient relationship harmony are also associated with more positive perceptions. Income matching efforts and perceptions of promotion fairness had positive impacts, while increasing severity of physical fatigue and psychological anxiety negatively influenced perceptions of the practice environment.ConclusionsThe APC analysis provided nuanced insights into evolving practitioner perceptions amid healthcare reforms in China. Tailored policies focused on career stage and generation are needed to address disruptions and sustain improvements. Monitoring feedback on reforms and changes is essential for optimizing the practice environment over time.

Climate change, climate disasters and oncology care: a descriptive global survey of oncology healthcare professionals.

Climate disasters and climate change have implications for healthcare globally. As the number and intensity of climate disasters increase, it is important to understand the effects on healthcare. We conducted a global survey of oncology healthcare providers to identify awareness, experiences, and educational needs related to climate change. An existing climate and health survey was adapted to oncology. This IRB- approved, 30-item survey measured demographics, climate disaster awareness, effects on cancer care and educational needs. Healthcare professionals employed in oncology settings (practice, research, or academic) were eligible. The survey was disseminated via social media and professional organizations. Descriptive statistics were computed using SPSS. 154 responses from 26 countries were received from nurses (56%), physicians (19%), and other healthcare professionals (25%). Common climate change-related events impacting oncology care were extreme heat (63.8%) and heavy rains (52.2%). Respondents reported their workplace has a disaster plan for climate-related weather events (50.4%) or has taken steps to prepare for a climate-related weather event (48.5%). Respondents were aware that the planet has warmed significantly (98.7%), that healthcare contributes to greenhouse gas emissions (98.6%) and reported wanting to learn more about how climate change affects cancer care (88.3%). Preferred educational modalities include webinars (69%), e-learning (55%), journal articles (48.3%), conferences (46.3%) and podcasts (38.9%). This global survey is the first to identify the awareness, experiences, and educational needs of oncology healthcare professionals related to climate change and climate disasters. Healthcare providers are positioned to take leadership roles related to climate and health.
