Professional attire at delivery Effect on postpartum and neonatal infection

The implementation of many hospital alternative birthing centers has been accompanied by a relaxation in the use of surgical drapes or sterile professional attire. To determine if the use or nonuse of sterile attire was accompanied by any difference in postpartum maternal or neonatal infectious morbidity, a retrospective review of records for 1 year was done in a hospital that permits variation in professional attire for spontaneous birth to women at low obstetric risk. A sample of 243 deliveries was reviewed. A comparison of the incidence of standard features of morbidity among six practitioners and three attire groups revealed no differences. Until contrary documentation is available, this supports the safety of allowing variation in attire under circumstances of low obstetric risk and minimal intervention deliveries.

Surgical Wound Environment

People are the major source of bacterial contamination in the hospital environment, and to control environmental bacteria, the shedding potential of people must be controlled. In a conventional operating room, the more complete the surgical attire worn by everyone in the room, the better the control; i.e., body exhaust equipment for the anesthesiologist and the scrub nurse is essential. The laminar airflow filter system and ultraviolet light are effective environmental controls for preventing wound contamination.

Professional attire at delivery part II: Delivery attire in chicago-area hospitals

In many settings, professional attire used during hospital delivery has changed from surgical apparel to clean clothing. To document current practice, a questionnaire was mailed to obstetric nursing supervisors of 30 metropolitan hospitals. Follow-up letters and phone calls resulted in a response rate of 100%. As the annual number of births per hospital increased, the use of surgical attire ( P = 0.056) and surgical drapes ( P = 0.03) decreased. No significant difference was found between physician's and certified nurse-midwife's (CNM) use of apparel ( P ⩾ 0.16). However, less frequent use of the cap and gown by the CNM did approach statistical significance ( P = 0.09). A significant difference ( P < 0.05) was found for the use of attire between delivery room and birthing room settings; more surgical attire was required for the more traditional delivery room births.

Total Hip Replacement in a Laminar Flow Environment with Special Reference to Deep Infections

A clean air enclosure with standard drapings was inadequate in the prevention of deep infection in total hip replacement and an overall deep infection rate of 2.2% still existed. This suggested that a sterile atmosphere under these circumstances could not ensure a bacteria-free deep wound at the end of a lengthy procedure. Additional antibiotic coverage, however, gave a deep wound infection rate of zero. A larger number of cases must be studied before final conclusions can be reached. Subsequent studies under these conditions have shown that the contamination appears to come from both patient and surgeon--principally the latter. Patient-derived organisms can be eliminated by impermeable draping. Contamination from the surgeon possibly can be eliminated only by drastic modification of conventional surgical attire.

Maintenance of aseptic barriers in the conventional operating room

Contamination of operative wounds during surgery occurs primarily through contact and secondarily through the airborne route. Inspection and maintenance of air-handling equipment is essential, with special reference to air intakes, filters, humidifiers, and exhaust systems. Doors should remain closed to maintain positive pressure. Surgical attire for all personnel must provide for complete coverage of hair and arms. Intensive interim cleaning of all horizontal sufaces between operations is essential. The infection rate is proportional to the duration of the operation and the number of personnel in the room, and inversely proportional to the air changes per hour.
