A randomized controlled trial of a mutual support group for family caregivers of patients with schizophrenia

This randomized controlled trial examined the effectiveness of a 12-session mutual support group conducted over 3-months for Chinese family caregivers of a relative with schizophrenia compared with routine family support services in Hong Kong. Forty-eight family caregivers from two psychiatric outpatient clinics were allocated randomly to an experimental (mutual support and usual outpatient care) group ( n=24) or a control (usual outpatient care only) group ( n=24). Data were collected prior to, 1 week and 3 months after the intervention. Families allocated to the mutual support group experienced decreased levels of family burden and increased family functioning and these changes were significantly greater than those of the controls at both post-intervention time points. The experimental group also showed a significant decrease in the duration of patient re-hospitalization (the total number of days of psychiatric hospitalization) at 3 months compared with the control group. This suggests that the mutual support group provided a more responsive service for patients than standard care. However, there was no significant difference in family service utilization between the two groups. The findings indicate that a mutual support group can provide benefits for family caregivers of people with schizophrenia that go beyond those provided by routine family support.

Health promotion behaviors in Chinese family caregivers of patients with stroke.

The purpose of this study was to explore the relationship between and among the caregiver's personal factors, the care recipient's functional status, the caregiver's perceived self-efficacy, social support, reactions to caregiving, and health promotion behaviors in family caregivers of community-dwelling stroke patients in Taiwan. A structured home-interview survey methodology was used to collect data from 134 primary caregivers responsible for care of stroke patients in Taipei, Taiwan. The study results indicated that, in general, caregivers were female spousal caregivers (mean age 52 years, average caregiving period 24 months). Regression analyses revealed that the caregiver's health status was the strongest positive predictor of caregiver self-efficacy. Spousal caregivers with a better-perceived health status were more satisfied with their resources of social supports. Spousal caregivers with poor perceived health status had a higher level of caregiving strain. Results for the overall model indicated caregiver's social support and the care recipient's functional status made significant contributions in explaining the caregiver's health promotion behaviors. Implications for further practice suggest establishing community training programs and support groups for family caregivers.

A Comparison of the Well-Being of Family Caregivers of Elderly Patients Hospitalized With Physical Impairments Versus the Caregivers of Patients Hospitalized With Dementia

This study investigated the differences between the level of well-being of caregivers of older patients hospitalized with dementia and the well-being of caregivers of older patients hospitalized with physical impairments. It also explored the relationships between social roles, religious faith, and well-being. Two groups were constructed to compare caregivers' well-being levels: caregivers of patients with dementia and caregivers of patients with physical impairments. The groups were caregivers of patients of two geriatric hospitals in central Israel. Patients were categorized by their medical files according to their medical diagnosis. A total of 107 caregivers participated in this research. Of that number, 46% were caregivers of patients with physical impairments while the other 54% were caregivers of patients with dementia. The patients with physical impairments suffered from stroke, hip fractures, Parkinson's disease, diabetes, and heart and lung diseases. The patients with dementia suffered from Alzheimer's disease or related dementia. Of the total number of patients, 67% were males and 33% were females. The caregivers were asked to fill out a questionnaire that included 90 questions. Some of the questionnaires were given to the caregivers to be filled out at home. The questionnaire integrated three subcategories: well-being, identity accumulation, and religious faith. Differences were found only in the dimensions of social activity. Caregivers of the patients with physical impairments were found to attend synagogues and clubs more than caregivers of the patients with dementia did. The number of the caregivers' social roles were found to have been significantly related to caregivers' mental health. A negative correlation was found between the intensity of religious faith and the level of the caregivers' spouses' self-rated physical and mental health: those of stronger faith rated their physical health lower and were reported to have more symptoms of stress. There are no significant differences between the level of well-being of caregivers of patients with dementia and caregivers of patients with physical impairments. Both groups of caregivers are at great risk of compromised physical and mental health, of decreased social activity, and of financial distresses. Various social roles and religious faith may enhance the caregiver's well-being. However, the spouse's religious faith has negative effects on their own well-being. These results call for welfare services and religious organizations to form support groups for family caregivers. They also make clear that all caregivers should benefit from the same social support if their older relatives are hospitalized.

Telephone support group intervention for persons with hemophilia and HIV/AIDS and family caregivers

The purpose of this pilot project was to test the feasibility of a telephone support group intervention for persons with hemophilia and HIV/AIDS and for their family caregivers. Their support needs were unique because they did not identify with predominant groups of persons with AIDS and were geographically dispersed from peers. The 12 week intervention involved separate telephone support groups for hemophiliacs and for family caregivers. The two groups, comprised of a predetermined maximum of six people, were co-led by a professional and a peer. The support group for family caregivers involved six people and the group for men with hemophilia included five people, including one peer facilitator and one professional facilitator in each group. The telephone support group discussions were taped, transcribed, and analyzed for prevalent themes. The peer and professional facilitators maintained weekly field notes. All participants reported that the telephone groups had a positive impact on meeting their support needs. They believed that they had benefitted from sharing information and that the support groups had decreased their feelings of isolation and loneliness. Participants, however, contended that the intervention should be longer than 12 weeks.

The German Alzheimer Family Groups. An assessment of current status

Counselling the relatives of psychiatric patients is becoming increasingly important. This includes the relatives of patients with Alzheimer's disease. Since little is known about the organization and function of support groups for family caregivers of Alzheimer patients, in 1994 we wrote to all such groups known to us and asked them to fill in a questionnaire. The response rate was 85% (84/99), and 66 of the forms could be used for further analysis. The results were as follow: Most groups for relatives of Alzheimer patients admitted new members at any time. 69% had existed for more than 1 year and 19% for more than 5 years. 81% consisted of 6-15 participants. 73% of the groups met once in a month. Health professionals organized the groups in 65% of cases. The main topics were trading information about the disease and support as well as daily care requirements. 73% of the groups were associated with health institutions or nationwide support organizations. Public relations were managed by personal contacts and press reports; did the groups rarely advertise. Based on a review of the literature, the impact of structure upon the function of groups is discussed.

The subjective experience of Alzheimer's disease: A sociocultural perspective

Although the North American population is multicultural, little is known of sociocultural variations in the subjective experience of Alzheimer's disease. In Canada, data on important health indicators from sociocultural minority groups are not readily available. The reasons for unavailability include problems with defining and measuring ethnicity - in particular, concern that the sociocultural background of a person is private information, linked to concerns about potential violation of the Canadian Charter of Rights. The current commitment to reducing inequalities in health is starting to stimulate more interest in research on health-related sociocultural issues. This focus on links between sociopsychological factors and health suggests that members of sociocultural minority groups, and especially seniors, may be neurologically vulnerable to the stresses associated with older age, particularly when compounded by stressesfrom immigration and from aging in an acquired culture. Cross-cultural psychiatry distinguishes between disease as malfunctioning biologic and psychophysiologic process in the individuals, and illness as a subjective experience of being ill. Consequently, Alzheimer's disease will appear universally across cultures, but may vary in the realm of subjective experience. A strategy for building support groups for family caregivers from the targetedpopulation is presented as one example of effective intervention. The urgent needfor research to inform professional caregivers about sociocultural variations in the subjective experience of Alzheimer's disease is noted. In order to provide responsible and supportive care for patients and their caregivers in sociocultural minority groups, there is the urgent need for research to inform professional caregivers about sociocultural variations in the subjective experience of Alzheimer's disease.

Support groups for family caregivers of dementia victims: Questions, directions, and future research

The author discusses some assumptions, dilemmas, and questions that have come out of her experience in facilitating support groups for caregivers whose relatives have Alzheimer's disease, talking with other such facilitators around the country, reviewing the literature, and talking with group members themselves. Suggestions are made for a wider variety of support-group models, in recognition that caregivers have different needs.
