Biceps brachii elastography in well-trained men post eccentric exercise-induced muscle damage.

Recent advancements in elastography techniques, specifically supersonic shearwave elastography (SWE), have enabled non-invasive assessment of muscle stiffness. However, there is limited research on the immediate and short-term effects of eccentric exercise-induced muscle damage (EIMD) in well-trained individuals. This study aimed to follow up on the effects of eccentric training on the biceps brachialis stiffness by supersonic shearwave imaging (SSI) as well as the soreness and elbow flexion maximal voluntary isometric contraction (MVIC), immediately post-intervention, at 10min, 48h, and 96h in well-trained men. Thirteen well-trained males participated in the study. Baseline measurements of elastography images, MVIC of the elbow flexors, and muscle soreness were obtained. The participants performed an eccentric exercise protocol (4 sets X 10 repetitions) on the dynamometer isokinetic and elastography measurements were repeated immediately post-exercise, at 10min, 48h, and 96h. Significant reductions in stiffness (measured by shear modulus (µ)) were observed immediately and at 10min post-exercise. MVIC exhibited significant reductions immediately after, 10min, and 48h compared to baseline measurements. Muscle soreness peaked at 48h, persisting until 96h. The BB stiffness and MVIC reduction immediately post-eccentric exercise in well-trained men, suggest the potential involvement of mechanical stress and sarcomere rupture. Trained individuals may exhibit a distinct response to EIMD compared to untrained individuals, highlighting the applicability of elastography in monitoring acute biomechanical changes following high-intensity exercise.

Comparison of ultrasound elastography, magnetic resonance elastography and finite element model to quantify nonlinear shear modulus

Objective. The aim of this study is to validate the estimation of the nonlinear shear modulus () from the acoustoelasticity theory with two experimental methods, ultrasound (US) elastography and magnetic resonance elastography (MRE), and a finite element method. Approach. Experiments were performed on agar (2%)—gelatin (8%) phantom considered as homogeneous, elastic and isotropic. Two specific setups were built to ensure a uniaxial stress step by step on the phantom, one for US and a nonmagnetic version for MRE. The stress was controlled identically in both imaging techniques, with a water tank placed on the top of the phantom and filled with increasing masses of water during the experiment. In US, the supersonic shear wave elastography was implemented on an ultrafast US device, driving a 6 MHz linear array to measure shear wave speed. In MRE, a gradient-echo sequence was used in which the three spatial directions of a 40 Hz continuous wave displacement generated with an external driver were encoded successively. Numerically, a finite element method was developed to simulate the propagation of the shear wave in a uniaxially stressed soft medium. Main results. Similar shear moduli were estimated at zero stress using experimental methods, = 12.3 ± 0.3 kPa and = 11.5 ± 0.7 kPa. Numerical simulations were set with a shear modulus of 12 kPa and the resulting nonlinear shear modulus was found to be −58.1 ± 0.7 kPa. A very good agreement between the finite element model and the experimental models ( = −58.9 ± 9.9 kPa and = −52.8 ± 6.5 kPa) was obtained. Significance. These results show the validity of such nonlinear shear modulus measurement quantification in shear wave elastography. This work paves the way to develop nonlinear elastography technique to get a new biomarker for medical diagnosis.

Supersonic shear wave elastography of human tendons is associated with in vivo tendon stiffness over small strains

Supersonic shear wave (SW) elastography has emerged as a useful imaging modality offering researchers and clinicians a fast, non-invasive, quantitative assessment of tendon biomechanics. However, the exact relationship between SW speed and in vivo tendon stiffness is not intuitively obvious and needs to be verified. This study aimed to explore the validity of supersonic SW elastography against a gold standard method to measure the Achilles tendon’s in vivo tensile stiffness by combining conventional ultrasound imaging with dynamometry. Twelve healthy participants performed maximal voluntary isometric plantarflexion contractions (MVC) on a dynamometer with simultaneous ultrasonographic recording of the medial gastrocnemius musculotendinous junction for dynamometry-based measurement of stiffness. The tendon’s force–elongation relationship and stress–strain behaviour were assessed. Tendon stiffness at different levels of tension was calculated as the slope of the stress–strain graph. SW speed was measured at the midportion of the free tendon and tendon Young’s modulus was estimated. A correlation analysis between the two techniques revealed a statistically significant correlation for small strains (r(10) = 0.604, p =.038). SW-based assessments of in vivo tendon stiffness were not correlated to the gold standard method for strains in the tendon>10 % of the maximum strain during MVC. The absolute values of SW-based Young’s modulus estimations were approximately-three orders of magnitude lower than dynamometry-based measurements. Supersonic SW elastography should be only used to assess SW speed for the detection and study of differences between tissue regions, differences between people or groups of people or changes over time in tendon initial stiffness (i.e., stiffness for small strains).

Clinical study of the value of shear wave elastography in evaluating the degree of liver fibrosis in children.

To explore the clinical application of shear wave elastography (SWE) in evaluating the degree of liver fibrosis in children. To explore the value of SWE in assessing liver fibrosis in children, the correlation between elastography values and the METAVIR grade of liver fibrosis in children with biliary system or liver diseases was studied. Children with significant liver enlargement were enrolled, and the fibrosis grade was analyzed to explore the value of SWE in assessing the degree of liver fibrosis in the presence of significant liver enlargement. A total of 160 children with bile system or liver diseases were recruited. The areas under the receiver operating characteristic curve (AUROCs) for liver biopsy from stage F1 to F4 were 0.990, 0.923, 0.819, and 0.884. According to the degree of liver fibrosis at liver biopsy, there was a high correlation between the SWE value and the degree of liver fibrosis (correlation coefficient 0.74). There was no significant correlation between the Young's modulus value of the liver and the degree of liver fibrosis (correlation coefficient 0.16). Supersonic SWE can generally accurately evaluate the degree of liver fibrosis in children with liver disease. However, When the liver is significantly enlarged, SWE can only evaluate liver stiffness based on Young's modulus values, and the degree of liver fibrosis must still be determined by pathologic biopsy.

Comparison of Two Kinds of Two-Dimensional Shear Wave Elastography Techniques in the Evaluation of Jaundiced Infants Suspected of Biliary Atresia.

Purpose: To compare the reliability and performance of Supersonic shear wave elastography (S-SWE) and Toshiba shear wave elastography (T-SWE) in the diagnosis of biliary atresia (BA) and assessment of liver fibrosis among jaundiced infants suspected of BA. Material and Methods: A total of 35 patients with suspected BA who underwent both S-SWE and T-SWE examinations were prospectively included. Diagnostic performances of S-SWE and T-SWE in identifying BA were evaluated. The correlation between two types of SWE values and histological liver fibrosis stages by Metavir scores were investigated in 21 patients with pathology results. The intraclass correlation coefficients (ICCs) were calculated in 16 patients for inter- and intra-observer agreement. The area under the receiver operating characteristic curve (AUC) analysis was compared using a DeLong test. Results: There were 22 patients with BA and 13 patients without BA. The diagnostic performance of S-SWE was comparable to that of T-SWE (AUC 0.895 vs. 0.822, p = 0.071) in diagnosing BA. The AUCs of S-SWE in predicting liver fibrosis stages were from 0.676 to 1.000 and showed no statistical differences from that of T-SWE (from 0.704 to 1.000, all p > 0.05). T-SWE provided higher inter-operator agreement (ICC 0.990) and intra-operator agreement (ICCs 0.966–0.993), compared with that of S-SWE in a previous study (ICC 0.980 for inter-operator and 0.930–0.960 for intra-operator). Conclusions: For infants suspected of BA, T-SWE had good performances in the diagnosis of BA and the assessment of liver fibrosis compared with S-SWE. Furthermore, T-SWE showed higher measurement reproducibility than S-SWE.

A three-dimensional finite element foot-ankle model and its personalisation methods analysis

Foot and ankle injuries are common as a result of overuse or trauma as well as various pathologies. This study aims to establish a three-dimensional foot-ankle model and investigate its suitability personalisation for the analysis of foot-ankle biomechanical responses during dynamic gait loading. The model was first established based on medical images and validated in both static standing posture and dynamic gait loading concerning regional plantar pressures and the centre of pressure curve (COP). Then, two model personalisation methods using magnetic resonance imaging (MRI) and fast optical scan data, respectively, were investigated and compared through analysing the dynamic gait responses of the scaled models. In both methods, material personalisation of plantar tissues was also combined through supersonic shear wave elastography (SWE) measurements. With regard to plantar pressure distribution and internal soft tissue strain, both personalisation methods correlated well with the subject-specific experimental results. Thus, we believe that the present model, with a fast optical scan personalisation method of specific SWE measurement, is a reasonable arrangement for subject-specific plantar deformation behaviour analysis during dynamic gait loading.

Diagnostic Accuracy of Four Levels of Manual Compression Applied in Supersonic Shear Wave Elastography of the Breast

To investigate the diagnostic accuracy of applying four levels of manual pressure in Shear Wave Elastography (SWE) of the breast and to assess inter-rater reliability. Single-center prospective preliminary study including patients receiving ultrasound examination of breast lesions as part of routine clinical practice. SWE was performed on 60 breast masses (26 benign and 34 malignant) in 54 patients by a breast fellowship trained radiologist. Stiffness values were compared between benign and malignant masses at four levels of manual compression: none, mild, moderate, and marked. Accuracy of SWE was assessed using receiving operating characteristics analysis at each level. In 18 patients, a second radiologist repeated the SWE acquisitions to evaluate reproducibility. Reproducibility was assessed using intraclass correlation coefficient. Without compression, we observed no significant difference in stiffness (p > 0.99) between benign and malignant lesions, and SWE demonstrated low accuracy (area under the curve = 0.64). Stiffness was higher in malignant lesions at all levels of compression (p < 0.001). SWE demonstrated good accuracy at all three levels of compression (from area under the curve = 0.71 to 0.84 across Emax and Emean), with high interobserver agreement. This preliminary study suggests that not using compression during SWE for breast lesion characterization offers suboptimal results. On the contrary, application of compression yields high diagnostic performance with good interobserver agreement and, as such, should be included in routine clinical practice.

Diagnostic Accuracy Evaluation of Two-Dimensional Shear Wave Elastography in the Differentiation Between Benign and Malignant Thyroid Nodules: Systematic Review and Meta-analysis.

To evaluate two-dimensional (2D) shear wave elastography (SWE) performance as an independent predictor of malignancy in the diagnostic differentiation of thyroid nodules (TNs), including subgroup analyses of different manufacturers and respective cutoffs points. The online databases MEDLINE (PubMed), Embase, and the Cochrane Library were searched for articles using 2D SWE in TN evaluation. After good-quality relevant thyroid-specific articles were selected, the main data, plus their sensitivity and specificity, were tabulated. Summary receiver operating characteristic curves were generated to verify the accuracy of data obtained from 3 manufactures. A meta-analysis was performed to evaluate whether clinical recommendations can be improved by the use of 2D SWE to differentiate TNs. The sensitivity and specificity parameters of 2D SWE for the differentiation between benign and malignant TNs according to different instruments were, respectively, as follows: Toshiba SWE (Toshiba Medical Systems, Tochigi, Japan), 0.77 (95% confidence interval [CI], 0.70-0.83) and 0.76 (95% CI, 0.72-0.81); Virtual Touch tissue imaging and quantification (Siemens Medical Solutions, Mountain View, CA), 0.72 (95% CI, 0.67-0.77) and 0.81 (95% CI, 0.78-0.84); and SuperSonic SWE (SuperSonic Imagine, Aix-en-Provence, France), 0.63 (95% CI, 0.59-0.66) and 0.81 (95% CI, 0.79-0.83). The summary receiver operating characteristic curves showed the following area under the curve syntheses: Toshiba SWE, 0.84 (Q* = 0.7707); Virtual Touch tissue imaging and quantification, 0.85 (Q* = 0.7809); and SuperSonic SWE, 0.88 (Q* = 0.8102). Positive and negative predictive values varied, respectively, from 16% to 94% and 29% to 100%, considering all included articles. The overall accuracy ranged from 53% to 93%. Two-dimensional SWE is a relevant and an important tool that supports ultrasound in clinical practice in the diagnostic differentiation between benign and malignant TNs.

Pulmonary Capillary Hemorrhage Induced by Super Sonic Shear Wave Elastography in Rats

The occurrence of the pulmonary capillary hemorrhage (PCH) bioeffect of diagnostic ultrasound in rats was investigated for a SuperSonic Imagine shear wave elastography system (Aixplorer, Supersonic Imagine, Aix-en-Provence, France). The elastography imaging repeated at 1 Hz and consisted of widely spaced B-mode image pulses, supersonic push (SSP) pulses and shear wave imaging (SWI) pulses. Groups of rats anesthetized with ketamine and xylazine, or with ketamine only, were imaged on their right side in a warm water bath for one frame, 30 s and 300 s. The image focus and region of interest were adjusted to give exposure only with the background B-mode imaging, or primarily with the SSP and SWI pulses. A sham group had only low power aiming scans. The lungs were removed 5 min after exposure and evaluated for PCH area and volume. The B mode was notably ineffective and produced significant PCH only at the maximum 0 dB output. The SSP pulses together with the SWI pulses produced significant PCH for 300 s, 30 s and even single image exposures. Peak rarefactional pressure amplitude PCH thresholds for 300 s exposure were the same with or without the B-mode pulses at 1.5 MPa (in situ mechanical index, MIIS = 0.67). A 30 s duration resulted in a slightly increased threshold of 1.7 MPa (MIIS = 0.76). The omission of xylazine from the anesthetic, which reduces the sensitivity of rat lung to PCH occurrence, resulted in an increased threshold of 2.1 MPa (MIIS = 0.94). The unique SSP pulses were much more effective than the B mode, but thresholds were comparable to previous results with other diagnostic ultrasound modes on other systems.

Supersonic shear-wave elastography and APRI for the detection and staging of liver disease in pediatric cystic fibrosis

BackgroundCurrent diagnostic methods for the diagnosis of Cystic fibrosis (CF)-associated liver disease (CFLD) are non-specific and assessment of disease progression is difficult prior to the advent of advanced disease with portal hypertension. This study investigated the potential of Supersonic shear-wave elastography (SSWE) to non-invasively detect CFLD and assess hepatic fibrosis severity in children with CF. Methods125 children were enrolled in this study including CFLD (n = 55), CF patients with no evidence of liver disease (CFnoLD = 41) and controls (n = 29). CFLD was diagnosed using clinical, biochemical and imaging best-practice guidelines. Advanced CFLD was established by the presence of portal hypertension and/or macronodular cirrhosis on ultrasound. Liver stiffness measurements (LSM) were acquired using SSWE and diagnostic performance for CFLD detection was evaluated alone or combined with aspartate aminotransferase-to-platelet ratio index (APRI). ResultsLSM was significantly higher in CFLD (8.1 kPa, IQR = 6.7–11.9) versus CFnoLD (6.2 kPa, IQR = 5.6–7.0; P < 0.0001) and Controls (5.3 kPa, IQR = 4.9–5.8; P < 0.0001). LSM was also increased in CFnoLD versus Controls (P = 0.0192). Receiver Operating Characteristic (ROC) curve analysis demonstrated good diagnostic accuracy for LSM in detecting CFLD using a cut-off = 6.85 kPa with an AUC = 0.79 (Sensitivity = 75%, Specificity = 71%, P < 0.0001). APRI also discriminated CFLD (AUC = 0.74, P = 0.004). Classification and regression tree modelling combining LSM + APRI showed 14.8 times greater odds of accurately predicting CFLD (AUC = 0.84). The diagnostic accuracy of SSWE for discriminating advanced disease was excellent with a cut-off = 9.05 kPa (AUC = 0.95; P < 0.0001). ConclusionsSSWE-determined LSM shows good diagnostic accuracy in detecting CFLD in children, which was improved when combined with APRI. SSWE alone discriminates advanced CFLD.

In Vivo Measurement of Plantar Tissue Characteristics and Its Indication for Foot Modeling.

Plantar heel pain is one of the most common musculoskeletal disorders and generally causing long term discomfort of the patients. The objective of the present study is to combine in vivo experimental measurements and finite element modelling of the foot to investigate the influences of stiffness and thickness variation of individual plantar tissues especially the heel pad on deformation behaviours of the human foot. The stiffness and thickness variance of individuals were measured through supersonic shear wave elastography considering detailed heel pad layers refered to in literature as: dermis, stiffer micro-chamber layer, softer macro-chamber layer. A corresponding foot model with separated heel pad layers was established and used to a sensitivity analysis related to the variance of above-mentioned tissue characteristics. The experimental results show that the average stiffness of the micro-chamber layer ranged from 24.7 (SD 2.4) kPa to 18.8 (SD 3.5) kPa with the age group increasing from 20-29years old to 60-69years old, while the average macro-chamber stiffness is 10.6 (SD 1.5) kPa that appears to slightly decrease with the increasing age. Both plantar soft tissue stiffness and thickness of male were generally larger than that of female. The numerical simulation results show that the variance of heel pad strain level can reach 27.5% due to the effects of stiffness and thickness change of the plantar tissues. Their influences on the calcaneus stress and plantar pressure were also significant. This indicates that the most appreciate way to establish a personalized foot model needs to consider the difference of both individual foot anatomic geometry and plantar soft tissue material properties.

Supersonic Shear Imaging for Quantification of Lateral Abdominal Muscle Shear Modulus in Pediatric Population with Scoliosis: A Reliability and Agreement Study

The aim of this study was to assess the intra- and inter-rater reliability/agreement of the lateral abdominal muscle (LAM) stiffness and thickness measurements at rest and during contraction and to determine the relationship between the superficial fat thickness and the LAM stiffness measurements. LAM stiffness and thickness were measured using supersonic shear wave elastography (SSI) in pediatric participants. The reliability of LAM stiffness and thickness measurements ranged from moderate to excellent. There was an inverse correlation between fat thickness and between-rater difference in the resting external oblique stiffness (r > –0.37) and the contracted external and internal oblique stiffness (r > –0.40). SSI is a reliable method for assessing LAM stiffness and thickness in pediatric populations. To remove potential systematic errors: (i) the first round of measurements should be performed to familiarize patients with procedures; (ii) the examiner should pay more attention while performing LAM measurements on the opposite side of the body.

Posture-related stiffness mapping of paraspinal muscles.

The paraspinal compartment acts as a bone-muscle composite beam of the spine. The elastic properties of the paraspinal muscles play a critical role in spine stabilization. These properties depend on the subjects' posture, and they may be drastically altered by low back pain. Supersonic shear wave elastography can be used to provide quantitative stiffness maps (elastograms), which characterize the elastic properties of the probed tissue. The aim of this study was to challenge shear wave elastography sensitivity to postural stiffness changes in healthy paraspinal muscles. The stiffness of the main paraspinal muscles (longissimus, iliocostalis, multifidus) was measured by shear wave elastography at the lumbosacral level (L3 and S1) for six static postures performed by volunteers. Passive postures (rest, passive flexion, passive extension) were performed in a first shear wave elastography session, and active postures (upright, bending forward, bending backward) with rest posture for reference were performed in a second session. Measurements were repeated three times for each posture. Sixteen healthy young adults were enrolled in the study. Non-parametric paired tests, multiple analyses of covariance, and intra-class correlations were implemented for analysis. Shear wave elastography showed good to excellent reliability, except in the multifidus at S1, during bending forward, and in the multifidus at L3, during bending backward. Yet, during bending forward, only poor quality was recorded for nine volunteers in the longissimus. Significant intra- and inter-muscular changes were observed with posture. Stiffness significantly increased for the upright position and bending forward with respect to the reference values recorded in passive postures. In conclusion, shear wave elastography allows reliable assessment of the stiffness of the paraspinal muscles except in the multifidus at S1 and longissimus, during bending forward, and in the multifidus at L3, during bending backward. It reveals a different biomechanical behaviour for the multifidus, the longissimus, and the iliocostalis.

Hepatic fibrosis: US elastography vs. MR elastography

Hepatic fibrosis is final results of a wide variety of types of liver injury and is a consequence of all chronic liver diseases and progresses more or less rapidly towards cirrhosis, and finally cancer. In terms of the tools used to quantitate fibrosis today, there are three kinds of tools; biochemical markers, conventional imaging, and functional imaging such as elastography (E) or stiffness imaging. Recent imaging tools for evaluation of hepatic fibrosis are US based techniques, MR based techniques, and perfusion imaging. US Elastography (USE) is classified into strain-based E and SW-based E. Strain-based E is subclassified into Static USE (Manual compression of the tissue) and Quasi-static USE (Physiologic vibrations (heartbeat, blood vessels pulse). SW-based E is subclassified into 1D such as fibroscan (Echosens), Point E such as virtual touch quantification (Siemens). ElastPQ (Philips) and 2D SWE such as supersonic shear imaging (Supersonic Imagine), and shear wave elastography (GE, Toshiba)). For the liver, USE is divided into 2 categories quasi-static E and shear wave based imaging. Quasi-static E is not popular. Shear wave based imaging is subdivided into two types based on generation method of shear wave: Controlled external vibration with a vibrator and acoustic radiation force impulse. Transient elastography is using mechanical push, and both ARFI imaging and supersonic shear wave elastography are using ARFI. Strengths of 1D SW-based elastography are the most extensively investigated technique with high portability. Limitations are no B-mode imaging, high technical failure rate (2.4-9.4%) and narrow applications outside of liver investigation. Strengths of point SW-based elastography are low technical failure rate ( In Summary, liver stiffness is determined by matrix deposition (fibrosis) and pressure change (hydrostatic, osmotic). Hepatic fibrosis (HF) is a consequence of all chronic liver diseases and progresses towards cirrhosis and finally cancer. Noninvasive diagnosis and staging is important clinical issue for hepatic fibrosis which is reversible. Elastrography is clinically acceptable for the prediction of liver fibrosis (≥F2) and confounders of stiffness measurement (intercostal approach to right lobe (S7/8), 1-3 cm below the liver capsule, biologic confounders can change stiffness values, fasting at least 2 hours, breath-hold at expiration). Elastography is one of important noninvasive methods to quantify HF more sensitively and objectively. USE is the first line examination for the assessment of HF. MRE is the second line examination, especially in cirrhosis. To avoid confounders of liver stiffness, comprehensive other imaging exams are needed.

Influence of thoracolumbar fascia stretching on lumbar back muscle stiffness: A supersonic shear wave elastography approach.

The lumbar paraspinal muscle compartment (PMC) is a stabilizing system of the spine whose efficiency depends on its elastic properties, which may be quantifiable by supersonic shear wave elastography (SWE). The thoracolumbar fascia (TLF) encapsulates the lumbar paraspinal muscles (LPM) and creates a PMC. Tensioning of the TLF via the stretching of the latissimus dorsi is supposed to increase stiffness within the PMC. The aims of this study were (1) to test the reliability of SWE in the multifidus and the erector spinae (ES) in prone and sited position; (2) to investigate the role of the tensioning of the pTLF, via stretching of the latissimus dorsi (LD), on LPM stiffness. Stiffness of ES and multifidus was measured using SWE at L3-L4 in procubitus and seated position in 15 participants. Stretching of LD was performed with arm elevation. Parametric paired tests, multiple analyses of variance, and intraclass correlation were used for statistical analysis. Reliability estimates were fair to excellent. Reliability was greater in ES than the multifidus, greater in seated position than during rest. Stiffness was greater in the ES than in multifidus, and in seated position than at rest. Tensioning of the TLF via LD stretching did not generate significant LPM stiffness changes. SWE is a reliable tool for assessing stiffness in the LPM. Reliability of SWE protocols is improved during seated position. Tensioning of the TLF via LD stretching did not influence LPM stiffness. Clin. Anat. 32:73-80, 2019. © 2018 Wiley Periodicals, Inc.

Breast lesion classification based on supersonic shear-wave elastography and automated lesion segmentation from B-mode ultrasound images

Supersonic shear-wave elastography (SWE) has emerged as a useful imaging modality for breast lesion assessment. Regions of interest (ROIs) were required to be specified for extracting features that characterize malignancy of lesions. Although analyses have been performed in small rectangular ROIs identified manually by expert observers, the results were subject to observer variability and the analysis of small ROIs would potentially miss out important features available in other parts of the lesion. Recent investigations extracted features from the entire lesion segmented by B-mode ultrasound images either manually or semi-automatically, but lesion delineation using existing techniques is time-consuming and prone to variability as intensive user interactions are required. In addition, rich diagnostic features were available along the rim surrounding the lesion. The width of the rim analyzed was subjectively and empirically determined by expert observers in previous studies after intensive visual study on the images, which is time-consuming and susceptible to observer variability. This paper describes an analysis pipeline to segment and classify lesions efficiently. The lesion boundary was first initialized and then deformed based on energy fields generated by the dyadic wavelet transform. Features of the SWE images were extracted from inside and outside of a lesion for different widths of the surrounding rim. Then, feature selection was performed followed by the Support Vector Machine (SVM) classification. This strategy obviates the empirical and time-consuming selection of the surrounding rim width before the analysis. The pipeline was evaluated on 137 lesions. Feature selection was performed 20 times using different sets of 14 lesions (7 malignant, 7 benign). Leave-one-out SVM classification was performed in each of the 20 experiments with a mean sensitivity, specificity and accuracy of 95.1%, 94.6% and 94.8% respectively. The pipeline took an average of 20 s to process a lesion. The fact that this efficient pipeline generated classification accuracy superior to that of existing algorithms suggests that improved efficiency did not compromise classification accuracy. The ability to streamline the quantitative assessment of SWE images will potentially accelerate the adoption of the combined use of ultrasound and elastography in clinical practice.

Effect of renal perfusion and structural heterogeneity on shear wave elastography of the kidney: an in vivo and ex vivo study

BackgroundTo evaluate the effect of perfusion status on elasticity measurements of different compartments in the kidney using shear wave elastography (SWE) both in vivo and ex vivo.MethodsThirty-two rabbit kidneys were used to observe the elasticity variation caused by renal artery stenosis and vein ligation in vivo, and six beagle kidneys were studied ex vivo to explore the effect of renal perfusion on elasticity. Supersonic SWE was applied to quantify the elasticity values of different renal compartments (cortex, medulla and sinus). Additionally, histopathological examination was performed to explore the possible mechanisms.ResultsThe elasticity of the cortex was higher than that of the medulla, and the elasticity of the sinus was lowest among the compartments in native kidneys. The Young’s modulus (YM) of the cortex, medulla and sinus increased gradually as the duration of renal vein ligation increased, from 16.34 ± 1.01 kPa to 55.06 ± 5.61 kPa, 13.71 ± 1.16 kPa to 39.63 ± 2.91 kPa, and 12.61 ± 0.84 kPa to 29.30 ± 2.04 kPa, respectively. In contrast, the YM of the three compartments respectively decreased with progressive artery stenosis, from 16.34 ± 1.83 kPa to 11.21 ± 1.79 kPa, 13.31 ± 1.67 kPa to 8.07 ± 1.37 kPa, and 12.78 ± 2.66 kPa to 6.72 ± 0.95 kPa. Artery perfusion was the main factor influencing elasticity in ex vivo. The cortical elasticity was more prone to change with renal perfusion both in vivo and ex vivo. Histopathological examination showed progressive changes in the structure and content of the three compartments, consistent with the elasticity variation.ConclusionsBoth the complex structure/anisotropy and the perfusion of the kidney obviously influence the evaluation of renal elasticity. The measurement of SWE should be performed at a specific location along a certain angle or direction, and renal perfusion status should also be taken into account to ensure reproducible detection.

The purpose of this study was to apply supersonic shear wave elastography, an ultrasound-based noninvasive modality that can quantitatively estimate the shear stiffness of the tissue, to investigate the spatial-dependent mechanical properties of the heel pad. Measurements were conducted in 40 heel pads of 20 normal participants aged between 20 and 30 years by shear wave elastography. The continuous change in local shear stiffness of the heel pad along the depth direction of the heel pad was measured. The result showed that the mechanical properties of the heel pad were highly heterogeneous but followed a simple and specific pattern that local heel pad shear stiffness was highest beneath the plantar skin and decreased continuously with increasing depth. This finding provides a better understanding of the heel pad biomechanics and basis for further investigation of the heterogeneous properties of the heel pad.
