Long‐Term Evaluation ofANKYLOS® Dental Implants, PartI: 20‐Year Life Table Analysis of a Longitudinal Study of More Than 12,500 Implants

Scientific evidence is limited regarding the long-term (>10 years) outcomes of large enough numbers of implants (>500) to allow for reliable comparison of subgroups. The purpose of this study was to analyze the outcomes of dental implants placed in an active University Clinic setting and followed for up to 20 years. Data documenting the implant placement, prosthetic reconstruction, and annual follow-up of patients treated at Frankfurt University were extracted from a Structured Query Language database and patients' written records and evaluated statistically. Between April of 1991 and May of 2011, 12,737 ANKYLOS® (DENTSPLY Implants Manufacturing GmbH, Mannheim, Germany) implants were placed in 4,206 patients for a variety of clinical indications. The Kaplan-Meier cumulative survival rate (CSR) was 93.3% after 204 months. Most of the failures (198/1.6%) occurred during the first year after implant placement and before prosthesis delivery. A significantly higher (p < .001) number of implants placed in the mandible and in hard quality bone failed than those placed in the maxilla or in weak and normal quality bone. Female patients had significantly higher CSRs (93.7% 204 months) than male patients (92.8% 204 months/p = .029). The implants showed low rates of peri-implant bone loss after 204 months (horizontal: ≤1 mm: 85.7%, vertical: ≤1 mm: 85.2%). ANKYLOS dental implants followed for up to 20 years have high CSRs and low rates of peri-implant bone loss.

Impact of obesity on the cost of major colorectal surgery9

The incidence of obesity is increasing in New Zealand. The aim of the study was to determine whether obesity impacts on the cost of treating patients undergoing major colorectal surgery. Between 1 February 2008 and 31 July 2009, consecutive patients undergoing major colorectal surgery at Christchurch Hospital, New Zealand, were enrolled in the study. Body mass index (BMI) and waist-to-hip ratios were assessed using standardized techniques. Patients with a high surgical risk were identified using established criteria and all patients were assessed using the Portsmouth modification of the Physiological and Operative Severity Score for the enUmeration of Mortality and morbidity (P-POSSUM). Cost analysis was performed using a structured query language database. Patients were analysed using accepted groupings for BMI, waist circumference and waist-to-hip ratio. A total of 372 patients were enrolled in the study, of whom 345 were included in the analysis. The incidence of diabetes was significantly higher with increased BMI (P = 0·002), whereas all other co-morbidities, and P-POSSUM values, did not differ between BMI groups. The groups were similar in terms of case mix. Treatment of obese patients (BMI at least 30 kg/m(2)) was significantly more expensive than that of normal weight patients (BMI 20-24·9 kg/m(2)): €10,036 versus €7390 (P = 0·005). Treatment costs for patients with a BMI of 25-29·9 kg/m(2) were next highest (€9048) followed by those for patients whose BMI was less than 20 kg/m(2) (€8884). Patients with a waist circumference above recognized standards for men and women also cost significantly more to treat (€10,063 versus €7836; P = 0·014). Excess body fat was associated with higher costs of major colorectal surgery.

Quantitative Statistical Analysis of Standard and Human Blood Proteins from Liquid Chromatography, Electrospray Ionization, and Tandem Mass Spectrometry

It will be important to determine if the parent and fragment ion intensity results of liquid chromatography, electrospray ionization and tandem mass spectrometry (LC-ESI-MS/MS) experiments have been randomly and independently sampled from a normal population for the purpose of statistical analysis by general linear models and ANOVA. The tryptic parent peptide and fragment ion m/z and intensity data in the mascot generic files from LC-ESI-MS/MS of purified standard proteins, and human blood protein fractionated by partition chromatography, were parsed into a Structured Query Language (SQL) database and were matched with protein and peptide sequences provided by the X!TANDEM algorithm. The many parent and/or fragment ion intensity values were log transformed, tested for normality, and analyzed using the generic Statistical Analysis System (SAS). Transformation of both parent and fragment intensity values by logarithmic functions yielded intensity distributions that closely approximate the log-normal distribution. ANOVA models of the transformed parent and fragment intensity values showed significant effects of treatments, proteins, and peptides, as well as parent versus fragment ion types, with a low probability of false positive results. Transformed parent and fragment intensity values were compared over all sample treatments, proteins or peptides by the Tukey-Kramer Honestly Significant Difference (HSD) test. The approach provided a complete and quantitative statistical analysis of LC-ESI-MS/MS data from human blood.

Informatics in Radiology: Use of CouchDB for Document-based Storage of DICOM Objects

Picture archiving and communication systems traditionally have depended on schema-based Structured Query Language (SQL) databases for imaging data management. To optimize database size and performance, many such systems store a reduced set of Digital Imaging and Communications in Medicine (DICOM) metadata, discarding informational content that might be needed in the future. As an alternative to traditional database systems, document-based key-value stores recently have gained popularity. These systems store documents containing key-value pairs that facilitate data searches without predefined schemas. Document-based key-value stores are especially suited to archive DICOM objects because DICOM metadata are highly heterogeneous collections of tag-value pairs conveying specific information about imaging modalities, acquisition protocols, and vendor-supported postprocessing options. The authors used an open-source document-based database management system (Apache CouchDB) to create and test two such databases; CouchDB was selected for its overall ease of use, capability for managing attachments, and reliance on HTTP and Representational State Transfer standards for accessing and retrieving data. A large database was created first in which the DICOM metadata from 5880 anonymized magnetic resonance imaging studies (1,949,753 images) were loaded by using a Ruby script. To provide the usual DICOM query functionality, several predefined "views" (standard queries) were created by using JavaScript. For performance comparison, the same queries were executed in both the CouchDB database and a SQL-based DICOM archive. The capabilities of CouchDB for attachment management and database replication were separately assessed in tests of a similar, smaller database. Results showed that CouchDB allowed efficient storage and interrogation of all DICOM objects; with the use of information retrieval algorithms such as map-reduce, all the DICOM metadata stored in the large database were searchable with only a minimal increase in retrieval time over that with the traditional database management system. Results also indicated possible uses for document-based databases in data mining applications such as dose monitoring, quality assurance, and protocol optimization.

Return on investment: workload, complexity and value of the CNS

The rheumatology nurse specialist (RNS) has become an integral and vital part of the multidisciplinary team and is valued by patients. Yet, a number of challenges regularly face all clinical nurse specialists (CNS) in the UK. The perception that CNS are an expensive and poorly defined nursing resource results in regular threats to their sustainability, particularly that of the RNS. This study examined return on investment of the RNS. An interrelational Structured Query Language (SQL) database collected data on the day-to-day activities of the RNS based on previous models of CNS and RNS work, and qualitative narrative data were then subjected to data mining. The RNS represented an excellent return on investment, in terms of income generation activity, patient safety and efficiency. These outcomes were achieved using key principles of proactive case management using vigilance, rescue work and brokering. The mean average per whole time equivalent (WTE) RNS per annum represents £ 175,168 in terms of income/savings to an employing NHS Trust. This figure is likely to be an underestimation, as calculations on reduction in bed days in hospital have not been included.

Configuration estimation method for preliminary cost of ships based on engineering bills of materials

Ships are complex engineering structures that are designed and built on the basis of technical experience. A shipowner will often be required to estimate the price of a new ship on the basis of the value of comparable ships identified in trade journals. Similarly, shipbuilders are often interested in estimating approximate costs during the tendering phase in order to determine whether a ship is likely to be competitive for a particular order. Thus, when designing a ship prior to having obtained a contract, one of the most important processes is the estimation of approximate costs, including materials, associated labor, and overhead. During this preliminary design phase, the design is temporary and subject to change based on variations in the shipowner’s requirements. Hence, quick and flexible responses are key during this period and an integral aspect of the competitive powers of the shipbuilder. Given this environment, we propose a “configuration estimation method.” Our method is based on the configuration design method that is widely used in three-dimensional (3D) computer aided design (CAD) systems. We assume that a product lifecycle management system is furnished and that the cost is then estimated via the configuration of the ship, using an engineering bill of materials (E-BOM). In referring to the E-BOM, we utilize technical parametric costs derived from similar ships built previously. Using the proposed method, it is possible to obtain an accurate list of materials from the quotation, as well as a detailed work assessment for labor costs and overhead rates, so that reliable cost estimates can be generated quickly and flexibly. To demonstrate the practical applicability and effectiveness of the proposed method, we implement the prototype of a shipbuilding configuration estimation system by using a Microsoft Structured Query Language database and an E-BOM from AVEVA Marine version 12.01, which is a representative CAD system for shipbuilding.

The Design and Implementation of an Automated System for Logging Clinical Experiences Using an Anesthesia Information Management System

Residents in anesthesia training programs throughout the world are required to document their clinical cases to help ensure that they receive adequate training. Current systems involve self-reporting, are subject to delayed updates and misreported data, and do not provide a practicable method of validation. Anesthesia information management systems (AIMS) are being used increasingly in training programs and are a logical source for verifiable documentation. We hypothesized that case logs generated automatically from an AIMS would be sufficiently accurate to replace the current manual process. We based our analysis on the data reporting requirements of the American College of Graduate Medical Education (ACGME). We conducted a systematic review of ACGME requirements and our AIMS record, and made modifications after identifying data element and attribution issues. We studied 2 methods (parsing of free text procedure descriptions and CPT4 procedure code mapping) to automatically determine ACGME case categories and generated AIMS-based case logs and compared these to assignments made by manual inspection of the anesthesia records. We also assessed under- and overreporting of cases entered manually by our residents into the ACGME website. The parsing and mapping methods assigned cases to a majority of the ACGME categories with accuracies of 95% and 97%, respectively, as compared with determinations made by 2 residents and 1 attending who manually reviewed all procedure descriptions. Comparison of AIMS-based case logs with reports from the ACGME Resident Case Log System website showed that >50% of residents either underreported or overreported their total case counts by at least 5%. The AIMS database is a source of contemporaneous documentation of resident experience that can be queried to generate valid, verifiable case logs. The extent of AIMS adoption by academic anesthesia departments should encourage accreditation organizations to support uploading of AIMS-based case log files to improve accuracy and to decrease the clerical burden on anesthesia residents.

Naturally Occurring Human Urinary Peptides for Use in Diagnosis of Chronic Kidney Disease

Because of its availability, ease of collection, and correlation with physiology and pathology, urine is an attractive source for clinical proteomics/peptidomics. However, the lack of comparable data sets from large cohorts has greatly hindered the development of clinical proteomics. Here, we report the establishment of a reproducible, high resolution method for peptidome analysis of naturally occurring human urinary peptides and proteins, ranging from 800 to 17,000 Da, using samples from 3,600 individuals analyzed by capillary electrophoresis coupled to MS. All processed data were deposited in an Structured Query Language (SQL) database. This database currently contains 5,010 relevant unique urinary peptides that serve as a pool of potential classifiers for diagnosis and monitoring of various diseases. As an example, by using this source of information, we were able to define urinary peptide biomarkers for chronic kidney diseases, allowing diagnosis of these diseases with high accuracy. Application of the chronic kidney disease-specific biomarker set to an independent test cohort in the subsequent replication phase resulted in 85.5% sensitivity and 100% specificity. These results indicate the potential usefulness of capillary electrophoresis coupled to MS for clinical applications in the analysis of naturally occurring urinary peptides.

Application of Medical Information System for Telepathology—Georgian Experience

The field of healthcare informatics is rapidly evolving. The new models and protocols of medical information system (MIS) are developed. Despite obvious advantages and benefits, practical application of MIS in everyday practice is slow. Much progress has been made around the world in the field of digital imaging and virtual slides, but in Georgia telepathology is still in evolving stages. It revolves around static telepathology. Practical application of MIS has been started in Georgia. The architecture of the mentioned system and its usage for telepathology will be presented. The MIS has been created with .Net technology and structure query language (SQL) database architecture. It involves a multiuser Web-based approach. By this, local (intranet) and remote (Internet) access of the system and management of databases can be achieved. Two hundred electronic medical records illustrated by images were selected for telepathology consultations. These electronic medical records were written in Georgian. This predetermines organization of regional second opinion consultations. For security reasons all experts have been registered as users at MIS. MIS has been launched in Georgia. Its primary goal is patient management. However, the system can be successfully applied for static telepathology purposes. The ideal of healthcare in the information age must be to create a situation where healthcare professionals spend more time creating knowledge from medical information and less time managing medical information. The application of easily available and adaptable technology and improvement of the infrastructure conditions is the basis for telemedical applications. The usage of MIS holds the potential to realize telepathology in the effective and comprehensive mode.

Development of a Kidney TeleUltrasound Consultation System

This paper presents the development of kidney TeleUltrasound consultation system. The TeleUltrasound system provides an innovative design that aids the acquisition, archiving, and dissemination of medical data and information over the internet as its backbone. The system provides data sharing to allow remote collaboration, viewing, consultation, and diagnosis of medical data. The design is layered upon a standard known as Digital Imaging and Communication in Medicine (DICOM). The DICOM standard defines protocols for exchanging medical images and their associated data. The TeleUltrasound system is an integrated solution for retrieving, processing, and archiving images and providing data storage management using Structured Query Language (SQL) database. Creating a web-based interface is an additional advantage to achieve global accessibility of experts that will widely open the opportunity of greater examination and multiple consultations. This system is equipped with a high level of data security and its performance has been tested with white, black, and gray box techniques. And the result was satisfactory. The overall system has been evaluated by several radiologists in Malaysia, United Arab Emirates, and Sudan, the result is shown within this paper.

Application of a statistical software package for analysis of large patient dose data sets obtained from RIS

For the purpose of patient dose audit, clinical audit and radiology workload analysis, data from Radiology Information Systems (RIS) at many hospitals are collected using a database and the analysis was automated using a statistical package and Visual Basic coding. The database is a Structured Query Language database, which can be queried using an off-the-shelf statistical package, Statistica. Macros were created to automatically format the data to a consistent format between different hospitals ready for analysis. These macros can also be used to automate further analysis such as detailing mean kV, mAs and entrance surface dose per room and per gender. Standard deviation and standard error of the mean are also generated. Graphs can also be generated to illustrate the trends in doses between different variables such as room and gender. Collectively, this information can be used to generate a report. A process that once could take up to 1 d to complete now takes around 1 h. A major benefit in providing the service to hospital trusts is that less resource is now required to report on RIS data, making the possibility of continuous dose audit more likely. Time that was spent on sorting through data can now be spent on improving the analysis to provide benefit to the customer. Using data sets from RIS is a good way to perform dose audits as the huge numbers of data available provide the bases for very accurate analysis. Using macros written in Statistica Visual Basic has helped sort and consistently analyse these data. Being able to analyse by exposure factors has provided a more detailed report to the customer.

Full predicate coverage for testing SQL database queries

AbstractIn the field of database applications a considerable part of the business logic is implemented using a semi‐declarative language: the Structured Query Language (SQL). Because of the different semantics of SQL compared with other procedural languages, the conventional coverage criteria for testing are not directly applicable. This paper presents a criterion specifically tailored for SQL queries (SQLFpc). It is based on Masking Modified Condition Decision Coverage (MCDC) or Full Predicate Coverage and takes into account a wide range of the syntax and semantics of SQL, including selection, joining, grouping, aggregations, subqueries, case expressions and null values. The criterion assesses the coverage of the test data in relation to the query that is executed and it is expressed as a set of rules that are automatically generated and efficiently evaluated against a test database. The Clinical Application of a PACS-Dependent 12-Lead ECG and Image Information System in E-Medicine and Telemedicine

This study presents a software technology to transform paper-based 12-lead electrocardiography (ECG) examination into (1) 12-lead ECG electronic diagnoses (e-diagnoses) and (2) mobile diagnoses (m-diagnoses) in emergency telemedicine. While Digital Imaging and Communications in Medicine (DICOM)-based images are commonly used in hospitals, the development of computerized 12-lead ECG is impeded by heterogeneous data formats of clinically used 12-lead ECG instrumentations, such as Standard Communications Protocol (SCP) ECG and Extensible Markup Language (XML) ECG. Additionally, there is no data link between clinically used 12-lead ECG instrumentations and mobile devices. To realize computerized 12-lead ECG examination procedures and ECG telemedicine, this study develops a DICOM-based 12-lead ECG information system capable of providing clinicians with medical images and waveform-based ECG diagnoses via Picture Archiving and Communication System (PACS). First, a waveform-based DICOM-ECG converter transforming clinically used SCP-ECG and XML-ECG to DICOM is applied to PACS for image- and waveform-based DICOM file manipulation. Second, a mobile Structured Query Language database communicating with PACS is installed in physicians' mobile phones so that they can retrieve images and waveform-based ECG ubiquitously. Clinical evaluations of this system indicated the following. First, this developed PACS-dependent 12-lead ECG information system improves 12-lead ECG management and interoperability. Second, this system enables the remote physicians to perform ubiquitous 12-lead ECG and image diagnoses, which enhances the efficiency of emergency telemedicine. These findings prove the effectiveness and usefulness of the PACS-dependent 12-lead ECG information system, which can be easily adopted in telemedicine.

Proteomics Analysis of the Excretory/Secretory Component of the Blood-feeding Stage of the Hookworm, Ancylostoma caninum

Hookworms are blood-feeding intestinal parasites of mammalian hosts and are one of the major human ailments affecting approximately 600 million people worldwide. These parasites form an intimate association with the host and are able to avoid vigorous immune responses in many ways including skewing of the response phenotype to promote parasite survival and longevity. The primary interface between the parasite and the host is the excretory/secretory component, a complex mixture of proteins, carbohydrates, and lipids secreted from the surface or oral openings of the parasite. The composition of this complex mixture is for the most part unknown but is likely to contain proteins important for the parasitic lifestyle and hence suitable as drug or vaccine targets. Using a strategy combining the traditional technology of one-dimensional SDS-PAGE and the newer fractionation technology of OFFGEL electrophoresis we identified 105 proteins from the excretory/secretory products of the blood-feeding stage of the dog hookworm, Ancylostoma caninum. Highly represented among the identified proteins were lectins, including three C-type lectins and three beta-galactoside-specific S-type galectins, as well as a number of proteases belonging to the three major classes found in nematodes, aspartic, cysteine, and metalloproteases. Interestingly 28% of the identified proteins were homologous to activation-associated secreted proteins, a family of cysteine-rich secreted proteins belonging to the sterol carrier protein/Tpx-1/Ag5/PR-1/Sc-7 (TAPS) superfamily. Thirty-four of these proteins were identified suggesting an important role in host-parasite interactions. Other protein families identified included hyaluronidases, lysozyme-like proteins, and transthyretin-like proteins. This work identified a suite of proteins important for the parasitic lifestyle and provides new insight into the biology of hookworm infection.

Dimensions of clinical nurse specialist work in the UK

To model the work of clinical nurse specialists (CNSs) in the UK. This article examines data mined as part of a national project. The Pandora database was initially collected on a Microsoft Office Access database and subsequently, a Structured Query Language database in several iterations from June 2006 to September 2008. Pandora recorded CNS activity as a series of events with eight dimensions to each event. Data from this were mined to examine the complexity of CNS work. This study represents the work of 463 CNSs over 2,778 days in England, Scotland and Wales. Clinical work, including physical assessment, referral, symptom control and 'rescue' work, accounted for a large part of the CNS's role. Administration was the second highest workload, with about half of these administrative tasks identified as being suitable for secretarial staff to undertake. Research, education and consultation accounted for less time. A significant proportion of the nurses' clinical work is undertaken by telephone. CNSs in this study spent much of their time doing complex clinical work. Payment by Results (Department of Health 2006) should recognise the work undertaken by CNSs, particularly that done on the telephone. Complex clinical work by CNSs takes place in many different contexts using a wide range of interventions. The role of the CNS is complex and diverse, making comparisons of it difficult. More research needs to be done in relation to quality, safety and efficiency.

Data Management System for Distributed Virtual Screening

High throughput docking (HTD) using high performance computing platforms is a multidisciplinary challenge. To handle HTD data effectively and efficiently, we have developed a distributed virtual screening data management system (DVSDMS) in which the data handling and the distribution of jobs are realized by the open-source structured query language database software MySQL. The essential concept of DVSDMS is the separation of the data management from the docking and ranking applications. DVSDMS can be used to dock millions of molecules effectively, monitor the process in real time, analyze docking results promptly, and process up to 10(8) poses by energy ranking techniques. In an HTD campaign to identify kinase inhibitors a low cost Linux PC has allowed DVSDMS to efficiently assign the workload to more than 500 computing clients. Notably, in a stress test of DVSDMS that emulated a large number of clients, about 60 molecules per second were distributed to the clients for docking, which indicates that DVSDMS can run efficiently on very large compute cluster (up to about 40000 cores).

Using a Web-Based Database to Record and Monitor Athletic Training Students' Clinical Experiences

Objective: The purpose of this article is to introduce a documentation recording system employing the Microsoft Structured Query Language (MS-SQL) database used by the Athletic Training Education Program (ATEP) for recording and monitoring of athletic training student (ATS) clinical experiences and hours. Background: Monitoring ATSs clinical experiences and hours can be a monumental task for an athletic training education program director (PD). This article seeks to explain and demonstrate how a web-based database is incorporated into an athletic training education program to manage the ATSs clinical experiences. To date, a thorough review of the literature did not reveal any published documentation of a web-based database being used for monitoring ATSs clinical experiences. In our ATEP, prior documentation of the ATSs clinical experiences consisted of a paper-based system which involved the ATS writing down their clinical hours and having their assigned approved clinical instructor (ACI) or clinical instructors (CI) verify them. After reviewing other software application programs (i.e., Microsoft Excel and Microsoft Access), it was determined that these programs did not provide the flexibility needed for a program-wide database. Therefore, time management and evaluation accessibility constraints played a pivotal role in the decision to move to a web-based database.

Building a data warehouse for infection control

Recently, the convergence of several themes in health care has stimulated interest in exploiting data collected during clinical encounters to monitor and improve the quality of health care. Infection control departments can be supported by assembling previously collected electronic data into a data warehouse to monitor infections, report communicable diseases, and detect clusters of organisms. I present the rationale for constructing a data warehouse and describe the steps and challenges of developing a data warehouse to support infection control activities.

Web-based method for translating neurodevelopment from laboratory species to humans

Biomedical researchers and medical professionals are regularly required to compare a vast quantity of neurodevelopmental literature obtained from an assortment of mammals whose brains grow at diverse rates, including fast developing experimental rodent species and slower developing humans. In this article, we introduce a database-driven website, which was created to address this problem using statistical-based algorithms to integrate hundreds of empirically derived developing neural events in 10 mammalian species (http://translatingtime.net/). The site, based on a statistical model that has evolved over the past decade, currently incorporates 102 different neurodevelopmental events obtained from 10 species: hamsters, mice, rats, rabbits, spiny mice, guinea pigs, ferrets, cats, rhesus monkeys, and humans. Data are arranged in a Structured Query Language database, which allows comparative brain development measured in postconception days to be converted and accessed in real time, using Hypertext Preprocessor language. Algorithms applied to the database also allow predictions for dates of specific neurodevelopmental events where empirical data are not available, including for the human embryo and fetus. By designing a web-based portal, we seek to make these comparative data readily available to all those who need to efficiently estimate the timing of neurodevelopmental events in the human fetus, laboratory species, or across several different species. In an effort to further refine and expand the applicability of this database, we include a mechanism to submit additional data.
