RADT-41. RADIOSURGERY FOR GLOMUS TUMORS: IMPACT OF MRI-PET FUSION ON TREATMENT PLANNING AND OUTCOMES

Abstract Paragangliomas present a challenge for stereotactic radiosurgery (SRS) treatment planning, particularly regarding the delineation of tumor boundaries. We propose that integrating 68Ga-DOTATATE PET scans alongside MRI in SRS planning will facilitate superior target identification and tumor coverage compared to reliance on MRI alone. A retrospective analysis of 9 patients diagnosed with paraganglioma who underwent MRI and 68Ga-DOTATATE PET imaging over a 20-month period was conducted. Images were imported into GammaPlan treatment planning software, partitioned into separate sessions: one with MRI and another with MRI-PET fusion. A single evaluator contoured paragangliomas as gross tumor volume (GTV) initially using MRI alone and subsequently with PET/MRI fusion. The pre- and post-PET fusion GTV volumes were compared. The standard deviation of the percentage increases in volume was calculated to assess variability across the sample. Of included 9 patients, 7 were females and median age was 58.3 years. Four patients had prior surgery. Seven patients were treated with Gamma Knife, while 2 with LINAC. Two patients were treated with single fraction SRS while 7 with hypofractionated SRS. Across all treated patients (n=9), GTV volumes significantly increased with PET fusion, with an average volume increase of 26.02% (n=8). One patient reported dryness of throat after SRS. Significant PET avidity over structures like temporal bone, internal jugular vein, petrous internal carotid artery and sigmoid sinus-IJV junction were included in PET/MRI fusion GTVs, which were missed on isolated MRI contours. At median 6-month follow up, all patients had a stable radiological response. Integration of 68Ga-DOTATATE PET scans with MRI enhances the visualization of paragangliomas in patients undergoing SRS. Further extensive studies are warranted to validate this observation and to better understand the variability in treatment response observed across the patient population. Our practice has initiated routine utilization of DOTATATE-PET imaging in paraganglioma patients undergoing SRS planning.

Calculating Optic Nerve Planning Organ at Risk Volume Margins for Stereotactic Radiosurgery Using Optic Nerve Motion determined using MRI.

The combination of sharp dose gradients in stereotactic radiosurgery (SRS) and minute optic nerve motion may significantly increase dose to the optic nerves when treating perioptic lesions. The aim of this study was to calculate optic nerve planning organ at risk volume (PRV) margins for CyberKnife SRS treatment planning. MRI scans were taken of 10 healthy volunteers looking left, right, up, down, and straight ahead to measure optic nerve motion. The measured optic nerve motion and the uncertainties in the technical accuracy of CyberKnife were used to calculate optic nerve PRV margins. Two optic nerve PRV margins were calculated: a non-isotropic margin of mL/R,PRV=3mm, mSup/Inf,PRV=2mm and mAnt/Post,PRV=1mm which considers the full range of motion measured in a worst case scenario; and an isotropic margin of mPRV=1mm which considers a scenario where patients are asked to look neutrally during imaging and treatment. Applying these PRVs to 8 historical sphenoid wing meningioma CyberKnife plans showed tolerance levels may be exceeded due to optic nerve motion. Optic nerve PRV margins may be needed in CyberKnife planning to reduce risk to the optic nerves. The use of a mPRV=1mm PRV to account for organ motion, along with instructing patients to hold their gaze neutrally during imaging and treatment, may be a suitable organ sparing strategy. Measured optic nerve motion and the technical accuracy of the CyberKnife system have been used to calculate optic nerve PRV margins.

Dosimetry study comparing single-isocenter high-definition dynamic radiosurgery with multiple-isocenter cone-based stereotactic radiosurgery in treating multiple brain metastases

PurposeWe sought to compare two radiotherapeutic strategies for treating multiple brain metastases (MBM), assessing delivery efficiency, plan quality, and dosimetry of planning target volume (PTV) and normal tissue. MethodsOur study population included 21 patients treated for MBM, each reliant on high-definition dynamic radiosurgery (Monaco-HDRS) or cone-based stereotactic radiosurgery (SRS) treatment plans. Monaco-HDRS plans called for one isocenter for all targets. In cone-based plans, each target corresponded with one isocenter. To compare these modalities, we assessed: monitor units (MU); new conformity index (nCI); dose gradient index (GI); homogeneity index (HI); PTV minimum/maximum (Dmax/Dmin) and mean (Dmean) doses; maximum doses to lenses, optic nerves, and brainstem; and brain dose volumes at 3, 6, 10, 12, and 22 Gy (V3Gy-V22Gy). ResultsThe Monaco-HDRS treatment plan proved more efficient, displaying lower MU, HI, and nCI values and better dosimetry (Dmax/Dmin and Dmean) for PTV. The cone-based plan yielded a lower GI value and dose volumes at 3 Gy and 22 Gy (V3Gy, V22Gy) for brain. There were no significant differences among other parameters. ConclusionsThe Monaco-HDRS plan improved treatment efficiency, conformity, and homogeneity, although dose fall-off was worse. The cone-based plan reduced normal brain dose volumes at 3 Gy and 22 Gy.

Impact of the Novel MRI Contrast Agent Gadopiclenol on Radiotherapy Decision Making in Patients With Brain Metastases.

The aim of this study was to assess the effect of gadopiclenol versus gadobenate dimeglumine contrast-enhanced magnetic resonance imaging (MRI) on decision-making between whole-brain radiotherapy (WBRT) and stereotactic radiosurgery (SRS) for treatment of brain metastases (BMs). Patients with BMs underwent 2 separate MRI examinations in a double-blind crossover phase IIb comparative study between the MRI contrast agents gadopiclenol and gadobenate dimeglumine, both administered at 0.1 mmol/kg. The imaging data of a single site using identical MRI scanners and protocols were included in this post hoc analysis. Patients with 1 or more BMs in any of both MRIs were subjected to target volume delineation for treatment planning. Two radiation oncologists contoured all visible lesions and decided upon SRS or WBRT, according to the number of metastases. For each patient, SRS or WBRT treatment plans were calculated for both MRIs, considering the gross target volume (GTV) as the contrast-enhancing aspects of the tumor. Mean GTVs and volume of healthy brain exposed to 12 Gy (V12), as well as Dice similarity coefficient scores, were obtained. The Spearman rank (ρ) correlation was additionally calculated for assessing linear differences. Three different expert radiation oncologists blindly rated the contrast enhancement for contouring purposes. Thirteen adult patients were included. Gadopiclenol depicted additional BM as compared with gadobenate dimeglumine in 7 patients (54%). Of a total of 63 identified metastatic lesions in both MRI sets, 3 subgroups could be defined: A, 48 (24 pairs) detected equal GTVs visible in both modalities; B, 13 GTVs only visible in the gadopiclenol set (mean ± SD, 0.16 ± 0.37 cm3); and C, 2 GTVs only visible in the gadobenate dimeglumine set (mean ± SD, 0.01 ± 0.01). Treatment indication was changed for 2 (15%) patients, 1 from no treatment to SRS and for 1 from SRS to WBRT. The mean GTVs and brain V12 were comparable between both agents (P = 0.694, P = 0.974). The mean Dice similarity coefficient was 0.70 ± 0.14 (ρ = 0.82). According to the readers, target volume definition was improved in 63.9% of cases (23 of 36 evaluations) with gadopiclenol and 22.2% with gadobenate dimeglumine (8 of 36), whereas equivalence was obtained in 13.9% (5 of 36). Gadopiclenol-enhanced MRI improved BM detection and characterization, with a direct impact on radiotherapy treatment decision between WBRT and SRS. Additionally, a more exact target delineation and planning could be performed with gadopiclenol. A prospective evaluation in a larger cohort of patients is required to confirm these findings.

Evaluating effectiveness of clustering algorithms in multiple target stereotactic radiosurgery

Objective. Single-isocenter-multiple-target technique for stereotactic radiosurgery (SRS) can reduce treatment duration but risks compromised dose coverage due to potential rotational errors. Clustering targets into two groups can reduce isocenter-target distances, mitigating the impact of rotational uncertainty. However, a comprehensive evaluation of clustering algorithms for SRS is absent. This study addresses this gap by introducing the SRS Target Clustering Framework (Framework), a comprehensive tool that utilizes commonly used clustering algorithms to generate efficient cluster configurations. Approach. The Framework incorporates four distinct optimization objectives based on two key metrics: the isocenter-target distance and the ratio of this distance to the target radius. Agglomerative and weighted agglomerative clustering are employed for minimax and weighted minimax objectives, respectively. K-means and weighted k-means are utilized for sum-of-squares and weighted sum-of-squares objectives. We applied the Framework to 126 SRS plans, comparing results to ground truth solutions obtained through a brute force algorithm. Main results. For the minimax objective, the average maximum isocenter-target distance from agglomerative clustering (4.8 cm) was slightly higher than the ground truth (4.6 cm). Similarly, the weighted agglomerative clustering achieved an average maximum ratio of 15.1 compared to the ground truth of 14.6. Notably, both k-means and weighted k-means clustering showed close agreement (within a precision of 0.1) with the ground truth for average root-mean-square target-isocenter distance and ratio (3.6 cm and 11.1, respectively). Significance. These results demonstrate the Framework’s effectiveness in generating clusters for SRS targets. The proposed approach has the potential to become a valuable tool in SRS treatment planning. Furthermore, this study is the first to investigate clustering algorithms for both minimizing maximum and sum-of-squares uncertainty in SRS.

Development and validation of a fully automatic tissue delineation model for brain metastasis using a deep neural network.

Stereotactic radiosurgery (SRS) treatment planning requires accurate delineation of brain metastases, a task that can be tedious and time-consuming. Although studies have explored the use of convolutional neural networks (CNNs) in magnetic resonance imaging (MRI) for automatic brain metastases delineation, none of these studies have performed clinical evaluation, raising concerns about clinical applicability. This study aimed to develop an artificial intelligence (AI) tool for the automatic delineation of single brain metastasis that could be integrated into clinical practice. Data from 426 patients with postcontrast T1-weighted MRIs who underwent SRS between March 2007 and August 2019 were retrospectively collected and divided into training, validation, and testing cohorts of 299, 42, and 85 patients, respectively. Two Gamma Knife (GK) surgeons contoured the brain metastases as the ground truth. A novel 2.5D CNN network was developed for single brain metastasis delineation. The mean Dice similarity coefficient (DSC) and average surface distance (ASD) were used to assess the performance of this method. The mean DSC and ASD values were 88.34%±5.00% and 0.35±0.21 mm, respectively, for the contours generated with the AI tool based on the testing set. The DSC measure of the AI tool's performance was dependent on metastatic shape, reinforcement shape, and the existence of peritumoral edema (all P values <0.05). The clinical experts' subjective assessments showed that 415 out of 572 slices (72.6%) in the testing cohort were acceptable for clinical usage without revision. The average time spent editing an AI-generated contour compared with time spent with manual contouring was 74 vs. 196 seconds, respectively (P<0.01). The contours delineated with the AI tool for single brain metastasis were in close agreement with the ground truth. The developed AI tool can effectively reduce contouring time and aid in GK treatment planning of single brain metastasis in clinical practice.

Lexicographic Optimization-Based Planning for Single and Multiple Brain Metastasis Radiosurgery with Coplanar Arcs

This study validated a not yet commercially available fully-automated lexicographic optimization planning system (LOps) for single and multiple lesions intracranial stereotactic radiosurgery (SRS). Forty-four consecutive SRS treatment plans (21 Gy/1 fx) delivered between November 2019 and August 2022 were retrospectively selected and automatically re-planned by LOps: 25 of them has 1 lesion, 13 had 2 lesions, 4 had 3 lesions, and 2 had 4 lesions. An a-priori assigned priority list, Wish List (WL), was used to define the sequential LO: 4 patient sets (tuning set) were necessary to tune each WL, for single lesion-plans (SLp) and multiple lesion-plans (MLp). While in manual plans (MP), the arc setup is freely chosen, the WL was tuned to use 2 coplanar arcs of 140° and 360° for SLp and MLp, respectively. A 0° and 90° collimator rotation were set for counter-clockwise and clockwise arcs, respectively. The fluence optimization is followed by a Monte Carlo calculation (MCc) with 1 mm-dose grid and 0.5%-statistical uncertainty. A target coverage as high as possible was requested, with at least 80% of the prescription dose covering 99% of the PTV. The main criteria for SLp approval were a brain V12Gy <10 cm3. In MLp this criterion can be overcome to get the minimum target coverage. The remaining 36 SRS plans (21 SL and 15 MLp) were automatically re-planned (testing set). Testing plans were compared in terms of dose-volume constraints, conformality, and monitor units (MUs). Statistical significance was assessed by performing the Wilcoxon test and plan delivery accuracy was verified by pre-treatment QA. WLs-tuning took 3 days. Overall manual and automatic MCc time can be estimated at 8 hours and 3 hours, respectively. Statistically significant increases in SLp and MLp target coverage (GTV_D98%: +3.0% SL, +5.7% ML, PTV_D98%: +4.4% SL, +13.4% ML) and conformity index were registered. Automatic plans showed acceptably higher median brain V12Gy (SL: MP 7.2 cm3, SLp 7.6 cm3; ML: MP 8.7 cm3, MLp 10.3 cm3). The SLp registered a lower median MU number (-4.2%) while MLp were obtained with a higher median number of MU (+9.8%). This not statistically significant difference did not affect gamma passing rates. The novel LOps produced high-quality clinically acceptable SRS SL and ML plans with coplanar arcs according to institutional-specific planning protocols, significantly reducing the overall planning time from about one working day for one MP to about 3 hours for one automatic plan. Together with comparable OAR sparing, the target coverage was significantly increased and the plan deliverability preserved.

P13.09.B FULLY AUTOMATED BRAIN METASTASES SEGMENTATION USING T1-WEIGHTED CONTRAST-ENHANCED MR IMAGES BEFORE AND AFTER STEREOTACTIC RADIOSURGERY

Abstract BACKGROUND Brain metastases (BM) represent the most common intracranial tumor in adults. An estimated 20% of all patients with cancer will develop BM. Stereotactic Radiosurgery (SRS) is a major treatment option for BM. For SRS treatment planning and outcome evaluation, magnetic resonance images (MRI) are acquired before and at multiple stages during the follow-up. Accurate segmentation of brain tumors on MRI is crucial for treatment planning and response evaluation. Detection and segmentation of BM which is a tedious and time-consuming task for many radiologists could be optimized with machine learning METHODS . The accuracy of the auto-segmentation, however, is influenced by the presence of false-positive and false-negative segmentation. There are studies which evaluated the segmentation performance of several deep learning algorithms, these were mainly focused on training and testing the models on either the pre-treatment or post-treatment images. The purpose of this study was to investigate a well-known deep learning approach (nnU-Net) for BM segmentation and to evaluate its performance on both pre-treatment and post-treatment images to assess if it could be a handy tool for the clinicians. METHODS Pre-treatment T1-weighted brain MRIs which were contrast-enhanced with triple-dose gadolinium were collected retrospectively for 266 patients with BM. Scans were made as part of clinical care at the Gamma Knife Center of the Elisabeth-TweeSteden Hospital (Tilburg, the Netherlands). All patients underwent Gamma Knife Radiosurgery, a form of SRS. This total of 266 patients were randomly split into 210 patients for model training/validation and 56 patients for testing. For these 56 patients used for testing, the post treatment follow-up T1-weighted brain MRI scans which were contrast-enhanced with single-dose gadolinium were also retrospectively collected. The nnU-Net model was trained with the pre-treatment training data, and then tested separately against the pre-treatment and follow-up data. RESULTS The model obtained a Dice score of 0.91 when tested with the pre-treatment images and a Dice score of 0.80 when tested with the follow-up after treatment T3 images. The False Negative Rate (FNR) when tested with the pre-treatment images was 0.07 and 0.24 when tested with post-treatment T3 images. CONCLUSION The model achieved a good performance score for pre-treatment images. The nnU-Net model can automatically detect and segment brain metastases with high sensitivity, and low FNR for treatment planning. This could be a beneficial tool for clinicians and assist SRS management for diagnosis and treatment planning. Though there is a decline in the Dice score and an increase in the FNR of the model for the post-treatment images, the performance still remained higher than in other similar studies.

Comparison of two optimization algorithms (VOLOTM , SEQU) for CyberKnife® treatment of acoustic neuromas, lung metastases, and liver metastases.

Two optimization algorithms VOLO™ and sequential optimization algorithm (SEQU) are compared in the Precision® treatment planning system from Accuray® for stereotactic radiosurgery and stereotactic body radiotherapy (SBRT) treatment plans. The aim is to compare the two algorithms to assess if VOLO™ is better of SEQU in certain treatment site. Sixty clinical treatment cases were compared. Entities include Acoustic neuroma (AN), lung metastases, and liver metastases. In each entity, 10 SEQU and 10 VOLO™ treatment plans were optimized. The Ray-Tracing calculation algorithm was used for all treatment plans and the treatments were planned exclusively with fixed cones (5-50mm). The number of nodes, beams, total MU, and treatment time were compared. Conformity index (CI), new conformity index (nCI), homogeneity index (HI), gradient index (GI), and target coverage were examined for agreement. Dmin , Dmean , Dmax , D100%, D98%, and D2% dose in the target volume as well as exposure to organs at risk was checked. To determine peripheral doses, the isodose volumes from V10% to V98% were evaluated. AN treatment plans showed significant differences for the number of nodes, beams, total MU, treatment time, D98%, D100% for the target volume, and the doses for all organs at risk. VOLO™ achieved better results on average. Total MU, treatment time, coverage, and D98% are significantly better for VOLO™ for lung metastases. For liver metastases, a significant reduction in number of nodes, total MU, and treatment time was observed for VOLO™ plans. The mean target coverage increased slightly with VOLO™, while the mean CI deteriorated slightly. The averages of Dmin , Dmean , D98%, D100%, and V80% resulted in a significant increase for VOLO™. The results of the present study indicate that VOLO™ should be used in place of SEQU as a standard for AN cases moving forward. Despite the lack of significance in the lung and liver cases, VOLO™ optimization is recommended because OAR sparing was similar, but coverage, Dmin , and Dmean were increased, and thus better tumor control can be expected.

Incorporation of Brain Connectomics for Stereotactic Radiosurgery Treatment Planning.

Neurosurgeons have integrated neuroanatomy-based tractography to avoid critical structures during dose planning. However, they have yet to integrate more comprehensive connectome networks for radiosurgical planning. A young man presented with a Spetzler-Martin Grade 3 right temporal arteriovenous malformation. As proof of concept, we incorporated connectomic networks including default mode network, optic radiation and central executive network into the Gamma Knife radiosurgical treatment planning workflow. Connectome networks were created from T1 anatomic and diffusion-weighted images magnetic resonance images using Quicktome software. The resulting networks were voxel-encoded in the magnetic resonance images, imported into GammaPlan, and segmented by image thresholding. The GammaPlan Lightning optimizer was used to create radiosurgical plans with a dose of 20 Gy to the 50% isodose line delivered to the arteriovenous malformation nidus both with and without treating these networks as risk structures. When taking into account the connectome networks, a maximum dose restriction of 14 Gy was placed on each network during lightning dose planning. With default mode network, optic radiation, and central executive network as risk structures, the maximum dose and V 12Gy were reduced by 23.4% and 88.3%, 20% and 34.3%, and 29.8% and 63.2%, respectively. We were able to incorporate connectomes into radiosurgical dose planning approaches. This allowed for dose reductions to the networks while still achieving delivery of a therapeutic dose to the target volume.

Evaluation of inverse treatment planning for gamma knife radiosurgery using fMRI brain activation maps as organs at risk.

Stereotactic radiosurgery (SRS) can be an effective primary or adjuvant treatment option for intracranial tumors. However, it carries risks of various radiation toxicities, which can lead to functional deficits for the patients. Current inverse planning algorithms for SRS provide an efficient way for sparing organs at risk (OARs) by setting maximum radiation dose constraints in the treatment planningprocess. We propose using activation maps from functional MRI (fMRI) to map the eloquent regions of the brain and define functional OARs (fOARs) for Gamma Knife SRS treatment planning. We implemented a pipeline for analyzing patient fMRI data, generating fOARs from the resulting activation maps, and loading them onto the GammaPlan treatment planning software. We used the Lightning inverse planner to generate multiple treatment plans from open MRI data of five subjects, and evaluated the effects of incorporating the proposed fOARs. The Lightning optimizer designs treatment plans with high conformity to the specified parameters. Setting maximum dose constraints on fOARs successfully limits the radiation dose incident on them, but can have a negative impact on treatment plan quality metrics. By masking out fOAR voxels surrounding the tumor target it is possible to achieve high quality treatment plans while controlling the radiation dose on fOARs. The proposed method can effectively reduce the radiation dose incident on the eloquent brain areas during Gamma Knife SRS of braintumors.

Evaluation of commercial devices for patient specific QA of stereotactic radiotherapy plans.

Stereotactic radiotherapy (SRT) methods have become common for the treatment of small tumors in various parts of the body. Small field dosimetry has a unique set of challenges when it comes to the pre-treatment validation of a radiotherapy plan that involves film dosimetry or high-resolution detectors. Comparison of commercial quality assurance (QA) devices to the film dosimetry method for pre-treatment evaluation of stereotactic radiosurgery (SRS), fractionated SRT, and stereotactic body radiation therapy treatment plans have been evaluated in this study. Forty stereotactic QA plans were measured using EBT-XD film, IBA Matrixx Resolution, SNC ArcCHECK, Varian aS1200 EPID, SNC SRS MapCHECK, and IBA myQA SRS. The results of the commercial devices are compared to the EBT-XD film dosimetry results for each gamma criteria. Treatment plan characteristics such as modulation factor and target volume were investigated for correlation with the passing rates. It was found that all detectors have greater than 95% passing rates at 3%/3mm. Passing rates decrease rapidly for ArcCHECK and the Matrixx as criteria became more strict. In contrast, EBT-XD film, SNC SRS MapCHECK, and IBA myQA SRS passing rates do not decline as rapidly when compared to Matrix Resolution, ArcCHECK, and the EPID. EBT-XD film, SNC SRS MapCHECK, and IBA myQA SRS maintain greater than 90% passing rate at 2%/1mm and greater than 80% at 1%/1mm. Additionally, the ability of these devices to detect changes in dose distribution due to MLC positioning errors was investigated. Ten VMAT SBRT/SRS treatment plans were created with 6MV FFF or 10MV FFF beam energies using Eclipse 15.6. A MATLAB script was used to create two MLC positioning error scenarios from the original treatment plan. It was found that errors in MLC positioning were most reliably detected at 2%/1mm for high-resolution detectors and that lower-resolution detectors did not consistently detect MLC positioning errors.

Evaluation of plan quality and treatment efficiency in cranial stereotactic radiosurgery treatment plans with a variable source-to-axis distance.

Radiotherapy deliveries with dynamic couch motions that shorten the source-to-axis distance (SAD) on a C-arm linac have the potential to increase treatment efficiency through the increase of the effective dose rate. In this investigation, we convert clinically deliverable volumetric modulated arc therapy (VMAT) and dynamic conformal arc (DCA) plans for cranial radiosurgery into virtual isocenter plans through implementation of couch trajectories that maintain the target at a shortened but variable SAD throughout treatment. A randomly sampled population of patients treated with cranial radiosurgery from within the last three years were separated into groups with one, two, and three lesions. All plans had a single isocenter (regardless of number of targets), and a single prescription dose. Patient treatment plans were converted from their original delivery at a standard isocenter to a dynamic virtual isocenter in MATLAB. The virtual isocenter plan featured a variable isocenter position based upon the closest achievable source-to-target distance (referred to herein as a virtual source-to-axis distance [vSAD]) which avoided collision zones on a TrueBeam STx platform. Apertures were magnified according to the vSAD and monitor units at a given control point were scaled based on the inverse square law. Doses were calculated for the plans with a virtual isocenter in the Eclipse (v13.6.23) treatment planning system (TPS) and were compared with the clinical plans. Plan metrics (MU, Paddick conformity index, gradient index, and the volume receiving 12 Gy or more), normal brain dose-volume differences, as well as maximum doses received by organs at risk (OARs) were assessed. The values were compared between standard and virtual isocenter plans with Wilcoxon Sign Ranked tests to determine significance. A subset of the plans were mapped to the MAX-HD anthropomorphic phantom which contained an insert housing EBT3 GafChromic film and a PTW 31010 microion chamber for dose verification on a linac. Delivering plans at a virtual isocenter resulted in an average reduction of 20.9% (p = 3×10-6 ) and 20.6% (p = 3.0×10-6 ) of MUs across all VMAT and all DCA plans, respectively. There was no significant change in OAR max doses received by plans delivered at a virtual isocenter. The low dose wash (1.0-2.0 Gy or 5-11% of the prescription dose) was increased (by approximately 20 cc) for plans with three lesions. This was equivalent to a 2.7%-3.8% volumetric increase in normal tissue receiving the respective dose level when comparing the plan with a virtual isocenter to a plan with a standard isocenter. Gamma pass rates with a 5%/1mm analysis criteria were 96.40% ± 2.90% and 95.07% ± 3.10% for deliveries at standard and virtual isocenter, respectively. Absolute point dose agreements were within -0.36% ± 3.45% and -0.55% ± 3.39% for deliveries at a standard and virtual isocenter, respectively. Potential time savings per arc were found to have linear relationship with the monitor units delivered per arc (savings of 0.009 s/MU with an r2 = 0.866 when fit to plans with a single lesion). Converting clinical plans at standard isocenter to a virtual isocenter design did not show any losses to plan quality while simultaneously improving treatment efficiency through MU reductions.

An Optimal Solution Study on a Non-Invasive Stereotactic Radiosurgery Instrument Optimizer for Stereotactic Radiosurgery of Single Brain Metastasis

<h3>Purpose/Objective(s)</h3> This study looks at a fast inverse planning (FIP) optimizer that enables more efficient and less planner dependent stereotactic radiosurgery (SRS) treatment planning. FIP generates a plan using multiple user-inputted variables that affect plan quality metrics, such as target coverage (TC), maximum dose to the target and organs-at-risk, low dose (LD), and beam-on time (BOT). The objective of this study is to systematically characterize each of these variables and determine which combination leads to the best SRS plan quality metrics. <h3>Materials/Methods</h3> Thirty patients with a single brain metastasis who were previously treated with SRS were evaluated. Each lesion was planned using FIP by varying the LD and BOT variables in 0.1 increments from 0.0 to 1.0 (i.e., plan #1 [0.0, 0.0], plan #2 [0.0, 0.1], ..., plan #120 [1.0, 0.9] and plan #121 [1.0, 1.0]). All plans were generated with the same prescription dose (24 Gy/1 fraction), TC (100%), maximum allowed dose (43.2 Gy), and Gamma angle (90°). The following metrics were recorded for each plan to evaluate plan quality: Paddick conformity index (PCI), gradient index (GI), and BOT. Heat maps for each of the above metrics as a function of LD and BOT penalties were analyzed. An objective score matrix was calculated for each plan using a linear weighted combination of scaled and normalized GI, PCI, and BOT. An optimal solution space containing the 5 lowest values of the score matrix per patient was extracted. A frequency histogram was subsequently created in the LD-BOT coordinate space. <h3>Results</h3> The target volumes in the study ranged from 1.0 cc to 2.5 cc. For every patient, 121 plans were generated across the range of LD and BOT penalties, resulting in 3630 plans included in this analysis. The range of PCI, GI, and BOT across all plans were 0.71 to 0.95, 2.5 to 2.9, and 17.2 to 221 minutes, respectively. The variation of the LD/BOT penalties had a much larger impact resulting in a higher deviation about the mean for BOT (169%), followed by PCI (28%). Heat map analysis revealed that the plans that maximized PCI and minimized GI values occupied the upper triangular matrix of the LD/BOT penalty space, while plans that minimized BOT occupied the lower triangular space. The optimal solutions frequently occupied the diagonal space in between these zones. Specifically, the histogram analysis showed that 4 highest frequencies occurred at the user-inputted settings (LD, BOT) of (0.6, 0.5), (0.8, 0.6), (0.7, 0.5), and (0.7, 0.6), in order of plan quality. <h3>Conclusion</h3> In this study, the user-inputted variables (LD and BOT) were systematically varied to determine the optimal solutions to simultaneously maximize PCI while minimizing GI and BOT. The results of this study may permit SRS planners to generate single brain metastasis plans more efficiently using the study-defined optimal settings of LD and BOT.

Validation of Lexicographic Optimization-Based Planning for Brain Metastases Radiosurgery with Coplanar Arcs

<h3>Purpose/Objective(s)</h3> This study aimed to validate a not yet commercially available fully-automated lexicographic optimization planning for intracranial stereotactic radiosurgery (SRS) with coplanar arcs. <h3>Materials/Methods</h3> Twenty-one single-lesion SRS treatment plans (21 Gy/1 fx) delivered between November 2019 and December 2021 were retrospectively selected and automatically re-planned. An a-priori assigned priority list, a so-called Wish List (WL), was used to define the sequential multi-criterial optimization: four patient sets were necessary to achieve a robust WL. All plans were optimized with 2 coplanar 140°-arcs and calculated with the Monte Carlo algorithm (1 mm-dose grid, 0.5%-statistical uncertainty). The main criteria for planning approval was a brain volume receiving more than 12 Gy less than 10 cm³ (V_12Gy<10 cm³). A target coverage as high as possible was requested, with at least the 80% of the prescription dose covering the 99% of the planning target volume (PTV). Manual plans (MP) and automatic plans (AP) were compared in terms of dose-volume constraints and monitor units (MUs). Statistical significance was assessed performing the Wilcoxon-Mann-Whitney test with Bonferroni correction for multiple tests (alpha=0.05). Plan deliverability was verified by pre-treatment QA and gamma index analysis. <h3>Results</h3> The 21 AP re-planning took only 5 working days. The median D_98% of the gross tumor volume (GTV) and the PTV for MP and AP was 20.4 Gy [18.5-21.6] and 18.1 Gy [16.8-19.9], and 21.1 Gy [19.2-21.7] and 19.1 Gy [17.6-19.4], respectively. These increases were statistically significant (p<0.03). A significant improvement in PTV Paddick's conformity index has been registered: 0.3[0.0-0.6] for MP and 0.5[0.3-0.9] for AP with a p<0.001. The brain V_12Gy was 7.3 cm³ [4.4-12.7] and 7.6 cm³[4.2-13.3] for MP and AP, respectively (p>0.05). Other organs at risk were never significantly interested by clinically relevant doses. These AP results were obtained with a lower median number of MU (-11.6%) even if this difference was not statistically significant and plans registered a comparable gamma analysis (local 2%/2mm). <h3>Conclusion</h3> The novel autoplanning produced high-quality clinically acceptable radiosurgery plans with coplanar arcs, significantly reducing the overall planning time from about one working day for one MP to about 2 hours for one AP. While the OAR sparing was comparable between MP and AP, the target coverage was significantly increased, reducing the MU number and preserving the plan deliverability. The validation proved the capability to generate high-quality deliverable plans according to institutional-specific planning protocols.

Stereotactic radiotherapy of brain metastases: clinical impact of three-dimensional SPACE imaging for 3T-MRI-based treatment planning

PurposeFor planning CyberKnife stereotactic radiosurgery (CK SRS) of brain metastases (BM), it is essential to precisely determine the exact number and location of BM in MRI. Recent MR studies suggest the superiority of contrast-enhanced 3D fast spin echo SPACE (sampling perfection with application-optimized contrast by using different flip angle evolutions) images over 3D gradient echo (GE) T1-weighted MPRAGE (magnetization-prepared rapid gradient echo) images for detecting small BM. The aim of this study is to test the usability of the SPACE sequence for MRI-based radiation treatment planning and its impact on changing treatment.MethodsFor MRI-based radiation treatment planning using 3T MRI in 199 patients with cerebral oligometastases, we compared the detectability of BM in post-gadolinium SPACE images, post-gadolinium MPRAGE images, and post-gadolinium late-phase MPRAGE images.ResultsWhen SPACE images were used for MRI-based radiation treatment planning, 29.8% and 16.9% more BM, respectively, were detected and included in treatment planning than in the post-gadolinium MPRAGE images and the post-gadolinium late-phase MPRAGE images (post-gadolinium MPRAGE imaging: ntotal = 681, mean ± SD 3.4 ± 4.2; post-gadolinium SPACE imaging: ntotal = 884, mean ± SD 4.4 ± 6.0; post-gadolinium late-phase MPRAGE imaging: ntotal = 796, mean ± SD 4.0 ± 5.3; Ppost-gadolinium SPACE imaging versus post-gadolinium MPRAGE imaging < 0.0001, Ppost-gadolinium SPACE imaging versus post-gadolinium late-phase MPRAGE imaging< 0.0001).ConclusionFor 3T MRI-based treatment planning of stereotactic radiosurgery of BM, we recommend the use of post-gadolinium SPACE imaging rather than post-gadolinium MPRAGE imaging.

Application of an in-house developed complementary metal-oxide-semiconductor-based optical computed tomography (CMOS-OCT) imaging system for stereotactic radiosurgery dosimetry using a PRESAGE® dosimeter

PRESAGE® dosimeter can be useful for 3D verification of radiotherapy treatment delivery when careful attention is given to the dosimeter measurement technique. Ideally, a low noise and fast optical CT scanning system is required for imaging the dosimeter for verification of advanced radiotherapy treatment delivery. We have developed a complementary metal-oxide-semiconductor (CMOS)-based optical computed tomography (OCT) system (CMOS-OCT), implementing the recent advances in CMOS active pixel sensor (APS) technology that offer on-chip image processing and high-speed readout. In this study, we present the dosimetric response of irradiated PRESAGE® dosimeter measured with the CMOS-OCT system and describe a feasibility study of using the calibrated system to verify stereotactic radiosurgery (SRS) treatment. PRESAGE® dosimeters were irradiated using a 6 MV Elekta Synergy linear accelerator to investigate dosimetric and geometric properties of PRESAGE® dosimeter with CMOS-OCT imaging system, in order to verify a clinical SRS treatment plan of the brain. The tomographic projection images of the PRESAGE® dosimeter were captured by the image sensor within 100 s for a 360-degree rotation. The CMOS-OCT imaging system showed good linearity in dose-response measured by PRESAGE® dosimeter. The correlation of the optical density (OD) changes measured from the CMOS-OCT with attenuation value measured using a calibrated UV/Vis spectrophotometer was found to be better than 0.996. The tomographic reconstruction of the dose distribution from the PRESAGE® showed good agreement when compared with the calculated dose distributions from the treatment planning software. The in-house CMOS-OCT imaging system is capable of visualising and verifying the dose distribution of stereotactic radiosurgery treatment. The CMOS-OCT imaging of the PRESAGE® dosimeter provides accurate dosimetric and geometric information on radiotherapy delivery in a three-dimensional volume, which is useful in advanced radiotherapy. The introduction of CMOS technology in the OCT improves the imaging speed of the dosimeters dose readout.

Brain metastases: the role of clinical imaging.

Imaging of brain metastases (BMs) has advanced greatly over the past decade. In this review, we discuss the main challenges that BMs pose in clinical practice and describe the role of imaging.Firstly, we describe the increased incidence of BMs of different primary tumours and the rationale for screening. A challenge lies in selecting the right patients for screening: not all cancer patients develop BMs in their disease course.Secondly, we discuss the imaging techniques to detect BMs. A three-dimensional (3D) T1W MRI sequence is the golden standard for BM detection, but additional anatomical (susceptibility weighted imaging, diffusion weighted imaging), functional (perfusion MRI) and metabolic (MR spectroscopy, positron emission tomography) information can help to differentiate BMs from other intracranial aetiologies.Thirdly, we describe the role of imaging before, during and after treatment of BMs. For surgical resection, imaging is used to select surgical patients, but also to assist intraoperatively (neuronavigation, fluorescence-guided surgery, ultrasound). For treatment planning of stereotactic radiosurgery, MRI is combined with CT. For surveillance after both local and systemic therapies, conventional MRI is used. However, advanced imaging is increasingly performed to distinguish true tumour progression from pseudoprogression.FInally, future perspectives are discussed, including radiomics, new biomarkers, new endogenous contrast agents and theranostics.

A Unique Volumetric Modulated Arc Stereotactic Radiosurgery (SRS)Treatment Planning Approach for Volume Staged SRS to Large Arteriovenous Malformation

The management of large Arteriovenous Malformation AVMs (> 10cc) using volume staged Stereotactic Radiosurgery (VS SRS) usually requires more than 2 VS SRS procedures. VS SRS downsizes the treatment volume per stage allowing effective higher dose of SRS to relative smaller volumes of target to reduce risk of SRS induced brain necrosis. It is imperative to maintain the SRS treatment plan quality matrix of conformity and dose gradient at optimal values to minimize the adverse radiation effects to surrounding brain tissues. However, VS SRS plans can present a challenge at treatment planning given the angioarchitecture of staged volumes and total volume, delivery technique and modality limitations.A 37-year-old female patient has history of headaches, intraventricular hemorrhage, and cerebral arteriovenous malformation AVM in the right parietal lobe. This large AVM (16.5cc) scored 4 on Spetzler-Martin grading system. Patient was simulated with large board CT simulator using an SRS mask. 1 mm thickness of CT images with IV contrast per CTA protocol were obtained and imported into the TPS. MRA images data sets and Angio CBCT scans from arteriogram were registered to CT Simulation scans for contouring AMV nidus. The nidus was divided into 3 sub volumes of 8 cc, 4.5cc and 4 cc, respectively. The dose of 20 Gy was prescribed to entire volume of nidus and portions of total dose were delivered to sub volumes with interval of 2-4 weeks. The VMAT inverse treatment plans were generated for each volume/isocenter using non-coplanar arcs. to be delivered on HDMLC. Collimator and arc angles were configured geometrically to minimize dose spillage between staged volumes. Planning objectives including generalized uniform equivalent dose gEUD were assigned to planning volumes and OARs. Normal tissue objective was enabled, structure resolution was set to 1.25mm, and AAA calculation grid was set to 1mm. Individual and composite plans isodoses and DVHs were examined repetitively during the planning and optimization iterations to confirm meeting the acceptable VS SRS plan quality matrix or to adjust if needed. All three individual sub volume plans passed IMRT QA (> 90%, DTA 1mm, %DD 2%) RESULTS: The composite plan maximum dose was 33.3 Gy, minimum dose for total nidus volume was 94%, and 95.2%, 100%, 109% for three sub volumes of nidus, respectively. V95%was > 95% and D95% was > 95% for all three sub volumes and total volume in the composite plan. The mean dose was 27.7 Gy. CI was 0.63, PITV was 1.57, R50 was 4.36 while GI was 2.77, V12 was 52.9cc (3.9%).The proposed unique VMAT VS SRS for large AVM achieves adequate and comparable treatment planning quality parameters when evaluated with published literature covering VS SRS delivery with different treatment modalities such as robotic radiosurgery systems. Technical note: A spiral fluid-attenuated inversion recovery magnetic resonance imaging technique for stereotactic radiosurgery treatment planning for trigeminal neuralgia.

Magnetic resonance imaging (MRI) is commonly used in treatment planning for stereotactic radiosurgery (SRS) of trigeminal neuralgia (TN). With current MRI techniques, the delineation of the trigeminal nerve root entry zone (REZ) may be degraded due to poor contrast and artifacts. The purpose of this work is to develop an MRI technique with better delineation of the trigeminal nerve REZ to improve SRS treatment planning for TN. A spiral fluid-attenuated inversion recovery (FLAIR) MRI technique was developed to improve image quality by improving tissue contrast, fluid suppression, artifact reduction, and signal-to-noise ratio (SNR). A concomitant-phase compensation method based on spiral gradient waveforms was implemented to minimize artifacts due to magnetic field change induced by the metal frame used in Gamma Knife treatment planning. The image quality of spiral FLAIR was assessed in four healthy volunteers. The geometric accuracy was quantitatively evaluated by registering spiral FLAIR to computed tomography (CT) images and comparing it with existing MRI techniques. The spiral FLAIR technique demonstrated better delineation of the trigeminal nerve REZ, improved tissue contrast of the brain stem, and minimized flow artifacts, compared to steady-state free precession (SSFP) MRI. Spiral FLAIR also improved fluid suppression, SNR, and artifacts, which contributed to better delineation of the trigeminal nerve REZ compared to conventional Cartesian FLAIR. The measured mean (± standard deviation) distance between spiral FLAIR and CT images is 0.98 ± 0.56mm, comparable to 0.40 ± 0.26mm in 3T T1 spoiled gradient echo (T1-SPGR), 0.59 ± 0.25mm in 3T SSFP, 0.66 ± 0.38mm in 1.5T T1-SPGR, and 0.61 ± 0.25mm in 1.5T Cartesian FLAIR. A spiral FLAIR technique with improved image quality and good geometric accuracy provides a potential alternative for treatment planning in SRS for TN patients.
