Needs-based selection and prioritization of Technologies to Aid and Assist Nursing Staff in Inpatient Care of Elderly

Abstract Inpatient care facilities globally are facing a critical shortage of staff, posing significant challenges to resident well-being and care quality. This issue is further compounded by demographic shifts and increasing care demands. While technological advancements offer promise in alleviating nursing staff burdens, their effective integration remains complex, with nursing staff acceptance playing a pivotal role. This paper describes a systematic approach designed to streamline the process of identifying, categorizing, and prioritizing suitable technologies in inpatient care settings. By taking into account the specific needs and requirements of nursing staff, this approach, validated through a comprehensive case study, aims to facilitate targeted technology adoption, thereby contributing to the successful digitization of this occupational domain.

Feasibility and outcomes of a trauma-informed model of care in residential treatment for substance use

IntroductionHistory of trauma and PTSD are both common among individuals with substance use disorder (SUD) and associated with poorer SUD treatment retention and outcomes. Recommended trauma-informed care (TIC) approaches in SUD treatment have been underutilized in residential substance use treatment services. MethodsThis study conducted a single-armed, phase 1 feasibility and outcomes trial for a novel TIC model developed and delivered in a young adult (18–35 years) residential substance use treatment service in Australia. Measures of client acceptability, staff acceptability, and treatment fidelity assessed feasibility. The study collected client substance use (global substance use and alcohol, methamphetamine, and cannabis use involvement) and mental health outcomes (depression, anxiety, and PTSD symptoms) at service entry, and 3, 6, and 12 months later. Staff professional quality of life, and perceptions and confidence in delivering TIC were collected at baseline (prior to staff training in TIC), and at 3, 6, 12 and 18 months following training in the model. ResultsThe program was delivered as per the TIC model approximately 88 % of the time, where 48 % of clients completed the full 6-week treatment program. Mixed effect models showed significant reductions in substance involvement, particularly for substance use at 3 months (d = 0.67). Improvements in depression, anxiety, and PTSD were also found (ps < .01). Staff levels of support and self-efficacy to implement TIC remained high over the duration of the evaluation, and trauma knowledge scores were higher at follow-up time points. Staff professional quality of life also remained high across the study. ConclusionThe novel TIC model was acceptable for residential clients and staff and was associated with improved mental and substance use outcomes. These findings support TIC as feasible in residential service, with likely benefits for staff and clients. Optimizing Efficiency: A Comparative Analysis of Project Management Tools with a Business Case Study of Trello Implementation

This paper provides functionality, usability, and suitability for various business environments of several well- known project management software are all thoroughly compared in this study. This review aims to highlight each tool's advantages and disadvantages while providing useful advice for companies looking to streamline their project management processes. The paper includes a business case study that focuses on the use of Trello in a digital marketingcompany in addition to comparative analysis. The company used Trello to improve its project management skills after encountering difficulties with task tracking, communication, and project deadlines. The installation of Trello is described in depth in the paper, from initial setup and customization to staff acceptance and training, including the incorporation of key performance indicators (KPIs) to gauge the tool's effectiveness. Task completion rate, project delivery time, customer happiness, teamwork, and campaign success indicators are among the KPIs considered in this study. The results show that task management and teamwork have significantly improved, leading to better project outcomes. Nevertheless, difficulties including early staff reluctance, a learning curve, and Trello's constrained reporting features were also faced and resolved. Ultimately, this paper addresses the challenges and solutions that come up during the implementation process and shows how choosing the appropriate project management tool may change company processes, increase team productivity, and deliver measurable advantages. SQuID (subcutaneous insulin in diabetic ketoacidosis): Clinician acceptability.

We previously implemented the SQuID protocol (subcutaneous insulin in diabetic ketoacidosis [DKA]) demonstrating safe, effective treatment of low- to moderate-severity DKA in a non-intensive care unit setting. Since success and sustainability of interventions rely on staff buy-in, we assessed acceptability of SQuID among emergency department (ED) and inpatient clinicians. We conducted a cross-sectional study in an urban academic hospital (March 2023-November 2023), surveying ED nurses (RNs) and physicians (MDs) and floor RNs and MDs treating patients on SQuID via emailed survey links. Clinicians could only take the survey once. We used Sekhon's Theoretical Framework of Acceptability, validated for staff acceptability of a new intervention, assessing eight domains with 5-point Likert responses. Clinicians were asked about prior experience with SQuID, and we assessed ED MD and RN preference (SQuID vs. intravenous [IV] insulin). Surveys included free-text boxes for comments. We present descriptive statistics including proportions with 95% confidence interval and medians with interquartile ranges (IQRs) and conducted thematic analysis of free-text comments. Our overall response rate (107/133) was 80% (34/42 ED RNs, 13/16 floor RNs, 47/57 ED MDs, 13/17 floor MDs), with first-time users of SQuID ranging from 7.7% (hospitalist MDs) to 35.3% (ED RNs) of participants. ED clinicians preferred SQuID over IV insulin (67% vs. 12%, 21% no preference). Acceptability was high across all domains and clinician types (median 4, IQR 4-5). Overall percentage of positive responses (4s and 5s) across domains was 92% (ED RNs [89%], floor RNs [89%], ED MDs [97%], floor MDs [87%]). We identified several themes among participant comments. Acceptability was high across clinician types; 65% of ED clinicians preferred SQuID to IV insulin. Clinicians liked SQuID (affective attitude), found it easy to use (burden), were confident in its use (self-efficacy), felt that it improved outcomes (perceived effectiveness), found that it was fair to patients (ethicality), found that it made sense (intervention coherence), and found that it did not interfere with other activities (opportunity cost).

ASSESSING HEALTHCARE PROVIDERS’ ACCEPTANCE OF THE HOSPITAL MANAGEMENT SYSTEM IN A PRIVATE HOSPITAL IN SELANGOR, MALAYSIA

Introduction: In the current technological era, the implementation and use of information and communication technology are crucial for productivity and organizational management in the health sector. This study aimed to investigate factors influencing staff acceptance of the Hospital Management System (HMS) using the Unified Theory of Acceptance and Use of Technology (UTAUT) in a private hospital in Selangor, Malaysia. Methods: Semi-structured interviews were conducted with 25 healthcare professionals, including doctors, nurses, and administrative staff, to explore their experiences and perceptions of the HMS. Data collection was performed through interviews and observations and analyzed using thematic analysis with ATLAS.ti. Results: The study identified five key themes influencing HMS acceptance: performance expectancy, effort expectancy, social influence, facilitating conditions, and behavioral intention. Participants emphasized the importance of system reliability, speed, and user-friendly interfaces for efficient workflow. Peer support and a technology-promoting organizational culture positively influenced system use. Comprehensive training and continuous technical support were crucial for user confidence. Despite recognizing the system's benefits, participants highlighted challenges such as inadequate system integration and the need for simultaneous digital and traditional record-keeping. Conclusion: Addressing these issues and tailoring the HMS to meet specific departmental needs can enhance user satisfaction and system effectiveness. The study underscores the importance of understanding user needs and providing robust support systems to facilitate the successful implementation and adoption of HMS in healthcare settings.

Factors Affecting Employees’ Intention to Work Through the Metaverse Platform

While the benefits of digital transformation are widely acknowledged, the uptake of Metaverse as a new digital workplace remains limited. This study focused on Self-Determination Theory to examine the factors influencing employees’ intention to use the Metaverse platform for their business within medium and large organisations in Malaysia. The analysis of 295 responses noted that competence and relatedness, and improvements in performance and well-being in the Metaverse platform can significantly enhance employees’ attitudes towards the platform while autonomy does not. The study further revealed that digital competence does not significantly moderate the relationship between employees’ attitudes and their behavioural intentions towards using the Metaverse. These results suggest that organisations should focus on fulfilling the psychological needs of their employees to encourage technology adoption. Thus, this study highlights the importance of addressing human factors when aiming to increase staff engagement and acceptance of the innovative digital workplace. By exploring the importance of employees’ intrinsic motivation factors on the adoption of a specific and current digital platform, the model was thus able to address a novel and practical aspect of technology

EVALUATION OF STAFF ACCEPTANCE OF THE USE OF E-PUSKESMAS AT UPT PUSKESMAS GONDANGLEGI IN 2023

Background: This research aims to evaluate staff acceptance of the use of E-Puskesmas at the UPT Puskesmas Gondanglegi. Subjects and Methods: This research uses the Technology Acceptance Model (TAM) method. Based on perceived usefulness and Perceived Ease of Use. The type of research used is descriptive. The data collection technique uses a questionnaire. The subject population of this research was 10 officers who used the E-Puskesmas information system. The sample used is the entire population used as a sample. Analysis of the questionnaire results was carried out using descriptive statistical methods. Results: The percentage results of the questionnaire based on perceived usefulness were 93.4% in the excellent category and Perceived Ease of Use was 62.83% in the sufficient category. Conclusion: There are several obstacles caused by users who are still confused about operating E-Puskesmas, inadequate facilities and infrastructure related to an insufficient number of computers, the workload of officers has increased because recording of patient medical records is done manually and electronically and socialization of the use of E-Puskesmas has already been carried out, but in its operation still not optimal.

Correlates of Staff Acceptability of a Novel Telemedicine-Delivered Medications for Opioid Use Disorder Program in a Rural Detention Center.

Opioid overdose death is significantly increased immediately following incarceration. Evidence-based medications are underutilized in rural jails and detention centers. We have reported our efforts to address this gap through telemedicine-based medications for opioid use disorder treatment (tele-MOUD) for incarcerated patients. Staff acceptance and perceptions are critically important factors in the assurance of program validation. We assessed tele-MOUD acceptability and perceptions of effectiveness and stigma in one detention center. Overall, we found that jail staff's general acceptability of the program was rather low, as was perceived effectiveness of MOUD, while stigmatizing beliefs were present. Furthermore, tele-MOUD acceptability was positively correlated with perceptions of MOUD effectiveness and negatively correlated with stigmatizing notions of MOUD (p's < 0.001). Findings suggest the need for educational interventions. Future research investigating the potential moderating effects of training on staff acceptability of jail-based tele-MOUD will support the implementation and sustainability of these life-saving programs.

Morality in Higher Education's AI Integration: Examining Ethical Stances on Implementation

The research focuses on transparency and education related to artificial intelligence (AI) in higher education, as well as how these factors affect student and academic staff acceptance of technology. The main objective of this study is to explore users' views and attitudes towards the use of AI, with a special emphasis on ethical and practical aspects. This study uses a mixed approach, combining quantitative surveys and qualitative interviews conducted at Superior University, Lahore, Pakistan. Quantitative data were analyzed using descriptive and inferential statistics, while thematic methods analyzed qualitative data. The study results showed that 75.2% of respondents supported education and transparency related to AI. 54.7% of respondents expressed concern that AI could threaten academic integrity if not used transparently and fairly. However, 45.3% of respondents see AI as an effective tool to detect plagiarism and improve academic supervision. The implications of this study underscore the importance of comprehensive education and transparency policies to ensure the ethical and practical use of AI in higher education. This research provides an empirical foundation for developing policies that can increase trust and acceptance of AI in academic contexts.

Virtual robotic telepresence early childhood mental health consultation to childcare centers in the aftermath of COVID-19: training approaches and perceived acceptability and usefulness.

Childcare center closures during COVID-19 impacted education for approximately 40 million children nationwide. Unfortunately, COVID-19 restrictions significantly limited the extent that outside personnel could provide in-person support to educators, resulting in the need for innovative approaches to meet childcare centers' needs. A virtual robotic telepresence approach was applied to early childhood consultation models to promote child resilience while mitigating COVID-19 risks. The goal of this study was to examine how training influenced consultants' and childcare staff uptake of the virtual robotic telepresence consultation approach and their acceptance of this technology. Ten early childhood consultants received multimedia/simulation training and weekly communities of practice related to virtual telepresence robotic consultation. Telepresence robotic consultation equipment was deployed to 16 childcare centers in a diverse multilingual metropolitan area as a part of a larger randomized controlled trial. Consultants trained childcare staff (14 center directors and 58 teachers) on how to receive virtual telepresence robotic consultation. Demographic information and measures of technology acceptability and uptake were collected from childcare staff and consultants. A mixed methods approach was used including multilevel modeling and focus groups to examine consultation uptake, acceptability, barriers, and facilitators of virtual telepresence robotic consultation implementation. Consultants and childcare staff generally perceived the virtual telepresence consultation approach to be useful and easy to use. Consultant perceptions of the acceptability of technology did not change over time. Childcare staff, center, and consultant factors impacted the uptake of the virtual robotic telepresence consultation approach and childcare staff acceptance of the technology. Focus groups revealed that consultants believed that additional hands-on training with childcare staff would have benefited implementation and expressed a desire for a hybrid approach for consultation. Perceptions of telepresence robotic consultation acceptability are discussed, including future recommendations for training.

Pro-Con Debate: Universal Versus Selective Continuous Monitoring of Postoperative Patients.

In this Pro-Con commentary article, we discuss use of continuous physiologic monitoring for clinical deterioration, specifically respiratory depression in the postoperative population. The Pro position advocates for 24/7 continuous surveillance monitoring of all patients starting in the postanesthesia care unit until discharge from the hospital. The strongest arguments for universal monitoring relate to inadequate assessment and algorithms for patient risk. We argue that the need for hospitalization in and of itself is a sufficient predictor of an individual's risk for unexpected respiratory deterioration. In addition, general care units carry the added risk that even the most severe respiratory events will not be recognized in a timely fashion, largely due to higher patient to nurse staffing ratios and limited intermittent vital signs assessments (e.g., every 4 hours). Continuous monitoring configured properly using a "surveillance model" can adequately detect patients' respiratory deterioration while minimizing alarm fatigue and the costs of the surveillance systems. The Con position advocates for a mixed approach of time-limited continuous pulse oximetry monitoring for all patients receiving opioids, with additional remote pulse oximetry monitoring for patients identified as having a high risk of respiratory depression. Alarm fatigue, clinical resource limitations, and cost are the strongest arguments for selective monitoring, which is a more targeted approach. The proponents of the con position acknowledge that postoperative respiratory monitoring is certainly indicated for all patients, but not all patients need the same level of monitoring. The analysis and discussion of each point of view describes who, when, where, and how continuous monitoring should be implemented. Consideration of various system-level factors are addressed, including clinical resource availability, alarm design, system costs, patient and staff acceptance, risk-assessment algorithms, and respiratory event detection. Literature is reviewed, findings are described, and recommendations for design of monitoring systems and implementation of monitoring are described for the pro and con positions.

Effects of Implementing a Barcode Information Management System on Operating Room Staff: Comparative Study.

Barcode information management systems (BIMS) have been implemented in operating rooms to improve the quality of medical care and administrative efficiency. Previous research has demonstrated that the Agile development model is extensively used in the development and management of information systems. However, the effect of information systems on staff acceptance has not been examined within the context of clinical medical information management systems. This study aimed to explore the effects and acceptance of implementing a BIMS in comparison to the original information system (OIS) among operating and supply room staff. This study was a comparative cohort design. A total of 80 staff members from the operating and supply rooms of a Northern Taiwan medical center were recruited. Data collection, conducted from January 2020 to August 2020 using a mobile-based structured questionnaire, included participant characteristics and the Information Management System Scale. SPSS (version 20.0, IBM Corp) for Windows (Microsoft Corporation) was used for data analysis. Descriptive statistics included mean, SD, frequency, and percentage. Differences between groups were analyzed using the Mann-Whitney U test and Kruskal-Wallis test, with a P value <.05 considered statistically significant. The results indicated that the BIMS generally achieved higher scores in key elements of system success, system quality, information quality, perceived system use, perceived ease of use, perceived usefulness, and overall quality score; none of these differences were statistically significant (P>.05), with the system quality subscale being closest to significance (P=.06). Nurses showed significantly better perceived system use than technicians (1.58, SD 4.78 vs -1.19, SD 6.24; P=.02). Significant differences in perceived usefulness were found based on educational level (P=.04) and experience with OIS (P=.03), with junior college-educated nurses and those with over 6 years of OIS experience reporting the highest perceived usefulness. The study demonstrates that using the Agile development model for BIMS is advantageous for clinical environments. The high acceptance among operating room staff underscores its practicality and broader adoption potential. It advocates for continued exploration of technology-driven solutions to enhance health care delivery and optimize clinical workflows.

Understanding paramedic work in general practice in the UK: a rapid realist synthesis

BackgroundGeneral practice in the UK is under substantial pressure and practices are increasingly including paramedics as part of their workforce. Little is known about how different models of paramedic working may affect successful implementation of the role, as viewed from patient, clinician and system perspectives. This realist synthesis developed theories about ‘models of paramedic working in general practice’ in different UK contexts to understand their impact.MethodsThe rapid realist synthesis comprised data from: (1) empirical and grey literature searches; (2) semi-structured realist interviews with system leaders involved with the implementation of the role; and (3) a stakeholder event with healthcare professionals and the public, to develop initial programme theories that can be tested in future work. Sources were analysed using a realist approach that explored the data for novel or causal insights to generate initial programme theories.ResultsEmpirical sources (n = 32), grey sources (n = 95), transcripts from system leader interviews (n = 7) and audio summaries from the stakeholder event (n = 22 participants) were synthesised into a single narrative document. The findings confirmed the presence of a wide variety of models of paramedic working in UK general practice. The perceived success of models was influenced by the extent to which the paramedic service was mature and embedded in practice, and according to four theory areas: (1) Primary care staff understanding and acceptance of the paramedic role; (2) Paramedic induction process, including access to training, supervision and development opportunities; (3) Patient understanding and acceptance of the role; (4) Variations in paramedic employment models.ConclusionsVariability in how the paramedic role is operating and embedding into general practice across the UK affects the success of the role. These findings provide a theoretical foundation for future research to investigate various ‘models of paramedic working’ in different contexts.

Enhancing radiation safety and efficiency in nuclear medicine through automated drug dispensing systems: A feasibility study with staff acceptance analysis

Nuclear medicine, a vital component of modern healthcare, harnesses the power of radioactive isotopes to enable precise diagnoses and treatments. However, the handling of radioactive materials poses inherent risks. This study introduces an automated drug dispensing system (ADDS) within a nuclear medicine department to enhance safety and efficiency. The research comprehensively evaluates ADDS implementation’s financial and operational feasibility, considering staff perceptions and acceptance. Findings reveal that RESCUEDOSE, an ADDS solution, offers a cost-effective, safety-enhancing option with rapid investment returns. Staff members exhibit favorable acceptance, contingent on adequate training. This study contributes to the broader trend of automation in healthcare, emphasizing its potential to elevate patient care and radiation safety.

The impact of the COVID-19 pandemic on primary health care practices and patient management in the Republic of Moldova - results from the PRICOV-19 survey

BackgroundThe COVID-19 pandemic has had an enormous impact on health systems in Europe and has generated unprecedented challenges for tertiary care. Less is known about the effects on the activities of local family doctors (FDs), who have shifted tasks and adapted their practice to accommodate the new services brought by the pandemic. The PRICOV-19 study was a multi-country survey aiming to understand the challenges posed by the pandemic in primary health care (PHC) practices around Europe. Within the framework of this study, we assessed the impact of the pandemic on PHC facilities in urban, rural, and mixed urban/rural areas in the Republic of Moldova.MethodsWe present the results from the PRICOV-19 questionnaire designed at Ghent University (Belgium) and distributed between January and March 2021 to PHC facilities from the 35 districts of the Republic of Moldova. This analysis presents descriptive data on limitations to service delivery, staff role changes, implementation and acceptance of COVID-19 guidelines, and incidents reported on staff and patient safety during the pandemic.ResultsResults highlighted the differences between facilities located in urban, rural, and mixed areas in several dimensions of PHC. Nearly half of the surveyed facilities experienced limitations in the building or infrastructure when delivering services during the pandemic. 95% of respondents reported an increase in time spent giving information to patients by phone, and 88% reported an increase in responsibilities. Few practices reported errors in clinical assessments, though a slightly higher number of incidents were reported in urban areas. Half of the respondents reported difficulties delivering routine care to patients with chronic conditions and a delay in treatment-seeking.ConclusionsDuring the pandemic, the workload of PHC staff saw a significant increase, and practices met important structural and organizational limitations. Consequently, these limitations may have also affected care delivery for vulnerable patients with chronic conditions. Adjustments and bottlenecks need to be addressed, considering the different needs of PHC facilities in urban, rural, and mixed areas.
