Clinicopathological characteristics and immune microenvironment of breast squamous cell carcinoma

Objective: To investigate the clinicopathological characteristics of breast squamous cell carcinoma and to analyze the relationship between its immune microenvironment tumor infiltrating lymphocytes (TILs) and prognosis. Methods: Forty-four cases of primary squamous cell carcinoma of the breast diagnosed and treated in the First Affiliated Hospital of Air Force Medical University, Xi'an, China from January 2006 to July 2022 were selected. Their clinicopathological characteristics were analyzed. The cell composition of TILs was evaluated using immunohistochemistry (Mainly markers of B lymphocytes, T lymphocytes and plasma cells). The relationship between TILs and prognosis was also analyzed. Results: The 44 patients of breast squamous cell carcinoma were all female and all were invasive carcinoma. Eight cases (8/44, 18.2%) were squamous cell carcinoma, while 36 cases (36/44, 81.8%) were mixed squamous cell carcinoma. The mixed components included non-specific carcinoma and spindle cell metaplastic carcinoma (17 cases each). One case contained ductal carcinoma in situ of the breast and 1 case contained tubular carcinoma. The proportion of squamous cell carcinoma was 10% to 90%. The cases with pure squamous cell carcinoma often had a large cystic cavity, which was lined by atypical squamous epithelium, while infiltrating squamous cell carcinoma nests were seen in the breast tissue around the cystic cavity. Immunohistochemical staining showed that p63 and CK5/6 were expressed in the squamous cell carcinoma component, but ER, PR and HER2 were not, except for one case of HER2 1+. The positive rates of TRPS1 and PDL-1 were 76% and less than 1%, respectively. Fifteen cases were in the high TILs group (TILs≥30%) and 29 cases were in the low TILs group (TILs<30%). Twenty-three patients were followed up for 5 to 118 months. Among them, 12 died within 3 years and 9 were alive at the end of the follow up. There was no significant difference in TNM stage, TILs and prognosis between simple squamous cell carcinoma and mixed squamous cell carcinoma. Conclusions: Breast squamous cell carcinoma can be divided into simple squamous cell carcinoma and mixed squamous cell carcinoma. There are differences in gross findings and histology between the simple and mixed squamous cell carcinoma of the breast. Sufficient samples should be taken to avoid missing the diagnosis of a minor squamous component. The prognosis of patients with high TILs is significantly better than that of patients with low TILs. The expression rate of TRPS1 in primary squamous cell carcinoma of breast is high and helpful to the differential diagnosis from metastatic squamous cell carcinoma.

Metaplastic Squamous Cell Carcinoma - A Very Rare Entity of Breast Carcinoma in an Elderly Female Patient

Metaplastic squamous cell carcinoma of breast is a rare malignancy and accounts to &lt; 1% of all breast cancers and it is most commonly seen in elderly women. As the tumor cells exhibit heterogeneous behavior, a standard therapeutic approach is difficult to establish. Age, size of the tumor, lymph node metastasis and receptor status are the important prognostic factors. Metaplastic squamous cell carcinoma has very unfavorable prognosis compared to other triple negative breast malignancies. Surgery combined with neoadjuvant radiotherapy can ameliorate the disease outcome. Early diagnosis and treatment play a crucial role in the management. Here we report a case of metaplastic squamous cell carcinoma of breast in a 67 years female who came with breast lump in the left breast for 6 months with palpable axillary lymph nodes. She underwent modified radical mastectomy with the diagnosis of Ductal cell carcinoma reported on fine needle aspiration cytology. Lymph node clearance done at all levels and the specimen was sent for histopathological examination. Gross examination revealed unifocal gray white irregular tumor measuring 9 x 4 x 3.6 cms located in the inner central quadrant and container labelled as level 1 revealed 7x 4 x 3 cms firm mass. Microscopic examination revealed infiltrating tumor tissue in nesting pattern, syncytial pattern and solid pattern. The tumor cells were having pleomorphic nuclei, increased N:C ratio and moderate to scant eosinophilic cytoplasm. Within the nesting pattern of tumor tissue keratin pearls were seen. Interstitium showed lymphocytic infiltration. One lymph node from level 1 showed metastatic deposits. The tumor was diagnosed as metaplastic Squamous cell carcinoma of left breast with TNM staging of PT3 N1 Mx. Keywords: Squamous, metaplastic, breast, poor prognosis.

Prognosis of squamous cell carcinoma of the breast by biological subtypes and the efficacy of radiotherapy: The Japanese Breast Cancer Registry analysis.

561 Background: Primary squamous cell carcinoma (SCC) of the breast is a rare disease, accounting for about 0.1% of all breast cancers. We aimed to determine the clinical features and prognosis of breast SCC, and the effect of radiotherapy by biological subtypes using the Japanese Breast Cancer Registry. Methods: We conducted a matched-pair analysis of 350,977 breast cancer patients with SCC or invasive ductal cancer (IDC) in the Japanese Breast Cancer Registry between 2004 and 2014. Inclusion criteria were female breast cancer patients who underwent surgery with stage I–III diseases, SCC or IDC, and age ≥ 18. Patients who received preoperative radiotherapy were excluded. We performed 1:1 SCC-IDC exact matching analysis by predominant subtypes in SCC: ER(-) and PgR(-)/HER2(-) or ER(+) and/or PgR(+)/HER2(-). Overall survival (OS), breast cancer-specific survival (BCSS), recurrence-free survival (RFS), and first locoregional or distant metastasis recurrence incidence were compared between IDC and SCC by Kaplan-Meier methods and log-rank test. In-field area recurrence was compared between those who received radiotherapy and those who did not. Results: We included 452 SCC and 182,707 IDC patients for the analysis. SCC patients were more likely to present with advanced stage disease compared to those with IDC. Among SCC patients, 59% was ER(-)/PgR(-)/HER2(-), 19% was ER(+)/PgR(+)/HER2(-), 8% was ER(-)/PgR(-)/HER2(+), 2% was ER(+)/PgR(+)/HER2(+), and 12% was data missing. Ten-year OS, BCS, and RFS rates were 70%, 80%, and 66% in SCC, and 88%, 93%, and 81% in IDC, respectively, and SCC carried a significantly worse prognosis than IDC (all P &lt; 0.001). For exact-matched analysis, 10-year BCS of ER(-)/PgR(-)/HER2(-) SCC patients was significantly worse than IDC (P = 0.02), whereas there were no significant differences between SCC and IDC of ER(+)/PgR(+)/HER2(-) patients in OS, BCS, and RFS. Among patients who received radiotherapy, no difference was found in in-field area recurrence between SCC and IDC in either ER(-)/PgR(-)/HER2(-) or ER(+)/PgR(+)/HER2(-) subtype. Conclusions: SCC of the breast was often diagnosed in an advanced stage and had a worse prognosis than IDC. More than half of them were triple-negative subtype. In the triple-negative subtype, SCC was an independent poor prognostic factor. No significant differences in in-field area recurrence after radiotherapy between SCC and IDC suggested that the effect of radiotherapy on local control for SCC was similar to IDC.

Squamous cell carcinoma of the breast: A case report and review of the literature

Breast squamous cell carcinoma (SCC) is a rare form of breast cancer and is highly malignant; diagnosis is based on histological examination after eliminating the primary tumor; clinical and radiological examination is not specific; treatment is the same as for invasive ductal carcinoma; prognosis is poor due to tumor size and lymph node involvement. We report a case of breast SCC in a 53-year-old patient who was examined in the obstetrics and gynecology department of the hospital in Casablanca. Through this case, we raise the main characteristics of this rare type of breast cancer. As a conclusion, the etiology and pathogenesis of SCC of the breast is still unclear.

Preoperative MRI of breast squamous cell carcinoma: diagnostic value of distinguishing between two subtypes

To retrospectively analyse the clinical and MRI data of primary squamous cell carcinoma (SCC), particularly pure squamous cell carcinoma (PSCC) and mixed squamous cell carcinoma (MSCC). The MRI data and clinicopathological characteristics of 20 patients with histopathologically confirmed SCC of the breast, including eight PSCC patients and 12 MSCC patients, from multiple centres between January 2013 and December 2020 were analysed retrospectively. Nine of 12 patients in the MSCC group showed hyperintensity on T1-weighted imaging (WI), while this feature was not observed in the PSCC group (p=0.001). Most of the PSCC group showed rim enhancement, whereas most of the MSCC group showed heterogeneous enhancement (p=0.007). In addition, there was no significant difference in the thickness of the rim enhancement and the percentage of necrotic components in the tumours between the two types of SCCs of the breast (p=0.545 and p=0.662, respectively). Four patients (4/12) in the MSCC group had sentinel lymph node metastasis, while only one patient (1/8) in the PSCC group showed lymph node metastasis (p=0.603). Metastatic disease occurred in 25% of patients with PSCC and in approximately 41.7% of patients with MSCC. The signal on T1WI and internal enhancement characteristics were the key features for differentiating PSCC and MSCC. Therefore, MRI phenotypes may provide additional information for the pathological classification of breast SCC.

A case of breast squamous cell carcinoma following breast augmentation with liquid silicone injection after 16\xa0years

BackgroundBreast augmentation has been linked to various complications, including cancerous tumors. The majority type of breast cancer associated with breast augmentation is adenocarcinoma. Primary squamous cell carcinoma (SCC) of the breast is extremely rare in both augmented and non-augmented women. Due to the low incidence, the possible origin and the mechanism of carcinogenesis of the breast SCC are not well understood. Here, we report a rare case of pure SCC 16 years after breast augmentation with liquid silicone injection.Case presentationA 51-year-old Japanese woman was suffered from prolonged breast fluid retention in her left breast. Multiple unknown foreign bodies caused difficulties to investigate the inflammatory focus with ultrasonography. After unsuccessful surgical drainage and antibiotics treatments, the long-standing fluid retention was surgically removed and pathologically investigated. SCC was found in the removed tissue, and the patient underwent a total left mastectomy followed by postoperative chemotherapy. Pathological analysis revealed multiple cystic structures with a hard shell which enclosed high viscous liquid. A qualitative analysis using a Fourier transform infrared spectroscope defined the liquid as pure silicon, which possibly caused the squamous cell carcinogenesis.ConclusionsAlthough liquid silicone injection is not a current option for breast augmentation, the injected silicone could result in cancerous tumor generation after years. This case revealed that unphysiological substances could lead to unexpected biological reactions, which caused difficulties in diagnosis with our routine examination. It will be required that accumulate information from more cases and develop novel diagnostic equipment and biomarkers to address these artificial substance-derived tumors.

Primary Squamous Cell Carcinoma of the Breast with Metastasis-A Rare Case Report

Primary squamous cell carcinoma of breast is extremely rare invasive breast carcinoma with rapid progression with incidence of 0.1% and worse prognosis. Careful assessment and diagnosis of the entity should be considered in a rapidly progressing breast tumor. We hereby reported a case of primary carcinoma breast in 80-year-old female who was managed at tertiary care centre of Rajindra Hospital, Patiala.

Squamous cell breast cancer: description of a rare clinical case

Squamous cell carcinoma of the breast is a rare, aggressive tumor with a poor prognosis. To date, due to the low incidence of morbidity, there is no single concept in the complex treatment of this pathology. The article describes the case of a 49-year-old patient after surgical treatment of squamous cell carcinoma of the left breast from 2019 on the anamnesis. A year later after the progression of the disease the patient underwent removal of a chest wall tumor with resection of IIIV ribs, allo- and autoplasty, axillary lymphadenectomy on the left. In most cases, the treatment of squamous cell carcinoma of the breast is surgery, but the role of adjuvant chemotherapy, radiation therapy and endocrinotherapy are still unambiguous.

Primary Squamous Cell Carcinoma of the Breast: A Rare Case Report and Management Decisions

Background Squamous cells are normally not found inside the breast. Therefore, a primary squamous cell carcinoma of breast is an exceptional phenomenon and the management of this type of disease is still debated. The prognosis is poor, and hence there is a clinical trend to offer multimodal treatment options of surgery, chemotherapy and radiation therapy. Aim Clinical outcome assessment of patient with squamous cell carcinoma of the breast and management decisions. Materials and methods: we report case of primary squamous cell carcinoma of the breast T2N0Mx in a 78-years-old woman who underwent Modified Radical Mastectomy (MRM) plus adjuvant chemotherapy and hormonal therapy without radiation therapy and review literature for appropriate management decisions. Results With a follow up of 24 months, the patient is alive with no of local or distant recurrence. Conclusion Pure primary cell carcinoma of the breast is rare and aggressive disease, often treatment-refractory, our case show that the hormonal therapy after surgery and chemotherapy, allows to achieve a high local control. Multidisciplinary approach seems to be the optimal management for early stages in this rare disease.

Clinical outcome of adjuvant radiotherapy for squamous cell carcinoma of the breast; a multicenter retrospective cohort study

BackgroundBecause primary squamous cell carcinoma (SCC) of the breast is a rare disease, the standard therapy has not been established. We examined the clinical outcomes of postoperative adjuvant radiotherapy for breast SCC. Material and methodsWe conducted a multicenter retrospective cohort study. Patients diagnosed with primary breast SCC who received adjuvant radiotherapy as part of their primary definitive treatment were included. Overall survival (OS), breast cancer-specific survival (BCSS), and recurrence-free interval (RFi) were evaluated. ResultsBetween January 2002 and December 2017, 25 breast SCC patients received adjuvant radiotherapy as a primary treatment were included. Median follow-up time was 43.5 months. Three (12%), fifteen (60%) and seven (28%) patients had clinical stage I, II and III disease, respectively. Fourteen patients underwent breast-conserving surgery and subsequent adjuvant radiotherapy. Eleven patients underwent mastectomy and post-mastectomy radiotherapy. Ten patients received regional lymph node irradiation. Nine (36%) patients had disease recurrence. The first site of recurrence was locoregional in five, but distant metastasis arose in one. Concurrent local and distant metastasis were seen in two. Six cases of local recurrence occurred within the irradiated site. Seven patients died, and six of the deaths were due to breast cancer. Five-year OS, BCSS, and Rfi were 69%, 70%, and 63%, respectively. In multivariate analysis, age and lymphatic invasion were associated with increased risk of recurrence. ConclusionBreast SCC has a high incidence of locoregional recurrence and poor prognosis. Age and lymphatic invasion are significant risk factors for recurrence.

SOX10 - A Novel Marker for the Differential Diagnosis of Breast Metaplastic Squamous Cell Carcinoma.

IntroductionDifferential diagnosis of metaplastic squamous cell carcinoma of breast (MSCCB) is difficult. In particular, in terms of metastatic MSCCB, because of the low speciality of traditional markers such as mammaglobin, gross cystic disease fluid protein-15 (GCDFP-15) and GATA binding protein 3 (GATA3), the most common problem is differentiating the spread of MSCCB to the lung from a primary lung squamous cell carcinoma. It is urgently required to explore a novel marker to aid in differential diagnosis.AimThe aim of this study is to explore a novel marker to aid in the differential diagnosis of MSCCB from other squamous cell carcinomas (SCC) in other organs.MethodsWe tested the expression of SOX10 in 375 human SCC specimens with immunohistochemistry (IHC).ResultsIn a series of 20 MSCCB, 9 (45%) were positive for SOX10. All of them were triple-negative MSCCB. Conversely, SOX10 was totally negative in another 205 SCC originating from lung, skin, cervix, oral mucosa, and esophagus. In a series of 150 triple-negative breast cancer and their metastatic foci, SOX10 labeling in the primary tumor and metastasis was 78% and 79.3%, respectively, and the agreement rate was 97.3% (P>0.05).ConclusionOur findings demonstrate that SOX10 was recommended for differentiating MSCCB from non-mammary metastasis to the breast, as well as for distinguishing primary SCC from metastatic MSCCB, and SOX10 may be valuable in the pathological diagnosis of breast-derived metaplastic squamous cell carcinoma.

Squamous Cell Carcinoma: A Stranger in The Breast

In routine clinical practice, invasive ductal carcinoma of breast is commonly encountered. However strangers do knock the door sometimes and squamous cell carcinoma of breast is one such entity. Although clinical and radiographic features are not specific to this tumour it follows an atypical presentation in terms of tumour size, lymphnode and systemic spread. Usually Squamous cell carcinomas of the breast are triple negative tumours with aggressive behavior and are resistant to standard chemotherapeutic regimens.

Squamous cell carcinoma of the breast, are there two entities with distinct prognosis? A series of 39 patients.

Squamous cell carcinoma (SCC) of the breast is a rare entity of breast cancer, with a very poor prognosis, and whose pathophysiology is still unwell established. Therapeutic management is very heterogeneous due to its incomplete understanding. Nevertheless, it seems that two histological entities can be distinguished: pure SCC close to the cutaneous origin, and metaplastic squamous breast cancer (MSBC). The aim of this study is therefore to assess the difference in survival according to the histological type (SCC or MSBC) and to describe the demographic, clinical, and therapeutic characteristics of the two underlying populations. Our data came from a monocentric retrospective series of 39 patients treated between 1985 and 2018 at the Gustave Roussy Institute (France) for a breast SCC. Of the 39 patients included, 64% had MSBC and 36% had a pure form. The overall and recurrence-free survival at 3years [CI 95%] was 72.3% [56.9%; 87.0%] and 67.2% [51.2%; 83.2%], respectively. The overall 3-year survival of patients with MSBC was significantly lower than that with pure SCC: HR [CI 95%] 9.5 [1.2; 73.1], p = 0.008. The 3-year recurrence-free survival of patients with MSBC was also poorer: HR [CI 95%] 11.9 [1.6; 90.7], p = 0.002. Patients with MSBC also tended to be younger, have a large lesion size, and be more metastatic. The histological nature of SCC seems to bring fundamental new elements to the therapeutic management as it impacts recurrence and survival. It should therefore be better characterized at diagnosis in order to possibly adapt treatments.

The variables obtained by univariate analysis were introduced into the Cox proportional hazard model for multivariate analysis. The risk factors affecting the prognosis of BSqCC were obtained. P Results: A total of 739 patients with BSqCC was included. The median age of diagnosis was 66 years. In most cases, the expression of estrogen receptor (ER), progesterone receptor (PR), and human epidermal growth factor receptor 2 (HER2) was negative. One-third of the cases underwent breast-conserving surgery, and more than half of the cases underwent mastectomy (unilateral or bilateral). The 1-year survival rate was 81.2%, the 3-year survival rate was 62.9%, the 5-year survival rate was 54.4%, and the 10-year survival rate was 41.4%. At the same time, we developed a prognostic nomogram with excellent discrimination for breast cancer squamous cell carcinoma; therefore, it could help clinicians make decisions on a personal basis.

Among them, marital status (HR: 0.502, 95% CI: 0.318–0.794), age (HR: 2.186, 95% CI: 1.234–3.875), surgical procedure (HR: 1.03, 95% CI: 1.01–1.051), tumor size (HR: 1.505, 95% CI: 1.083–2.091) and the number of positive lymph nodes (HR: 1.277, 95% CI: 1.087–1.499) were independent risk factors for the survival of BSqCC. Five independent prognostic factors were then integrated for the construction of nomograms.ConclusionsBSqCC was a malignant tumor with a low survival rate. Higher efficacy and complete response with administration of eribulin for recurrent squamous cell breast carcinoma: A case report.

Squamous cell carcinoma is a rare histological type of breast cancer classified as metaplastic carcinoma. Metaplastic carcinoma involves differentiation of the breast glandular duct cells into mesenchymal tissues. While the chemotherapy regimen for metaplastic carcinoma is often similar to that for invasive ductal carcinoma, recurrence is associated with a poor prognosis due to the inadequate therapeutic efficacy of the regimen. We herein present the case of a 42-year-old female patient who underwent mastectomy of the right breast and right axillary node lymphadenectomy for T4N3aM0 breast cancer stage. The cancer was histopathologically diagnosed as squamous cell carcinoma of the breast. Adjuvant cyclophosphamide-epirubicin-fluorouracil (CEF) postoperative chemotherapy was administered, and lymphadenectomy of right-sided parasternal lymphatic metastases with pleural drainage was subsequently performed. Radiotherapy was administered to the thoracic wall and supraclavicular lymph nodes at 60 Gy. Positron emission tomography (PET)-computed tomography (CT) examination 3 months after the radiotherapy identified accumulation of fluorodeoxyglucose (FDG) in a supraclavicular lymph node and the thoracic wall; hence, a chemotherapeutic regimen with eribulin was initiated. At 11 months after initiation of eribulin, complete response was achieved, indicated by the absence of FDG accumulation in both the supraclavicular lymph node and the thoracic wall on PET-CT. The treatment efficacy of eribulin is considered to be a result of the mixed morphology of squamous cell carcinoma, including the presence of an epithelial component, such as adenocarcinoma cells, and a mesenchymal component, in the form of sarcomatoid cells. Eribulin displayed an effect similar to that of adriamycin against malignant soft tissue tumors and was shown to effectively target mesenchymal components. In cases of reduced expression of the DNA repair pathway components, such as in metaplastic carcinomas, eribulin may be more effective compared with adriamycin, the mechanism of action of which involves inhibition of DNA synthesis. A superior therapeutic effect was obtained with eribulin in squamous cell carcinoma. Therefore, eribulin appears to be a promising, effective therapeutic choice for the management of metaplastic carcinomas, including squamous cell carcinomas.

Primary Squamous Cell Carcinoma of Breast in a Young Female: An Institutional Experience with Review of Literature

Invasive or infiltrating ductal carcinoma is the most common type of breast cancer while primary squamous cell carcinoma of breast is extremely rare occurring in 0.04%-0.1% of all breast cancer cases known for aggressive behaviour and poor prognosis. We hereby report a case of primary squamous cell carcinoma of breast in a 31-yearold female who presented with a left breast lump of 2 months duration. Lumpectomy done at another non-oncology centre showed invasive ductal carcinoma with margin positivity for which she underwent modified radical mastectomy at our institute. Post-operative immune-histopathology revealed triple negative squamous cell carcinoma. Her metastatic work-up with whole-body positron emission tomography scan was negative. She was treated with adjuvant chemotherapy doxorubicin, cyclophosphamide and docetaxel followed by loco-regional radiotherapy to left chest wall which she tolerated well. No hormonal therapy was given in view of hormone receptor negativity. She is presently on follow-up for more than 6 months without any evidence of recurrence or distant metastasis. This case is being presented to highlight its extreme rarity, it’s occurrence in a young female, the diagnostic and therapeutic challenges it presented and the overall prognosis of this neoplasm.
