Sarcomatoid Squamous Cell Carcinoma of Alveolus: A Rare Case Presentation

To describe a rare case of sarcomatoid squamous cell carcinoma of alveolus. A 53-year-old female with complaints of non-healing ulcerative growth over the left side of the hard palate, which was associated with pain for two months. She underwent extraction and a Histo-pathological examination of teeth with attached soft tissue was suggestive of Sarcomatoid Carcinoma. The patient underwent Left infrastructure maxillectomy with wide local excision with left modified radical neck dissection type II. Histopathology showed lympho-vascular infiltration and perineural infiltration. All margins were free. Out of 8 lymph nodes that were dissected, 1 showed metastatic disease. The patient received adjuvant concurrent radio-chemotherapy 66Gy in 33 fractions (2Gy/day) in two phases along with weekly Injection Cisplatin 40 mg/m2. Sarcomatoid squamous carcinoma is a rare and uncommon variant of squamous cell carcinoma. It is aggressive in nature and shows early recurrence and metastasis. Thus, associated with a poor prognosis. There is no defined treatment protocol for this variant. We have discussed a case of sarcomatoid carcinoma of the alveolus that was operated and followed by adjuvant chemo-radiotherapy.

Prosthetic management of a Hemi-mandibulectomy patient with Definitive Removable Prosthesis - A case report

The loss of continuity of the lower jaw following surgical excision of tumor results in deviation of remaining mandibular segment toward the resected side. Swallowing, speech, mastication, and mandibular movements are adversely affected by mandibular surgery. Prosthetic rehabilitation plays a major role in these patients, by fabricating the whole array of prostheses to meet specific patient needs. This case report describes about the patient diagnosed with squamous cell carcinoma of alveolus on the left side. The patient had undergone hemimandibulectomy followed by soft tissue reconstruction. As a result, mandible deviated to the affected side with trismus due to scar contraction. Second surgery was planned to remove the scar tissue and to improve mouth opening. Few days following second surgery, guiding flange prosthesis was fabricated to reduce the deviation and to guide the mandible close to occlusion. After three months, a removable cast partial prosthesis was fabricated, for the defect side which was supported by a wing to compensate for lack of underlying structures. It is imperative to provide such patients with guiding flange prosthesis three to four weeks after surgical management to guide mandible to occlusion and minimize the mandibular deviation. Failure to do so will complicate the occlusion. Subsequently patient can be rehabilitated with removable prosthesis to restore the function

Case report of metastatic presence of lingual lymph nodes in carcinoma alveolus of mandible

Lingual node metastasis is seen most commonly in carcinoma of tongue and carcinoma floor of mouth. Literature elicits about such cases, but rarely reported is lingual node metastasis in Squamous cell carcinoma of alveolus. Our case report is an elderly male patient with Squamous cell carcinoma of alveolus. Management included hemi-mandibulectomy because of the extension. Metastatic lingual node was found during the surgery as it is rare finding so is this case report presented to reappraise the literature. Metastatic papillary carcinoma thyroid co-existing with oral cavity squamous cell carcinoma: A case report and review of literature

The incidental discovery of metastatic papillary carcinoma thyroid in lymph node while the patient is being investigated for primary squamous cell carcinoma of the oral cavity is an unusual clinical situation and the appropriate management in such clinical situation is controversial and confusing. We report a case of a 65year old male with primary squamous cell carcinoma of alveolus with bilateral neck nodes. Fine needle aspiration cytology showed metastatic squamous cell carcinoma in lymph nodes on the left side and metastatic papillary carcinoma in the lymph node on the right side. We present the diagnostic dilemma and the confusion in planning the treatment of such difficult situation.

Malignant orofacial neoplasms in Lagos, Nigeria

To analyse general differences in age, gender, anatomic location of the various histological types of malignant neoplasm involving the oral and maxillofacial region at a tertiary referral centre in Nigeria and to compare this with reports in the literature. Retrospective analysis. A tertiary referral centre in Lagos, Nigeria. All patients with a histological diagnosis of malignancy were included provided the final surgical histology reports as well as the biopsy reports were available for review. Squamous cell carcinoma (SCCA) with a peak age incidence in the 6th decade of life and a male to female ratio of 1.65 to 1 was the most common cancer (42.8%). Males with SCCA (mean = 48.7 years) were relatively younger than females (mean = 58.8 years). Eighteen percent of patients with SCCA were below 40 years at presentation and males were predominantly affected at a ratio of 5.2:1. Majority of the SCCA (75%) were well/moderately well differentiated. However, in patients below 40 years, 45.9% had poorly differentiated SCCA. The Most commonly affected sites for SCCA were the maxillary antrum (36.7%), mandibular gingiva/ alveolus (23.0%) and tongue (12.04%). Patients who presented with antral SCCA were relatively younger (mean = 48.9 years) than those who had SCCA involving the mandibular gingiva/ alveolus (mean = 57.1 years) as well as floor of mouth (mean = 56.3 years). Furthermore, males with antral SCCA were younger than their female counterparts. Similarly, males who had mandibular gingiva/ alveolus SCCA (mean = 48.4 years) were younger than their female counterparts (mean = 62.8 years). Majority of the glandular carcinomas (GLDCAs) arose from minor salivary glands (63.0%). Thirty seven per cent of these patients were below 40 years at presentation. Females who had antral GLDCA (mean = 32.4 years) were relatively younger than their male counterparts (mean = 49 years). Most of the patients who presented with sarcoma were below 40 years (77.4%) and males were younger (22. 8 years) than females (mean = 35.4 years). The proportion of orofacial cancers that is SCCA in Nigerians is relatively low because of a relatively high proportion of GLDCA and Burkitt's lymphoma. A relatively high proportion of patients with SCCA were below 40 years at presentation. They were predominantly males who presented with poorly differentiated tumours.
