Sertraline Improves Executive Function in Patients With Vascular Cognitive Impairment

The authors reviewed 35 open-label sertraline trials for executive impairment in ischemic cerebrovascular disease. Outcomes included clock-drawing, the Executive Interview (EXIT25), the Geriatric Depression Scale, and the Mini-Mental State Examination. Clinically "meaningful" improvement was defined as a >3.0 EXIT25 point decline from baseline. "Remission" was defined as the achievement of an EXIT25 score <15/50. Only EXIT25 scores improved significantly. Twenty patients (57.1%) experienced a clinically meaningful improvement in executive control function. Twelve (34.3%) achieved remission. Our findings suggest that sertraline may have both statistical and clinically meaningful effects on executive control function in ischemic cerebrovascular disease. The authors discuss the implications for future clinical trials.

Antipsychotic Medication Utilization Trends Among Texas Veterans: 1997–2002

An antipsychotic utilization pattern has evolved substantially over the past 20 years or so due to the introduction of the second-generation antipsychotics (SGAs) and the increasing understanding of their adverse effect profile. To understand antipsychotic utilization trends (including monotherapy, antipsychotic switching, and combination therapy) and to investigate factors associated with antipsychotic index medication selection (SGAs vs first-generation antipsychotics [FGAs]) among Texas veterans. Data were taken from the Veterans Administration North Texas Health Care System (VANTHCS) and South Texas Veterans Health Care System (STVHCS) from January 1996 to December 2003. Adults with continuous enrollment (1 y before and after the index date) who had newly initiated antipsychotic therapy were included. Prescriptions were followed for up to 12 months. Descriptive analyses examined utilization trends; logistic regression evaluated factors associated with antipsychotic index medication selection. A total of 8096 patients were included in the study (VANTHCS n = 4477; STVHCS n = 3619), with the majority being male (93.6%) and white (62.6%) and nearly half aged 55 years or older (44.1%). Between 1997 and 2002, antipsychotic prescriptions changed from primarily FGAs (1997: 71.7%; 1999: 25.2%; 2002: 5.7%) to SGAs. Over the 6-year time frame, risperidone (31.0%) and olanzapine (30.7%) were most commonly prescribed. The overall combination therapy slightly increased over time (4.3%), switching to another antipsychotic remained stable (14.2%), and antipsychotic monotherapy remained dominant (81.5%). Hispanic and black patients were less likely than white patients to be initiated on SGAs. Patients who were older, had hypertension, and were in STVHCS were less likely to start on SGAs. Patients with dyslipidemia, bipolar disorder, and treatment in recent years were more likely to start on SGAs. SGAs have replaced FGAs as first-line medications for patients with mental disorders. Race, age, physical comorbidities (ie, dyslipidemia, hypertension), and treatment initiation year were important factors in index medication selection.

However, this summary overplayed the strength of the article by Oosterheert and colleagues (2), and we disagree with the recommendations. Oosterheert and colleagues' article (2) was a ground-breaking, randomized, controlled trial that examined 2 strategies of conversion from intravenous to oral antimicrobial therapy in patients hospitalized with community-acquired pneumonia. Unfortunately, this article had several limitations, such as lack of a true cohort of participants with severe pneumonia—the overall 28-day mortality rate was 5% versus the generally accepted rate of 20% to ...

Multidrug-Resistant Gram-Negative Pathogens Multiple Approaches and Measures for Prevention

Despite the predominance of gram-positive healthcare-associated infections in the past 2 decades, gram-negative pathogens continue to be a concern because of the high mortality from these infections among seriously ill patients. Although several alternative antimicrobials active against multidrugresistant gram-positive pathogens are now available, multidrug resistant gram-negative pathogens continue to emerge and new alternative therapies are not available. Preventive measures, then, become all the more important. Pseudomonas aeruginosa, Klebsiella pneumoniae, and Enterobacter species are among the most problematic gram-negative pathogens with emerging resistance. Four articles in this month’s journal address important issues in the prevention of the emergence of resistance in these pathogens. Lautenbach et al. and Fortaleza et al. study risk factors associated with imipenem resistance and ceftazidime resistance 3 in P. aeruginosa. The setting of the study by Fortaleza et al. was a 400-bed general teaching hospital in Campinas, Brazil. Cases in which imipenem-resistant P. aeruginosa strains were recovered (108 patients) and in which ceftazidime-resistant strains were recovered (56 patients) were selected during the years 1999-2002. Case-control studies showed that independent risk factors associated with imipenem resistance in their hospital were transfer from another hospital (perhaps a marker for longer time at risk), receipt of hemodialysis, and use of imipenem, amikacin, or vancomycin. Only transfer from another hospital and amikacin use were independent risk factors for recovery of ceftazidimeresistant P. aeruginosa; ceftazidime use was not. The setting of the study by Lautenbach et al. was a 625bed tertiary care hospital in Philadelphia, Pennsylvania. Cases in which imipenem-resistant P. aeruginosa strains were recovered (142 patients) were identified during the years 19992000. Results of a case-control study showed that fluoroquinolone use, and not imipenem use, was an independent risk factor for recovery of an imipenem-resistant strain. Why was imipenem use a risk factor in one study and not in the other? Both were case-control studies that evaluated imipenem resistance in P. aeruginosa at large teaching hospitals. There was a similar distribution of anatomical sites from which the clinical specimens were isolated and a similar amount of intensive care unit exposure in both studies. Recent studies have discussed the importance of the method used to select control subjects in case-control studies of antibiotic resistance. In the study by Lautenbach et al., the question being asked was “Among clinical isolates of P. aeruginosa, what are the risk factors associated with imipenem resistance in a given isolate?” Thus, patients colonized or infected with imipenem-resistant P. aeruginosa were classified as case patients and patients colonized or infected with imipenem-susceptible P. aeruginosa were classified as control subjects. The study by Fortaleza et al. was designed to evaluate risk factors associated with the recovery of imipenemresistant and ceftazidime-resistant P. aeruginosa isolates from patients admitted to the authors’ hospital. Case patients were those from whom imipenem-resistant or ceftazidime-resistant strains of P. aeruginosa were recovered, and control subjects matched to case patients were selected from the patients admitted to the same ward. The questions asked were different, and thus the procedures for selection of control subjects were different. Harris et al. have reviewed the methodologies for casecontrol studies of antibiotic resistance that may lead to varying results with respect to risk factors. Both Lautenbach et al. and Fortaleza et al. acknowledged the impact of methodology and chose control subjects on the basis of their intended question. The novel case-case-control study design described by Kaye et al. addresses some limitations in study design and has been used to study risk factors for colonization or infection with imipenem-resistant P. aeruginosa among hospitalized patients. Findings of the study by Kaye et al. showed that receipt of imipenem, piperacillin-tazobactam, vancomycin, and/or aminoglycosides were risk factors for isolation of imipenem-resistant P. aeruginosa.
