Expert Clinical Decision Support Systems to Enhance Antimicrobial Stewardship Programs: Insights from the Society of Infectious Diseases Pharmacists

Health care-associated infections (HAIs) are a leading cause of in-hospital mortality and adverse events such as antimicrobial resistance. These infections place tremendous burdens on the health care system and create situations for misuse of antimicrobial drugs. Recognition of these factors has led professional societies, clinicians, and hospitals to develop programs to improve the management of HAIs and the use of antimicrobial drugs. The clinical literature is replete with examples of these programs, often referred to as antimicrobial stewardship. Traditionally, antimicrobial stewardship programs have relied on manual methods combined with clinical oversight and intervention. The advent of modern health care information technology offers the opportunity to expand the breadth and depth of these programs. Expert clinical decision support systems are the most promising of these information technology advances.

Gram-Positive Resistance: Pathogens, Implications, and Treatment Options: Insights from the Society of Infectious Diseases Pharmacists

Despite the advent of new antibiotics, resistance in gram-positive pathogens, including staphylococci and enterococci, continues to increase. This is evident with the recent emergence of vancomycin-resistant Staphylococcus aureus . Newer treatment agents are available, including quinupristin-dalfopristin, linezolid, and daptomycin. In addition, investigational agents are being explored. Clinical trials have been conducted for various infections, such as skin and skin structure infections, pneumonia, and bloodstream infections. Antibacterial activity, site of infection, and potential for adverse effects must be taken into account when making decisions regarding therapy.

Current Issues in the Development of a Vaccine to Prevent Human Immunodeficiency Virus: Insights from the Society of Infectious Diseases Pharmacists

Infection with the human immunodeficiency virus (HIV) continues to affect millions of people worldwide. Preventive measures have done little to slow the transmission of the virus. Discovery of an effective vaccine to prevent HIV could have a tremendous impact on this global pandemic. However, the complex interactions between HIV and the host immune system have limited the progress in vaccine development. Traditional vaccination strategies have shown little promise. Currently, subunit vaccines, DNA vaccines, recombinant vector vaccines, and prime-boost strategies are being evaluated in clinical trials. Although many breakthroughs have been made in HIV vaccine research, only three candidate HIV vaccines have been studied in phase III clinical trials. The current strategies being investigated in the development of preventive HIV vaccines are reviewed.

Intravenous Ciprofloxacin: A Position Statement by the Society of Infectious Diseases Pharmacists

Intravenous ciprofloxacin is approved for the treatment of urinary tract, bone and joint, skin and soft tissue, and lower respiratory tract infections. Few large, randomized studies comparing its effectiveness with that of other available agents exist; most of these have been published in non-peer-reviewed journal supplements. Intravenous ciprofloxacin is nearly ten times more expensive than the "equivalent" oral dose. Based on the limitations of currently available clinical data, iv ciprofloxacin does not appear to be superior, and is at best comparable in efficacy to other currently available antibiotics. Bacterial resistance, especially in serious infections secondary to P. aeruginosa and S. aureus, is becoming more prevalent. Intravenous ciprofloxacin should be considered an alternative for the treatment of infections of the urinary and lower respiratory tracts only when the following conditions exist: (1) documented bacterial resistance to less-costly regimens with proven efficacy for these indications (e.g., beta-lactams, aminoglycosides, trimethoprim/sulfamethoxazole) and known susceptibility to ciprofloxacin, (2) documented hypersensitivity to first-line agents, or (3) inability to ingest or absorb oral ciprofloxacin. Further clinical trials using higher doses are required before iv ciprofloxacin can be recommended routinely for treatment of serious systemic infections. Because of its poor activity against streptococci, marginal activity against some strains of P. aeruginosa, and the rapidly developing resistance of staphylococci, iv ciprofloxacin should not be used empirically for infections in settings (such as the intensive care unit) where these organisms are likely to be pathogenic.
