Marked response to imatinib mesylate in metastatic acral lentiginous melanoma on the thumb

We report a case of melanoma that had a marked response to treatment with imatinib mesylate (IM). The patient was a 61-year-old man who presented with a small red nodule on the thumb and destruction of the nail plate. On histological examination, this lesion was diagnosed as a melanoma, and computed tomography revealed lymph-node swelling in the left axilla and nodules in both lung fields. Although the patient received intratumoral injections of interferon-β and systemic administration of dacarbazine, both primary and metastatic lesions increased in size. Immunochemistry detected a KIT mutation, which was confirmed by DNA sequencing analysis, and patient was given IM. Within 2 weeks of starting the IM regimen, the size of the nodule on the nail plate markedly decreased, and the axillary lymph-node swelling and lung-nodule formation regressed. This case suggests that IM may be a promising treatment option for KIT mutation-positive melanoma.

Painful red nodules in female patient with recent travel history: a case report

IntroductionDermatologic pathology can be both challenging and frustrating in the Family Practice setting.Case presentationWe report the case of a 38-year-old female that presented initially with a few, small red nodules on both the upper and lower extremities, which were painful to touch. This patient had an extremely vague picture, which included recent upper respiratory infection and recent travel to Europe. Erythema Nodosum was suspected and work-up initiated to determine underlying cause. ASO titer ultimately confirmed recent Streptococcal infection. Although the primary diagnosis was not made until follow-up visit, treatment was started based on understanding of common causes of Erythema Nodosum.ConclusionUsing the patient's history, a differential diagnosis knowing the common causes of EN can help direct diagnostic evaluation.

Intestinal tuberculosis of the terminal ileum causing obstructive ileus and tuberculous peritonitis and presenting numerous peritoneal small red nodules: a case report

A 38-year-old woman suffering from lower abdominal pain was referred to our hospital. Abdominal computed tomography showed marked thickening of the terminal ileum to the cecum, localized collection of ascites, and multiple mesenteric lymphadenopathy. A barium contrast small bowel series showed solitary severe stenosis of the terminal ileum with marked swelling of the ileocecal valve, where colonoscopy could not pass through, suggesting that ileal stenosis was caused by intestinal tuberculosis. She also showed strongly positive tuberculin skin test. Laparoscopy-assisted ileocecal resection was performed for confirmation of diagnosis and removal of the stenotic intestinal lesion. Laparoscopically, numerous small red nodules scattered on the stenotic ileal serosa, peritoneum, and mesenterium. Histopathological examination revealed ileal tuberculosis causing ulcerative stricture, and mesenteric tuberculous lymphadenitis. The small red nodules were formed of hemorrhagic tuberculous nodules.

Small Red Nodule on the Leg of a Young Woman

REPORT OF A CASE A 24-year-old white woman presented with a slowly enlarging nodule on her left calf. The lesion was of 6 months' duration and was generally asymptomatic with only occasional mild pruritus and slight tenderness. There was no history of trauma. The physical examination revealed a firm red nod- ule (diameter, 0.6 cm), with a slightly irregular contour (Figure 1). A biopsy specimen was obtained (Figure 2andFigure 3). What is your diagnosis? HISTOLOGIC FINDINGS The microscopic examination revealed a superficial and middermal haphazard infiltrate of collapsed small vascular structures extending between collagen bundles. These vessels showed irregular branching and consisted mostly of plump endothelial cells. Focally, some of the vessels had more visible lumens, giving a sinusoidal appearance. Significant atypia and mitotic figures were not observed.
