Dosimetric commissioning of a high-resolution CMOS 2D detector array for patient-specific QA of single-isocenter multi-target VMAT stereotactic radiosurgery.

Stereotactic radiosurgery (SRS) using the single-isocenter-multiple-target (SIMT) technique by volumetric modulated arc therapy is increasingly popular for treating multiple brain metastases. However, the complex nature of SIMT SRS necessitates rigorous patient-specific quality assurance (PSQA). This study presents a multi-institutional dosimetric commissioning of a high-resolution complementary metal oxide semiconductor (CMOS) 2D detector array, the myQA SRS device for SIMT SRS PSQA. Basic dosimetric properties such as dose-rate, field-size, energy and angular dependencies were characterized for the CMOS detectors. Additionally, gamma index analyses were performed between the measured dose and the films for nine simulated and clinical plans. The results showed that the CMOS detector was dose-rate, field-size, energy and beam-angle dependent. Specific to SIMT SRS, angular dependence on gantry rotations was invariant to couch rotations but was sensitive to off-isocenter distances. With appropriate dose calibration and angular corrections, myQA SRS showed a high dosimetric correlation with films. The average gamma index pass rates were 99.9 ± 0.03% and 99.2 ± 1.1% at 3%/2mm/10%thr(global) and 1mm/1%/10%thr(local) criteria, respectively. The average dose difference between myQA SRS and films was 0.4 ± 1.3%. In conclusion, the CMOS 2D detector array has demonstrated its potential as a reliable tool for PSQA for SIMT SRS. The excellent dosimetric agreement with the films was consistent in multiple institutions, further validating the dosimetric accuracy and reproducibility. It provides a timely alternative to film dosimetry for commissioning and quality assurance.

Evaluation of a novel patient-specific quality assurance phantom for robotic single-isocentre, multiple-target stereotactic radiosurgery, and stereotactic radiotherapy.

To evaluate patient-specific quality assurance (PSQA) of 3 targets in a single delivery using a novel film-based phantom. The phantom was designed to rotate freely as a sphere and could measure 3 targets with film in a single delivery. After identifying the coordinates of 3 targets in the skull, the rotation angles about the equator and meridian were computed for optimal phantom setup, ensuring the film plane intersected the 3 targets. The plans were delivered on the CyberKnife system using fiducial tracking. The irradiated films were scanned and processed. All films were analysed using 3 gamma criteria. Fifteen CyberKnife test plans with 3 different modalities were delivered on the phantom. Both automatic and marker-based registration methods were applied when registering the irradiated film and dose plane. Gamma analysis was performed using a 3%/1 mm, 2%/1 mm, and 1%/1 mm criteria with a 10% threshold. For the automatic registration method, the passing rates were 98.2% ± 1.9%, 94.2% ± 3.7%, and 80.9% ± 6.3%, respectively. For the marker-based registration approach, the passing rates were 96.4% ± 2.7%, 91.7% ± 4.3%, and 78.4% ± 6.2%, respectively. A novel spherical phantom was evaluated for the CyberKnife system and achieved acceptable PSQA passing rates using TG218 recommendations. The phantom can measure true-composite dose and offers high-resolution results for PSQA, making it a valuable device for robotic radiosurgery. This is the first study on PSQA of 3 targets concurrently on the CyberKnife system.

Dosimetric effects of rotational errors for single isocenter multiple targets in HyperArc plans: A phantom and retrospective imaging analysis study.

This study uses a phantom to investigate the dosimetric impact of rotational setup errors for Single Isocenter Multiple Targets (SIMT) HyperArc plans. Additionally, it evaluates intra-fractional rotational setup errors in patients treated with Encompass immobilization system. The Varian HyperArc system (Varian Medical systems) was used to create plans targeting spherical PTVs with diameters of 5, 10, and 15mm and with offsets of 1.3-5.3cm from the isocenter. Dosimetric parameters, including mean and maximum dose, D99% and D95% were evaluated for various rotational setup errors ranging from 0.5° to 2° for the PTVs and certain CTVs created within PTVs. These rotational errors were applied in an order and direction that resulted in the maximum displacement of targets. The rotation was applied both uniformly around all three axes and individually around each axis. Furthermore, to link the findings to actual treatment scenarios, the intra-fractional rotational setup errors were obtained for stereotactic cranial patients treated with the Encompass system using CBCT images acquired during treatments. The maximum displacement of 2.7mm was observed for targets located at 4.4 and 4.5cm from the isocenter with rotational setup errors of 2°. The dose reduction for D99% values corresponding to this displacement were about 50%, 40%, and 30% for PTVs with diameters of 5, 10, and 15mm, respectively. Both D99% and D95% showed a consistent trend of dose reduction across various rotational errors and PTV volumes. While the maximum dose remained consistent for different targets with various rotational errors, the mean dose decreased by approximately 25%, 12%, and 6% for PTVs with diameters of 5, 10, and 15cm, respectively, with rotational errors of 2°. In addition, by analyzing CBCT images, the absolute mean rotational setup errors obtained during treatment with Encompass for pitch, roll, and yaw were 0.17°±0.13°, 0.11°±0.10°, and 0.12°±0.10° respectively. This data, combined with existing studies, suggest that a 0.5° rotational setup error is a safe choice to consider for calculating additional PTV margin to ensure adequate CTV coverage. Therefore, the assessment of maximum displacement and dosimetric parameters in this study, for a 0.5° rotational error, highlights the need for an additional 0.7mm PTV margin for targets positioned at distances of 4.4cm or greater from the isocenter. For SIMT Plans, a 0.5° rotational setup error is recommended as a basis for calculating additional PTV margins to ensure adequate CTV coverage when using the Encompass system.

A clinical application of fair value estimate (FVE) for R50%: Separating overlapping 50% isodose volumes in single isocenter multiple targets SRS.

In the treatment of single isocenter multiple targets (SIMT) stereotactic cranial cases with linac-based, multi-leaf collimated delivery, one encounters cases when the 50% isodose clouds (IDC50%s) of planning target volumes (PTVs) in close proximity overlap and cannot easily be separated. In such cases, it is difficult to assign an IDC50% to each individual PTV, which is necessary to allow evaluation of individual PTV intermediate dose spill for comparison to established intermediate dose spill metrics for plan quality assessment. The Fair Value Estimate (FVE) for R50% (R50%FVE ) is a method to unambiguously apportion the overlapping volume of IDC50% to allow calculation of the intermediate dose spill metric R50% (defined as volume of IDC50% / volume of PTV). Full application of R50%FVE requires knowing the surface area of the PTVs. Since surface area information is not always available, we develop a spherical PTV approximation to R50%FVE-sphere and compare this to R50%FVE . Then we apply the R50%FVE-sphere to clinical data from the University of Alabama, Birmingham (UAB) that catalogs 68 PTVs from various SIMT plans with overlapping IDC50%. The UAB dataset reports intermediate dose spill as Falloff Index. While Falloff Index looks mathematically equivalent to R50%, the Falloff Index attributes the "entire overlapping IDC50% of PTVs in close proximity" to each individual PTV in the cluster. R50%FVE-sphere provides a value that is conceptually correct and numerically smaller relative to the Falloff Index data reported by UAB in all cases. This reprocessing of the UAB data places many of the PTVs with very high intermediate dose spill within recently proposed R50% guidelines.

Evaluation of Dosimetry Metrics in Single Isocenter Multiple Targets and Single Isocenter Single Target in the Treatment of Brain Metastases

Purpose: This study compares two LINAC-based techniques for treating multiple brain lesions using quantitative evaluation of plan dosimetric indices. Material and Methods: Twenty-one patients with a total of 168 targets were selected. All plans were designed with five non-coplanar arcs in the RayStation treatment planning system (TPS). The composite dose of Multiple Isocenter Multiple Targets (MIMT) was compared to that of single isocenter multiple targets (SIMT) by evaluating conformity indices, Paddick gradient-index (PGI), V8, V12, and Delivery Time (DT).

A Comprehensive Evaluation of Single Isocenter Multiple Target SRS Plans and the Analytical Relationship between Plan Quality Indices with the Number and Volume of Targets.

To study the relationship of plan quality indices with the number and volume of target for 5 to 10 brain metastases treated with LINAC-based Single Isocenter Multiple Target (SIMT) Stereotactic radiosurgery (SRS) planning and to determine the maximum volume of spherical targets treated without exceeding the normal tissue tolerances. Spherical targets of 5 to 10 numbers per plan, with individual target volumes ranging from 0.025 cc to 11.5 cc, were simulated with randomly drawn planning target volumes (PTVs) within the brain. SIMT SRS plans were generated for the 21 Gy prescription dose with a 6 MV Flattening Filter Free photon beam. Target coverage, organ at risk sparing, plan quality indices, R50%, and gradient measure were studied. Mean brain dose, V12 for Brain minus PTV (BmP), V10, V12, V15, V18, V20, and V24 for brain volume were evaluated. Equations relating the gradient index, mean brain dose, and V12 (BmP) to the given number and volume of the targets were constructed. PTV coverage D98 was 98.77 ± 1.37 %. The mean CIRTOG, QRTOG, HIRTOG, CIP, GI, and R50% of the individual targets were 1.02 ± 0.08, 0.94 ± 0.02, 1.49 ± 0.11, 0.91 ± 0.06, 4.74 ± 2.3, and 4.95 ± 2.67, respectively. The gradient measure achieved was in the range of 0.49 to 1.35 cm. The mean brain dose was in the range of 1.62 to 6.69 Gy. The mean V12 (BmP) per target obtained was 3.85 ± 2.83 cc. Equations relating the number and volume of targets to the gradient measure, mean brain dose, V12 (BmP), and V10-24 can serve as a baseline for multiple brain metastases SIMT planning. The target volume for 5, 6, 7, 8, 9, and 10 targets that can be treated without exceeding V12 (BmP) is 6, 5, 4.7, 4, 3.7, and 3.4 cc, respectively, for the 21 Gy prescription dose.

How to estimate R50% for cranial SRS/SRT cases with overlapping 50% isodose volumes: A proposed system.

Inevitably in clinical stereotactic cranial single isocenter multiple target cases treated with linac‐based multi‐leaf collimated (MLC) delivery, one encounters planning target volumes (PTVs) in close proximity with overlapping 50% isodose clouds (IDC50%). In such cases, it is very difficult to separate the IDC50% attributable to each individual target and, thus, assess the intermediate dose conformality or R50%. Such scenarios happen regardless of what metric is used to measure intermediate dose spill. Now that universal standards for intermediate dose spill have been proposed, it is important to have a consistent method for apportioning these overlapping IDC50% volumes to allow comparison with the proposed standards when multiple PTVs have overlapping IDC50%. We propose a systematic method for apportioning the IDC50% of multiple targets with overlapping IDC50% based on the relative surface area of each target volume; we call this the fair value estimate (FVE). This FVE system of apportionment is tested for reasonableness by comparing the apportionment of multiple target single isocenter stereotactic treatment with widely spaced targets where the IDC50% can be obviously assigned to demonstrate that the FVE results are very similar to the actual R50% results. We then demonstrate how the FVE system would be applied to cases with overlapping IDC50%. We propose this FVE system for consideration by the cranial stereotactic community for apportioning the intermediate dose spill when that intermediate dose spill overlaps among multiple targets.

Systematic end-to-end testing of multiple target treatments using the modular RUBY phantom

The RUBY head phantom in combination with the System QA insert MultiMet can be used for simultaneous point dose measurements at an isocentric and two off-axis positions. This study investigates the suitability of the system for systematic integral end-to-end testing of single-isocenter multiple target stereotactic treatments. Several volumetric modulated arc therapy plans were optimized on a planning CT of the phantom positioned in a stereotactic mask on the stereotactic treatment board. The plans were created for three artificial spherical target volumes centred around the measurement positions in the MultiMet insert. Target diameters between 5 and 40 mm were investigated. Coplanar and non-coplanar plans were optimized using the collapsed cone algorithm of the Oncentra Masterplan treatment planning system and recalculated with the Monte Carlo algorithm of the Monaco treatment planning system. Measurements were performed at an Elekta Synergy linear accelerator. The head phantom was positioned according to clinical workflow comprising immobilization and CBCT imaging. Simultaneous point dose measurements at all target positions were performed with three PinPoint 3D chambers (type 31022) as well as three microDiamond detectors (type 60019) and compared to the treatment planning system calculations. Furthermore, the angular dependence of the detector response was investigated to estimate the associated impact on the measured point dose values. Considering all investigated plans, PTV diameters and positions, the point doses calculated with the Monaco treatment planning system and the microDiamond measurements differed within 3.5%, whereas the PinPoint 3D showed differences of up to 6.9%. Point dose differences determined in comparison to the Oncentra Masterplan dose calculations were larger. The RUBY system was shown to be suitable for end-to-end testing of complex treatment scenarios such as single-isocenter multiple target plans.

The use of collimator angle optimization and jaw tracking for VMAT-based single-isocenter multiple-target stereotactic radiosurgery for up to six targets in the Varian Eclipse treatment planning system.

PurposeIsland blocking occurs in single‐isocenter multiple‐target (SIMT) stereotactic radiotherapy (SRS) whenever targets share multi‐leaf collimator (MLC) leaf pairs. This study investigated the effect on plan quality and delivery, of reducing island blocking through collimator angle optimization (CAO). In addition, the effect of jaw tracking in this context was also investigated.MethodsFor CAO, an algorithm was created that selects the collimator angle resulting in the lowest level of island blocking, for each beam in any given plan. Then, four volume‐modulated arc therapy (VMAT) SIMT SRS plans each were generated for 10 retrospective patients: two CAO plans, with and without jaw tracking, and two plans with manually selected collimator angles, with and without jaw tracking. Plans were then assessed and compared using typical quality assurance procedures.ResultsThere were no substantial differences between plans with and without CAO. Jaw tracking produced statistically significant reduction in low‐dose level parameters; healthy brain V10% and mean dose were reduced by 9.66% and 15.58%, respectively. However, quantitative values (108 cc for V10% and 0.35 Gy for mean dose) were relatively small in relation to clinical relevance. Though there were no statistically significant changes in plan deliverability, there was a notable trend of plans with jaw tracking having lower gamma analysis pass rates.ConclusionThese findings suggest that CAO has limited benefit in VMAT SIMT SRS of 2–6 targets when using a low‐dose penalty to the healthy brain during plan optimization in Eclipse. As clinical benefits of jaw tracking were found to be minimal and plan deliverability was potentially reduced, a cautious approach would be to exclude jaw tracking in SIMT SRS plans.

Distance to isocenter is not associated with an increased risk for local failure in LINAC-based single-isocenter SRS or SRT for multiple brain metastases

PurposeTo evaluate the impact of the distance between treatment isocenter and brain metastases on local failure in patients treated with a frameless linear-accelerator-based single-isocenter volumetric modulated arc (VMAT) SRS/SRT for multiple brain metastases. Methods and materialsPatients treated with SRT for brain metastases (BM) between April 2014 and May 2019 were included in this retrospective study. BM treated with a single-isocenter multiple-target (SIMT) SRT were evaluated for local recurrence-free intervals in dependency to their distance to the treatment isocenter. A Cox-regression model was used to investigate different predictor variables for local failure. Results were compared to patients treated with a single-isocenter-single-target (SIST) approach. ResultsIn total 315 patients with a cumulative number of 1087 BM were analyzed in this study of which 140 patients and 708 BM were treated with SIMT SRS/SRT. Median follow-up after treatment was 13.9 months for SIMT approach and 11.9 months for SIST approach. One-year freedom from local recurrence was 87% and 94% in the SIST and SIMT group, respectively. Median distance to isocenter (DTI) was 4.7 cm (range 0.2–10.5) in the SIMT group. Local recurrence-free interval was not associated with the distance to the isocenter in univariable or multivariable Cox-regression analysis. Multivariable analysis revealed only volume as an independent significant predictor for local failure (p-value <0.05). ConclusionSRS/SRT using single-isocenter VMAT for multiple targets achieved high local metastases control rates irrespective of distance to the isocenter, supporting efficacy of single-isocenter stereotactic radiation therapy for multiple brain metastases.

Positioning accuracy of a single-isocenter multiple targets SRS treatment: A comparison between Varian TrueBeam CBCT and Brainlab ExacTrac.

This study compared the positioning accuracy between cone-beam CT (CBCT) and ExacTrac (ETX) for a single-isocenter multiple target stereotactic radiosurgery (SRS) on two TrueBeam STx systems. A single-isocenter treatment plan was simulated on an anthropomorphic head phantom with six spherical steel ball bearings (BBs). One of the BBs was chosen to be the isocenter. The five off-isocenter targets were located at various distances from the isocenter. MV portal images were generated to evaluate the deviations between the expected and the real center of the targets after CBCT and ETX positioning, respectively. The evaluation of the positioning accuracy for the isocenter target showed that CBCT and ETX positioning provided comparable, sub-millimetric results. Deviations in positioning accuracy were also calculated for all other targets, also showing comparable results for CBCT and ETX. Moreover, our study showed that the deviation between CBCT and ETX positioning were in better agreement for TBSTx1 and deviated slightly higher on TBSTx2 (maximum: 1.23mm at S/I direction), due to a less perfect alignment between the CBCT coordinate system and the ETX coordinate system on TBSTx2 compared to TBSTx1. This study also showed a correlation between the target positioning accuracy and the distance to the isocenter. The positioning accuracy of ETX and CBCT for targets located at isocenter and off-isocenter locations was compared on two treatment machines and found comparable. Our study highlights the importance of a proper calibration procedure, to ensure correct alignment between the CBCT, ETX and machine coordinate systems.

Initial Experience With Single-Isocenter Radiosurgery to Target Multiple Brain Metastases Using an Automated Treatment Planning Software: Clinical Outcomes and Optimal Target Volume Margins Strategy

PurposeOur purpose was to assess the clinical outcomes and target positioning accuracy of frameless linear accelerator single-isocenter multiple-target (SIMT) dynamic conformal arc (DCA) stereotactic radiosurgery (SRS) for multiple brain metastases (BM). Methods and MaterialsBetween October 2016 and September 2018, 31 consecutive patients ≥18 years old with 204 BM <3 cm in maximum size receiving SIMT DCA SRS were retrospectively evaluated. All plans were created using a dedicated automated treatment planning software (Brainlab, Munich, Germany), and treatments were performed with a Truebeam STx or a Novalis Tx (Brainlab and Varian Medical Systems, CA). The accuracy of setup and interfraction patient repositioning was assessed by Brainlab ExacTrac radiograph 6-dimensional image system and the risk of compromised target dose coverage evaluated. Brain control and overall survival were estimated by Kaplan-Meier method calculated from the time of SRS. ResultsFourteen patients were treated for 4 to 6 and 17 patients for 7 to 10 BM. The mean gross tumor volume (GTV) was 0.65 cm3 and the mean planning target volume (PTV) was 0.89 cm3. Mean V95 (the volume of the PTV covered by 95% of the prescription dose) and D95 (the prescription dose covering 95% of the PTV) were 99.5% and 21.1 Gy, respectively. With a median clinical follow-up of 11 months (range, 4-26 months), the 1-year survival was 68% and local control was 89%. As a consequence of plan isocenter residual errors, a loss of target coverage, defined as V95 < 95%, occurred in 28 PTVs (10 patients); using a 1 mm GTV-to-PTV margin, adequate dose coverage was maintained for all lesions. ConclusionsSIMT DCA SRS represents a fast and effective approach for patients with up to 10 BM. The dosimetric effects of residual set-up and intrafraction positioning errors are modest, although a GTV-to-PTV margin of 1 mm is recommended.

Neurological outcome and memory performance in patients with 10 or more brain metastases treated with frameless linear accelerator (LINAC)-based stereotactic radiosurgery.

To assess the neurocognitive function and neurological toxicity of frameless linear accelerator (LINAC)-based stereotactic radiosurgery (SRS) in patients with 10 or more brain metastases (BM). Forty consecutive adult patients who received SRS for ten or more 10 BM < 3cm in maximum size were evaluated. All plans were generated using a single-isocenter multiple-target (SIMT) SRS technique with doses of 22Gy for lesions < 2cm and 16-18Gy for those ≥ 2cm in size. Survival analyses were estimated by Kaplan-Meier method from the date of SRS. Neurocognitive function using the Hopkins verbal learning test-revised (HVLT-R) and activity of daily living scale (ADLS) were collected prospectively at baseline and at 3,6 and 12-month follow-up. Toxicity was assessed by the National Cancer Institute Common Toxicity Criteria for Adverse Events (Version 5.0). With a median follow-up of 10.8months, 1-year survival and local control rates were 65% and 86%, respectively. Grade 2 or 3 toxicity occurred in eleven patients, being associated with radiological changes suggestive of radiation necrosis in seven patients. Three months after SRS, the mean relative decline was 14.2% for HVLT-R delayed recall, 12.3% for HVLT-R recognition, and 9.8% for HVLT-R total recall. A significant deterioration of HVLT-R scores ranged from 5.5 to 18.7% of patients at different time points. ADLS scores declined over time, but changes were not significant. SRS is an effective and safe approach for patients with 10 or more BM able to maintain the pretreatment neurocognitive function in the majority of patients.

Optimization of MLC parameters for TPS calculation and dosimetric verification: application to single isocenter radiosurgery of multiple brain lesions using VMAT

Linac and MLC-based stereotactic radiosurgery (SRS) using single-isocenter-multiple-target (SIMT) VMAT has become increasingly popular in the management of multi-focal cranial metastases. However, significant geometrical and dosimetric challenges exist due to the typically small target volumes and in most cases, non-isocentric locations. To the best of our knowledge, there hasn’t been a study in the optimization of MLC parameters, in the context of SIMT SRS, to ensure TPS calculation accuracy. In this work, we set out to optimize the dosimetric leaf gap (DLG) for the HD MLC installed on dedicated stereotactic Varian STx systems using a diverse group of 21 clinical SRS and SBRT plans. These plans featured a broad range of target sizes and target-to-isocenter distances that are typical of the stereotactic cases treated on these systems. Dose discrepancies between TPS calculations and verification measurements using a previously validated diode array Delta4 (ScandiDos) were minimized in a balanced manner to accommodate the variety of stereotactic plans. A DLG of 0.6 mm was found to be ‘optimal’ for the HD MLC and for the ‘typical’ plans treated on our STx systems. The finding was independently verified using commercially available 3D polymer gel dosimeter CrystalBallTM (MGS Research Inc.). 3D verification for 6 SIMT SRS plans, consisted of 5 to 15 targets, achieved an average gamma score of 97.3% (σ = 2.0%) on 3%/2 mm criteria with a cutoff isodose level of 20%. We further examined the practice of routine dosimetric verifications including the selection of appropriate detectors and optimal gamma parameters. We found that the commonly used standard 3%/3 mm criteria would have resulted in all but 4 (out of 2840) clinical plans achieving a gamma score of 95% or better, and therefore, losing sensitivity to detect potential dosimetric discrepancies. Based on the characteristics of stereotactic plans, a more stringent distance-to-agreement parameter is needed.
