Intraspousal transmission of GB virus C/hepatitis G virus in an hepatitis C virus hyperendemic area in Japan

OBJECTIVE: An immunoassay for antibodies against an hepatitis G virus (HGV) protein (anti-E2) was recently developed that might serve as a useful marker for diagnosing recovery from HGV infection. METHODS: We investigated the intraspousal transmission of GB virus C/hepatitis G virus (GBV-C/HGV) using both reverse transcription hemipolymerase chain reaction (RT-hemi-PCR for the 5′ untranslated region) and a recently developed anti-E2. RESULTS: Thirty-two GBV-C/HGV-infected index subjects were selected from an hepatitis C virus hyperendemic area in Japan. Of the 32 subjects, seven (6.4%) were GBV-C/HGV RNA-positive, 24 (21.8%) were anti-E2-positive, and one (0.9%) was both GBV-C/HGV RNA- and anti-E2-positive. Among the 32 spouses of these subjects, GBV-C/HGV RNA, anti-E2, and both GBV-C/HGV RNA and anti-E2 positivity were detected in 0, 6, (18.8%), and one (3.1%) spouses, respectively (the total prevalence of GBV-C/HGV was 7 spouses [21.9%]). Thus, the intraspousal transmission of GBV-C/HGV was undeniable in these seven couples. The respective positive rates of 175 sex- and age-matched controls were 7 (4.0%), 26 (14.9%), and 0 (the total prevalence of GBV-C/HGV was 34 [19.4%]). No significant difference in positive rates was observed between the subjects/spouses and the controls. Five spouses among the seven couples who were positive for any of GBV-C/HGV markers had parenteral risk factors such as blood transfusion, acupuncture, and major surgery. CONCLUSION: Based on these observations, we cannot draw a definitive conclusion that intraspousal transmission of GBV-C/HGV had occurred among these seven couples.

The relationship between constitutive nitric oxide synthase (cNOS) and pregnancy induced hypertension.

To study the relationship between constitutive nitric oxide synthase (cNOS) and pregnancy induced hypertension (PIH), cNOS expression and localization in placental villi of PIH patients (n = 15) and normal pregnancy patients (n = 15) were immunohistochemically studied. The positive immunostaining of cNOS was located in trophocytes and cytoplasm of vascular endothelial cells. The positive rate in PIH patients was much higher than that in cases of normal pregnancy. The positive rates were 34.40% in mild, 44.74% in middle and 50.14% in severe PIH patients respectively. There was a significant difference in positive rate among the mild, middle and severe PIH patients (P < 0.01). It is concluded that the increase of cNOS activity probably was the results of protective or compensatory mechanism of PIH course.

The Furosemide Test and Vestibular Status in Meniere's Disease

A total of 62 ears of patients with typical Meniere's disease was examined by the furosemide test to detect endolymphatic hydrops. In 95% of the normal control group, the per cent change in the maximum velocity of the slow phase of caloric nystagmus (MVS) after injection of furosemide was under 10%. Therefore, a positive furosemide test was defined as a change in MVS of more than 10%. Thirty-five (56%) of the 62 ears with typical Meniere's disease showed a positive furosemide test. When the affected ears were divided into two groups according to vestibular symptoms, only 11 (38%) of 29 inactive ears were positive while 24 (73%) of 33 active ears were positive. There was a significant difference in the positive rate of the furosemide test between the ears with clinically inactive and active vestibular disease. The per cent canal paresis (CP%) was determined to assess canal excitability and a CP%>25% was defined as canal paresis. There was no significant difference in the furosemide test positive rate between ears with canal paresis and ears with a normal CP%, although the former tended to show a greater MVS change. The response to the furosemide test showed no relationship to the results of pure tone audiometry. In conclusion, the furosemide test appears to indicate the vestibular status in various stages of Ménière's disease.

Effects of Closed-SystemDrain in Surgery:Focus on Methicillin-ResistantStaphylococcus aureus

Objective: The emergence of methicillin-resistant Staphylococcus aureus (MRSA) has made a strong impact on the strategy of peri-operative antibiotic prophylaxis, since MRSA has become one of the most common causative organisms of nosocomial infection in recent years. In this study, we conducted a bacteriological evaluation of surgical drains before and after introducing strategies to decrease MRSA infection rates. Design and Patients: Between January 1987 and December 1994, we performed a total of 2,755 surgical operations on inpatients, including 1,635 major and 1,120 minor operations. Almost all surgical drains were examined bacteriologically when they were removed. The number of drains examined was 460 ± 47 (mean ± SEM) per year. Since the increased incidence of MRSA infection, we started exclusively using a closed drainage system and first-generation cephalosporins in 1991. The strategy was evaluated by comparing the positive rates of drain cultures, changes in bacteriological features, and incidence of MRSA infection for the 4-year periods before and after 1991. Results: The positive rate of bacteria in the drains decreased significantly (p < 0.01) from 25 ± 2 to 16 ± 1%. Bacteriologically, the positive rate of Staphylococcus spp. decreased significantly (p < 0.05) from 7 ± 2 to 3 ± 0.3%. Positive rates of MRSA decreased significantly (p < 0.05) from 2.1 ± 0.3 to 1.3 ± 0.3%. Streptococcus declined dramatically from 3.0 ± 0.3 to 0.3 ± 0.1%. Of gram-negative strains, Pseudomonas and Escherichia coli were most often isolated. They showed no significant difference in positive rates between the terms. Conclusion: A closed drainage system and thorough use of the first-generation cephalosporins for prophylaxis were effective in decreasing positive bacterial culture of drains and reducing the incidence of MRSA on drains after surgery.

Serodiagnosis of tuberculosis and leprosy by enzyme immunoassay.

OBJECTIVE: To evaluate the use of serodiagnosis for tuberculosis and leprosy using mycobacterial antigen 38 kDa, with kits from Omega laboratories, to detect IgG by enzyme immunoassay (EIA). METHODS: The study population consisted of 58 patients with evidence of tuberculous infection (culture of Mycobacterium tuberculosis complex or microscopic evidence), of whom 23 had pulmonary and 35 had extrapulmonary disease. There were six subjects who had recently been treated for tuberculosis, 11 patients on treatment for leprosy and 137 patients suspected of having tuberculosis on clinical or radiologic grounds (without laboratory evidence). A control group comprised 35 healthy individuals or patients suffering from diseases other than tuberculosis. RESULTS: The tests showed that there was a significant difference in antibody levels between the patients with active pulmonary disease, extrapulmonary tuberculosis and leprosy in comparison with the control group (p<0.001). The sensitivities of the two tests together for proven pulmonary tuberculosis were 100% and 95.7% at 1.0--1.5 and >1.6 EIA cut-off points respectively, while the specificities were 88.5% and 100% at the same cut-off points. The sensitivities for extrapulmonary tuberculosis were 71.4% and only 51.4% at 1.0--1.5 and >1.6 EIA cut-off points. The test was positive in 30 (21.9%) of the 137 suspected patients, while 43 (31.4%) had an equivocal result and the remaining 64 (47.7%) suspects were definitely negative. There was again a significant difference in positivity rates between suspects and the control group. CONCLUSIONS: Omega IgG test is useful in the serodiagnosis of active pulmonary tuberculosis and leprosy, but less sensitive in extrapulmonary disease, particularly in children. Equivocal results may only add to the evidence of tuberculosis in early or minimal disease.

Lack of screening test sensitivity during HIV-1 non-subtype B seroconversions.

To evaluate the serological consequences of HIV-1 group M diversity we studied the ability of screening tests to detect anti-HIV antibodies in early seroconverters infected by different HIV subtypes. Virology Department, Bichat-Claude Bernard Hospital, Paris, France. Symptomatic patients with serial samples and with infective strains characterized by heteroduplex mobility assay. In each case, two sera were selected. The first (pre-seroconversion sample) was the last p24 antigen-positive/Western blot-non-reactive sample. The second (seroconversion sample) was the first Western blot-reactive sample. One second-generation enzyme immunoassay (EIA; Abbott) based on anti-human immunoglobulin (Ig) G-conjugate and three third generation EIA (Abbott; Enzygnost; Genscreen) based on the double antigen sandwich principle, detecting IgM and IgG, were used. Ten patients had subtype B strains and nine had non-B strains (seven were A, one E and one G). The Abbott third-generation test was more sensitive than the second generation test for pre-seroconversion subtype B samples (nine versus four out of 10; P < 0.05), but not for non-B subtypes; only two of the nine non-B sera tested were positive by both EIA. Positivity rates and optical densities differed (P < 0.05) between B and non-B subtypes in all third-generation EIA. There was no significant difference between the subtype B and non-B groups with regard to the interval between the pre-seroconversion sample and the seroconversion sample (subtype B, 6.7 +/- 2.6 days; non-B, 5.2 +/- 1.7 days). No significant difference in positivity rates and optical densities were found between B and non-B subtypes in these seroconversion samples. The shorter time since HIV infection required for sera to become reactive in third-generation EIA screening tests is due to better sensitivity for subtype B strains only. These results stress the importance of strict donor selection, the need to test screening kits against large panels of all subtypes, and the place of p24 antigen testing in closing the window of seroconversion.

Distinguishing primary and metastatic carcinomas of the liver: Immunohistochemical updates and reminders

Distinguishing primary hepatocellular carcinoma (HCC) from metastatic carcinomas to the liver is often difficult, if not impossible, particularly in needle biopsy and fine-needle aspiration specimens. In an attempt to identify a specific immunohistochemical profile that would distinguish HCC from metastatic carcinomas, we studied 56 HCCs, 8 cholangio-carcinomas, and 24 metastatic adenocarcinomas with monoclonal antibodies to alpha-fetoprotein (AFP), keratin (AE1, AE3, and CAM5.2), Leu-M1, human milk fat globule (HMFG-2), tumor-associated glycoprotein-72 (B72.3), epithelial specific membrane antigen (Ber-EP4), and BCA-225 (CU-18). Both monoclonal and polyclonal (mCEA and pCEA) antibodies to carcinoembryonic antigen also were used. Metastatic adenocarcinomas were often positive for CU-18 (71%), Leu-M1 (75%), B72.3 (50%), HMFG-2 (67%), Ber-EP4 (83%) and mCEA (71%). Using these antibodies, the frequency of positivity for HCC was 9%, 16%, 11%, 20%, 36%, and 11%, respectively. CU-18 was the only monoclonal antibody in which there was a significant difference in positive rates between HCC and metastatic adenocarcinomas. Most HCCs (71%) revealed a bile canalicular staining pattern with pCEA. Because this staining pattern was absent in metastatic carcinomas, pCEA appears to be useful in confirming a diagnosis of HCC, AE1, AE3 and CAM5.2 antibodies were not useful in distinguishing HCC from metastatic carcinomas. Each cholangiocarcinoma shared a staining profile similar to that of metastatic carcinomas. Hepatocyte paraffin 1 is a monoclonal antibody that has been developed specifically to react with hepatocytes in routine formalin-fixed and paraffin-embedded surgical pathology tissues. It results in a distinct, granular cytoplasmic staining of hepatocytes but fails to react with bile ducts and nonparenchymal liver cells. The antibody decorates a majority of hepatocellular carcinomas, including fibrolamellar variants. It fails to react with a wide variety of other adult malignancies, with the exception of focal staining in a few gastrointestinal malignancies, including a sub-population of gastric carcinomas.

Immunohistochemical study of c-erbB-2 proto-oncogene product in prostatic cancer

The c-erbB-2 proto-oncogene product is expressed in adenocarcinomas of breast cancer and ovarian cancer, and its significance as a prognostic factor has been increasingly noted. We immunohistochemically studied the expression of c-erbB-2 proto-oncogene product using anti-c-erbB-2 gene product polyclonal antibody (Nichirei), which was produced using a synthetic peptide at the C-terminal portion as the immunogen. The subjects consisted of 52 patients with prostatic cancer who were treated at the Department of Urology, Shiga University of Medical Science, from 1982 to 1990. The expression of c-erbB-2 gene was observed in 40 of the 52 patients (76.9%). The positive rate was highest in patients with poorly differentiated cancer and in stage D2 patients, but there were no significant differences in positive rates among patients with different histological types or clinical stages. The probability that progression would occur was significantly (p < 0.05) lower in the group that tested positive for c-erbB-2 than in the group that tested negative among 33 stage D2 patients after 5 years of treatment. When cause specific survival rates were calculated using the Kaplan-Meier method, the group that tested positive had a significantly (p < 0.001) poorer outcome than the group that tested negative after 3 years and 6 months of treatment. The above results suggest that c-erbB-2 expression in prostatic cancer may be useful in predicting the prognosis of the disease.

Epidemiological Studies On Protozoan Infection In Gyeong-Gi Do And Jeonra Bug Do

The present study was designed to elucidate the recent prevalence of the intestinal protozoa and to demonstrate the relationship on prevalence of the surveyed population with their academic career and living conditions. During the period from May to July 1981, total 2,735 fecal specimens were collected from the inhabitants of 11 areas in Gyeong-gi Do (province) near by Seoul City, and 6 areas in Jeonra Bug Do, southwestern part of Korea. The samples were fixed with SAF solution and were examined by formalin-ether sedimentation technique. The results are summarized as follows: Overall positive rate of intestinal protozoa was 11.1 %: Gyeong-gi Do, 10.7 %; Jeonra Bug Do, 11.7 %. Five species of intestinal protozoa were detected: Entamoeba histolytica 0.69 %, Entamoeba coli 5.52 percent, Endolimax nana 2.71 %, Giardia lamblia 2.23 %, Iodamoeba butschlii 0.l8 %. Highest positive rates among the examined areas were recognized: Incheon City of Gyeong-gi Do, 26.7 %; Im-sil Gun(county) of Jeonra Bug Do, 19.9 %. By species, E. nana showed the highest prevalence rate (13.74 %) in Incheon City, and E. histolytica (2.37 %) and E. coli (9.95 %) in Im-sil Gun. Prevalence of E. histolytica in rural areas was higher than in urban areas. No significant difference in the prevalence rate by sex and age was recognized, although the group under 10 years and over 60 years were lower than in other age groups. The highest positive rate of E. histolytica was observed in pauper group and significant linearity of prevalence rate of E. histolytica by monthly income was observed. The inhabitants who used conventional latrine showed higher prevalence, 11.5 %, than the group of inhabitants who used modified privy. Although no significant difference of positive rate was recognized by academic career, but the lowest prevalence rate was observed in college graduates. Over half of the surveyed population, 64.8 % has no experience on anthelmintic practice. Number of anthelmintic application resulted no difference in prevalence. In conclusion, prevalences of intestinal protozoa in Gyeong-gi Do and Jeonra Bug Do decreased remarkably in comparison with those of one decade before. However, economic level, academic career and anthelmintic administration seemed no essential reasons for such declination.

An altered lung volume and electrical resistance of the tissues surrounding the heart can affect vectorcardiographic changes. This study was designed to evaluate whether the altered lung volume by expiratory and inspiratory breath-holding can produce any change of vectorcardiogram. Patients with chronic obstructive pulmonary disease (COPD), but without respiratory failure, were divided into 3 groups according to forced expiratory volume in one second (FEY 1.0 % ): Group 1, consisted of 15 cases with FEV1.0% of 71% or more; Group 2,19 cases of 55% to 70%; Group 3,24 cases of 55% or less. Vectorcardiographic changes, anatomical axis of the heart, diaphragmatic movement determined by chest X-P and gas analysis by exercise testing were investigated in 58 COPD patients and 15 normal men.1) There was max QRS vector showing backward rotation by deep inspiration in Group 1, Group 3 and normal men. The downward movement changes of max QRS vector by dee p inspiration were seen in Group 1, Group 2 and normal men.2) The magnitude of max QRS vector by deep inspiration was reduced significantly in Group 2 and normal men and correlated with a decrement of lung triangle area. (Lung triangle area is an area of left side chest X-P; this triangle is formed by three points of bifurcatio trachea, posterior phrenicocostal sinus and a point of intersection of diaphragram and posterior margin of the heart.)3) The max QRS vector of frontal plane in Group 3 showed greater downward deviation by deep expiration than normal men. The max QRS vector of horizontal plane showed greate r backward deviation by deep expiration than normal men. The magnitude of max QRS vector by deep expiration in Grpup 3 showed significant lower value than normal men. This can be explained adequately on the basis of irreversible inflated lung in patients w ith COPD.4) Anatomical axis was closely related to electrical axis in normal men in deep expiratory and inspiratory breath-holding, but there was no significant relation in patients with COPD.5) A decrement of max QRS vector magnitude in frontal plane correlated with a decrement of lung triangle area in Group 2.6) A good relation was found between movement of diaphragm and FEV1.0%, between an area change of lung triangle and FEV1.0%.7) PO2 showed a more significant decrease after Master's 2 steps exercise in patients with poor diaphragramatic movement than in patients with good diaphragmatic movemen t. There was no significant difference in positive rate of Master's 2 steps exercise among the groups. Group 3 showed lower value of PO2and higher value of PCO2 than normal me n, induced by exercise.In the light of the evidence, it would seem most probable that the experimental data presented for these supine subjects can most simply be explained on the assumption that the vectorcardiographic changes in COPD, in which max QRS vector become oriented more to the right posterior and inferor direction, are the direct result of excessive lung volume around the heart at the extreme of breath-holding

Incidence of bluetongue virus precipitating antibodies in sera of some domestic animals in the Sudan

To determine the presence and prevalence of bluetongue (BT) infection in a variety of domestic animal species in different geographical regions of the Sudan, a serological study using the agar gel precipitation technique was initiated. A total of 2142 serum samples were examined. Of the numbers tested approximately 28% of sheep, 11.2% of goats, 8% of cattle and 4.9% of camels were positive for group-specific antibodies to BT virus antigen, indicating previous exposure to BT infection. None of the samples tested from horses or donkeys were positive. The findings suggest that the disease is widely distributed in most parts of the Sudan where possible insect vectors prevail and may be endemic in sheep in Juba District, Equatoria Province, Southern Region. Goats appeared to have some degree of resistance to infection compared with sheep, and there seemed to be no significant differences in positive rates between farm and free-range cattle. It is concluded that BT infection may cause clinical disease in sheep, while it is probably subclinical or inapparent in goats, cattle and camels of the Sudan.

Five trials were carried out with more than 15, 000 chickens, including approximately 10, 000 birds inoculated with Marek's disease vaccine (Deptavac-HVT). Each trial lasted for 180 days.The disease broke out in 7.5% of the vaccinated birds and 14.4% of the unvaccinated control ones. The rate of decrease in outbreak of the disease in the vaccinated birds was 48.1%. Death and culling occurred from a variety f causes, including weakness accidental death, cannibalism, Marek's disease, leucosis, and chronic respiratory diseease.There was no significant difference in positive rate for gel-precipitating antibody against Marek's disease, mean titer of neutralizing antibody against infectious bronchitis virus, or positive rate for agglutination againstMycoplasma gallisepticunzbetween the vaccinated and control groups. When some vaccinated birds were subsequently inoculated with killed or live Newcastle disease vaccine 2 to 4 times during a period from 4 to 143 days of age no hemagglutinatio-ninhibiting antibody response to this vaccine was suppressed in any of 4 vaccinated groups.
