Parent’s Satisfaction Level without Antibiotic Prescription in Children with Clinical Diagnosis of Viral URTI

ABSTRACT Objective: Objective of this study is to assess parent satisfaction levels when healthcare providers do not prescribe antibiotics for children with clinical diagnoses of viral upper respiratory tract infection (URTI). Methodology: 200 children having ages between 6 months to 12 years, clinically diagnosed, as viral upper respiratory tract infections (based on clinical sign and symptoms) without respiratory distress signs were included. Structured questionnaire included items related to parent’s demographic information, their understanding of URTIs, their expectations regarding antibiotic prescriptions and their satisfaction levels with the healthcare provider's decision. A medical officer conducted face-to-face interviews of parents or guardian after their child's diagnosis and consultation with a Consultant Pediatrician, in a friendly atmosphere. Results: The average age of the children included in the study was 5.12 ± 2.93 years. There were 124 males (62.0%) and 76 females (38.0%). 18 participants (9.0%) reported being highly unsatisfied with the healthcare provider's decision not to prescribe antibiotics for their children's viral URTI. 152 (76.0%), expressed being unsatisfied, while 30 participants (15.0%) were satisfied. The majority of participants, 192 (96.0%), believed that antibiotics could help in the treatment of viral URTIs. Only a small percentage, 8 (4.0%), held the correct belief that antibiotics do not help in viral URTI. Conclusion: Majority of parents expressed dissatisfaction with the healthcare provider's decision not to prescribe antibiotics for their children's viral URTIs. Well-Preserved Urinary Bladder Anatomy in Rats After Minimally Invasive Surgery

Background: The setup of experimental protocols able to preserve the anatomical integrity also in terms of organ microarchitecture is mandatory to ensure result translatability. Also, the maintenance of structural integrity perfectly aligns with the refinement implementation aiming to reduce procedure severity, a key issue in animal studies deemed compulsory from both ethical and legal standpoints. Here we report a detailed description of all peri-operative and post-operative care and clinical evaluation in a surgical rat model to test the efficacy of a catheter functionalized by a peptide coating with antimicrobial and antibiofilm properties, whose efficacy was previously tested in vitro. Methods: We used male and female adult Fischer 344 rats (tot n = 44, n = 22 each sex), which were divided into four experimental groups. Each animal underwent refined surgery for the implantation of a functionalized or standard catheter, depending on the group, and was observed for 7 and 14 days. The surgical refinement strategy was based on the placement of the catheter into the bladder lumen rather than in the urethra. Still in the refinement perspective, ultrasound examination of the bladder was conducted to confirm the in situ position of the medical device at an intermediate time point, 4 or 10 days post-surgery depending on the group, while, at the same time, but also at days 0, 7, or 14 post-surgery, an ultrasound-guided cystocentesis was performed to collect sterile urine. The imaging approach was used in place of metabolic cages to minimize distress to the animals and to ensure reliable and unbiased scientific outcomes. Hematological and biochemical parameters were monitored along the preclinical trial; namely, blood sampling was performed at the beginning (day 0) and at the end of the trial (day 7 or 14 post-surgery depending on the group). Results: Clinical scores and biochemical analyses of all animals did not reveal signs of distress or disease. At the endpoints, histological analyses of urinary bladder displayed a well-preserved anatomical structure of the organ without significant signs of inflammatory infiltration into the urothelium. Conclusions: Our model represents a refined strategy to achieve the scientific goals required by the preclinical setting related to catheter-associated urinary tract infections. In particular, it ensures the preservation of bladder morphology and urothelium microarchitecture, maintaining an adequate level of animal health and welfare while monitoring the onset of urinary tract infections through the sterile collection of urine in long-lasting experiments.

ACUTE POST HEMORRHAGIC ANEMIA AND SHOCK IN NEONATE DUE TO RUPTURE UMBILICAL CORD DURING DELIVERY

Umbilical cord rupture in utero is a very rare and critical emergency that can cause fetal death within minutes. Umbilical cords rupture can be associated with normal or adverse perinatal outcomes leading to stillbirth, asphyxia, fetal distress, and neurologic damage. When rupture does occur in utero signs of fetal distress appear and fetal mortality rate is approximately 50%. We present the rare case of severe anaemia and shock immediately after delivery related to severe neonatal blood loss due to intrapartum umbilical cord rupture where there was a tight nuchal cord and the placenta was bilobed with a Succenturiate lobe. The umbilical cord ruptures close to the babys side near the abdominal wall.This is an unusual site, as most cases tend to rupture from the weakest site at the insertion of the cord to the abnormal placenta.Following stabilization and resuscitation, the baby survived and recovered well.

Idiopathic Left Posterior Posterior Fascicular Ventricular Tachycardia in a Pregnant. A case report

Idiopathic left fascicular ventricular tachycardias (VT) are a rare form of ventricular tachycardia. In general, they represent 10-15% of all idiopathic ventricular tachycardias, and verapamil is useful in their treatment and in the prevention of recurrences. They are typical of young males (15-40 years) and although they usually occur at rest, they can be induced by catecholamines in situations of emotional stress or exercise. We present the case of an 18-year-old woman, 38.2 weeks pregnant, admitted to the Emergency Department of our center for the presence of palpitations and elevated heart rate in the context of emotional stress. She had no previous evaluations for similar symptomatology. In the electrocardiogram performed, repolarization alterations, incomplete right bundle branch block pattern, narrow QRS, electrical axis deviated to the left, atrioventricular dissociation, and sustained tachycardia refractory to treatment, deciding the termination of pregnancy due to signs of fetal distress.

Assessment and Management of a Gifted Child Presenting Mental Distress and Disruptive Behaviors: Clinical Report.

This article aims to report on the clinical care of a gifted child in the context of Health and Education. The child showed signs of emotional distress and disruptive behaviors, which were related to the school context. Based on the assessment that revealed this child was gifted, his acceleration by one school year was recommended, which in turn resulted in complete remission of symptoms. This experience exemplifies the importance of identifying giftedness in childhood as a guide for appropriate conduct, both in cultivating the good development of the child's skills and in promoting their emotional well-being.

Comparison of the effects of in-person and internet-delivered mindfulness-based stress reduction on the burden of psychosomatic symptoms in nurses.

There is some evidence comparing the efficacy of telehealth to in-person mental health care, but there is limited research specifically comparing these modalities in nurses. The study aimed to compare the effects of Mindfulness-based Stress Reduction (MBSR) and Internet-delivered Mindfulness-based Stress Reduction (iMBSR) on burden of psychosomatic symptoms of nurses working at Al-Alhamzeh general hospital, Aldiwaniyeh, Iraq. The study was a semi-experiment study with a pre-posttest design on 72 registered nurses. Subjects were randomly allocated in group A, in-person MBSR and group B, internet-delivered MBSR. Intervention in two groups was held at 8 weekly sessions. The data collection instrument included sociodemographic, Patient Questionnaire Health-15 (PHQ-15), and General Health Questionnaire-12 (GHQ-12). Data were analyzed with SPSS version 24 by descriptive and non-parametric inferential tests. The study found that 50% of the nurses in both groups reported mild somatic symptoms, and 40% reported moderate symptoms, with the majority showing no signs of mental distress. A more significant reduction in GHQ scores compared to PHQ scores was observed from pre-to post-intervention. Notably, the online MBSR group showed a significant decrease in GHQ scores, both between groups (p = 0.04) and within the online MBSR group itself (p = 0.02), highlighting the greater impact of the intervention in this group. The study highlights the positive effects of both in-person and online MBSR interventions on reducing depressive symptoms and improving mental health outcomes among nurses. Online MBSR, in particular, shows promise in addressing medically unexplained symptoms and enhancing mental well-being.

Evaluating the impact of financial worry on mental health: a cross-sectional study among Kenyan radiographers

BackgroundPsychological distress is a major public health concern that has many influencing factors. One of them is the financial capability of an individual. Despite the integral role of radiographers in healthcare delivery, there is limited literature about radiographers in Kenya and more so regarding their mental well-being. A descriptive cross-sectional study was conducted to evaluate the effect of perceived financial worry on mental health among Kenyan radiographers.MethodsData was collected from registered radiographers in Kenya through an online survey conducted between December 2023 and January 2024. Multivariable binary logistic regression was used to evaluate the association between financial worry and psychological distress.ResultsOut of the 2055 radiographers invited to participate in the survey, 245 responded (11.92% response rate). The gender distribution of the respondents was 39.2% female and 60.8% male, closely mirroring the latest data on registered radiographers (37.3% female and 62.7% male). Most respondents in our study were under 30 years old (56.7%). The majority of radiographers (62.4%) reported poor social support. Additionally, most radiographers experienced some level of financial concern across various issues, with an average financial worry score of 16.75 (standard deviation: 4.97; minimum 3, maximum 24). A significant proportion (36.7%) also showed signs of psychological distress. Individuals with higher levels of financial worry were noted to be more likely to experience psychological distress after adjusting for the socioeconomic and health-related variables (adjusted odds ratio, AOR 1.20, 95% confidence interval, CI 1.10–1.31, p < 0.001). Furthermore, individuals with at least moderate social support (AOR: 0.39; 95% CI 0.18–0.86, p = 0.019) and larger families (AOR: 0.11; 95% CI 0.02–0.78, p = 0.027) were less likely to experience psychological distress compared to their peers.ConclusionsA significant number of radiographers reported experiencing financial worry which was associated with an increased likelihood of psychological distress. This emphasizes the need for policies and mechanisms to address financial worry and psychological distress to have a more resilient medical workforce in Kenya.

By utilizing advanced computer vision algorithms, the system analyzes video feeds from cameras to continuously assess students' health, focusing on facial expressions and behaviors. The DeepFace model is employed to detect and recognize individual students and analyze their facial expressions for signs of distress or other health-related issues. When the system identifies concerning health indicators, it generates immediate alerts, which are sent to school administrators through a user-friendly web interface. This interface allows administrators to monitor health analytics, view real-time alerts, and access historical data for effective decision-making and intervention. The proposed framework offers a proactive approach to student health monitoring, ensuring a safer and more supportive school environment. High flow nasal cannula and low level continuous positive airway pressure have different physiological effects during de novo acute hypoxemic respiratory failure

BackgroundLarge tidal volumes during de novo acute hypoxemic respiratory failure (AHRF) may promote patient self-inflicted lung injury. Tidal volume assessment under high flow nasal cannula (HFNC) is not routinely feasible at the bedside. Our objective was to determine whether tidal volume during low-level continuous positive airway pressure (CPAP) could predict tidal volume during HFNC and to compare the physiological effects of HFNC and low-level CPAP.MethodsProspective, single-center study including 29 de novo AHRF patients treated with HFNC (50 to 60 L.min− 1). Patients were monitored using electrical impedance tomography during HFNC then CPAP at 4 cmH2O. Tidal volume during HFNC was calculated based on tidal impedance variation. The ability of tidal volume under low-level CPAP to predict tidal volume under HFNC was explored using Bland-Altman analysis. CPAP and HFNC were compared in terms of tidal volume, minute ventilation, respiratory comfort, dyspnea, oxygenation, ventilation distribution, end-expiratory lung volume, thoraco-abdominal asynchrony and recruitment.ResultsUnder HFNC, patients had a tidal volume of 6.6 (5.9–8.7) mL.kg− 1 PBW. 20 (69%) patients exhibited a tidal volume between 4 and 8 mL.kg− 1 PBW, while in 5 (17%) patients it exceeded 9 mL.kg− 1 PBW. Tidal volume under CPAP was higher (9.4 (8.3–11) mL.kg− 1 PBW, p < 0.001). Tidal volumes under CPAP and under HFNC were modestly correlated (Spearman r = 0.50, p = 0.005). Bland-Altman analysis showed a bias of 2.4 mL.kg− 1, with limits of agreement ranging from − 1.1 mL.kg− 1to 5.9 mL.kg− 1. Nevertheless, a larger (> 11.5 mL.kg− 1 PBW ) tidal volume under low-level CPAP predicted a larger (> 9 mL.kg− 1 PBW ) tidal volume under HFNC with 80% sensitivity and 96% specificity. Low-level CPAP was associated with increased minute ventilation, end-expiratory lung volume, and oxygenation as compared to HFNC. It decreased signs of respiratory distress in the most severe patients but was associated with lower comfort compared to HFNC.ConclusionAmong ICU patients with de novo AHRF, tidal volume under HFNC was mostly protective. Tidal volume during CPAP at 4 cmH2O did not predict tidal volume during HFNC. Such low-level CPAP was associated with increased tidal volume, minute ventilation, end-expiratory volume, and oxygenation.Trial registrationClinicalTrials.gov ID NCT03919331. Echocardiographic evaluation of ventricular performance in VA-ECMO weaning: results from a observational monocentric prospective study

Abstract Background Although temporary mechanical circulatory support (MCS) devices are increasingly used in the management of cardiogenic shock (CS), several open question regarding the kind of support and timing of institution/weaning remains open. Echocardiography is widely applied to drive the weaning process and decision although there are no standardized protocols for the trial nor accordance on the parameters to apply. Purpose We sought to evaluate the overall accuracy of echocardiographic indices in predicting successful weaning from VA ECMO in patients with CS. Specifically, we tested total isovolumic time (tIVT), an index of ventricular performance, and longitudinal function (MAPSE) on top of the commonly used parameters (left ventricular ejection fraction, LVEF; left ventricular velocity time integral, LV VTI; TAPSE; right ventricular S’ at tissue Doppler imaging, RV S’). Methods Monocentric prospective study on adults receiving VA-ECMO due to refractory CS and/or cardiac arrest (CA) from 2012 to 2020. Echocardiography was performed at VA-ECMO placement and during the weaning trials. Weaning trials were performed daily, starting 48 hours after VA-ECMO institution, when patients were hemodynamically stable: mean arterial pressure &amp;gt;65 mmHg, no or minimum adrenergic support (vasoactive inotropic score &amp;lt;10), heart rate &amp;lt;110 bpm, lactate &amp;lt;2 mmol/L, and no signs of respiratory distress. Weaning trials were performed with a stepwise decrease of VA-ECMO blood flow until reaching 1 or 1.5 L/min under adequate anticoagulation. Successful weaning was defined as removal of VA-ECMO with no requirement for further MCS in the following 30 days. Results 76 patients were enrolled (23.8% female, 54.2±10.6 years-old), 52.6% due to CA. Median duration of VA-ECMO was 58 hours; 47 (61.8%) had intra-aortic balloon pump (IABP) inserted upfront. In-hospital mortality was 62.4% overall, 40% for CS without CA. 6 of the 32 weaned patients died for multiorgan failure due to septic shock. The baseline echocardiographic parameters associated with successful weaning were (table): stroke volume &amp;gt;13.9 mL (p 0.002), tIVT &amp;gt;23.6 min/sec (p 0.053), LV VTI &amp;gt;6.6 cm (p&amp;lt;0.001), MAPSE &amp;gt;6.15 mm (p 0.001), TAPSE &amp;gt;11 mm (p 0.023), RV S’ &amp;gt;10 cm/s (p 0.012). LVEF did not reach the statistical significance. Figure shows Kaplan-Meier analysis on echocardiographic indices collected at time of cannulation (panel A) and during the last weaning trial (panel B). Conclusions Echocardiography is pivotal in defining cardiac recovery and monitoring the weaning trial. Indices reflecting ventricular performance and delivered flow from LV were associated with weaning success both at VA-ECMO institution and throughout the weaning trials. Considering the known limitations of LVEF (heart rate, loading and left ventricular dyssynchrony dependence), tIVT and MAPSE should be introduced in the standard evaluation of patients in the acute setting.TableFigure

Depression, anxiety, psychological distress, and perceived social support among Ugandan palliative care providers during the COVID-19 pandemic.

To identify the prevalence of depression, anxiety, and psychosocial distress among Ugandan palliative care providers during the COVID-19 pandemic, measure providers' perceived levels of social support, and identify factors affecting a provider's likelihood of being depressed, anxious, distressed, or perceiving various levels of social support. Data was collected from 123 palliative care providers using an online survey. Depression, anxiety, and psychological distress were measured using the Patient Health Questionnaire-9, the Generalized Anxiety Disorder-7, and the Self-Reporting Questionnaire, respectively. Information on perceived level of social support was gathered through the Multidimensional Scale of Perceived Social Support. The survey also asked about mental health resources available to providers at their place of work and what resources are still needed. Participants ranged in age, gender, religion, marital status, clinical position, and years of experience in palliative care. Results indicate that 20% of respondents show signs of moderate to severe depression, 14% show signs of moderate to severe anxiety, and 33% show signs of psychological distress. Additionally, 50% of respondents reported a low total level of social support. Depression, anxiety, and psychological distress scores were all negatively correlated with perceived social support scores. Over 50% expressed a desire for additional mental health resources at their place of work. In 2021-2022, the severity of depression, anxiety, and psychological distress varied among palliative care providers in Uganda, with some experiencing moderate to severe mental health effects. Higher degrees of depression, anxiety, and psychological distress were correlated with lower levels of perceived social support, highlighting the importance of social support during times of crisis. The results highlight a desire for improved access to mental health resources and will help providers and organizations provide better support and better prepare for future crises.

Neonatal tetanus in South Sudan: a case series

Introduction: Neonatal tetanus is a life-threatening disease of public health importance; it is yet to be eliminated and is still occurring in South Sudan. It is caused by a neurotoxin from a bacterium Clostridium tetani whose spores exist in the environment. Compared to high-income countries, most low-income countries lack intensive care units, and magnesium sulfate shown to improve neonatal tetanus outcomes. Objectives: We aimed to determine the outcome of neonatal tetanus in Al Sabah Children’s Hospital, Juba, South Sudan. Methods: We conducted a case series study in Al Sabah Children’s Hospital in Juba, South Sudan. It described three neonates who presented within the first seven days of life with a history of excessive crying, inability to breastfeed, and tetanic muscle spasms when stimulated. They had signs of respiratory distress, fever, and labile heart rates. We made a diagnosis of neonatal tetanus with autonomic dysfunction, and started them on phenobarbitone because diazepam was not available, oxygen, and antibiotics. However, tetanus immune globulin and anti-tetanus serum were not available. Results: Unfortunately, all three neonates died within 24 hours of admission. Conclusion: Our primary focus as a nation is to improve access to quality health services and the prevention of neonatal tetanus through encouraging appropriate antenatal care, facility delivery, clean delivery and healthy umblical cord care practices, and tetanus vaccinations in teenage girls and young adults. When prevention fails, there is a need for neonatal intensive care with recommended medicines to significantly decrease mortality in this preventable tragic illness. The purpose of this research is to address the challenges faced by visually impaired individuals, particularly in handling household appliances independently. With approximately 285 million visually impaired individuals worldwide, technological solutions are crucial to enhancing their accessibility and independence. This paper introduces a Smart Assistance System designed to empower visually impaired individuals to interact with household appliances in real-time without assistance. In this study, three Convolutional Neural Network (CNN) algorithms are compared to develop the system. The evaluation metrics include accuracy, precision, recall, F1 score, and hamming loss on validation images. The performance comparison reveals that the custom architecture CNN, MobileNetv2, and YOLO models achieve F1 scores of 0.43, 0.63, and 0.24, respectively. To enhance object detection and classification, the paper suggests implementing bounding box buttons categorization using YOLOv8, which demonstrates superior performance with a 95% classification accuracy on testing images of home appliance buttons. They face similar difficult while in public and accessing public property. Expanding upon the proposed system’s capabilities, the paper introduces the concept of panic button detection and activation in a bus environment tailored for blind individuals. This system relies on various factors such as the number of people onboard, heart rate monitoring, and the detection of distress signals or SOS sounds emitted by the user. By integrating advanced sensing technologies and intelligent algorithms, this panic button detection system aims to provide prompt assistance and ensure the safety of visually impaired passengers in public transportation settings.

Clinical Profile and Outcome of Bronchiolitis in Children With 1-24 Months of Age.

Introduction Bronchiolitis poses a significant challenge in pediatric critical care. It is an acute illness affecting the lower respiratory tract in children under the age of two. The most common cause of bronchiolitis is the seasonal respiratory syncytial virus, with influenza and adenovirus also notable contributors. It is characterized by various clinical symptoms and indicators, such as an upper respiratory prodrome, increased respiratory effort, and wheezing in younger children under two years old.This study primarily examines the clinical profile, risk factors, severity, and outcomes of bronchiolitis in children under two years, excluding the neonatal age group. Materials and methods Children under two years of age who presented with upper respiratory symptoms and their first episode of wheezing were evaluated. Those with pre-existing systemic conditions such as cardiac, respiratory, or immunodeficiency disorders were excluded. A detailed history was gathered using a questionnaire, and risk factors were analyzed. The severity of the condition was measured using the Wood-Downes-Ferres score. Data analysis was performed using IBM SPSS Statistics for Windows, Version 26.0 (Released 2019; IBM Corp., Armonk, New York, United States). The relationship between risk factors, severity, and outcomes was examined using the chi-squared test. A two-sided probability of p<0.05 was considered statistically significant for all tests. Results Among 54 children aged 1-24 months with bronchiolitis, the average age was 10.18 months, with a standard deviation of 4.8 months. The severity of the condition was greater in younger children (1-12 months) and tended to decrease with age. Bronchiolitis was more common in males (33 cases) than females (21 cases). Approximately 50 children (92.6%) exhibited signs of respiratory distress, and 45 children (83.3%) showed cough as an initial symptom. Severity was notably higher in children with a history of irritability, which was statistically significant (chi-squared value: 8.169; p-value: 0.017). Only 16 children (29.65%) had a history of poor feeding. Bronchiolitis was more prevalent among infants with a birth weight under 1500 grams (63%). Non-exclusive breastfeeding and early bottle feeding were significant risk factors for bronchiolitis and its severity (chi-squared values: 18.794; p-value: 0.000 and 7.795; p-value: 0.020, respectively). Only two children (3.7%) had been exposed to passive smoke, and the severity was slightly higher in these cases. There was also a statistically significant association between socioeconomic status and bronchiolitis (chi-squared value: 11.917; p-value: 0.018). Conclusion This study aims to raise awareness among parents and clinicians about the high-risk age group for bronchiolitis, its typical presentations, and predictors of severity. It underscores the impact of both biological and environmental risk factors, such as bottle feeding, non-exclusive breastfeeding, and socioeconomic status, on the severity of the condition.
