The Power of Speech in the Wild

Mobile phone sensing is increasingly being used in clinical research studies to assess a variety of mental health conditions (e.g., depression, psychosis). However, in-the-wild speech analysis -- beyond conversation detecting -- is a missing component of these mobile sensing platforms and studies. We augment an existing mobile sensing platform with a daily voice diary to assess and predict the severity of auditory verbal hallucinations (i.e., hearing sounds or voices in the absence of any speaker), a condition that affects people with and without psychiatric or neurological diagnoses. We collect 4809 audio diaries from N=384 subjects over a one-month-long study period. We investigate the performance of various deep-learning architectures using different combinations of sensor behavioral streams (e.g., voice, sleep, mobility, phone usage, etc.) and show the discriminative power of solely using audio recordings of speech as well as automatically generated transcripts of the recordings; specifically, our deep learning model achieves a weighted f-1 score of 0.78 solely from daily voice diaries. Our results surprisingly indicate that a simple periodic voice diary combined with deep learning is sufficient enough of a signal to assess complex psychiatric symptoms (e.g., auditory verbal hallucinations) collected from people in the wild as they go about their daily lives.

Coping Strategy Enhancement for Auditory Verbal Hallucinations Within Routine Clinical Practice.

Auditory verbal hallucinations (AVH) are often multiple distressing experiences. Emerging evidence suggests that interventions informed by the principles of cognitive behavioral therapy, such as brief Cognitive Strategy Enhancement (brief-CSE), can reduce the distress related to AVH. The benefits of brief-CSE have been demonstrated for English-speaking patients. This uncontrolled pilot study, conducted in routine clinical practice, evaluated the benefits of brief-CSE within a group of French-speaking AVH hearers. Thirty-two patients were offered the brief-CSE intervention. Self-administered questionnaires were completed pre-post intervention. A significant reduction was observed in AVH distress, with a large effect size, and for more than half of the patients, this reduction was clinically meaningful. AVH severity and anxiety also decreased significantly. This study demonstrates that brief-CSE can be implemented in non-English-speaking routine clinical practice and can reduce several aspects of AVH subjective experience. There is a need to confirm these findings in a larger sample.

Local Alterations of Left Arcuate Fasciculus and Transcallosal White Matter Microstructure in Schizophrenia Patients with Medication-resistant Auditory Verbal Hallucinations: A Pilot Study

Auditory verbal hallucinations (AVH) in schizophrenia (SZ) have been associated with abnormalities of the left arcuate fasciculus and transcallosal white matter projections linking homologous language areas of both hemispheres. While most studies have used a whole-tract approach, here we focused on analyzing local alterations of the above-mentioned pathways in SZ patients suffering medication-resistant AVH. Fractional anisotropy (FA) was estimated along the left arcuate fasciculus and interhemispheric projections of the rostral and caudal corpus callosum. Then, potential associations between white matter tracts and SZ symptoms were explored by correlating local site-by-site FA values and AVH severity estimated via the Auditory Hallucinations Rating Scale (AHRS). Compared to a sample of healthy controls, SZ patients displayed lower FA values in the rostral portion of the left arcuate fasciculus, near the frontal operculum, and in the left and right lateral regions of the rostral portion of the transcallosal pathways. In contrast, SZ patients showed higher FA values than healthy controls in the medial portion of the latter transcallosal pathway and in the midsagittal section of the interhemispheric auditory pathway. Finally, significant correlations were found between local FA values in the left arcuate fasciculus and the severity of the AVH’s attentional salience. Contributing to the study of associations between local white matter alterations of language networks and SZ symptoms, our findings highlight local alterations of white matter integrity in these pathways linking language areas in SZ patients with AVH. We also hypothesize a link between the left arcuate fasciculus and the attentional capture of AVH.

Pilot-RCT Finds No Evidence for Modulation of Neuronal Networks of Auditory Hallucinations by Transcranial Direct Current Stimulation.

Background: Transcranial direct current stimulation (tDCS) is used as treatment for auditory verbal hallucinations (AVH). The theory behind the treatment is that tDCS increases activity in prefrontal cognitive control areas, which are assumed to be hypoactive, and simultaneously decreases activity in temporal speech perception areas, which are assumed to be hyperactive during AVH. We tested this hypofrontal/hypertemporal reversal theory by investigating anatomical, neurotransmitter, brain activity, and network connectivity changes over the course of tDCS treatment. Methods: A double-blind, randomized controlled trial was conducted with 21 patients receiving either sham or real tDCS treatment (2 mA) twice daily for 5 days. The anode was placed over the left dorsolateral prefrontal cortex (DLPFC) and the cathode over the left temporo-parietal cortex (TPC). Multimodal neuroimaging as well as clinical and neurocognitive functioning assessment were performed before, immediately after, and three months after treatment. Results: We found a small reduction in AVH severity in the real tDCS group, but no corresponding neuroimaging changes in either DLPFCD or TPC. Limitations: The study has a small sample size. Conclusion: The results suggest that the currently leading theory behind tDCS treatment of AVH may need to be revised, if confirmed by studies with larger N. Tentative findings point to the involvement of Broca’s area as a critical structure for tDCS treatment.

The alternations of nucleus accumbent in schizophrenia patients with auditory verbal hallucinations during low-frequency rTMS treatment.

Low-frequency repetitive transcranial magnetic stimulation (rTMS) has been shown to reduce the severity of auditory verbal hallucinations (AVH) and induce beneficial functional and structural alternations of the brain in schizophrenia patients with AVH. The nucleus accumbens (NAcc) as an important component of the ventral striatum is implicated with the pathology in AVH. However, the induced characteristic patterns of NAcc by low-frequency rTMS in schizophrenia with AVH are seldom explored. We investigated the functional and structural characteristic patterns of NAcc by using seed-based functional connectivity (FC) analysis and gray matter volume (GMV) measurement in schizophrenia patients with AVH during 1 Hz rTMS treatment. Although low-frequency rTMS treatment did not affect the volumetric changes of NAcc, the abnormal FC patterns of NAcc, including increased FC of NAcc with the temporal lobes and decreased FC of NAcc with the frontal cortices in the pretreatment patients compared to healthy controls, were normalized or reversed after treatment. These FC changes were associated with improvements in clinical symptoms and neurocognitive functions. Our findings may extend our understanding of the NAcc in the pathology of schizophrenia with AVH and might be a biomarker of clinical effect for low-frequency rTMS treatment in schizophrenia.

Abnormal interhemispheric and intrahemispheric functional connectivity dynamics in drug‐naïve first‐episode schizophrenia patients with auditory verbal hallucinations

Numerous studies indicate altered static local and long‐range functional connectivity of multiple brain regions in schizophrenia patients with auditory verbal hallucinations (AVHs). However, the temporal dynamics of interhemispheric and intrahemispheric functional connectivity patterns remain unknown in schizophrenia patients with AVHs. We analyzed resting‐state functional magnetic resonance imaging data for drug‐naïve first‐episode schizophrenia patients, 50 with AVHs and 50 without AVH (NAVH), and 50 age‐ and sex‐matched healthy controls. Whole‐brain functional connectivity was decomposed into ipsilateral and contralateral parts, and sliding‐window analysis was used to calculate voxel‐wise interhemispheric and intrahemispheric dynamic functional connectivity density (dFCD). Finally, the correlation analysis was performed between abnormal dFCD variance and clinical measures in the AVH and NAVH groups. Compared with the NAVH group and healthy controls, the AVH group showed weaker interhemispheric dFCD variability in the left middle temporal gyrus (p < .01; p < .001), as well as stronger interhemispheric dFCD variability in the right thalamus (p < .001; p < .001) and right inferior temporal gyrus (p < .01; p < .001) and stronger intrahemispheric dFCD variability in the left inferior frontal gyrus (p < .001; p < .01). Moreover, abnormal contralateral dFCD variability of the left middle temporal gyrus correlated with the severity of AVHs in the AVH group (r = −.319, p = .024). The findings demonstrate that abnormal temporal variability of interhemispheric and intrahemispheric dFCD in schizophrenia patients with AVHs mainly focus on the temporal and frontal cortices and thalamus that are pivotal components of auditory and language pathways.

Cingulum bundle integrity in schizophrenia with auditory verbal hallucinations: A diffusion tensor imaging tractographic study.

Cingulum bundle (CB) is frequently implicated in schizophrenia; however, its role in specific symptoms of schizophrenia such as auditory verbal hallucinations (AVHs) is less explored. Few studies have reported association between reduced integrity of CB and severity of AVH. Using a symptom-based approach, this diffusion tensor imaging (DTI) tractographic study was aimed to assess and compare the integrity of CB in schizophrenia with AVH, schizophrenia without AVH and healthy controls. A total of 92 right-handed adult individuals (aged 18-50 yr) were recruited across three study groups. Those with Diagnostic and Statistical Manual of Mental Disorders-5 (DSM-5) diagnosis of schizophrenia with AVH (group I; n=30) were compared to those with DSM-5 schizophrenia without lifetime AVH (group II; n=32) and healthy controls (group III; n=30; screened using Mini International Neuropsychiatric Interview version-7.0.0. and negative family history). Clinical assessments (groups I and II) included scale for assessment of positive symptoms, scale for assessment of negative symptoms, clinical global impression-schizophrenia and psychotic symptom rating scale. All participants underwent DTI, and quantitative tract-based measurements of fractional anisotropy (FA) were obtained for images using DTI studio version-3.0. All groups were comparable for age, gender, education and severity of illness. Group I had significantly lower FA values in the cingulate gyrus (CG) part of the left CB compared to groups II and III. No significant difference was found between groups II and III. The findings of this study suggest that the disruption in the left CB appears to be specific for AVH-positive schizophrenia. The finding is, however, preliminary subject to replication in future studies. Further investigations are needed to understand its relevance in the context of AVH-positive schizophrenia.

A Study of Fractional Amplitude of Low Frequency Fluctuation in Schizophrenia Patients with Persistent Verbal Auditory Hallucinations

Background： Auditory verbal hallucinations (AVHs), one of the hallmark positive symptoms of schizophrenia (SCZ), affect 60-80% of SCZ patients. However, the neuroimaging mechanisms underlying AVHs, particularly persistent AVHs, are currently unknown. The aim of this study was to explore the neuroimaging mechanisms of persistent AVHs by comparing the characteristics of changes in fractional amplitude of low frequency fluctuations (fALFF) values of subjects. Methods： 64 SCZ patients with persistent AVHs, 39 patients with non-AVHs, and 60 healthy controls (HC) underwent a resting-state functional magnetic resonance imaging (MRI) study. The fALFF was used to assess localized spontaneous neural activity in the brain of three groups of subjects. The Positive and Negative Symptoms scale (PANSS) was used to rate the severity of patients' psychiatric symptoms; the P3 item on the PANSS scale was used to assess the severity of persistent AVHs. Results： SCZ patients with persistent AVHs had lower fALFF values in the right lingual gyrus, right postcentral, and left supplementary movement area (SMA) than in the HC, and higher fALFF values in the right medial superior frontal, left inferior parietal lobule, left precuneus, and right supramarginal than in the HC. SCZ patients with non-AVHs had lower fALFF values in the left lingual gyrus, right postcentral, and left supplementary motor area than in the HC group, while higher fALFF values were found in the right medial superior frontal only. However, there was no difference in fALFF values between patients with persistent AVHs and non-AVH patients. Conclusions： These results suggest that local spontaneous neural activity was significantly impaired in the brains of patients in the AVH and non-AVH groups compared to those in HC, and that altering spontaneous neural activity in these brain regions may be of value in the treatment of SCZ.

Negative valence of hallucinatory voices as predictor of cortical glutamatergic metabolite levels in schizophrenia patients.

ObjectivesNegative emotional valence of auditory verbal hallucinations (AVHs) in schizophrenia can be a source of distress and is considered a strong predictor of illness severity. Previous studies have found glutamate to mediate AVH severity in frontal and temporal brain regions, however, they do not specifically address emotional valence of AVH. The role of glutamate for the experience of negative‐ versus positive emotional valence of AVH is therefore unknown and was investigated in the current study.MethodsUsing magnetic resonance spectroscopy (MRS), 37 schizophrenia patients had Glx (glutamate+glutamine) measured in the left superior temporal gyrus (STG), and additionally in the anterior cingulate cortex (ACC) and the right STG, or in the left inferior frontal gyrus (IFG). Self‐reported emotional valence in AVH was measured with the Beliefs About Voices Questionnaire (BAVQ‐R).ResultsResults from linear mixed models showed that negative emotional valence was associated with reduced Glx levels across all four measured brain regions in the frontal and temporal lobe. More specifically, voices that were experienced to be omnipotent (p = 0.04) and that the patients attempted to resist (p = 0.04) were related to lower Glx levels. Follow‐up analysis of the latter showed that voices that evoked emotional resistance (i.e., fear, sadness, anger), rather than behavioral resistance, was a significant predictor of reduced glutamate (p = 0.02).ConclusionThe findings could indicate aberrant glutamatergic signaling, or increased NMDA‐receptor hypoactivity in patients who experience their voices to be more emotionally negative. Overall, the study provides support for the glutamate hypothesis of schizophrenia.

One-year randomized trial comparing virtual reality-assisted therapy to cognitive\u2013behavioral therapy for patients with treatment-resistant schizophrenia

The gold-standard cognitive–behavioral therapy (CBT) for psychosis offers at best modest effects. With advances in technology, virtual reality (VR) therapies for auditory verbal hallucinations (AVH), such as AVATAR therapy (AT) and VR-assisted therapy (VRT), are amid a new wave of relational approaches that may heighten effects. Prior trials have shown greater effects of these therapies on AVH up to a 24-week follow-up. However, no trial has compared them to a recommended active treatment with a 1-year follow-up. We performed a pilot randomized comparative trial evaluating the short- and long-term efficacy of VRT over CBT for patients with treatment-resistant schizophrenia. Patients were randomized to VRT (n = 37) or CBT (n = 37). Clinical assessments were administered before and after each intervention and at follow-up periods up to 12 months. Between and within-group changes in psychiatric symptoms were assessed using linear mixed-effects models. Short-term findings showed that both interventions produced significant improvements in AVH severity and depressive symptoms. Although results did not show a statistically significant superiority of VRT over CBT for AVH, VRT did achieve larger effects particularly on overall AVH (d = 1.080 for VRT and d = 0.555 for CBT). Furthermore, results suggested a superiority of VRT over CBT on affective symptoms. VRT also showed significant results on persecutory beliefs and quality of life. Effects were maintained up to the 1-year follow-up. VRT highlights the future of patient-tailored approaches that may show benefits over generic CBT for voices. A fully powered single-blind randomized controlled trial comparing VRT to CBT is underway.

White Matter Microstructural Differences between Hallucinating and Non-Hallucinating Schizophrenia Spectrum Patients.

The relation between auditory verbal hallucinations (AVH) and white matter has been studied, but results are still inconsistent. This inconsistency may be related to having only a single time-point of AVH assessment in many studies, not capturing that AVH severity fluctuates over time. In the current study, AVH fluctuations were captured by utilizing a longitudinal design and using repeated (Positive and Negative Symptoms Scale) PANSS questionnaire interviews over a 12 month period. We used a Magnetic Resonance Diffusion Tensor Imaging (MR DTI) sequence and tract-based spatial statistics (TBSS) to explore white matter differences between two subtypes of schizophrenia patients; 44 hallucinating (AVH+) and 13 non-hallucinating (AVH-), compared to 13 AVH- matched controls and 44 AVH+ matched controls. Additionally, we tested for hemispheric fractional anisotropy (FA) asymmetry between the groups. Significant widespread FA-value reduction was found in the AVH+ group in comparison to the AVH- group. Although not significant, the extracted FA-values for the control group were in between the two patient groups, for all clusters. We also found a significant difference in FA-asymmetry between the AVH+ and AVH- groups in two clusters, with significantly higher leftward asymmetry in the AVH- group. The current findings suggest a possible qualitative difference in white matter integrity between AVH+ and AVH- patients. Strengths and limitations of the study are discussed.

Exploring concomitant neuroimaging and genetic alterations in patients with and patients without auditory verbal hallucinations: A pilot study and mini review.

ObjectiveTo explore concomitant neuroimaging and genetic alterations in patients with schizophrenia with or without auditory verbal hallucinations (AVHs), and to discuss the use of pattern recognition techniques in the development of an objective index that may improve diagnostic accuracy and treatment outcomes for schizophrenia.MethodsThe pilot study included patients with schizophrenia with AVHs (SCH-AVH group) and without AVHs (SCH-no AVH group). High throughput sequencing (HTS) was performed to explore RNA networks. Global functional connectivity density (gFCD) analysis was performed to assess functional connectivity (FC) alterations of the default mode network (DMN). Quantitative long noncoding (lnc) RNA and mRNA expression data were related to peak T values of gFCDs using Pearson’s correlation coefficient analysis.ResultsCompared with the SCH-no AVH group (n = 5), patients in the SCH-AVH group (n = 5) exhibited differences in RNA expression in RNA networks that were related to AVH severity, and displayed alterations in FC (reflected by gFCD differences) within the DMN (posterior cingulate and dorsal-medial prefrontal cortex), and in the right parietal lobe, left occipital lobe, and left temporal lobe. Peak lncRNA expression values were significantly related to peak gFCD T values within the DMN.ConclusionAmong patients with schizophrenia, there are concomitant FC and genetic expression alterations associated with AVHs. Data from pattern recognition studies may inform the development of an objective index aimed at improving early diagnostic accuracy and treatment planning for patients with schizophrenia with and without AVHs.

Altered Effective Connectivity in Schizophrenic Patients With Auditory Verbal Hallucinations: A Resting-State fMRI Study With Granger Causality Analysis.

PurposeAuditory verbal hallucinations (AVH) are among the most common and prominent symptoms of schizophrenia. Although abnormal functional connectivity associated with AVH has been reported in multiple regions, the changes in information flow remain unclear. In this study, we aimed to elucidate causal influences related to AVH in key regions of auditory, language, and memory networks, by using Granger causality analysis (GCA).Patients and MethodsEighteen patients with schizophrenia with AVH and eighteen matched patients without AVH who received resting-state fMRI scans were enrolled in the study. The bilateral superior temporal gyrus (STG), Broca’s area, Wernicke’s area, putamen, and hippocampus were selected as regions of interest.ResultsGranger causality (GC) increased from Broca’s area to the left STG, and decreased from the right homolog of Wernicke’s area to the right homolog of Broca’s area, and from the right STG to the right hippocampus in the AVH group compared with the non-AVH group. Correlation analysis showed that the normalized GC ratios from the left STG to Broca’s area, from the left STG to the right homolog of Broca’s area, and from the right STG to the right homolog of Broca’s area were negatively correlated with severity of AVH, and the normalized GC ratios from Broca’s area to the left hippocampus and from Broca’s area to the right STG were positively correlated with severity of AVH.ConclusionOur findings indicate a causal influence of pivotal regions involving the auditory, language, and memory networks in schizophrenia with AVH, which provide a deeper understanding of the neural mechanisms underlying AVH.

T134. THE ROLE OF THE DEFAULT MODE NETWORK IN SCHIZOPHRENIA AND AUDITORY VERBAL HALLUCINATIONS – AN INVESTIGATION OF DYNAMIC FMRI RESTING STATE CONNECTIVITY

BackgroundThere is a wealth of evidence showing aberrant functional connectivity (FC) in schizophrenia but with considerable variability in findings across studies. Dynamic FC is an extension of traditional static FC, in that such analyses allow for explorations of temporal changes in connectivity. Thereby they also provide more detailed information on connectivity abnormalities in psychiatric disorders such as schizophrenia.MethodsThe current study investigated dynamic FC in a sample of 80 schizophrenia patients and 80 matched healthy control subjects. Furthermore, relationships with auditory verbal hallucinations (AVH), a core symptom of schizophrenia, were explored. Two measures of AVH were used, one measure of current AVH severity assessed on the day of scanning, and one trait-measure where AVH were assessed repeatedly over the course of one year.ResultsCompared to healthy controls, schizophrenia patients showed increased dwell times in states with high connectivity within the default mode network (DMN). Current AVH severity did not show a significant relationship with dynamic FC. However, the trait-measure of AVH proneness over one year showed a significant relationship with dynamic FC. Patients with high AVH proneness spent less time in connectivity states characterized by strong anti-correlation between the DMN and task-positive networks.DiscussionThe results provide further evidence for a DMN dysfunction in schizophrenia, which could be linked to thought disturbances in relation to an increased internal focus of cognitive processing. The effects of AVH proneness on dynamic FC support theoretical models of AVH which have proposed an instability of the DMN and impaired cognitive control in AVH patients. The results also point to AVH proneness as a potential marker for identifying distinct subgroups of schizophrenia patients.

Distinct hemispheric specialization of functional connectivity in schizophrenia with and without auditory verbal hallucinations.

As a basic organizing principle of the human brain, hemispheric specialization is an important perspective to explore the pathology of schizophrenia. However, it remains unclearly whether the hemispheric specialization of functional connectivity plays a role in mediating auditory verbal hallucinations in schizophrenia. In this study, 18 schizophrenic patients with auditory verbal hallucinations, 18 patients without auditory verbal hallucinations, and 18 matched healthy controls underwent resting-state functional MRI scans, and seed-based voxel-wise functional connectivity was calculated to quantify the degree of hemispheric specialization. The results revealed that both the auditory verbal hallucinations and non-auditory verbal hallucinations groups exhibited significantly increased specialization in the left middle temporal gyrus and left precuneus, and significantly reduced specialization in the right precuneus relative to healthy controls, and that the auditory verbal hallucinations severity was significantly correlated with the hemispheric specialization of the right precuneus in the auditory verbal hallucinations group. Moreover, the left frontal lobe exhibited reduced hemispheric specialization in the auditory verbal hallucinations group compared with non-auditory verbal hallucinations group, and the patients with and without auditory verbal hallucinations could be clustered into two groups with an accuracy of 80.6% based on the brain regions exhibiting significant specialization changes. The findings indicate that the hemispheric specialization of the aforementioned regions may play a role in mediating auditory verbal hallucinations in schizophrenia, and the distinct hemispheric specialization patterns of functional connectivity may provide a potential biomarker to differentiate schizophrenic patients with and without auditory verbal hallucinations.

Fronto-temporal transcranial direct current stimulation (tDCS) for auditory verbal hallucination in schizophrenia

Objective Up to 25% of patients with schizophrenia present with auditory verbal hallucinations (AVH) that are refractory to drug treatment. In such cases, evidence suggests that fronto-temporal transcranial direct current stimulation (tDCS) could alleviate AVH but these results were mixed and controversial. Here, we aimed to investigate the clinical effect of tDCS on AVH and its physical effects by measuring injected electric field distribution in the brain of patients. We hypothesized the superiority of active tDCS over sham to decrease AVH and that patients who will respond to tDCS would display greater electric field distribution in the brain areas involved in AVH than non-responders. Method Sixty patients with schizophrenia and daily refractory AVH were randomly allocated to receive either active (2 mA, 20 min) or sham tDCS applied twice daily (5 days). The anode was placed over the left dorsolateral prefrontal cortex and the cathode over the left temporo-parietal cortex. AVH severity was assessed with the Hallucination Change Scale (HCS). A modelling of the injected electrical field based on the MRI of each patient was compared between responders (HCS ≤ 5) and non-responders. Results Active tDCS can significantly decrease AVH [F(1,54) = 4.52; P = 0.03] as compared to sham. The effect was significant immediately after the tDCS, remain significant 1 month after the tDCS but did not reach significance 3 months later. At baseline, patients of the active group who will respond to tDCS displayed higher electric field strength in the left superior temporal gyrus (STG) than non-responders. Conclusion Fronto-temporal tDCS may alleviate treatment-resistant AVH. High distribution of the electric field within the left STG was observed in responders.

Brain function differences in drug-naïve first-episode auditory verbal hallucination-schizophrenia patients with versus without insight.

Background:Few studies have reported brain function differences in drug-naïve first-episode schizophrenia patients who had auditory verbal hallucinations (AVH) with insight vs. those without insight. This study aimed to investigate brain function differences between drug-naïve first-episode AVH-schizophrenia patients with and without insight.Methods:Forty first-episode drug-naïve AVH-schizophrenia patients with or without insight and 40 healthy controls between December 2016 and December 2018 were recruited in this study. The auditory hallucinations rating scale (AHRS) was used to assess AVH severity, while the insight and treatment attitudes questionnaire was used to distinguish insight. The global functional connectivity density (gFCD) between different groups was compared using a voxel-wise one-way analysis of covariance. The relationship between gFCD and AHRS total scores were analyzed using voxel-wise multiple regression.Results:Finally, 13 first-episode drug-naïve AVH-schizophrenia patients with insight, 15 AVH-schizophrenia patients without insight, and 20 healthy controls were included for analysis. Except for global assessment of functioning scores, there were no significant differences in sociodemographic information between the AVH-schizophrenia and healthy groups (P > 0.05). Compared to the healthy controls, AVH-schizophrenia patients with insight demonstrated a decreased gFCD in the supra-marginal gyrus within the primary auditory cortex, while those without insight demonstrated an increased gFCD in the inferior frontal gyrus and superior temporal gyrus and decreased gFCD in the supplemental motor area. Compared to the AVH-schizophrenia patients with insight, those without insight demonstrated an increased gFCD in the supra-marginal gyrus and posterior superior temporal lobule and a decreased gFCD in the frontal lobe. No significant correlation between gFCD and AVH severity (AHRS total score: r = 0.23, P = 0.590; and frequency: r = 0.42, P = 0.820) was found in both AVH-schizophrenia groups.Conclusions:The gFCD-aberrant brain regions in the AVH-schizophrenia patients without insight were wider compared to those with insight, although the AHRS scores were not significantly different. The AVH-schizophrenia patients without insight had wide functional impairment in the frontal lobule, which may underlie the lack of insight and the abnormal hyperactivity in the inferior frontal gurus and temporal lobe related to the AVH symptoms.

Self-voice perception and its relationship with hallucination predisposition

ABSTRACTIntroduction: Auditory verbal hallucinations (AVH) are a core symptom of psychotic disorders such as schizophrenia but are also reported in 10–15% of the general population. Impairments in self-voice recognition are frequently reported in schizophrenia and associated with the severity of AVH, particularly when the self-voice has a negative quality. However, whether self-voice processing is also affected in nonclinical voice hearers remains to be specified.Methods: Thirty-five nonclinical participants varying in hallucination predisposition based on the Launay-Slade Hallucination Scale, listened to prerecorded words and vocalisations differing in identity (self/other) and emotional quality. In Experiment 1, participants indicated whether words were spoken in their own voice, another voice, or whether they were unsure (recognition task). They were also asked whether pairs of words/vocalisations were uttered by the same or by a different speaker (discrimination task). In Experiment 2, participants judged the emotional quality of the words/vocalisations.Results: In Experiment 1, hallucination predisposition affected voice discrimination and recognition, irrespective of stimulus valence. Hallucination predisposition did not affect the evaluation of the emotional valence of words/vocalisations (Experiment 2).Conclusions: These findings suggest that nonclinical participants with high HP experience altered voice identity processing, whereas HP does not affect the perception of vocal emotion. Specific alterations in self-voice perception in clinical and nonclinical voice hearers may establish a core feature of the psychosis continuum.

Gray matter abnormalities in language processing areas and their associations with verbal ability and positive symptoms in first-episode patients with schizophrenia spectrum psychosis.

Impaired verbal communication is a prominent feature in patients with schizophrenia. Verbal communication difficulties adversely affect psychosocial outcomes and worsen schizophrenia's clinical manifestation. In the present study, we aimed to investigate associations among gray matter (GM) volumes in language processing areas (LPAs), verbal ability, and positive symptoms in first-episode patients (FEPs) with schizophrenia spectrum psychosis. We enrolled 94 FEPs and 52 healthy controls (HCs) and subjected them to structural magnetic resonance imaging. The GM volumes of the bilateral pars opercularis (POp), pars triangularis (PTr), planum temporale (PT), Heschl's gyrus (HG), insula, and fusiform gyrus (FG), were estimated and compared between the FEPs and HCs. Verbal intelligence levels and positive symptom severity were examined for correlations with the left LPA volumes. The GM volumes of the left POp, HG, and FG were significantly smaller in the FEPs than in the HCs, while the right regions showed no significant between-group difference. A multiple linear regression model revealed that larger left PT volume was associated with better verbal intelligence in FEPs. In exploratory correlation analysis, several LPAs showed significant correlations with the severity of positive symptoms in FEPs. The left FG volume had a strong inverse correlation with the severity of auditory verbal hallucinations, while the left PT volume was inversely associated with the severity of positive formal thought disorder and delusions. Moreover, the volume of the left insula was positively associated with the severity of bizarre behavior. The present study suggests that GM abnormalities in the LPAs, which can be detected during the early stage of illness, may underlie impaired verbal communication and positive symptoms in patients with schizophrenia spectrum psychosis.

The Effects of Add-on Fronto-Temporal Transcranial Direct Current Stimulation (tDCS) on Auditory Verbal Hallucinations, Other Psychopathological Symptoms, and Insight in Schizophrenia: A Randomized, Double-Blind, Sham-Controlled Trial.

BackgroundThe efficacy of fronto-temporal transcranial direct current stimulation in treating auditory verbal hallucinations and other psychopathological symptoms of schizophrenia patients has been examined in a small number of clinical trials with limited sample sizes, but the results are mixed. Fronto-temporal transcranial direct current stimulation has also been demonstrated to enhance patients’ insight into their mental illness in an open-label pilot study. The current investigation aimed to investigate the therapeutic effects of fronto-temporal transcranial direct current stimulation on the severity of auditory verbal hallucinations, other schizophrenia symptoms, and insight in a large double blind, randomized, sham-controlled trial.MethodsSixty patients with medication-refractory auditory verbal hallucinations were randomized over 2 conditions: transcranial direct current stimulation with 2-mA, twice-daily sessions for 5 consecutive days, with anodal stimulation to the left prefrontal cortex and cathodal stimulation to the left temporo-parietal junction, and sham treatment.ResultsFronto-temporal transcranial direct current stimulation failed to cause significant changes in the severity of auditory verbal hallucinations and other schizophrenia symptoms. The levels of insight into illness (effect size=0.511, P<.001) and positive symptoms (effect size=0.781, P<.001) were largely promoted by 5 days of transcranial direct current stimulation relative to sham treatment. The beneficial effects on the 2 insight dimensions remained 1 month after transcranial direct current stimulation.ConclusionsFronto-temporal transcranial direct current stimulation is not more effective for auditory verbal hallucinations and other schizophrenia symptoms than sham treatment. But the results of transcranial direct current stimulation-associated improvement in awareness of illness and positive symptoms show promise and provide a new direction for future research into insight promotion interventions in schizophrenia.
