Radiographic Assessment of the Vesicourethral Anastomosis Directing Early Decatheterization Following Nerve-Sparing Radical Retropubic Prostatectomy

Early decatheterization directed by postoperative gravity cystography in 55 consecutive radical prostatectomy patients is described. The catheter-free status was 22 per cent by postoperative day 8, 62 per cent by postoperative day 11 and 80 per cent by postoperative day 14. Cystograms performed beginning on postoperative day 7 identified 3 groups of patients and dictated their management: 1) no extravasation resulting in immediate catheter removal (36 patients), 2) moderate extravasation requiring repeat cystography leading to decatheterization by postoperative day 15 (9 patients) and 3) severe extravasation necessitating prolonged catheter drainage (8 patients). A decatheterization protocol is presented.

Extravasations of radiographie contrast material in the upper extremity

Five significant extravasations of radiographic contrast material in the upper extremity were treated with early surgical drainage in an attempt to prevent extensive skin necrosis. Four of the five patients underwent operation within 6 hours of the extravasations, and all of these patients had excellent functional and cosmetic results without any evidence of skin necrosis. One patient who had a poor result did not undergo operation until 20 hours after a severe extravasation to the dorsum of the right hand. Most small-volume, less-extensive extravasations can be satisfactorily managed conservatively. Our clinical experience indicates that large subcutaneous extravasations of radiographic contrast material (volume more than 20 ml) should be surgically drained within 6 hours to prevent extensive damage to tissue.

Etiology and treatment of chemotherapeutic agent extravasation injuries: a review.

While extravasation from intravenous (IV) lines is common and usually benign, leakage of certain drugs can cause severe skin ulceration. These ulcerogenic drugs can be conveniently divided into two categories, depending on whether they bind to DNA. Chemotherapeutic agents such as doxorubicin, which bind to DNA, are especially prone to cause severe extravasation skin ulcers. These ulcers are often chronic and progressive. Neither clinical nor experimental studies have shown an antidote to doxorubicin extravasation, which is best prevented by careful technique. If extravasation is suspected, the infusion should be immediately stopped. In the event of extravasation, elevation and ice are the currently recommended treatment. While small ulcerations may on occasion heal, large ulcerations require surgical excision for relief of pain and salvage of underlying tissues.

Adriamycin extravasation: surgical treatment and possible prevention of skin and soft-tissue injuries.

In a material consisting of approximately 6,000 injections of Adriamycin during a 2-year period, eleven moderate and five severe extravasation injuries have been observed. Even a small leakage of the drug may cause a permanent lesion. The most serious injuries were caused by large doses and on the dorsum of the hand. Plastic surgical operations were successfully performed in the severe cases. An active surgical intervention after primary neutralization of the extravasated Adriamycin is recommended.
