Purpose: to determine the severity and dynamics of changes in the concentration of biochemical markers of muscle tissue damage in the blood serum of an experienced ultramarathon runner after overcoming a distance of 165 kilometers.Materials and methods: measurement of serum concentrations of markers of muscle tissue damage (aspartate aminotransferase (AST), alanine aminotransferase (ALT), creatine kinase (CK), creatine kinase MB (CK-MB), myoglobin) in serum before and 16 and 144 hours after ultramarathon in an experienced runner (age 36, height 186 cm, weight 76 kg, body mass index 21.96).Results: 16 hours after the end of the race, extremely high levels of AST, ALT, CK, CK-MB and myoglobin were observed, the combination of which indicates severe rhabdomyolysis. After 144 hours, a decrease in the level of all markers of muscle tissue damage, including myoglobin, was noted, however, they continued to remain significantly higher than the reference values. At the same time, the athlete did not have any significant pathological symptoms, and he continued to train in the recovery mode.Conclusion: ultramarathon races lead to significant muscle damage, which can lead to rhabdomyolysis. Its severity may not correlate with any significant clinical manifestations.

The immediate effects of cervical spine manipulation on pain and biochemical markers in females with acute non-specific mechanical neck pain: a randomized clinical trial

ABSTRACTStudy Design: Randomized clinical trial with pre-test, post-test control group design.Objectives: To examine the immediate effects of cervical spinal manipulation (CSM) on serum concentration of biochemical markers (oxytocin, neurotensin, orexin A, and cortisol).Background: Several studies have found an association between spinal manipulation (SM) and pain perception. However, the mechanism by which SM modulates pain remains undefined.Methods: Twenty-eight female subjects with non-specific mechanical neck pain were randomly assigned to one of two interventions (CSM versus sham CSM). Blood samples were drawn before and immediately after the respective interventions. Oxytocin, neurotensin, orexin A, and cortisol were measured from the blood and serum using the Milliplex Map Magnetic Bead Panel Immunoassay on the Luminex 200 Platform.Results: In the CSM group, there were significant increases in pre- versus post-manipulation mean oxytocin (154.5 ± 60.1 vs. 185.1 ± 75.6, p = .012); neurotensin (116.0 ± 26.5 vs.136.4 ± 34.1, p < . 001); orexin A (52.2 ± 31.1 vs. 73.8 ± 38.8, p < .01) serum concentration; but no significant differences in mean cortisol (p = .052) serum concentration. In the sham group, there were no significant differences in any of the biomarkers (p > .05).Conclusion: The results of the current study suggest that the mechanical stimuli provided through a CSM may modify neuropeptide expression by immediately increasing the serum concentration of nociception-related biomarkers (oxytocin, neurotensin, orexin A, but not cortisol) in the blood of female subjects with non-specific mechanical neck pain.Trial registration: ClinicalTrials.gov identifier: NCT03176654.

Hepatoprotective Effect of &lt;i&gt;Croton macrostachyus&lt;/i&gt; Hochst. Ex Del Leaves Against CCl4-Induced Liver Damage

Croton macrostachyus Hochst is a medicinal plant widely used in Ethiopia for the treatment of dysentery, abdominal pain, hepatitis and jaundice. The aim of this study was to evaluate the hepatoprotective activity of aqueous extract of leaves of C. macrostachyus (AECM) in CCl4-induced hepatotoxicity in rats and mice. Seven groups of animals (n = 6) were used. Distilled water was given to group I; animals in group II were given distilled water along with CCl4 (1 mg/kg) every 72 h for 16 days, third group served as a positive control and received silymarin 100 mg/kg, and for the other groups 100, 200, 400 and 800 mg/kg AECM were given orally once daily for 16 days. Hepatotoxicity was induced by intra-peritoneal administration of 1 ml/kg of CCl4 every 72 h. At the 18th day blood samples were collected from the jugular vein. Serum concentrations of biochemical markers and histopathological analysis of the liver were conducted. Thiopentone-induced sleep time test was also conducted. The crude extract at all doses produced significant hepatoprotection by decreasing the level of aspartate transaminase (AST), alanine transaminase (ALT), total bilirubin (TBIL), direct bilirubin (DBIL) and indirect bilurbin (IBIL). The extract at 100, 200 and 400 mg/kg also produced significant hepatoprotection by decreasing levels of alkaline phosphatase (ALP) as well as produced significant elevation of serum albumin (ALB) level. These results correlated well with the histopathological studies and thiopentone-induced sleeping time assessment. Evaluation of serum biochemical marker concentrations and survival time in dogs with protein-losing enteropathy.

To evaluate serum concentrations of biochemical markers and survival time in dogs with protein-losing enteropathy (PLE). Prospective study. 29 dogs with PLE and 18 dogs with food-responsive diarrhea (FRD). Data regarding serum concentrations of various biochemical markers at the initial evaluation were available for 18 of the 29 dogs with PLE and compared with findings for dogs with FRD. Correlations between biochemical marker concentrations and survival time (interval between time of initial evaluation and death or euthanasia) for dogs with PLE were evaluated. Serum C-reactive protein concentration was high in 13 of 18 dogs with PLE and in 2 of 18 dogs with FRD. Serum concentration of canine pancreatic lipase immunoreactivity was high in 3 dogs with PLE but within the reference interval in all dogs with FRD. Serum α1-proteinase inhibitor concentration was less than the lower reference limit in 9 dogs with PLE and 1 dog with FRD. Compared with findings in dogs with FRD, values of those 3 variables in dogs with PLE were significantly different. Serum calprotectin (measured by radioimmunoassay and ELISA) and S100A12 concentrations were high but did not differ significantly between groups. Seventeen of the 29 dogs with PLE were euthanized owing to this disease; median survival time was 67 days (range, 2 to 2,551 days). Serum C-reactive protein, canine pancreatic lipase immunoreactivity, and α1-proteinase inhibitor concentrations differed significantly between dogs with PLE and FRD. Most initial biomarker concentrations were not predictive of survival time in dogs with PLE.

Biochemical markers of bone resorption are present in human milk: implications for maternal and neonatal bone metabolism.

This study investigated breast milk and maternal serum concentrations of biochemical markers of bone resorption, which may be implicated in both maternal and neonatal bone metabolism. Tests were carried out on 85 parturients 3-4 days after they gave birth. We measured their breast milk and serum concentrations for soluble receptor activator of nuclear factor kappaB ligand (sRANKL) and cross-linked N-telopeptide of type I collagen (NTx). The sRANKL and NTx concentrations were associated with several perinatal parameters. Soluble receptor activator of nuclear factor kappaB ligand was detectable in breast milk at considerably lower concentrations than in maternal serum (p < 0.001), and these breast milk sRANKL concentrations were decreased in maternal diabetes (b = -0.366, 95% CI -0.622 to -0.110, p = 0.006). Breast milk NTx concentrations were higher in exclusive lactation (b = 0.269, 95% CI 0.014-0.524, p = 0.039), but lower in Caesarean sections (b = -0.224, 95% CI -0.428 to -0.019, p = 0.032). Soluble receptor activator of nuclear factor kappaB ligand is downregulated in breast milk, particularly in the case of diabetes. Breast milk NTx upregulation characterises exclusive lactation, and its downregulation characterises Caesarean section deliveries. Nutritional interventions in foetal life and early infancy may programme adult bone health and ameliorate diseases with developmental origins, such as osteoporosis.

Age-Related Biomarkers Can be Modulated by Diet in the Rat

This study seeks to establish the normal serum concentrations of biochemical markers related to nutrition, inflammation and disease, and to investigate how the levels change with age and diet in the rat. To this end, we fed rats from weaning on three diets differing in their protein, carbohydrate and fatty acid content. The diets consisted of a control, nutritionally balanced diet, this same diet supplemented with 10% (wt/wt) beef tallow, and a diet that was high in fat and carbohydrate and low in protein. Blood samples from rats at two different ages, 3 months and 12 months, were then analysed. In control rats, with advancing age there was a general decrease in potassium, iron and serum albumin concentrations and in the activities of aspartate aminotransferase and alanine aminotransferase, and an increase in total and HDL cholesterol. These changes were modulated by diet: many of the age-related changes (serum concentrations of potassium, iron and cholesterol, and liver enzyme activities) were not observed in animals eating the high fat diet. In contrast, the high carbohydrate, high fat, low protein diet-fed animals showed several additional changes (serum concentrations of sodium, urea, creatinine and TG, and activity of alkaline phosphatase) that can be related to kidney, liver and cardiovascular health.

Abstract B8: Correlation of the expression of hepcidin mRNA and the serum iron, ferritin, and TIBC levels in hepatocellular carcinoma

Abstract Background: The present study correlated the expression of hepcidin mRNA and the levels of serum iron, ferritin, and TIBC in hepatocellular carcinoma (HCC). Methods: Samples of cancerous and noncancerous liver tissue were taken from patients with HCC who underwent hepatectomy. Expression of hepcidin mRNA was evaluated by realtime PCR, and compared in tumors differing in their degree of differentiation and number of tumors. Correlations between hepcidin expression and the serum concentration of hepcidin were evaluated, together with the expression of the serum concentration of biochemical markers of iron metabolism. Results: Hepcidin mRNA expression in non-cancerous and cancerous tissues was 1791.6 (34.3- 25187.4) and 58.7 (1.93235.8), respectively (P &amp;lt; 0.0001). There were no significant differences in hepcidin expression among tumors differing in their degree of differentiation and number of tumors. The serum concentration of hepcidin did not correlate with hepcidin-mRNA expression. The serum iron level was found to be normal (125.4 ± 21.4 mg/dL). The serum ferritin level was also found to be normal 193.5(14.0-232.9 ng/mL). TIBC was (284.8 ±28.3 ng/mL). There were significant correlations between the serum levels of hepcidin and iron, hepcidin-25 and ferritin and hepcidin and TIBC. Conclusion: The serum hepcidin concentration was correlated with the levels of serum iron and ferritin, but not with the level of hepcidin mRNA expression in either cancerous or non- cancerous liver tissue. Hepcidin is produced in patients with HCC, from non-cancerous liver tissue, even though production is inhibited in cancerous tissue. Changes of OPG and RANKL concentrations in Crohnʼs disease after infliximab therapy

Osteoporosis is a well-known complication of Crohn's disease (CD). Osteoprotegerin (OPG) concentration is elevated in patients with CD compared to healthy controls. Long-term infliximab (IFX) maintenance therapy improves the patients' bone mineral density. The effect of IFX on bone metabolism has not yet been clarified. Our aim was to evaluate IFX effects on bone pathology in CD patients. Twenty-nine patients were treated with IFX as an induction therapy according to international guidelines at weeks 0, 2, and 6. Serum concentrations of biochemical markers of bone formation (osteocalcin, OC) and bone resorption (beta-crosslaps, bCL), and serum concentrations of OPG and receptor activator of nuclear factor kappa B ligand (sRANKL) were measured before every treatment at days 1, 14, and 42. Serum levels of OC and sRANKL increased after treatment. OC concentrations were 28.93 +/- 14.95 ng/mL versus 36.33 +/- 20.05 ng/mL (P < 0.005) at days 1 and 42, respectively; sRANKL concentrations were elevated from 0.0112 +/- 0.028 ng/mL to 0.0411 +/- 0.123 ng/mL (NS) by the end of the study. The concentrations of both bCL and OPG decreased. bCL concentrations were 0.636 +/- 0.594 versus 0.519 +/- 0.235 (NS) at days 1 and 42, respectively, while OPG concentration decreased from 3.739 +/- 1.485 to 3.491 +/- 1.618 (P < 0,05). IFX therapy decreased the OPG concentration in CD patients significantly. In parallel, the serum bone resorption marker (bCL) also decreased. Concentrations of bone formation marker (OC) and sRANKL increased during the same period; however, those changes were not statistically significant. Elevated OPG in CD could be a counter-regulatory response to inflammatory cytokines or may reflect T-cell activation.

Effect of sodium valproate monotherapy on serum uric acid concentrations in ambulatory epileptic children: A prospective long-term study

Purpose Hyperuricemia has been shown to be related to cardiovascular morbidity and mortality. There is controversial data about the effect of sodium valproate (VPA) monotherapy on serum uric acid concentrations. The purpose of this study was to investigate by a long-term, prospective method, whether treatment with VPA monotherapy may alter serum uric acid concentrations and liver function tests in ambulatory epileptic children. Material and methods Serum uric acid concentrations were determined in 28 ambulatory epileptic children before and at 6, 12 and 24 months of VPA monotherapy. Serum concentrations of biochemical markers of liver and renal function, such as alanine aminotransferase (ALT), aspartate aminotransferase (AST), lactate dehydrogenase (LDH), gamma-glutamyltransferase ( γ-GT) and creatinine (Cr) were also measured before and at 6, 12 and 24 months of VPA monotherapy. Serum VPA concentrations remained within the therapeutic range (50–100 mg/L) during the period of study. Results No statistically significant changes in serum uric acid concentrations were found at 6, 12 or 24 months of treatment. Serum ALT concentrations were significantly increased at 6 and 12 months of treatment, AST concentrations at 6 and 12 months of treatment and LDH concentrations at 12 months of treatment. Conclusions VPA monotherapy does not have a significant effect on serum uric acid concentrations in ambulatory epileptic children. Further studies are needed to definitively address whether it would be useful for physicians to routinely check for elevated serum uric acid levels in children treated with VPA.

Early and persistent increase in serum lipoprotein (a) concentrations in epileptic children treated with carbamazepine and sodium valproate monotherapy

Purpose The aim of this study was to investigate by a prospective, self-controlled method, whether treatment with carbamazepine (CBZ) and sodium valproate (VPA) monotherapy may alter serum lipoprotein (a) [Lp(a)] concentrations in epileptic children. Methods Serum Lp(a) concentrations have been determined in 18 epileptic children before and at 6, 12 and 24 months of treatment with CBZ monotherapy and in 30 epileptic children before and at 6, 12 and 24 months of treatment with VPA monotherapy. Serum total cholesterol, triglycerides, high-density lipoprotein cholesterol, low-density lipoprotein cholesterol, apolipoproteins A-I and B concentrations and serum concentrations of biochemical markers of liver and renal function were also measured in the study participants. Results Serum Lp(a) concentrations were significantly increased at 6, 12 and 24 months of CBZ and VPA monotherapy. There were no significant correlations between serum Lp(a) and serum lipids, lipoproteins, apolipoproteins, concentrations of biochemical markers of liver and renal function or antiepileptic-drugs concentrations. Conclusions Children who receive CBZ or VPA monotherapy may have significant and persistent increase in serum lipoprotein (a) concentrations, occuring early in the course of therapy. It may be useful to measure serum Lp(a) concentrations routinely in epileptic children taking these antiepileptic drugs, especially in those that are already at higher atherosclerotic risk.

Evaluation of serum concentrations of biochemical markers of bone metabolism and insulin-like growth factor I associated with treadmill exercise in young horses

To evaluate changes in serum concentrations of biochemical markers of bone metabolism and insulin-like growth factor I (IGF-I) associated with treadmill exercise in young horses. 12 two-year-old Thoroughbred mares. During a 20-week study period, 6 horses were exercised on a treadmill 3 times a week (exercise group) and 6 horses received walking exercise 6 days a week (controls). Serum concentrations or activity of biochemical markers and IGF-I were assessed biweekly. Bone mineral density and content of the first phalanx were measured by dual-energy X-ray absorbiometry (DEXA) on completion of the study. Compared with values in controls, bone mineral density and content were higher and serum concentrations of osteocalcin (a marker of bone formation) and the carboxy-terminal telopeptide of type I collagen (a marker of bone resorption; ICTP) were lower in exercised horses. Serum concentration and activity of the bone formation markers carboxy-terminal propeptide of type I collagen and bone-specific alkaline phosphatase (BAP) were not different between the 2 groups. Serum IGF-I concentration was lower in the exercise group, compared with control values; there was a significant correlation between change in IGF-I values and changes in osteocalcin, ICTP, and BAP values at the end of the study. Treadmill exercise over 20 weeks induced adaptive changes in bones of 2-year-old Thoroughbreds; training appears to increase bone mineral density, thereby enhancing mechanical strength of bone, but decreases bone turnover. Results indicated an association between changes in serum IGF-I concentration and bone cell activity in horses.

Calcium supplementation provides an extended window of opportunity for bone mass accretion after menarche

High calcium intakes during adolescence may increase bone acquisition. The magnitude of the effect of dietary calcium supplementation and the timing of its administration to achieve significant effects on bone health are still incompletely defined. The objective of this study was to assess the effect of calcium supplementation on bone mass accretion in postmenarcheal adolescent girls with low calcium intakes. A double-blind, placebo-controlled calcium supplementation study was implemented. One hundred girls with a mean (+/- SD) age of 14 +/- 0.5 y with habitual calcium intakes < 800 mg/d completed a 12-mo protocol. The treatment group received a daily supplement containing 1000 mg elemental calcium. Bone mineral density (BMD) and bone mineral content (BMC) of the total body, lumbar spine, and femoral neck were determined at inclusion, 6 mo, and 12 mo. Also measured were serum concentrations of biochemical markers of bone turnover (osteocalcin and deoxypyridinoline), parathyroid hormone, and vitamin D. The calcium-supplemented group had greater accretion of total-body BMD and lumbar spine BMD but not BMC than did the control group. Calcium supplementation appeared selectively beneficial for girls who were 2 y postmenarcheal. Calcium supplementation significantly decreased bone turnover and decreased serum parathyroid hormone concentrations. Calcium supplementation of postmenarcheal girls with low calcium intakes enhances bone mineral acquisition, especially in girls > 2 y past the onset of menarche.

Evaluation of serum biochemical markers of bone metabolism for early diagnosis of nonunion and infected nonunion fractures in rabbits.

To evaluate the use of serum concentrations of biochemical markers of bone metabolism (osteocalcin [OC], bone-specific alkaline phosphatase [BS-ALP], and deoxypyridinoline [DPYR]) to compare healing in infected versus noninfected fractures and in fractures with normal repair versus delayed (nonunion) repair in rabbits. 32 female 9- to 10-month-old New Zealand White rabbits. A femoral fracture defect was made in each rabbit. Rabbits were assigned to the following groups: the bone morphogenetic-2 gene treatment group with either noninfected nonunion or infected (ie, inoculation of defects with Staphylococcus aureus) nonunion fractures or the luciferase (control) gene treatment group with either noninfected nonunion or infected nonunion fractures. Serum samples were obtained before surgery (time 0) and 4, 8, 12, and 16 weeks after surgery. Callus formation and lysis grades were evaluated radiographically at 16 weeks. Serum OC and BS-ALP concentrations decreased from time 0 at 4 weeks, peaked at 8 weeks, and then decreased. Serum DPYR concentration peaked at 4 weeks and then decreased, independent of gene treatment group or fracture infection status. Compared with rabbits with noninfected fractures, those with infected fractures had lower serum OC and BS-ALP concentrations at 4 weeks, higher serum OC concentrations at 16 weeks, and higher serum DPYR concentrations at 4, 8, and 16 weeks. Combined serum OC, BS-ALP, and DPYR concentrations provided an accuracy of 96% for prediction of fracture infection status at 4 weeks. Measurement of multiple serum biochemical markers of bone metabolism could be useful for clinical evaluation of fracture healing and early diagnosis of osteomyelitis.

Extent of correlation between first and second trimester markers for Down's syndrome screening.

Antenatal screening for Down’s syndrome estimates a woman’s risk of having an aVected pregnancy on the basis of factors found to be associated with Down’s syndrome—that is, maternal age, serum concentrations of biochemical markers, and ultrasound measurements. The GH secretagogues ipamorelin and GH-releasing peptide-6 increase bone mineral content in adult female rats.

Growth hormone (GH) is of importance for normal bone remodelling. A recent clinical study demonstrated that MK-677, a member of a class of GH secretagogues (GHSs), increases serum concentrations of biochemical markers of bone formation and bone resorption. The aim of the present study was to investigate whether the GHSs, ipamorelin (IPA) and GH-releasing peptide-6 (GHRP-6), increase bone mineral content (BMC) in young adult female rats. Thirteen-week-old female Sprague-Dawley rats were given IPA (0.5 mg/kg per day; n=7), GHRP-6 (0.5 mg/kg per day; n=8), GH (3.5 mg/kg per day; n=7), or vehicle administered continuously s.c. via osmotic minipumps for 12 weeks. The animals were followed in vivo by dual X-ray absorptiometry (DXA) measurements every 4th week. After the animals were killed, femurs were analysed in vitro by mid-diaphyseal peripheral quantitative computed tomography (pQCT) scans. After this, excised femurs and vertebrae L6 were analysed by the use of Archimedes' principle and by determinations of ash weights. All treatments increased body weight and total tibial and vertebral BMC measured by DXA in vivo compared with vehicle-treated controls. However, total BMC corrected for the increase in body weight (total BMC:body weight ratio) was unaffected. Tibial area bone mineral density (BMD, BMC/area) was increased, but total and vertebral area BMDs were unchanged. The pQCT measurements in vitro revealed that the increase in the cortical BMC was due to an increased cross-sectional bone area, whereas the cortical volumetric BMD was unchanged. Femur and vertebra L6 volumes were increased but no effect was seen on the volumetric BMDs as measured by Archimedes' principle. Ash weight was increased by all treatments, but the mineral concentration was unchanged. We conclude that treatment of adult female rats with the GHSs ipamorelin and GHRP-6 increases BMC as measured by DXA in vivo. The results of in vitro measurements using pQCT and Archimedes' principle, in addition to ash weight determinations, show that the increases in cortical and total BMC were due to an increased growth of the bones with increased bone dimensions, whereas the volumetric BMD was unchanged.
