PROSTHETIC REHABILITATION AFTER ENUCLEATION OF CYSTIC LESION WITH SEMI PRECISION ATTACHMENT AND TELESCOPIC CROWN

The demand for a quality dental practice is increasing in today’s era. Along with the form and function of the prosthesis, esthetics is also a matter of concern. Prosthetic rehabilitation of an enucleated cystic lesion becomes challenging in some conditions. Here, we report the case of prosthetic rehabilitation after enucleation of the cystic lesion with semi-precision attachment and telescopic crown in a 30-year-old female. In this case after enucleation of Odontogenic Keratocyst, the prosthesis was fabricated using semi-precision attachment along with the telescopic crown which provided proper retention, stability, and aesthetics.

PROSTHETIC REHABILITATION OF AN OSTEOPOROSIS CASE WITH SEMI PRECISION ATTACHMENTS AND CAST PARTIAL DENTURES - A CASE REPORT

Osteoporosis is a skeletal disorder that has emerged as a major health problem affecting mostly in the postmenopausal women and older individuals. In osteoporosis, there is an imbalance between the bone formation and bone resorption where in, there is an increase in resorption. It even affects the jaw bones making a challenging task for the prosthetic rehabilitation. Here, we present a case report of prosthetic rehabilitation of an osteoporosis case of 65-year-old woman, using semi precision attachments with Cast partial dentures.

Stresses Induced By Integrated and’ Nonintegrated Extracoronal Semi-Precision Attachments For Maxillary Distal Extension Bases

Purpose: The aim of the present study is to compare the stresses induces on the splinted abutments by two different reciprocation designs in conjunction with extracoronal semi-precision attachments retained removable partial denture (RPD) using strain gauge analysis. Material and methods: For this study, two RPD designs were made A, and B. representing maxillary bilateral distal extension edentulous area. The two abutments, canine and first bicuspid was fixed splinted on both sides with porcelain facing crowns. For design (A) an extra coronal attachment with integrated interlock was used at the distal aspect of the splinted first bicuspid and canine. For design (B) a parallel interlock extra coronal attachment was used with a conventional lingual bracing arm. A strain gauge was used for this study to measure the micro-strain induced on the apical and lateral sides of the canine and first-bicuspid abutments. Results: SPSS software program was used in the statistical analysis of the results. The results revealed that maximum stresses induced at the apex of the canine and firstbicuspid abutments were in case of design (B). While no significant difference between the two designs when the lateral load was applied Conclusion: A more favorable stress applied on the canine and bicuspids obtained with integrated interlock attachment.

A Simplified Approach to Semi-Precision Attachment

AbstractPrecision attachments open a new horizon of possibilities in prosthodontic rehabilitation. Varieties in size, shape and configuration allows them to be utilized in many clinical scenarios. However, the complex prosthesis with attachments does not serve the purpose of retention and esthetics but makes it cumbersome for the patient. Two cases in which a simplified approach to semi-precision attachments have been adopted, are discussed here.

COMPARATIVE STUDY BETWEEN TWO TYPES OF ATTACHMENTS USED FOR ROOT-SUPPORTED OVERDENTURE

INTRODUCTION: The incorporation of attachments in overdentures into everyday dental practice will open up another dimension in dental treatment planning and patient satisfaction. Teeth that might be considered for extraction may be considered as long or short term alternatives to implant or total edentulousness. Some teeth are maintained to support and/or retain the prosthesis and therefore, maximizing prosthesis stability, besides preserving proprioception of the periodontal ligament and reducing bone loss. Tooth-supported overdentures can be retained with attachments and can improve both retention and stability while simultaneously reducing alveolar bone resorption. They may also be more cost-effective and maintain more dental proprioception than implant-supported overdentures. OBJECTIVES: To compare clinically and radiographically between flex pivot (precision attachment with flexible male sphere) and castable pivot (semiprecision attachment) used for root-supported overdenture.MATERIALS AND METHOD: This randomized parallel controlled clinical study was conducted on twelve patients having bilateral mandibular single rooted teeth; canines or first premolars. Those were divided into two groups of six subjects. Group A (study group) each of six patients received flex pivots (precision attachment with flexible male sphere) bilaterally as test group. Group B (control group) six patients received castable pivots (semiprecision attachment) bilaterally as control group.RESULTS: When the data of changes in the clinical and radiographic parameters of periodontal status over a 6 months period were compared at immediate post-treatment, 1, 3, and 6 months intervals relative to type of attachment, the findings revealed there were no significant differences among precision attachment group in most of the mean values, while the opposite was shown in semiprecision attachment.CONCLUSIONS: Precision attachment with flex pivot was associated with more superior clinical periodontal parameters than precision attachment, and it is more biocompatible, hygienic, and maintaining healthy, stable periodontal soft tissue and crestal alveolar bone level.

RANDOMIZED CONTROL TRIAL FOR RADIOGRAPHIC ASSESSMENT OF TWO DIFFERENT ATTACHMENT MATERIAL RETAINING BAR SUPPORTED OVERDENTURE

Objectives: this research was carried out to evaluate two different attachment material retainers bar supported overdenture regarding the peri-implant crestal bone level changes. materials and methods: Twenty edentulous female patients participated in this study For each patient 3 endosteal implants were inserted in the interforamenal region using computer guided surgical Stent. Four month later implants were uncovered, healing abutments were secured and then transfer coping with long heads were used to make open tray impression technique. Plastic abutments were secured to implant analogue then plastic bars was attached to plastic abutments using durallay with two semi precision attachments at the distal ends of the bar. Then the bar was casted. Patients were divided into two groups. Group one the CAD CAM wax pattern of housing and the female part of the attachments were casted using nickel chrome alloy. Group two patients the CAD CAM wax pattern of housing and the female part of the attachments were transferred into Acetal resin using the injection molded technique. Then per implant crestal bone level were assessed using long cone parallel technique after six, twelve and twenty four months and then statistically analyzed. Results: This study showed that there was a statistically insignificant difference in between follow up periods for each group and between groups in between follow up period in interval 12-24 and significant difference between two groups after 24 months. Conclusion: Based on the results obtained from this study it can be concluded that acetal resin is favorable long term solution for implant retained mandibular overdentures

Precision attachments for aesthetics and function: a case report.

Successful restoration of the dentition requires plenty of contemporary and conventional treatment techniques and planning and attachment retained partial dentures are one such kind of treatment modality in prosthodontics. Limited space for extracoronal attachments is a serious gap in the design and the fabrication of a precision attachment Removable Partial Denture (RPD). A custom semi-precision attachment with a partial denture offers strength and improved aesthetics in cases with minimal space. This article presents a method of fabrication of semi-precision attachment to eliminate metal display and enhance aestheticity.

Nonrigid semi-precision connectors for FPD

In the practice of fixed prosthodontics, certain situations like pier abutments, tilted abutments pose problems with rigid connectors. If rigid connectors are used in this situation, the pier abutment will act as a fulcrum because of physiologic tooth movement, arch position of the abutment, and the retentive capacity of the retainers. A nonrigid connector may be preferred in fabrication of fixed partial denture (FPD) with pier abutment. In this prosthesis, the movement of the nonrigid connector is enough to prevent the pier abutment from serving as a fulcrum. This clinical report describes rehabilitation of a patient with FPD with pier abutment using semi-precision attachment. The technique used is simple, economical, and feasible in an ordinary laboratory set up.

A unique method of retaining orbital prosthesis with attachment systems – A clinical report

Diminution of the orbital contents post-surgical removal of a malignant tumor can have a severe psychological impact on the patient in terms of function and esthetics. Therefore, esthetic remedy should be planned subsequently, since tumor obliteration precedes cosmetic concern. A convenient option for successful rehabilitation in such patients is a simple, user-friendly, removable orbital prosthesis. Retention of the prosthesis is one of the key factors for the successful rehabilitation. Spectacle frame, conformers, adhesives, osseointegrated implants, magnets or buttons have been used to impart retention to the prosthesis. The use of semi precision attachments in maxillofacial prostheses is limited to the osseointegrated prostheses. This case report describes a conventional spectacle frame technique, to retain the silicone orbital prosthesis using two different types of stud attachments viz., dalla bona and O-ring attachment systems.

OBJECTIVE: Use of the fixed partial denture is more favorable than the removable partial denture for my patient, she complained about edentulism in upper right side and a little discomfort with second upper premolar. Clinical examination shows the tooth was crown less and radio graphically tooth had a big periapical lesion with poor prognosis. METHOD: After root canal treatment and extraction the tooth, clean and irrigate the socket, we are implanted it with hydroxyapatite again and follow the patient for six months. After six months, radio graph shows that the tooth was in ideal condition without mobility, then the tooth has used as an abutment for fixed partial denture with canine and 2nd molar in order to have a better prognosis we used semi precision attachment, and follow her for three years. RESULT: Three years follow the patient shows the tooth is in an acceptable condition radio-graphically without mobility and patient was comfortable esthetically and functionally. CONCLUSION: In my opinion in this case use of the hydroxyapatite, crown lengthening during the re- implantation and use the semi precision attachment was the reason of increase the prognosis after treatment INTRODUCTION: Fixed partial denture nowadays are more acceptable for patients than removable because of aesthetic, function and more confidence purposes. But not all cases are suitable for fixed partial denture, like the patients with long span edentulous area or with not reliable abutment. So save the good abutments and provide other conditions of oral cavity are the best things that the dentist can do for the patients to have an ideal fixed partial denture (7). Save the natural teeth is more comfortable for the patients and is near to our goal in conservative dentistry, in previous study the researchers was able to use a new technique with aid of hydroxyapatite to save a irremediable tooth, and recommended to continue observation of the tooth condition when it was loaded under the force, In this case, used it as an abutment for fixed partial denture. Before somebody used the reimplanted tooth, as an abutment for fixed bridge, but the most problems with the reimplanted teeth without medium objective like hydroxyapatite is increase the risk of root resorption,(1) and also because of ankylosis phenomenon in most reimplanted teeth use the rigid fixed bridge in long span edentulous with more than two abutment will cause apply extra force to another abutments that are not reimplanted and have normal periodontal ligament. So in these cases like my case the best option is use a medium like hydroxyapatite to have a better prognosis after reimplantation and reduction chance of rejection by the body as a foreign body and also prevention of root resorption due to osseointegration of bone. A simplified technique for custom made overdenture semi-precision attachments

Overdentures are the choice of treatment for most of them, but the only snag is its cost. In India, the rural areas are still deprived of quality treatment. This manuscript is showcasing a study, which is a cheaper, but effective way to provide the best to the patient. The aim of this study was to develop a custom made system, which can be used as an alternative treatment for the management of patients who need overdentures. The primary objective of this study was to provide a cost-effective attachment with the best functional efficacy. The semi-precision attachments were made by using the conventional technique of preparation of post space. The patterns were prepared by using the pattern resin material and the rest of the male and female attachments were fabricated by using innovative methods fulfilling the criteria. The attachments were successfully used in patients with remarkably good function and comfort. It was a successful attempt to provide the specialized treatment in affordable cost. There is a need of innovative work to make the sophisticated systems more economical and readily available.

Use of a Stud Attachment to Retain a Silicone Orbital Prosthesis: A Clinical Report

Orbital defects with total loss of eyelids and eyeball cannot be satisfactorily repaired by reconstructive surgery. Prosthetic replacement is the treatment of choice owing to its acceptable lifelike appearance. The use of semiprecision attachments in maxillofacial prostheses is limited to osseointegrated prostheses. Therefore, this article describes a conventional glasses frame technique, to retain the silicone orbital prosthesis using a Dalla Bona stud attachment.

An innovative method to anchor mandibular overdenture by OT Cap semiprecision attachment: A clinical report

Since the establishment of implant in dentistry, the use of removable prostheses in replacing missing teeth apparently becomes less popular, still the preventive prosthodontics emphasizes the importance of any procedure that can delay or eliminate future prosthodontic problems. Most patients with removable partial or complete denture are dissatisfied with their retention and stability. Semiprecision attachments offer considerable advantages such as increased retention of a denture, preservation of teeth which are otherwise indicated for extraction, as a viable alternative to implant retained overdenture and the ability to obtain parallelism in divergent abutments. A prosthodontist who familiarizes himself with semiprecision attachments will be in a position to suggest a better treatment options in retaining teeth which might otherwise be considered for extraction or immediate implant placement. However, lack of proper knowledge, overwhelming number of attachments available in the market, multiple adjustments and repairs are making dentist reluctant to offer and provide attachment-retained prostheses to their patients. This case report describes an innovative technique for improving overall prognosis of mandibular overdenture using advanced and prefabricated housing attachment system.

Effect of Insertion and Removal of Tooth Supported Overdentures on Retention Strength and Fatigue Resistance of Two Commercially Available Attachment Systems

ABSTRACT Purpose The main problem with semiprecision attachment retained prosthesis is that they need frequent servicing in terms of replacing the female component. There is a need for a study to assess the life span and frequency of replacement of elastic components in the attachment systems. Aim The aim of the study was to test the retention strength and fatigue resistance of Rhein OT cap and Ceka sagix attachment systems fabricated on the models. Materials and methods Rhein OT cap (micro) and Ceka sagix attachments were procured. Acrylic resin mandibular models fabricated with teeth embedded at overdenture positions. Bar framework with attachments were cast and cemented on the model. Denture fabricated on this model was subjected to universal testing machine (UTM Instron 5900 series) for retention and fatigue test. The retention strength was measured at various cycles namely 1,440, 2,880, 4,320 and 5,760 cycles to check for the loss of retention and development of fatigue on the basis of average number of removals and placements per day for 1,2,3 and 4 years, respectively. The values of both the attachment systems were recorded at the respective cycles and a graphical analysis was carried out representing the retention loss and development of fatigue. Results One-way ANOVA was used as part of the statistical analysis to compare the retention strength and fatigue resistance of both the attachment systems used. The results obtained proved that the retention values of both the attachment systems on tooth supported overdentures had a significant variation as they were subjected to increased number of cycles. Conclusion Sagix and Rhein attachment systems showed adequate retention values up to the first 2 to 3 years (2,880- 4,320 cycles). Fatigue test simulating 4 years of denture insertion and removal did cause subsequent reduction in the retention values but no component fracture of attachment systems. Conversion of a Partial Removable Dental Prosthesis from Kennedy Class II to Class III Using a Dental Implant and Semiprecision Attachments

This article presents a design to convert a partial removable dental prosthesis (PRDP) from Kennedy class II to class III using a dental implant. Incorporating semiprecision attachments, this design provides desired esthetics, phonetics, and function.

When compared to a conventional attachment, a (semi)precision attachment for a cast metal frame removable partial denture can perform a number of functions better. Proper assessment of the condition of the abutment teeth is needed for an adequate indication. Selecting the type of (semi-)precision attachment is primarily determined by the available maxillomandibular space, the crown length as well as the possibility of adequate oral self-care. With regard to technical characteristics, (semi-)precision attachments are classified according to measures of freedom. The distinct types are cast intra- and extra-coronal, adhesive and overdenture (semi-)precision attachments. With the lapse of time, some wear of the different parts of an attachment may occur, requiring specific subsequent care. Whether or not in combination with oral implants, (semi-)precision attachments often offer patients improved self-confidence and self-image. However, the relatively high cost is a barrier to apply (semi-)precision attachments widely.

Evaluation of the retentive characteristics of semi-precision extracoronal attachments

The aims of this in vitro study were: to evaluate the repeatability of producing the semi-precision attachment; to establish their estimated longevity by a simulated, repeated denture placement/removal procedure of denture; and to investigate the wear of their components. Four types of semi-precision attachments were evaluated. The attachments represented polymer-to-metal type of friction: Vario-Kugel-Snap, Vario-Soft, Strategy OT and ASC-52, which exhibited metal-to-metal type of friction. Tensile dislodging force was applied to sample to register peak load-to-dislodgment and also to determine the displacement curve. Then the cyclic axial loading tests were performed with insertion/separation machine. To detect worn areas, the surface of each sample was examined under a scanning electron microscope. The large variability of initial retentive force and shape of displacement curves was noted within Vario-Soft and Vario-Kugel-Snap group. After the first period of loading (up to 300 cycles), the mechanical properties of all samples were stabilised. The mean retention force of attachments with polymer matrix after 3000 cycles was approximately half the recommended value, while samples of ASC-52 (with metal-to-metal friction) withstood 8000 cycles. Results suggest that the inaccuracies of casting and laboratory procedure have only limited influence on the fit of the semi-precision attachments. The attachment with metal-to-metal type of friction exhibits the highest wear resistance (simulated up to 8 years of usage) compared to attachments with polymeric matrix (up to 3 years). All attachments exhibited wear of metal patrix, what can minimise the chance of re-activation in a long run.

The Use of Dental Implants in Combination with Removable Partial Dentures. A Case Report

Implant treatment protocols for fully edentulous patients include implant-supported fixed restorations with a minimum of five to six implants or overdentures on two to four implants. However, implant placement in the posterior areas is sometimes not permitted by anatomic and/or financial restraints, or patients' unwillingness to have extensive surgical procedures. The purpose of this case report is to present a treatment option where bilateral distal extension removable partial dentures (RPDs) are used in combination with anterior fixed implant prostheses with semi-precision attachments. The initial treatment plan of a 65-year-old female with failing dentition involved the fabrication of overdentures supported by four implants placed in the interforaminal area in both arches. Because of inadequate space for the retentive elements of the overdenture and the patient's objection to surgical procedures for the placement of additional implants to support a fixed restoration, the treatment plan was modified. Fixed ceramometal cement-retained implant restorations with semi-precision attachments adjacent to the distal abutments were fabricated in the maxilla and the mandible. Bilateral distal extension RPDs were placed in both arches. The patient's benefits were increased comfort, good esthetics in the anterior area, improved phonetics, and masticatory function. Retentive element maintenance requirements were similar to conventional RPDs. More extended controlled clinical studies are needed to establish the long-term success of this treatment option. Removable partial dentures may offer an attractive treatment option for an edentulous patient, as they may combine an FPD in the anterior segment with a removable appliance in the posterior areas. A satisfying restoration can be achieved with fewer implants.

Heightened Telescopic Copings in Cleft Palate Rehabilitation: A Case Report

To increase retention of overdentures in cleft palate patients by means of heightened telescopic copings with a newly designed attachment. In this study, a "precision attachment" is described to develop the stability and retention of telescopic overdentures. A cleft palate patient was treated with a newly designed overdenture. Intraoral examination revealed poor tooth structure and retention. Therefore, a new semiprecision attachment was designed in order to increase retention and stabilization. The patient's tolerance was increased with the new overdenture.
