Performance-based attentional control, but not self-reported attentional control, predicts changes in depressive symptoms in short-term psychotherapy

ObjectiveAlthough impairments in attentional control are pervasive across psychopathology, there is substantial individual differences. In the current study, we examined whether individual differences in self-reported and performance-based measures of attentional control predict changes in depressive symptoms and well-being in a diagnostically diverse sample of patients attending a CBT-based partial hospital program. MethodParticipants were 89 patients (56.2% men, 75.3% non-Hispanic White). At baseline, patients completed the self-reported Attentional Control Scale and the Rapid Serial Visual Presentation task (RSVP), a behavioral measure of attentional control. Depressive symptoms were assessed daily using the Patient Health Questionnaire and well-being was assessed using the Mental Health Continuum Short Form. ResultsOn average, greater self-reported attentional control was significantly associated with lower depressive symptoms, β = −0.49, t(52) = 4.84, p < .001, 95% CIs [−0.69, −0.29], and greater well-being, β = 0.45, t(53) = 3.90, p < .001, 95% CIs [0.22, 0.67]. More accurate task-based performance was associated with a decline in depressive symptoms over time, β = −0.02, t(32) = 2.50, p = .02, 95% CIs [−0.04, −0.01]. Neither self-reported nor performance-based measures of attentional control predicted changes in well-being. Finally, exploratory analyses suggest that depressive symptoms also improved over time for individuals who underestimated self-reported attentional control abilities relative to task-based performance, β = −0.19, t(32) = 2.23, p = .03, 95% CIs [−0.36, −0.02]. ConclusionsResults suggest that performance-based attentional control may be an important target for assessment and intervention, as well as a potential mechanism underlying risk and recovery.

Investigating the specific domains of the cognitive attentional syndrome that predict posttraumatic stress symptoms: The moderating effect of attentional control

The metacognitive model of posttraumatic stress disorder (PTSD) suggests that persistent use of the maladaptive self-regulation strategies that comprise the cognitive attentional syndrome (CAS) increases the likelihood of developing PTSD symptoms following trauma exposure. The metacognitive model also suggests that flexible regulation of attention might be protective against developing maladaptive outcomes that are associated with the CAS. The aims of this study were to (1) examine associations between all seven domains of the CAS and PTSD symptoms using a recently developed, multidimensional measure of the CAS, and (2) examine the moderating effect of self-reported attentional control on associations between the CAS and PTSD symptoms. Participants were trauma-exposed community adults (N = 237) who completed a battery of self-report measures. Results from linear regression analyses showed that worry, substance use, and internal threat monitoring accounted for unique variance in PTSD symptoms when all seven CAS domains were entered into the same model. Moderation analyses showed that attentional control dampened the effect of the CAS, specifically external threat monitoring, on PTSD symptoms. Study results support attentional control as a protective factor against the maladaptive effects of the CAS on PTSD symptoms.

The role of performance beliefs in the difference between self-report and behavioural measures of attentional control and their relationship with anxiety

While empirical findings closely link poor attentional control with elevated anxiety, this relationship is more consistently evident and stronger when attentional control is measured through self-report than through behaviour. One possible explanation for these diverging findings is that people lack insight into their attentional control capabilities, and people with elevated anxiety hold more negative beliefs about their level of attentional control, resulting in lower self-reported levels of attentional control. In two studies, participants (N = 78 and N = 207) completed the attentional control scale, the attentional network test (ANT), a questionnaire measuring beliefs about attentional control in the ANT, and a measure of anxiety. In both studies, no significant associations were present between beliefs about attentional control in the ANT and participants' performance on the ANT, suggesting a lack of insight in attentional control capabilities. Both studies further demonstrated that only beliefs about attentional control but not performance in the ANT were related to self-reported attentional control and anxiety. We thus show that evidence supporting the relationship between self-reported attentional control and anxiety is driven by biased beliefs about ability to control attention in people with heightened anxiety, and not by behavioural indices of attentional control.

Dependent Stress Generation Mediates the Relation Between Poor Cognitive Control and Repetitive Negative Thinking in Emerging Adults

Poor cognitive control has been associated with maladaptive thinking, like rumination and worry, that increase risk for internalizing psychopathology. However, little research has investigated how cognitive control is associated with commonalities between rumination and worry (i.e., repetitive negative thinking; RNT). The current study aimed to investigate how cognitive control predicts engagement in a common component of RNT over time via an indirect mechanism of dependent stress generation in a one-semester longitudinal study of emerging adult college students ( N = 224). Executive functioning task performance and self-reported attentional control (not working memory capacity task performance) prospectively predicted RNT, mediated by dependent stress, but did not predict change in stress or RNT from baseline. These findings suggest that aspects of cognitive control relevant for successful goal pursuit may be involved with maintaining levels of stressful life events and subsequent RNT.

Are reinforcement sensitivity personality constructs and attentional control important predictors of restrictive disordered eating?

The revised Reinforcement Sensitivity Theory (rRST) perspective on personality suggests that a neuropsychological behavioural inhibition system (BIS), behavioural approach system (BAS), and a fight-flight-freeze system (FFFS) produce the key personality traits involved in approach and avoidance behaviours. This perspective on personality can be used as a framework for understanding psychopathology. Self-report research on rRST suggests that restrictive disordered eating relates to elevated BIS sensitivity, elevated FFFS sensitivity, and possibly dysfunctional BAS sensitivity. Disordered eating can also relate to reduced trait mindfulness, which is a broadly defined construct. Trait mindfulness is positively correlated with attentional control (AC) which is a more specific component of our cognitive architecture that incorporates attentional focusing and attentional shifting processes. It is unknown how BIS and AC interact to predict restrictive disordered eating. We tested how self-reported BIS, BAS, and FFFS sensitivity, AC, and trait mindfulness relate to restrictive disordered eating in 464 healthy participants, and 177 participants with a history of psychiatric disorder. We provide new evidence that elevated restrictive disordered eating relates to reduced self-reported AC abilities (in addition to elevated BIS, and elevated FFFS sensitivity). We illustrate that the combination of high BIS and low AC predicts high levels of restrictive disordered eating (but not in all participants), whereas low BIS and high AC predicts lower levels of restrictive disordered eating (but not in all participants). We discuss how understanding the relationship between personality and attentional control can inform the design of future intervention studies.

A longitudinal trait-state model of attentional control: Implications for repetitive negative thinking

BackgroundAttentional control refers to the ability to direct, focus, and shift attention voluntarily, and poor attentional control may confer risk for various affective disorders by increasing repetitive negative thinking. Although attentional control has been described as a trait, it is unclear if it is a time-varying (TV) or state-like factor versus a time-invariant (TI) or trait-like personality characteristic. MethodsIn a 6-wave, 5-month longitudinal study, community participants (n = 1,251) completed the Attentional Control Scale (Derryberry & Reed, 2002), the most commonly used measure of attentional control that includes two components: Focusing and Shifting. A latent variable (trait-state-occasion) model was applied to the two components. ResultsThe results showed that although estimates of TI factor variance and TV factor variance were both significant for Focusing and Shifting, the proportion of TI factor variance (0.81, 0.77) was significantly greater than the amount of TV factor variance (0.18, 0.22). Furthermore, although TV factor stability was statistically significant for Focusing and Shifting, the size of the coefficients was small to moderate in magnitude. In predicting latent repetitive negative thinking at each of the six time points, regression weights for the attentional control TI factor were significant and larger than those for the TV factor (which were generally not significant). LimitationsRelatively short timeframe of 5 months and exclusive reliance on self-report measures. ConclusionsThese findings suggest that self-reported attentional control is largely TI and that it is this TI component that predicts repetitive negative thinking.

The Role of Attentional Control in Attention to Emotional Stimuli Among Middle-Aged and Older Adults

Prior studies have examined age differences in attention to emotional stimuli; in the current study, we considered how this might relate to dispositional measures of attentional control across age groups. Participants were 116 middle-aged (aged 35 – 64 years) and 39 older (aged 65-86 years) adults in the United States. In the study, participants filled in the Emotional Attentional Control Scale. Then participants watched fearful, happy, neutral, and disgusting videos. The gaze time for each video was measured by an eye tracker. Results did not show significant age difference in attention to happy and neutral videos. However, middle-aged adults gazed relatively more to the disgusting video and relatively less to the fearful video, t (115) =2.16, p =.03. The opposite pattern was found among older adults, t (38) =5.85, p<.001. Self-reported emotional attentional control was not significantly related to attention in either age group. These findings suggest that different stimuli may yield age differences in fixation that are a less consistent pattern with the age-related positivity effect reported in previous studies, and also that self-reported emotional attentional control may not relate to gaze patterns.

One Step Ahead-Attention Control Capabilities at Baseline Are Associated With the Effectiveness of the Attention Training Technique.

BackgroundAttentional control has been observed to play an important role in affective disorders by impacting information processing, the ability to exert top–down control in response to distracting stimuli, and by affecting emotional regulation. Prior studies demonstrated an association between attentional control and response to psychotherapy, thereby identifying attentional control as an interesting prognostic pre-treatment factor. Improving attentional control and flexibility is a cornerstone in metacognitive therapy (MCT), which is trained by the use of the Attentional Training Technique (ATT). However, as of yet, it remains unclear if pre-treatment attentional control is related to the effect of ATT.MethodsAn aggregated sample of 139 healthy participants [study 1: 85 participants, mean age 23.7 years, previously published (Barth et al., 2019); study 2: 54 participants, mean age 33.7 years, not previously published] performed an attentional performance test battery before and after applying ATT. Before ATT was administered, attentional control was measured using a well-established self-report instrument, i.e., the Attentional Control Scale (ACS; Derryberry and Reed, 2002). ATT was given in 2, 4, or 15 doses and compared to sham ATT. The test battery comprised a selection of established neurocognitive tasks: emotional dot probe, Stroop, 2-back, and dichotic listening.ResultsSham ATT showed no interaction with ACS score on performance outcome in all tests. At four doses of ATT, ACS score was associated with training response, i.e., subjects with high self-reported attentional control before training showed the largest improvements post-training (all P-values <0.05; see Figure 3). At 2 and 15 doses of ATT, the ACS score was unrelated to training response.ConclusionThis is a first attempt in understanding the optimal dosage in which ATT should be administered dependent on the individual characteristics of each subject pre-training. The current data suggest self-reported attentional control pre-training as a marker to determine an optimal individual ATT training profile. Future studies should investigate if other domains of metacognitions also interact with training outcome and evaluate the extent to which this relationship transfers to clinical samples. If successful, assessing attentional control prior to treatment in clinical samples could be of use regarding personalized therapy plans and treatment outcome.

Emotional conflict adaptation predicts intrusive memories

Why do some individuals experience intrusive emotional memories following stressful or traumatic events whereas others do not? Attentional control may contribute to the development of such memories by shielding attention to ongoing tasks from affective reactions to task-irrelevant emotional stimuli. The present study investigated whether individual differences in theability to exert cognitive control are associated with experiencing intrusive emotional memories after laboratory trauma. Sixty-one healthy women provided self-reported and experimentally derived measures of attentional control. They then viewed a trauma film in the laboratory and recorded intrusive memories for one week using a diary. Gaze avoidance during trauma film exposure was associated with more intrusive memories. Greater attentional control over emotion prior to film viewing, as assessed with the experimental task, predicted fewer intrusive memories while self-reported attentional control was unrelated to intrusive memories. Preexisting capacity to shield information processing from distraction may protect individuals from developing intrusive emotional memories following exposure to stress or trauma. These findings provide important clues for prevention and intervention science.

Examining differential relationships among self-reported attentional control, depression, and anxiety in a transdiagnostic clinical sample

BackgroundPoor attentional control, defined as difficulty focusing attention on a task or shifting attention flexibly between tasks, is a transdiagnostic construct theorized to confer risk for, and maintain, depression and anxiety. Research to date in non-clinical samples has suggested a dissociable relationship between the two factors of self-reported attentional control and psychopathology, with depression being associated with difficulties shifting and anxiety being associated with focusing. However, to our knowledge no study has tested this differential set of relationships in a clinical sample. MethodsAdults (N = 493) presenting for psychiatric treatment completed measures of depressive and anxiety symptom severity and self-reported attentional control. Hierarchical linear regression and Zou's (2007) confidence interval method were used to examine the relationship between clinical symptoms and attentional control. ResultsBoth shifting and focusing were significantly correlated with anxiety and depressive symptoms in this sample. However, focusing was more strongly associated with clinical symptomatology than shifting, which showed a weak correlation. LimitationsAll constructs were measured cross-sectionally by self-report questionnaires. ConclusionsIn contrast to studies conducted in non-clinical samples, attentional focusing appears to be more relevant than attentional shifting in a clinical sample for both depression and anxiety symptoms. These findings lend support to efforts to develop neurocognitive interventions that improve focusing. Replication of these findings, particularly in longitudinal studies, is warranted.

Does rumination mediate the relationship between attentional control and symptoms of depression?

Background and objectivesIt has been suggested that impaired attentional control (AC) promotes the use of maladaptive emotional regulation strategies, such as rumination, with subsequent increase in risk of depression. MethodThis study examined this hypothesis in a healthy community sample. Questionnaire measures of depression, anxiety, rumination and self-reported AC (shifting and focusing) were used, as well as an attention performance task (Attention Network Task; ANT). ResultsWhile self-report and performance measures of AC were not significantly related, both depression and rumination were associated with reduced self-reported AC. Depression was specifically associated with poorer attentional shifting. Depression and brooding were also associated with better performance on the conflict component of the ANT. Importantly, the relationships of ANT conflict and self-reported AC to depression were mediated by brooding. LimitationsThe current study used a community sample, and it is unclear if results would generalise to a clinical population. All measures were taken concurrently and so it is not possible to confidently ascertain causality or direction of effects. ConclusionsThese results are consistent with the suggestion that impaired AC, particularly a narrow and inflexible attentional focus, may increase risk of depression by promoting ruminative thinking. The results highlight the importance of considering both self-report and performance measures of AC, as well as different components of attentional performance.

On the validity of self-report assessment of cognitive abilities: Attentional control scale associations with cognitive performance, emotional adjustment, and personality.

Individual differences in attentional control involve the ability to voluntarily direct, shift, and sustain attention. In studies of the role of attentional control in emotional adjustment, social relationships, and vulnerability to the effects of stress, self-report questionnaires are commonly used to measure this construct. Yet, convincing evidence of the association between self-report scales and actual cognitive performance has not been demonstrated. Across 2 independent samples, we examined associations between self-reported attentional control (Attentional Control Scale; ACS), self-reported emotional adjustment, Five-Factor Model personality traits (NEO Personality Inventory-Revised) and performance measures of attentional control. Study 1 examined behavioral performance on the Attention Network Test (ANT; Fan, McCandliss, Sommer, Raz, & Posner, 2002) and the Modified Switching Task (MST; Suchy & Kosson, 2006) in a large sample (n = 315) of healthy young adults. Study 2 (n = 78) examined behavioral performance on standardized neuropsychological tests of attention, including Conner's Continuous Performance Test-II and subtests from the Wechsler Adult Intelligence Scales, Third Edition (WAIS-III; Psychological Corporation, 1997) and Delis-Kaplan Executive Function System (D-KEFS; Delis, Kaplan, & Kramer, 2001). Results indicated that the ACS was largely unrelated to behavioral performance measures of attentional control but was significantly associated with emotional adjustment, neuroticism, and conscientiousness. These findings suggest that although self-reported attentional control may be a useful construct, researchers using the ACS should exercise caution in interpreting it as a proxy for actual cognitive ability or performance. Attentional focus moderates the relationship between attention to threat bias and delusion-like experiences in healthy adults

BackgroundThe role of cognitive biases in delusion and delusion-like experiences has been widely investigated in recent years. However, little is known about individual differences, which may influence association between cognitive biases and formation of delusional beliefs. The aim of this study was to examine the moderating effect of self-reported attentional control on the relationship between attention to threat bias (ATB) and delusion-like experiences (DLEs) in healthy adults. MethodsParticipants (n=138) completed the Davos Assessment of the Cognitive Biases Scale (DACOBS), the Attentional Control Scale (ACS) and the Peters et al. Delusions Inventory (PDI). The moderation analysis was performed to check the influence of different components of attentional control (i.e. general ability to allocate attention, focusing, shifting and divide attention) on the interplay between ATB and DLEs. ResultsThe results supported the moderation model. Specifically, we found that a higher level of ability to focus attention is associated with a stronger effect of attention to threat bias on the overall frequency of DLEs. Our results indicate that ATB contributes to the number of DLEs only in individuals with high and moderate capacity to focus attention, whereas in those who scored low on the ACS focusing attention subscale, the presence of attentional bias does not influence the frequency of DLEs. ConclusionsOur findings show that the individual difference variable, such as ability to voluntarily focus attention, may moderate the relationship between attention to threat bias and delusion-like experiences in healthy adults.

Emotional attentional control predicts changes in diurnal cortisol secretion following exposure to a prolonged psychosocial stressor

Hypothalamic–pituitary–adrenal (HPA) axis irregularities have been associated with several psychological disorders. Hence, the identification of individual difference variables that predict variations in HPA-axis activity represents an important challenge for psychiatric research. We investigated whether self-reported attentional control in emotionally demanding situations prospectively predicted changes in diurnal salivary cortisol secretion following exposure to a prolonged psychosocial stressor. Low ability to voluntarily control attention has previously been associated with anxiety and depressive symptomatology. Attentional control was assessed using the Emotional Attentional Control Scale. In students who were preparing for academic examination, salivary cortisol was assessed before (time 1) and after (time 2) examination. Results showed that lower levels of self-reported emotional attentional control at time 1 (N=90) predicted higher absolute diurnal cortisol secretion and a slower decline in cortisol throughout the day at time 2 (N=71). Difficulty controlling attention during emotional experiences may lead to chronic HPA-axis hyperactivity after prolonged exposure to stress. These results indicate that screening for individual differences may foster prediction of HPA-axis disturbances, paving the way for targeted disorder prevention.

Transdiagnostic mechanisms in depression and anxiety: The role of rumination and attentional control

BackgroundDeficits in attentional control have been hypothesized to cause rumination, suggesting that the relationships between attentional control and clinical symptoms may be mediated in part by rumination. However, to date, no clinical study has examined these constructs transdiagnostically in a path analysis model. MethodsFifty-one adults presenting for treatment completed measures of self-reported attentional control, rumination, and depression and anxiety symptoms. A bias-corrected path analysis-based approach was employed to test whether indirect (i.e., mediating) effects of rumination were significantly associated with the direct effects of attentional control on depression and anxiety symptoms. Separate models for depression and anxiety symptoms were tested along with reverse models using attentional control as a proposed mediator. ResultsThe relationship between attentional control and clinical symptomatology (i.e., both depression and anxiety symptoms) was mediated by rumination. Poor attentional control was associated with more rumination and consequently more severe symptoms of depression and anxiety. The reverse relationship (i.e., attentional control mediating the relationship between rumination and depression or anxiety symptoms) was not significant. LimitationsStudy design did not allow testing of temporal precedence for the mediation models. All constructs were assessed via self-report. ConclusionsAttentional control appears to impact depression and anxiety symptoms through rumination. The pathway between poor attentional control and emotion dysregulation via rumination suggests that interventions targeting attentional control may decrease maladaptive ruminative processes, leading to improved emotion regulation and reduced clinical symptomatology. Future studies should examine the stability of this mediational relationship over time (and in the face of targeted interventions).

Shared and Distinct Cognitive/Affective Mechanisms in Intrusive Cognition: An Examination of Worry and Obsessions.

Generalized anxiety disorder and obsessive-compulsive disorder are defined by chronic intrusive thoughts. The aim of the present study was to evaluate the relationship between cognitive (attentional control) and motivational (negative urgency) mechanisms potentially underlying worry and obsessions. Participants (N = 526) completed an online questionnaire battery consisting of self-report measures of worry, OCD symptoms, attentional control (AC), negative urgency (NU), and trait negative affect. After controlling for trait negative affect, self-reported AC was negatively related to worry, repugnant obsessions, and ordering symptoms. Greater NU was associated with increased worry and repugnant obsessions. Further, self-reported AC and NU interacted such that greater NU was associated with greater worry at high but not low levels of AC. AC and NU were independently associated with repugnant obsessions. Perceived executive functioning impairments may confer risk for intrusive thoughts, particularly worries, whereas distress-driven impulsivity may contribute to the involuntary, ego-dystonic features of intrusions.

Spontaneous EEG theta/beta ratio and delta-beta coupling in relation to attentional network functioning and self-reported attentional control.

Relationships between electroencephalographic (EEG) slow- and fast-wave frequency bands are considered to be of interest in the study of dispositional affective traits, emotion regulation, and attentional phenomena. However, to date, no previous studies had explored whether both state performance-based and self-reported attentional control (AC) measures potentially relate to different patterns of spontaneous EEG measures, in the absence of emotional stimuli. In the present study, individual differences in spontaneous EEG theta/beta ratio and delta-beta coupling at frontal and parietal sites were explored in a sample of 110 healthy volunteers as potential correlates of individual differences in performance-based attentional network functioning, as measured through the Attentional Network Test for Interactions (ANT-I) and self-reported AC. We found that stronger delta-beta coupling at parietal sites was associated with higher self-reported AC. However, no significant associations were found between executive control network functioning and the EEG ratio or coupling measures. Furthermore, a lower spontaneous fronto-parietal theta/beta ratio was found to be associated with better orienting network functioning. These results are discussed with a focus on the potential utility of spontaneous EEG measures in several cortical regions for capturing trait-like individual differences in temperament-related factors.

Spontaneous emotion regulation and attentional control.

The current study aimed to explore whether self-reported attentional control (AC) and the attentional network functioning would predict spontaneous emotion downregulation after emotional induction. A total of 117 healthy volunteers were asked to continuously rate their discomfort while looking at affective pictures, as well as for a period of time after exposure. After controlling for trait anxiety, higher self-reported AC significantly predicted a greater spontaneous emotional downregulation after exposure to aversive pictures. Both higher self-reported AC and lower executive control network functioning (i.e., greater interference) predicted a faster spontaneous emotional downregulation after exposure to affectively neutral pictures. EEG theta/beta ratio as a potential biomarker for attentional control and resilience against deleterious effects of stress on attention

Anxious stress compromises cognitive executive performance. This occurs, for instance, in cognitive performance anxiety (CPA), in which anxiety about one's cognitive performance causes that performance to actually deteriorate (e.g., test anxiety). This is thought to result from a prefrontal cortically (PFC) mediated failure of top-down attentional control over stress-induced automatic processing of threat-related information. In addition, stress-induced increased catecholamine influx into the PFC may directly compromise attentional function. Previous research has suggested that the ratio between resting state electroencephalographic (EEG) low- and high-frequency power (the theta/beta ratio) is related to trait attentional control, which might moderate these effects of stress on attentional function. The goals of the present study were to test the novel prediction that theta/beta ratio moderates the deleterious effects of CPA-like anxious stress on state attentional control and to replicate a previous finding that the theta/beta ratio is related to self-reported trait attentional control. After recording of baseline frontal EEG signals, 77 participants performed a stress induction or a control procedure. Trait attentional control was assessed with the Attentional Control Scale, whereas stress-induced changes in attentional control and anxiety were measured with self-report visual analogue scales. The hypothesized moderating influence of theta/beta ratio on the effects of stress on state attentional control was confirmed. Theta/beta ratio explained 28% of the variance in stress-induced deterioration of self-reported attentional control. The negative relationship between theta/beta ratio and trait attentional control was replicated (r = -.33). The theta/beta ratio reflects, likely prefrontally mediated, attentional control, and should be a useful biomarker for the study of CPA and other anxiety-cognition interactions.

Attentional control, attentional network functioning, and emotion regulation styles

Attentional network functioning in emotionally neutral conditions and self-reported attentional control (AC) were analysed as predictors of the tendency to engage in dysfunctional emotion regulation strategies. Diminished attentional orienting predicted an increased tendency to engage in brooding rumination, and enhanced alertness predicted a greater chance of suppression, beyond trait anxiety and self-reported AC, which were not predictive of either rumination or suppression. This is the first study to show that some forms of dysfunctional emotion regulation are related to the attentional network functioning in emotionally neutral conditions. Results are discussed in relation to regulatory temperament and anxiety-related attentional biases literature.
