Development and Implementation of a Geriatric Oncology Interdisciplinary Case-Based Educational Intervention for Cancer Care Providers.

To develop and implement a continuing professional development (CPD) activity focused on geriatric assessment (GA) in oncology for oncologists and geriatricians. We evaluated the impact of this activity on knowledge, skills, and performance regarding GA in oncology, as well as its feasibility and acceptability. We included teams composed of an oncologist and a geriatrician working in Mexico. Curriculum content was selected from geriatric oncology guidelines. We used Project Extension for Community Healthcare Outcome (ECHO)'s model to create a 12-week online course. A one-group pretest post-test quasi-experimental design was used to evaluate the intervention's effectiveness. At baseline, participants answered a multiple-choice knowledge assessment, a survey on self-perceived competence in GA, and an adaptation of the Association for Community Cancer Centers Geriatric Oncology Gap Assessment Tool, evaluating self-perceived performance in conducting geriatric interventions. These assessments and a satisfaction questionnaire were also completed postintervention. Baseline and postintervention scores were compared using paired t-tests. We included 40 participants (20 oncologists and 20 geriatricians). Median attendance was 10 sessions (range 2-12). Thirty-eight participants completed the satisfaction questionnaire, with a median score of 10/10 (range 8-10). The mean baseline and postintervention knowledge scores were 59.5 ± 12.8 and 74.4 ± 9.7, respectively (P < .001, effect size 1.14). The mean baseline and postintervention competence scores were 6.42 ± 2.5 and 9.02 ± 0.8, respectively (P < .001, effect size 1.03). The mean baseline and postintervention performance scores were 2.58 ± 0.65 and 3.29 ± 0.5, respectively (P < .001, effect size 1.64). A CPD activity for oncologists and geriatricians on the basis of the Project ECHO model was feasible and acceptable, leading to increased knowledge, competence, and performance in geriatric oncology. This could represent a novel method for increasing the geriatric competence of the cancer care workforce in Latin America and globally.

Novel Occupational Therapy Program (Step Up) to Facilitate the Transition from Early Intervention in Psychosis Services.

The purpose of this program evaluation was to examine preliminary outcomes associated with a novel stepdown program for clients of early intervention in psychosis services ("Step Up") that featured occupational therapy (OT) as a critical treatment component. Clients participated in Step Up for at least 6 months and were administered pre-post assessments of clinician-rated performance of daily living activities and self-perceived performance and satisfaction with daily occupational functioning. Paired samples Wilcoxon tests were used to compare outcomes across the two time points. Data from 23 participants of Step Up were analyzed. Clinician-rated performance of daily living (especially in the areas of money and time management and leisure engagement) and client-rated performance and satisfaction with daily occupational functioning improved significantly over time. Results demonstrate the promise of programs such as Step Up that capitalize on OT and promote functional outcomes during the transition from early intervention.

What matters to you? A mixed-method evaluation of goal setting and attainment within reablement from a client perspective

Background Goal setting is an essential component of reablement programmes. At the same time it is also an important aspect in the evaluation of reablement from the perspective of clients. Objectives As part of the TRANS-SENIOR project, this research aims to get an in-depth insight of goal setting and goal attainment within reablement services from the perspective of the older person. Material and methods A convergent mixed methods design was used, combining data from electronic care files, and completed Canadian Occupational Performance Measure (COPM) forms with individual interviews. Results In total, 17 clients participated. Participants’ meaningful goals mainly focused on self-care, rather than leisure or productivity. This mattered most to them, since being independent in performing self-care tasks increased clients’ confidence and perseverance. Regarding goal attainment, a statistically significant and clinically relevant increase in self-perceived performance and satisfaction scores were observed. Conclusion Although most goals focused on self-care, it became apparent that these tasks matter to participants, especially because these often precede fundamental life goals. Significance Reablement can positively contribute to goal setting and attainment of clients and may contribute to increased independence. However, effectiveness, and subsequently long-term effects, are not yet accomplished and should be evaluated in future research.

The Impact of the SARS-COVID-19 Lockdowns on the Subjectively Perceived Performance Level of Amateur Athletes after Returning to the Gyms.

Since December 2019, few issues have garnered as much global attention as severe acute respiratory syndrome coronavirus 2 (SARS-CoV-19). The imposed lockdowns in 2020/21, which led to the closure of all gyms, barred people from participating in their favourite sports activities. This study explores athletes' self-evaluations of their performance levels upon return to training facilities post-reopening. Data were collected in May 2021, after the end of the second lockdown, using a national online questionnaire. The study recorded 20 demographic and training-specific parameters to discern the factors influencing self-perceived performance upon resuming gym activities after the 2020/2021 lockdown. A total of 1378 respondents participated in the study. Of the total number of participants, 27.5% (365) reported regaining 100% of their original performance level after reopening their studios, a proportion that comprised 212 males, 150 females, and 3 individuals of unspecified gender. Additionally, 35.7% (474) estimated their performance level to be up to 75%, followed by 30% (398) recording their performance level at 50%, and a minority of 6.8% (90) determining their performance level to be up to 25%. Exercise intensity prior to lockdown, training experience, sex, and concurrent practice of another sport significantly influenced the athletes' self-assessment of their current fitness levels (p > 0.001, p > 0.001, p > 0.001, and 0.006, respectively). We need to understand the factors that shape self-perception, especially in case of another lockdown, in order to provide preventive assistance concerning mental and physical well-being. Positive influences on self-perception include prior athletic experience, intensive training before the lockdown, and continued participation in sports throughout the lockdown. Younger age is also favourable, but this may not necessarily reflect the benefits of youth; rather, it could indicate the current lack of accessible online sports activities for older individuals. Women, however, might have a less favourable perception of their own athletic performance.

Changes in self-perceived performance and satisfaction with performance of daily activities following interdisciplinary rehabilitation in people with late effects of polio.

People with late effects of polio (LEoP) may need rehabilitation to manage everyday life but knowledge of the benefits of interdisciplinary rehabilitation is limited. To evaluate changes in performance and satisfaction with performance of activities among people with LEoP following interdisciplinary rehabilitation. A pre-post retrospective study based on data on 102 participants with LEoP from a rehabilitation clinic. Changes in performance and satisfaction with performance of daily activities before and after interdisciplinary rehabilitation were assessed with the Canadian Occupational Performance Measure (COPM). There were statistically significant increases in the mean performance and mean satisfaction with performance COPM scores from admission to discharge. Twenty-three percent and 19% of the participants, respectively, had improved their performance and satisfaction with performance, 25% and 26% of the participants had no changes, and 19% and 22% of the participants, respectively, rated their performance and satisfaction lower at discharge compared to admission. Interdisciplinary rehabilitation can enhance self-rated performance and satisfaction with performance of daily activities among people with LEoP. Future studies of rehabilitation for people with LEoP should use a prospective design and capture the participants' process of change related to their rehabilitation period.

‘Gear up and get ready’: Collaborative curriculum for radiographers supporting percutaneous nephrolithotomy in the operating theatre

IntroductionNewly qualified radiographers often find working in the operating theatre (OT) challenging and intimidating. These perceptions, which inhibit confidence, may hinder their effectiveness in interprofessional teamwork, which may in turn adversely affect patient outcomes. A collaborative education programme was designed, building upon the foundations of competency-based education (CBE) and simulation-based mastery learning (SBML) to examine its potential in mitigating these perceptions. The objective of this research was to assess participants’ experience and level of competency after attending the curated collaborative educational programme. MethodsThe programme was developed based on the Analysis, Design, Development, Implementation, and Evaluation (ADDIE) model and comprises two teaching and learning phases: educational session and simulation. A collaborative approach was undertaken to develop an assessment checklist for the interprofessional simulation. Requirements for the simulation, such as scenario design, information and storyboard, task trainer, logistics, and learners' briefing, debrief, and feedback, were identified and assembled. The radiographers’ performance was recorded using a practical skills assessment checklist and a theory assessment. ResultsTwelve radiographers participated and showed improvement in their self-rating of learning objectives before and after the programme. The median (interquartile range) score achieved in the theory assessment, out of a possible of 11, was 9.00 (7.75–9.50). The median (interquartile range) score achieved in the simulation component, out of a possible of 16, was 15.00 (14.00–15.00). There was statistically significant difference in self-perceived performance in all learning objective domains. ConclusionThe findings from the programme were promising. The use of simulation and an assessment checklist proved to be useful learning tools in preparing newly qualified radiographers for work in the OT. Implications for practiceAssessment checklists are valuable tools that should be considered to facilitate teaching and learning. The use of interprofessional simulation activities can support radiographers in developing knowledge, professional skills, and clinical competency. It should be conducted in a timely manner to facilitate the introduction to role understanding and effective communication.

Clinical and Psychosocial Predictors of Post-Event Processing in Adults Who Stutter.

Post-event processing, defined by self-critical rumination following a stressful communication event, is significantly associated with reduced quality of life. However, despite its relevance to the stuttering experience, to date, only a few studies have investigated post-event processing among adults who stutter, and no study has identified clinical and psychosocial predictors of post-event processing. The purpose of this study was to determine the contributions of clinical markers of stuttering and psychosocial variables to post-event processing. Adults who stutter (N = 96) participated in two virtual sessions. After completing the Trier Social Stress Test, a standardized social stress task in Session 1, participants completed measures of post-event processing, clinical markers of stuttering (i.e., the experience of stuttering, self- and observer-rated stuttering severity), and psychosocial characteristics (i.e., self-perceived performance, self-esteem, social anxiety, trait, and state self-compassion) in Session 2. Hierarchical linear regression models indicated that a more negative experience of stuttering, higher self-rated stuttering severity, and greater social anxiety predicted more post-event processing. Greater self-perceived performance and state self-compassion predicted less rumination. Observer-rated severity, self-esteem, and trait self-compassion were not significantly associated with post-event processing behavior. Findings reveal clinical and psychosocial variables to consider in the assessment and mitigation of post-event processing behavior in adults who stutter, and to bolster resiliency to social stress. https://doi.org/10.23641/asha.24223213.

A comparison of the effects of occupation-based interventions with and without responsibility feedback and conventional interventions on participation in people with idiopathic Parkinson’s disease: study protocol for a randomized controlled trial

BackgroundParkinson’s disease (PD) is a neurodegenerative disorder with debilitating motor and non-motor symptoms which affect participation in meaningful occupations. Occupation-based interventions can improve participation in people with PD. Evidence for incorporating structured and intensive occupational therapy by considering the concept of responsibility is lacking for this population. This trial will compare the effects of occupation-based interventions with and without responsibility feedback and conventional interventions on participation in people with idiopathic PD.MethodsA total of 45 people with PD, between 35 and 85 years old and Hoehn and Yahr stages between I to III, will be recruited from movement disorder centers for this three-armed study. Participants will be randomized into three groups (occupation-based interventions with responsibility feedback, occupation-based interventions without responsibility feedback, and conventional interventions). All participants will receive intervention for 24 sessions during a period of 12 weeks (2 sessions per week). The primary outcome measure will be participation satisfaction. Participation frequency and restriction, self-perceived performance, performance satisfaction, motivation, volition, sense of agency, responsibility, physical activity, community integration, activities of daily living (ADL), instrumental ADL, upper extremity function, balance, fatigue, and quality of life will be measured as secondary outcome measures. All outcomes will be measured at baseline, session 9, session 17, post-intervention (week 13), and follow-up (week 25).DiscussionThis home-based high-intensity, structured, client-centered, and occupation-based intervention will be conducted by utilizing the concept of responsibility. This proposed trial may result in enhanced participation that would benefit other motor and non-motor symptoms in people living with PD. Findings from this proposed study are expected to expand the knowledge of clinicians and help them in evidence-based decision-making processes.Trial registrationIranian Registry of Clinical Trials IRCT20140304016830N13. The Effects of General Listening Anxiety and Listening Test Anxiety on Self-Perceived Listening Performance Among Chinese English Learners.

This research seeks to conceptualize foreign language listening anxiety (FLLA) and provide its measurement, and further explore the influences of FLLA on self-perceived listening performance. In Study 1, semi-structured interviews explored FLLA-arousal situations. Follow-up reliability and validity tests for the newly-developed scale were testified. In Study 2, structural equation modeling explored the relationship between FLLA and self-perceived performance, which was followed by the comparison of the effects of different types of FLLA on self-perceived performance between English and non-English major students. The componential factors of FLLA included two factors, namely general listening anxiety and listening test anxiety, and general listening anxiety was represented by FLLA in classroom, daily usage, and media learning. The results also showed that listening test anxiety negatively affected self-perceived performance; general listening anxiety positively affected listening test anxiety but did not affect self-perceived performance, and listening test anxiety played a full mediation role. Moreover, findings revealed that non-English major students' general listening anxiety was higher than that of English major students. However, the multi-group analysis showed that these two groups did not differ in the effect of general listening anxiety on listening test anxiety, and in the effects of listening anxieties on self-perceived performance. For the two groups, the mechanism of anxiety-and-performance relationship was consistent. The results of this research have expanded the knowledge of listening anxiety by distinguishing general listening anxiety from listening test anxiety. Moreover, by testifying the mediator of listening test anxiety, this research deepened the understanding of the effects of different types of FLLA on self-perceived listening performance and the intensity differences of listening anxieties in English and non-English majors. Furthermore, the research has contributed to the literature on FLLA research based on Complex Dynamic Systems Theory, and has practical pedagogical implications for future studies.

Effects of Optical See-Through Head-Mounted Display Use for Simulated Laparoscopic Surgery

This study investigated the influence of using optical see-through head-mounted displays (OST-HMDs) as a replacement for conventional monitors for an extended duration, as required for laparoscopic surgery. We developed the experiment scenario including two displays (surgical field and patient vitals) in three different combinations of the HMD and conventional monitors. An experiment was conducted to compare the task performance, situation awareness, physical discomfort, and perceived workload in the three different display settings. From this study, it was shown that even though the OST-HMD resulted in higher physical discomfort and lower self-perceived performance rating, the surgical task performance, and situation awareness were not worse, and in some conditions even better than conventional monitors. The main obstacle to using OST-HMDs to replace conventional monitors in laparoscopic surgery would be the weight of the HMDs and the head discomfort caused by wearing them for a long time.

Development and feasibility of first- and third-person motor imagery for people with stroke living in the community

BackgroundImpairment of arm movement occurs in up to 85% of people post-stroke, affecting daily living activities, and quality of life. Mental imagery effectively enhances hand and daily function in people with stroke. Imagery can be performed when people imagine themselves completing the movement or imagine another person doing it. However, there is no report on the specific use of first-person and third-person imagery in stroke rehabilitation.AimsTo develop and assess the feasibility of the First-Person Mental Imagery (FPMI) and the Third-Person Mental Imagery (TPMI) programs to address the hand function of people with stroke living in the community.MethodsThis study comprises phase 1—development of the FPMI and TPMI programs, and phase 2—pilot-testing of the intervention programs. The two programs were developed from existing literature and reviewed by an expert panel. Six participants with stroke, living in the community, participated in the pilot-testing of the FPMI and TPMI programs for 2 weeks. Feedback collected included the suitability of the eligibility criteria, therapist’s and participant’s adherence to intervention and instructions, appropriateness of the outcome measures, and completion of the intervention sessions within the specified time.ResultsThe FPMI and TPMI programs were developed based on previously established programs and included 12 hand tasks. The participants completed four 45-min sessions in 2 weeks. The treating therapist adhered to the program protocol and completed all the steps within the specified time frame. All hand tasks were suitable for adults with stroke. Participants followed the instructions given and engaged in imagery. The outcome measures selected were appropriate for the participants. Both programs showed a positive trend towards improvement in participants’ upper extremity and hand function and self-perceived performance in activities of daily living.ConclusionsThe study provides preliminary evidence that these programs and outcome measures are feasible for implementation with adults with stroke living in the community. This study outlines a realistic plan for future trials in relation to participant recruitment, training of therapists on the intervention delivery, and the use of outcome measures.Trial registrationTitle: Effectiveness of first-person and third-person motor imagery in relearning daily hand tasks for people with chronic stroke: a randomised controlled trial.Registration No: SLCTR/2017/031. DOES MINDFULNESS HAVE A ROLE IN THE RELATIONSHIP BETWEEN EMOTIONAL INTELLIGENCE AND SELF-PERCEIVED JOB PERFORMANCE?

ABSTRACT This article contributes to the existing body of literature on emotional intelligence (EI) in the workplace by empirically studying its effects on employees’ job performance. Different studies on EI and job performance have produced conflicting results. To have a better understanding of the relationship between EI and job performance, some scholars suggested that pairing EI with another individual trait would be beneficial to that effect. This study sought to examine mindfulness as the mediating factor between trait EI and employees’ self-perceived job performance, utilizing the Trait Emotional Intelligence Questionnaire – Short Form (TEIQue-SF; Petrides, 2009), Work Effort Scale (WESC; De Cooman et al., 2009), and mindfulness at work (Zivnuska et al., 2016) for data collection. A total of 111 participants participated in the study on SurveyMonkey. The results show that there is no positive relationship between trait EI and employees’ self-perceived job performance. Conversely, the results indicate a significant relationship between mindfulness and employees’ self-perceived job performance, as well as identifying mindfulness as the mediator between trait EI and employees’ self-perceived job performance. Common sources of occupant dissatisfaction with workspace environments in 600 office buildings

Previously unpublished data from over 600 office buildings in the Center for the Built Environment (CBE) Occupant Survey database are used to perform a systematic analysis of dissatisfaction in contemporary workspaces. A total of 81% of respondents expressed dissatisfaction with at least one aspect of their workspace, and 67% with more than one. Acoustics were the most common source of dissatisfaction, particularly related to people talking, speech privacy, and phones. Other challenges included a perceived lack of control over the temperature and insufficient space, along with other associated problems of densely populated offices. The analysis shows that context matters when understanding occupant dissatisfaction. Occupants of open-plan offices with low or no partitions were almost twice as likely to complain about their workspace than someone in a private, enclosed office. Being near a window decreased the likelihood of dissatisfaction compared with those who were not near a window. There was a clear relationship between self-perceived performance and satisfaction with the indoor environment. Dissatisfaction profiles found that acoustics, space, and privacy-related items co-occur for many occupants dissatisfied with more than one workspace aspect. <em><strong>Practical relevance</strong></em> Post-occupancy surveys are a useful tool for evaluating whether an office environment supports occupants while conducting their work. While highlighting the successes is important, complaints from dissatisfied occupants can identify issues and pinpoint reasons why spaces do not meet expectations. The reported challenges generally relate to the simultaneous reduction in control and personalization with increasingly open and densely populated layouts. Occupant dissatisfaction may impact performance given the reported relationship between satisfaction with the environment and feeling supported by the workspace to complete work tasks. The themes emerging from this analysis identify common dissatisfaction sources that can serve as an empirical basis to identify common problems in contemporary workspace designs.

The Effects of an Empathic Communication Workshop on Internal Medicine Residents' Self-Perceived Empathy and Their Patients' Perception of Physician's Empathy: A Single-Group Experimental Study.

Background: Empathetic communication improves the physician-patient relationship and enhances patient and physician satisfaction. This study aims to evaluate the impact of empathic communication skills training on physicians' self-perceived performance and patient satisfaction regarding the empathetic quality of their relationship with their physicians. Methods: In this single-group before-after experimental study, we recruited 50 internal medicine residents at a large teaching hospital. We assessed the residents' empathy using the Jefferson Scale of Empathy before and 3 weeks after an 8-hour workshop on empathic communication skills. We also recruited 50 of their patients before and another 50 patients 3 weeks after the training to assess the patient's perceptions of their physician's empathy using the Consultation and Relational Empathy scale. Physicians' and patients' mean scores on empathetic care at the beginning of the study were then compared using paired t-tests with their scores after the workshop. Results: The residents' mean score on Jefferson Empathy Scale increased from 81.1(95%CI:78.8-83.3) at baseline to 96.8(95%CI:93.6-100) following the workshop (p < 0.001). Before the empathetic communication skills training, patients assessed their doctors' empathy at 68.3(95%CI:63.5-73.2). After the intervention, this improved to 84.9(95%CI:82.2-87.5) (p < 0.001). Conclusion: In this study, both the residents and their patients stated that the residents' empathy skills had significantly improved after an empathetic communication workshop for internal medicine residents.

Caregivers' Profiles Based on the Canadian Occupational Performance Measure for the Adoption of Assistive Technologies.

The COPM (Canadian Occupational Performance Measure) is a tool that is based on the identification of self-perceived performance and satisfaction problems in the performance of occupations, allowing the creation of a hierarchy in the order of the interventions to be carried out, and speeding up the identification of the necessary AT (Assistive Technologies). Given the importance of the caregiver’s perception about their own performance in the design of AT, this research examines the caregiver’s profile through the COPM. A sample of 40 caregivers volunteered to participate in the study. A cluster analysis was carried out on the COPM scores. Two caregiver profiles were found in relation to the COPM measure, one with low scores on performance and satisfaction and another with high scores on both of these two variables. The main predictor was found to be the self-perception of performance. The structure was replicated through a hierarchical cluster analysis, where the role of caregivers was of interest. These results are relevant on both a theoretical and practical level.

Dare to ask in front of others? Women initiating salary negotiations

Previous research has shown that gender pay differences in the labor market may be caused by women’s reluctance to initiate salary negotiations, which is often influenced by gender-related social norms and self-perceived gender identity. In the real world, others in work environments can know “who has asked.” In a laboratory experiment, we manipulated whether the decision to initiate salary negotiations was publicly observable. We found that in a context in which negotiation is not costly, women and men showed no difference in initiating salary negotiations. When asked to publicly express their willingness to negotiate salaries, a moderately lower number of women stepped up, an effect that was not significant for men. Furthermore, women tend to tie salary negotiation decisions to their self-perceived performance even when pay raises are not bound to performance. In contrast, men were not impacted by this factor.

The Predictive Role of Hand Section of Fugl-Meyer Assessment and Motor Activity Log in Action Research Arm Test in People With Stroke.

Background:Recent findings of clinical studies have demonstrated a significant positive relationship between Fugl–Meyer Assessment of upper extremity score and the action research arm test (ARAT) score in people with stroke. Although the motor activity log (MAL) can assess the self-perception of motor performance, which can affect the performance of the upper limb, the relationship between MAL score and ARAT score still remains unclear. The objective of this study is to quantify the independent contribution of MAL score and FMA-hand score on the ARAT score in people with stroke.MethodsThis is a cross-sectional study. There were a total of 87 subjects (50 males, 37 females; mean age = 61.12 ± 6.88 years, post-stroke duration=6.31 ± 2.84 years) included in this study. Self-perceived performance in using the paretic limb was measured by MAL, including subscale of the amount of usage (MAL-AOU) and quality of movement (MAL-QOM). Functional performance of the upper limb was measured by action research arm test (ARAT). Upper limb motor control of the hand was measured by hand section of Fugl–Meyer assessment (FMA-hand).ResultsThe result showed that MAL-QOM (r = 0.648, p < 0.001), MAL-AOU (r = 0.606, p < 0.001), FMA-hand scores (r = 0.663, p < 0.001), and the use of a walking aid (r = −0.422, p < 0.001) were significantly correlated with the ARAT scores. A total 66.9% of the variance in the ARAT scores was predicted by the final regression model including MAL-QOM, MAL-AOU, FMA-hand scores, and walking aid. The FMA-hand score was the best predictor of ARAT scores, which can predict a 36.4% variance of ARAT scores in people with stroke, which controlled the effect of using a walking aid. After controlling for use of a walking aid and FMA-hand scores, the multiple linear regression modeling showed that MAL-QOM and MAL-AOU scores could also independently predict an additional 10.4% of the variance in ARAT scores.ConclusionIn addition to the FMA-hand score, the MAL score was significantly correlated with the ARAT score. Improving self-perceived performance should be one goal of rehabilitation in people with stroke. Further work developing and testing techniques to do so is clearly warranted.

Implanted Peroneal Nerve Stimulator Treatment for Drop Foot Caused by Central Nervous System Lesion: A Twelve-Month Follow-up of 21 Patients.

We report data on patient self-perceived performance, satisfaction with performance, walking ability, and adverse effects after surgical implantation of the ActiGait® drop foot stimulator.DesignProspective case study with a 12-month follow-up.SubjectsTwenty-one participants with drop foot caused by central nervous system lesion.MethodsThe patients’ self-perceived performance and satisfaction with performance were evaluated using the Canadian Occupational Performance Measure (COPM). Walking ability was assessed using a 10-m walk test and a 6-min walk. Nerve conduction of the peroneal nerve was examined in 10 patients.ResultsAt follow-up, COPM self-percieved performance from 3.2 to 6.7 points, the median increase being 2.8 (interquartile range (IQR) 2.2–5.0), p < 0.001. Likewise, the COPM satisfaction with performance increased from 2.6 to 6.9 points, the median increase being 4.2 (IQR 2.8–5.8), p < 0.001. Walking velocity increased 0.1 m/s from a baseline measurement of 0.73 m/s (95% confidence interval (95% CI) 0.03–0.2), n = 21, p < 0.01, and walking distance increased by 33 m, from a baseline measurement of 236 m (95% CI 15–51), n = 21, p < 0.001.ConclusionStimulation of the peroneal nerve by an implantable stimulator increases self-perceived performance, satisfaction with performance, and ambulation in patients with long-lasting drop foot caused by a central nervous system lesion.LAY ABSTRACTDrop foot is a common impairment following stroke or other causes of disease in the nervous system. This study of 21 patients with drop foot caused by various diseases in the central nervous system reports data on patient self-perceived performance, satisfaction with performance, walking ability, and adverse effects after surgical implantation of the ActiGait® peroneal nerve stimulator. The patients’ self-perceived performance and satisfaction with performance were evaluated using the Canadian Occupational Performance Measure. Walking ability was assessed using a 10-m walk test and a 6-min walk test before implantation and at follow-up. At follow-up, patient self-perceived performance, satisfaction with performance, walking velocity, and walking distance increased. No changes in nerve function were demonstrated. In conclusion, implantable peroneal nerve stimulation increases self-perceived performance, satisfaction with performance, and ambulation in patients with long-lasting drop foot caused by central nervous system lesion.

E-government information search by English-as-a Second Language speakers: The effects of language proficiency and document reading level

A rapid increase in the use of web-based technologies – and corresponding changes in government and local council policies – in recent years, means that many vital services are now provided solely online. While this has many potential benefits, it can place additional burdens on certain demographic groups, some of whom may become considerably disadvantaged or even disenfranchised. This is particularly problematic for English-as-a Second Language (ESL) speakers, who are often immigrants or refugees and thus have a greater need to access these e-government services, and who may struggle to understand and assess the relevance of complex documents. In this work we investigate the search behaviours and performance of native English speakers and two different groups of ESL speakers when completing e-government tasks, and the effect of document readability/complexity. In contrast with previous work, our results show significant differences between groups of varying language proficiency in terms of objective search performance, time on task, and self-perceived performance and confidence. We also demonstrate that document reading level moderates the effect of language proficiency on objective search performance. The findings contribute to our existing understanding of how English language proficiency affects search for e-government topics, and have important implications for the future development of e-government services to ensure more equitable access and use.

Maximal vs. explosive knee extensor strength in professional soccer players: inter-limb asymmetries and relationship with knee function

ABSTRACT The main aims of this study were to compare the magnitude of inter-limb asymmetry (ILA) and the relation with self-reported knee function between maximal and explosive knee extensor strength outcomes in professional soccer players. Forty-six male soccer players completed different maximal isokinetic and isometric contractions of the knee extensors for the assessment of maximal strength (peak torque and maximal voluntary contraction (MVC) torque) and explosive strength (early, intermediate, late, and peak rate of torque development (RTD)). Self-reported knee function was assessed with the International Knee Documentation Committee (IKDC) and Lysholm knee scoring scales. Peak torque and MVC torque showed comparable ILAs (8-9%), both being significantly lower than all RTD ILAs (16% on average; p < 0.001). ILAs for early RTD (21%) and peak RTD (19%) were significantly higher than all the other variables (p < 0.05). Only early and intermediate RTD were significantly correlated – though weakly – with both IKDC (rho = 0.32 for both) and Lysholm (rho = 0.36 and 0.30, respectively) scores. We conclude that explosive knee extensor strength – early RTD in particular – exhibited larger ILAs and better relations with self-reported knee function than peak torque and MVC torque in professional soccer players. These results confirm the validity and functional relevance of early RTD and the need for its inclusion in routine performance testing for soccer players. Highlights Professional soccer players exhibited larger inter-limb deficits in knee extension strength for explosive actions than for the widely-used isokinetic test. Self-reported knee function was significantly correlated with explosive strength of the knee extensor muscles but not with maximal strength. The first 50 ms of an explosive knee extension seem to be crucial for self-perceived sport performance and possibly for injury prevention.
