Retrievable Cemented Implant Restorations

Prostheses can be attached to implants or implant abutments by screw retention, cementation, or with the use of set-screws. The indications for non-screw-retained restorations are discussed with respect to implant body position and surgical limitations for implant placement. The advantages and disadvantages of each method are outlined. A technique that uses a screw threaded into the implant restoration to displace the cemented restoration is presented. Use of this technique will allow predictable removal of the cemented restorations without damage to the prosthesis or abutment.

Revision cruciate ligament surgery with retention of femoral interference screws

As the popularity of cruciate ligament reconstruction continues to escalate, the need for revision surgery has also increased. Revision cruciate ligament surgery is often associated with difficult technical challenges including removing previously placed femoral interference screws. Not only can this hardware be difficult to remove, interference screw extraction often creates a large bony defect immediately adjacent to the revised femoral tunnel that can jeopardize the fixation of the new cruciate ligament graft. It is often possible to perform revision cruciate ligament reconstruction without removing femoral interference screws. This technical note provides several examples of this technique. IMPLANT ANGULATION AND POSITION AND SCREW OR CEMENT RETENTION

The choice between cement and screw retention in implant prosthodontics may be a matter of personal preference or dictated by specific clinical situations. As set of clinical guidelines based on implant angulation and arch position and clinical considerations are presented to aid the clinician in determining the most appropriate method of retention for implant prostheses.

Five techniques for single tooth replacement on endosseous root-form implants

Internally hexed and threaded root-form implants provide the clinician with many new treatment options for single tooth restorations. Coupled with an interlocking hex on the abutment, they provide stability for single tooth restorations while maintaining the options of screw or cement retention. Five techniques for fabricating single tooth restorations are presented. Clinical and laboratory evaluations of implant angulation, quantity and quality of the gingival tissues, and limitations in vertical height are discussed in relation to design, abutment selection, and laboratory procedures for each restoration. These techniques permit design of each restoration to meet the unique anatomic, functional and esthetic needs. Both screw-retained and cement-retained abutments and prostheses are discussed.

Lag screws in sagittal split osteotomies: should they be removed?

The use of stainless steel lag screws to provide rigid internal fixation for sagittal split osteotomies was first described more than 17 years ago. It was shown that the need for intermaxillary fixation could be eliminated and that lower relapse rates could be produced. However a decade later, removal of the screws was still being advocated. In this study, 48 patients had a total of 259 screws inserted over a 5 year period. Twenty six screws were removed 10 months following insertion. Most of these were removed in the absence of any symptoms although a few were palpable or tender to touch. None of the screws removed were loose nor was there any evidence of infection or corrosion associated with them. The long term consequences of screw retention are discussed and it is suggested that they do not need to be removed routinely.
